The article deals with the issue of health care, focusing on expenditure allocated to it. The first part of the article reviews some selected European Union countries, analyzing their expenditure on their health care sectors. The second part brings an analysis of expenditure on health care in Poland. Basing on this analysis we determine the state of the Polish health care sector. Moreover, as regards the Polish health care sector, we indicate the expenditure forecasts. The article concludes with a comparison of the state of the Polish system to the state of the systems in other European countries.

A full understanding of and a competent approach to dying patients may lead to a more qu-alitative service delivery, an enhanced quality of life paradigms, and the patients’ well-being, all of which remain the ultimate goal of health care practice. The modern world has developed in parallel with secularism and religious diversity. This paper aims to illustrate the secularization process in Britain (with indications of generalized meanings) and juxtaposes it with a descrip-tion of the needs of dying patients regarding the meanings of religion and non-religion. Altho-ugh this paper draws on and provides a review of selected theoretical literature, it also addres-ses a signifi cant challenge: the lack of scientifi c research on the subject. Hence, this paper aims to give an overview of the issues, but not synthesise them. The arguments that are elaborated in the paper are also supported by the author’s current research project in the city of London. The approach here is client oriented, and concerns social and health care. Practitioners ought to become competent, and maintain their competence throughout their professional career. Religious competence seems to have not been at the centre of discussions, regardless of the historical pathway that religious discourse has drawn since the beginnings of huma-nity. The paper concludes with certain suggestions for future research and inclusive appro-aches regarding religious matters.

Purpose: To estimate the frequency of aggressive behaviors in health care institutions, and determine their influence on emotional reactions of medical workers. Materials and methods: The study involved 201 health care professionals from the regions of Lower Silesia and Opole in Poland. The authors employed the exposure to patient aggression inventory, based on the questionnaires of Merecz and Nowicka & Kolasa; this inventory divides patient aggression towards medical workers into seven different forms. Results: In over 90% of cases, health care professionals fell victim to patient aggression in a workplace. It mostly took forms of verbal aggression, a raised voice and shouting. A physical attack was reported by 45.6% of the surveyed; it resulted in physical injuries in 27.9% of psychiatric hospital workers and 24.7% of general hospital workers. As a reaction to patients’ verbal aggression, medical workers usually calmly explained that such behavior was improper. Violence and aggressive behaviors of patients evoked workers’ anger, fear, a feeling of resignation and the loss of their sense of safety. In most case's victims of patient aggression either coped with the problem themselves or asked their co-workers and superiors for help. Conclusions: Aggressive behaviors of patients arouse in medical staff, mostly anger. Medical workers usually cope with patient aggression themselves; nurses more often than other health care professionals ask their superiors and co-workers for help. It is necessary to conduct further research into the problem of patient aggression towards medical staff so that actions ensuring safety for workers can be taken.

The purpose of this paper is to analyse different views on interpersonal relations and team composition among managers and medical professionals with respect to the transition of professional roles in healthcare in Poland. To achieve that goal, a description based on a quantitative and qualitative questionnaire was conducted. Since the questionnaire covered various areas of health care, only its small fraction was used for the analysis. The main result is that most of the medical professionals and medical managers consider technology to be the single most important external factor influencing the team work efficiency and team composition in health care, and the managers consider skillset as the crucial factor determining whether a person would be a good team member. Based on the literature on professional roles in health care and their evolution in recent years, one can assume that constant development and lifelong learning would play a significant role in the healthcare systems reform. The findings are an important contribution to the discussion of the healthcare reform and its possible directions in future years as well a reference point for policy makers.

Objective. To examine disparities in access to the public in Poland, in association of the resources of health care, socioeconomic development of Provinces and their level of urbanization. Materials and method. The Provinces were divided into four groups, using the quartile method, according to the urbanization and level of socio-economic development. The socio-economic development level was identified by the Human Development Index (HDI) for each Province. The urbanization level of each Province was determined by the Index of Urbanization (URBI). Disparities level to access to public health care was identified by taxonomic measure of accessibility (TMA), calculated using resources of health care data. TMA index was compared in the different regions, depending on the level of HDI and URBI. Results. There was no linear relationship between accessibility to public healthcare and socio-economic development of each tested region, nor between accessibility to public health care in the Provinces and their urbanization level. During the study, the correlation between the TMA and HDI and URBI alsdo produced a negative result. Conclusions. An insufficient number of physicians and the limited value of contracts within the public health service may cause a drop in the availability of the public medical service sector, regardless of regional level of urbanization and socioeconomic development.

This paper addresses the issue of integrated care services as a current challenge for primary health care in Europe. It is focused on an operational definition of integrated care and documents its relevance to the recent declaration of the World Health Organization regarding primary health care. The paper also reports on experiences gained and lessons learned in Greece, a country where initial attempts towards integration of public health into primary care are currently unfolding. Additionally, it discusses the limited involvement of patients, families, and communities in health care, as well as relative absence of advocacy and care coordination at a policy level. The need for training stakeholders to define and promote integrated care is highlighted as an essential component of translating new concepts into concrete health care actions. Finally, when discussing development and implementation of a well-coordinated and integrated primary health care system, the paper provides ideas for further consideration. The present report is anticipated to open the dialogue between health care professionals, stakeholders, policy makers, and the public towards the integration of health services in contemporary Europe.

I discuss the comments from Professor Piotr Lichacz on my book, Setting Health-Care Priorities. What Ethical Theories Tell Us (New York: OUP, 2019). The idea that our reluctance to let go of life and abstain from marginal life extension is irrational is defended against his criticism. The methodology used in the book—urging us to rely in our testing on ethical theories on the content of our considered moral intuitions—is explained at length and the notion of cognitive psychotherapy involved in it is defended.

Bronię krytykowanego przezeń poglądu, że nasz opór przed rezygnacją z życia i wstrzymania marginalnego wydłużenia życia jest irracjonalny. Szeroko wyjaśniam używaną w tej książce metodologię, skłaniającą do oparcia naszego testowania teorii etycznych na zawartości naszych przemyślanych intuicji moralnych. W obronę biorę również związane z tą metodologią pojęcie psychoterapii kognitywnej.

Nursing aides are particularly susceptible to manual handling injuries because they have the primary responsibility for heavy lifting. The aim of this study was to determine why a specific group of nursing aides have the highest manual handling injury rate in their hospital. The study investigated the adequacy of the manual handling training program, knowledge of manual handling, mechanical aid availability, and use of mechanical aids. Results indicated that the nursing aides’ manual handling knowledge was adequate but that they rarely used mechanical aids. This lack of use of aids was in part due to an over-reliance on their own strength and abilities. This may have been due to a lack of suitable mechanical aids on the wards or a lack of familiarity with the available aids. This study suggests that neither training alone nor the purchase of equipment alone is likely to resolve manual handling problems.

Quality in primary health care is gaining a predominant role. Both public and private providers of primary health care services increasingly focus on quality issues, which allows them to gain a competitive advantage. In the article, fifty attributes determining the quality of services identified in literature are considered and evaluated in the context of primary health care services. Both technical attributes relating to the medical process itself and functional attributes resulting from patient experience as a client are studied. An analysis aiming to rank the importance of attributes was made on the basis of surveys carried out among 98 primary health care PHC managers. The research allowed for the identification of the most and the least important primary health care service attributes determining quality from the perspective of PHC managers. Functional attributes relating to patients` experience of care are being noticed as important and desired by patients. The findings resulting from this study can be practically applied at primary health care entities in order to maximise the satisfaction of patients through proper adjustment of business models.

The aim of their treatment is the minimum of harm to the body and then, if possible, patient return to optimal functioning in the society. Empathy and resilience have an indirect effect on raising the level of the quality of care, as empathy is the basis for good interactions between medical workers and patients, and resilience, as a dynamic process, helps caregivers adapt well to difficult working conditions, risks or traumas experienced. The development of empathy and resilience of these medical workers is possible by a training method already employed among medical students in order to both improve the quality of care, but also to more effectively prevent professional burnout. The purpose of the study was to determine the level of empathy and the level of resilience, their possible correlations with each other, as well as the relationship of these variables with socio-demographic variables. The study was carried out in 2021 among 31 doctors and 45 nurses using two main tools - the Emotional-Cognitive Empathy Questionnaire (EEP) by Ewa Wilczek-Rużyczka and the Resilience Measurement Scale(SPP-25) by Nina Oginska-Bulik and Zygfryd Juczynski, as well as a socio-demographic questionnaire. The most valuable results of the study turned out to be the demonstration of statistically significant concordant associations of empathy level with resilience level both in general, and their individual factors as statistically significant intercorrelations. In addition, statistically significant differences were shown in the case of cognitive empathy - higher levels of empathy were found among people with longer work experience and women when compared to men. In the case of the level of other components of empathy, as well as resilience, differences so as to be statistically significant could not be confirmed. It is recommended that empathy and resilience training sessions be implemented for health care workers, and that the study be continued with larger research groups, with additional control groups, to refine the programs of these training sessions.

This article presents the quality, safety and assessment system as the important tools to supervise the health care organization. Quality is the sum of the properties and characteristics of a product, process or service that is appropriate to meet the complex requirements. In the case of health care is difficult to valuate quality, the universal indicators become useful tool. The development of quality indicators requires collecting of data and their proper processing. The high quality of medical services requires continuous improvement and adaptation to patients’ needs. The quality indicators, for example, refer directly to the effects of therapy and are used to measure the success or failure of the applied therapeutic methods. Measuring the results of activities indicates only the level of quality of the services provided. The data helps to analyze information and improve the quality and safety of health care services.

The Canadian health care system is a publicly financed system administered by ten provincial and three territorial governments. The purpose of this article is to provide an overview of the universal health care system in Canada, including its history, the health status of Canadians, health care funding and spending, and health research and data collection. Health care spending in Canada amounts to 11.6% of the country’s gross domestic product and is estimated to have been $200.5 billion Canadian dollars in 2011. Hospitals account for the largest source of health care spending (29%), followed by drugs (16%) and physician spending (14%). Of the total health care spending, 70% is paid for by the public system. Due to the Canadian population being covered by the universal health care system, health data are being collected and can be used to monitor the health care system and inform evidence-based medicine.

Ankle sprains are common soft-tissue injuries that are often treated in emergency departments. These injuries can have significant consequences for the patient, including long-term morbidity and loss of productivity. The objective of this study was to examine the direct and indirect health resource utilization associated with ankle sprains. 296 adult patients with acute ankle sprains participated in the study in Kingston, Ontario, Canada. Data were collected using a one-month productivity questionnaire. Overall, 11% (95% CI, 8-15%) of the participants visited a physician following the initial emergency department visit. Almost all (95%; 95% CI, 92-97%) of the participants used medications or supportive treatments and 55% (95% CI, 50-61%) reported taking time off from work, school, or housework. The use of unpaid assistance was indicated by 56% (95% CI, 50-62%). Findings from this analysis highlight the significant patient-related and health care system burden of acute ankle sprains.

Working as a doctor, nurse, and midwife is associated with great responsibility for the health and life of patients. It is a source of many burdens, which result in marked, chronic stress leading to professional burnout and related consequences affecting all spheres of life. The research aimed to analyze healthcare workers’ preferred stress-coping strategies. The study encompassed 134 healthcare workers practicing the profession of doctor, nurse, and midwife, working in the cities of Lublin in the period from May 2022 to February 2023. We employed a self-made questionnaire and the Stress Coping Inventory (Mini-COPE). The PS IMAGO 9.0 program was used for the calculations and analysis of the results. The strategies of coping with stress most often used by doctors, nurses, and midwives were based on active coping and planning. The three surveyed groups of healthcare workers differ statistically significantly in using the denial strategy. Nurses are more likely than physicians to use denial as a coping strategy. There were no statistically significant differences in the remaining strategies of coping with stress. Healthcare workers in stressful situations often choose active ways of dealing with them, especially active coping and planning. An important factor that strengthens the choice of active strategies for coping with stress seems to be training aimed at improving the professional qualifications of healthcare workers.

Occupational health practitioners deal with the questions of confidentiality, relation between the right to work and the right to health, between individual freedom and the risks other can run. An audit of pre-employment health assessment of health care workers, in the United Kingdom and in France, discussed the efficiency of pre-employment screening. Screening tests and medical examinations should not be used as a pretext to avoid implementing effective preventive measures. The pre-employment examination has mostly to be used for education of the future employees and for collection of baseline data. Appropriate procedures such as developing preventive policies, health promotion, and control of hazards in the work environment are efficient for the promotion of equal employment rights for women, older workers, or people with disabilities.

Purpose: To present issues treating digital transformation in the health sector and highlight the marketing dimension of this process. Design/Methodology: The article is a literature review of digital transformation, healthcare, management, marketing and health economics. The analysis was carried out using professional, scientific databases based on the desk research method. The study consists of the following parts: introduction; methodology of the study; issues concerning the process of digital transformation, ; digitisation of the health sector in the context of data security and benefits for stakeholders; practical aspects of implementing digital technologies in health care; generations X and Y- in the process of digitisation of health care in the marketing dimension; limitations of the study; conclusions and practical implications. Findings: The COVID-19 pandemic has significantly accelerated the digitisation of medical services, translating into the rapid development of various e-health platforms and tools. The latest techno-technological solutions for mobile health help in daily patient care and support prevention and preventive health care. The marketing transformation process parallels the digital transformation process in health care. The availability of digital health tools would not be possible without marketing. Limitations: Limitations of the study were identified that may have affected the overall picture of considerations. First, only articles indexed in selected databases were used: Google Scholar, ResearchGate, Taylor and Francis Online and ScienceDirect. Second, the literature search used a specific combination of words using Boolean operators. Originality/value: The treatment of issues treating the digital transformation of the health sector and its marketing dimension is based on the latest literature on the subject, enriching the existing body of scholarly work on the health and marketing industry.

Purpose - To do the research of competitiveness, balanced scorecard theoretical aspects, to indicate how balanced scorecard concept can increase competitiveness of private health care companies in Latvia.Design/methodology/approach - Research methodology includes the analysis of theoretical background and two survey data about private health care companies in Latvia.Findings - Globally recognized is the balanced scorecard concept. Latvian health care companies are not using in evaluation all of the main indicators influencing the company’s competitiveness and they do not group them into four perspectives of balanced scorecard for analyzing health care companies, as well as by evaluating the factors which are significant for patients in choosing the health care company. Price level, quality of health care services and the company’s images in Latvia are significant factors that can influence the health care company’s competitiveness not only in the local markets, but in global ones as well.Originality/value - The paper combines the research of competitiveness and balanced scorecard. Competition on value is crucial for health care companies, because they have to take into account the immaterial value - patient’s health.

Purpose: The scientific objective of this research was to determine social groups affected by exclusion in Polish health care. Materials and methods: Survey was carried out among local government units and nongovernmental organizations by using authorial questionnaire distributed towards representative research group selected. Results: This work depicts activities of social welfare centers in cooperation with non-profit sector entities, in the field of exclusion from the access to health care benefits in Poland, appointing circumstances, causes and the range of this exclusion. It presents the results of the countrywide research in the context of structure and tasks of the health care, but also two points of view (institutional and social one) for resolving the same population issues. Conclusions: On the basis of the conducted analyses it has been stated that social exclusion, in the field of health care, is a significant social problem, but the biggest difficulty is the access to the rehabilitation benefits and pharmacological therapy

Health care is a fundamental element of each country’s social policy. It is mainly organised and implemented through the adoption of a certain political framework (defined objectives and priorities), strategic and operational management (planning, organising, motivating and controlling), and generation of resources (e.g. defined activities of collection and distribution of financial resources, training of medical professionals, and the purchase of technology and pharmaceuticals). These principles are either formulated on the basis of already functioning health care models or bespoke models are being created. An important element of a given model is to define its mission (the reasons for its creation and operation), while such elements as the functions, objectives, resources and methods of operation attribute to its individual properties and values. Health care systems may be organised differently. Their main distinguishing features are the ownership (public, private or mixed), sources of financing (public, private or mixed) and management (centralised, dispersed), or they can be structured with regard to political aspects (single-centric, multi-centric and pluralistic). This article, based on the latest scientific developments, presents the historical outline of the selected models for health care systems and the new concepts regarding their classification. The article also offers theoretical analyses of those health systems, which have become the models for others. The aim of this article is to present the classification and the characteristics of the selected models for health care systems, both from the historical perspective and the perspective of those currently in operation. The documents analysis method was used, which included the leading positions in the Polish and foreign literature, in the field of the issues addressed, as well as the literature published by the related institutions. This topic is already being discussed within the literature of the subject, nevertheless it is still relevant and, due to its undeniable importance, deserves further examination because it directly or indirectly concerns every human being.

Usage of the economic analysis in the study of the performance of health care system does not surprise anyone nowadays. Trends that are drawn over the years fluctuate from the technology assessment of health programs - in terms of efficiency, costs or utility for patients, through methods to establishing copayment for health services and the demand for medical services. Much of the interest is devoted to analysis of the shape of the health care system: the amount of contributions to the National Health Fund, the managing the system, both at the micro and macro level, or restructuring. Any method that allows to show dependencies, identify weaknesses/strengths of the health care system is appreciated by health policy makers. The aim of this article is an attempt of the use of models of input-output type in the analysis of the performance of the health care sector in Poland. The construction of input-output model is based on the observed data for the specified, variously defined area - it may concern: country, region, municipality, etc., hence with the appropriate designed database, it may be possible to examine the flow of health benefits - for example, expressed in zlotys. Part of the article is dedicated to theoretical aspects of the input-output models and the problems this usage can cause.

The article offers a synthetic presentation of the results of analyses of diverse data resources and other scientific information concerning the healthcare in Poland. These studies were conducted in order to put forward positive changes that are not only to prevent the fall of the system, which is highly probable, but also to improve the availability of medical services and increase their quality. These studies are of major importance because the availability of medical (healthcare) services is largely determined by whether it is possible to implement the idea of sustainable development among employees, which indirectly determines the possibility of implementing sustainable development principles incorporated into legal circulation under one of the first articles of the Constitution of the Republic of Poland. The concept of sustainable development is not only one of the important reasons which call for modernisation of the functioning of the healthcare system, but also an idea which, once applied, may be helpful in solving healthcare problems. Considering the fact that the largest problem of this system in Poland lies in the shortage of employees, it becomes clear that by trying to ensure sustainable development of employees working in other industries, first, it is necessary to take care of their health, which must lead through sustainable development of healthcare workers. The aim of the article is to present the comprehensive nature of the idea of sustainable development of employees, the role of health protection in achieving a balance by employees, and to show the way to build integrated programmes to maintain the health of employees and their families against the background of the limitations of the Polish healthcare system.

Health care facility (In Polish ZOZ) is a basic, yet complex organizational and legal model within which, health service can be provided to patients in Poland. In the light of law ZOZ can receive funds from paid health benefits obtainable on the basis of bilateral agreement. Those contracts include agreement for the provision of health care service concluded between the service provider and the director of provincial department of National Health Fund (In Polish NFZ), commonly known as a contract. The subject of this paper is to present current legal basis and the rules for entering into agreement with National Health Fund by public and private health care facilities. The aim of this article is to attract the attention to the role of contract with NFZ as of the basic source of financing health care facilities and also to explain methods of financing health services awarded on the basis of contract.

Introduction and objective. The Polish health service is in need of improvement because of the increasing number of geriatric patients. Identifying the resources available to patients is important for ameliorating deficits. The goal of this research was to measure the health and wellbeing of seniors and to investigate to what extent health services meet their needs. Differences in access and scope of received services between patients from different localities (i.e villages, small towns and big cities) were analysed. Materials and method. Questionnaires (Satisfaction with Life Scale; VAS Numeric Pain Distress Scale) and a structured interview assessing the quality of healthcare were carried out in 2015 on a sample of 459 seniors. Each candidate gave consent for participation in the study. Statistica 12.5 software was used for analyses. Results. The overall measure of satisfaction with life of the respondents (M=22.34 SD=5.78) was good. Inhabitants of small towns had a significantly lower sense of quality of life than seniors from big cities (Tukey HSD = 0.047, p<.05). Multimorbidity and polypharmacy were present to a limited extent. The issue of discrimination of older individuals by medical personnel affected a fifth of seniors living in villages, and one- third of those living in big cities. Geriatric patients did not have equal opportunities for access to medical services. Conclusions. State-provided geriatric care is insufficient and does not meet the real needs of patients. A holistic care approach and/or individualized care for older adults are often indicated but are only theoretical constructs in Poland. This inefficient system places the burden of caregiving on the family of the ageing patient. Anxiety among geriatric patients is growing regarding exclusion and lack of care.

Podmioty lecznicze stawiają czoła zmieniającym się warunkom działania. Zarządzający tymi organizacjami coraz chętniej sięgają po narzędzia z zakresu zarządzania, w tym zarządzania strategicznego. Jednym z nich może być analiza KCS, której zadaniem jest m.in. wskazanie krytycznych obszarów funkcjonowania organizacji, na których należy skupić uwagę i doskonalić je, by zdobyć przewagę konkurencyjną. Przegląd literatury wskazał, iż problematyka KCS w ochronie zdrowia jest szczególnie słabo rozpoznana. Autorów zainteresowało zagadnienie na ile różnice występujące między sektorem prywatnym i publicznym wpływają na identyfikację i hierarchizację KCS podmiotów leczniczych. W artykule przedstawiono wyniki badań pilotażowych dotyczących identyfikacji kluczowych czynników sukcesu dla podmiotów realizujących świadczenia okulistyczne oraz podobieństwa i różnice pomiędzy KCS świadczeniodawcy prywatnego i publicznego.
Health care providers face the challenges of changing activities condition. Providers’ management more often uses the managerial tools, including strategic management schemes. One of the useful tool is Critical Success Factors (CSFs), which is to be applied to point critical areas of organizations’ function which should be focus on and improve to achieve competition advantage. Review of the literature revealed that CSFs methodology among health care organizations is only narrowly explored. The authors were interested whether the differences between public and private sector leaded to identification and hierarchization CSFs of health care providers. In the article are presented the results of pilot research which was set to identify CSFs for two ophthalmology clinics representing public and privet sector.

Purpose: The aim of the article is to present the theoretical assumptions of the model of professional competencies in health care units. It was assumed that competences are a multidimensional concept and require an integrated approach that allows for the construction of a competency model that reflects their real complexity. A list of professional competencies will be presented, which will be subject to empirical verification in the course of future research by the authors in order to identify key competencies. Design/methodology/approach: The proposed lists of professional competencies (six domains) was created and are based on the analysis of healthcare competencies models the study of the literature - and the Authors’ observations of the analyzed entities. Findings: Presented model of professional competencies in health care units contains six domains with sub-competencies. The importance of assessing competences is undeniable. Competence recognition offers a way to develop workforce planning and career opportunities of practicing medical staff. Having an instrument that identifies existing competences and those that need to be acquired becomes significant for distinguishing the singularity of actions for a professional practice which is safe, humane and with no risk to the client, the medical staff or the health care organization. Originality/value: An identification the professional competencies of health care units managers significantly shaping competences of such organizations especially relevant in pandemic time.

Findings: Presented model of managerial competencies in health care units contains six domains with 32 competencies. These domains capture the dynamics and complexity of health care unit’s manager’s role and reflect the dynamic realities in health leadership today. Originality/value: An identification the managerial competencies of health care units managers significantly shaping competences of such organizations especially relevant in pandemic time.

Opis:
The objective of efficient management in the public sector is first of all the rational use of limited resources that sector has. It is only possible when the performance of activities is entrusted to qualified managers specializing in the management of public sector organizations. It also depends on the implementation of modern management methods, for example procedures which enforce the efficient use of material (especially financial) resources: it is so-called management technology. Health protection and the management of public health care system units is one of the key elements of the state policy carried out as part of its constitutional duty regarding the common good. Within the system of managing these resources, territorial self-governments, particularly communal self-governments, are responsible for performing the activities which have the fundamental importance for citizens. The problem discussed in this article is first of all the clarity of planning in self-governmental independent health care centers and the availability of information on the management of public financial resources. It presents the results of research carried out in selected independent public health care centers in Warmia and Mazury region. Apart from drawing certain conclusions, the aim of the article was to propose changes in the financial management system of self-government health care units, especially planning which involves the performance budget execution system of the revenue and expenditure plan.

The healthcare industry is a growth driver. However, the health system is facing a crisis, affected by the financial development in Europe. An almost completely regulated market is just as little use, as a largely deregulated market such as in the U.S.A. Both lead to gaps in the sustainable and comprehensive patient care. Based on the German Healthcare System, an analysis is performed. Currently, the German health care system is in a transformation process. Traditional forms of health care services provision and the existing governance system are coming to their limits. The current health care system no longer meets the requirements for ensuring accessible and affordable health care services. As new players on the German hospital market, commercial hospital groups have emerged. To get more informed on the effects at the regional level, a scenario analysis was performed. A trend scenario which shows a clear trend toward a substantial increase of regional imbalances was developed. On one hand, there are highly profitable regions with excellent medical service provision by commercial hospital groups, and on the other hand, there are peripheral regions with a second-rate medicine, which are left to the public sector. The paper derives first ideas about a new structure of the healthcare system for ensuring accessible and affordable health care services for the citizens. The paper shows first ideas about the transformation of healthcare as an opportunity for growth.

The terminally ill children have been perceived as the patients having special needs who require special care. Most of them have been oncologically or neurologically sick children, also those with different types of congenital physical abnormalities and metabolic diseases. In addition to the Paediatric Department, the terminally ill children have also been provided with care by health care professionals and mobile hospic and some children have been located in the Children's Home in the palliative care sector since birth. The priority objective of the health and nursing care is to ensure an adequate ethics not only to a child but also to its parents (relatives) and implement palliative care focused on life quality and eliminate suffering in a terminally ill child.

Pojęcie efektywności w ochronie zdrowia jest używane przez różnych przedstawicieli tego sektora w odniesieniu do różnych jego poziomów: od leczenia jednostki chorobowej, przez pojedyncze jednostki organizacyjne, po system ochrony zdrowia jako całość. W przedstawionym artykule zaprezentowano rozważania na temat pojęcia efektywności w ochronie zdrowia jako celu. Przedstawiono, jakie zjawiska zachodzące w ochronie zdrowia wpływają na uzyskiwane efekty i co może być postrzegane jako efektywne. Pokazano to wraz z dyskusją dotyczącą celów systemu ochrony zdrowia oraz możliwości ich realizacji, a także efektywności ich osiągania. Zmiany zachodzące w systemie ochrony zdrowia w ostatnich latach wskazują, że udaje się osiągać najważniejszy cel tego systemu: poprawę zdrowia populacji. Dzieje się tak, mimo że działania stosowane przez państwo i jego agendy (motywacje dla zakładów) - wymuszając wzmacnianie efektywności pojedynczych organizacji ochrony zdrowia - przyczyniają się do pogorszenia warunków funkcjonowania dla tych organizacji i powodują problemy systemu jako całości.
The term: effectiveness in healthcare is used by different representatives of the sector in relation to its different levels: from the treatment of a disease entity, by single organizational units to the health care system as a whole. The discussion on effectiveness in health care as a goal of the health care system is presented in this article. The phenomena which take place in health care and affect it as well as factors that can be perceived as effective have been presented. The discussion on health care system goals, ways to achieve them and effectiveness of achieving them has also been presented. Changes in the health care system in the recent years show that the most important goal of the system (improvement in the population's health) is being achieved. This happens in spite of activities undertaken by the state and state agencies (incentives for health care units) - forcing an increase of individual health care organizations' effectiveness - which contribute to the deterioration of the conditions, in which these organizations work and which cause problems for the system as a whole.

The scope of this paper is that it investigates and proposes a new clustering method thattakes into account the timing characteristics of frequently used feature words and thesemantic similarity of microblog short texts as well as designing and implementing mi-croblog topic detection and detection based on clustering results. The aim of the proposedresearch is to provide a new cluster overlap reduction method based on the divisions ofsemantic memberships to solve limited semantic expression and diversify short microblogcontents. First, by defining the time-series frequent word set of the microblog text, a fea-ture word selection method for hot topics is given; then, for the existence of initial clusters,according to the time-series recurring feature word set, to obtain the initial clustering ofthe microblog.

Introduction and objective. Measuring the level of patient satisfaction is a useful tool in delivering quality care that is responsive to consumer preferences. Various socio-demographic factors might be considered as potential predictors of patient satisfaction. The aim of the study was to assess whether place of residence (rural/urban) affects patient satisfaction with hospital health care. Materials and method. Data were obtained using face-to-face questionnaire, administered in five large, urban hospitals in Podlaskie Province, north-eastern Poland, during 2014–2018. The study sample comprised of 1,624 participants (585 rural, 1039 urban) who assessed satisfaction with 28 hospital health care items. The means and standard deviations were presented to compare 28 satisfaction items between the rural and the urban samples. Regression analysis was used to determine whether location difference (rural vs urban) affected patient satisfaction with various domains of hospital health care. Results. The mean results of 28 satisfaction items on the 1–5 scale were similar among the rural and the urban samples, and generally skewed towards positive experiences. In the univariate analysis, significant associations between place of residence and patient satisfaction were identified with regard to three components of inpatient care: 1) hospital settings and staff care, 2) doctors’ professional skills, and 3) hospitalization outcomes. After adjusting for socio-demographic variables, the association remained significant only with respect to satisfaction with hospitalization outcomes (b = 0.121; SE = 0.055; p = 0.028). Conclusions. Some evidence for differences in patient satisfaction by place of residence was found. Study findings may be helpful in implementing care quality improvement programmes.

Aim/purpose – The aim of this study was to estimate the effect of health insurance on Out-of-Pocket (OOP) health care expenditure in Kenya. It is informed by persistence in the challenges of access and utilization of quality and affordable health care services. Previously, researchers have estimated the effects of different forms of health care financing on its demand and shown that affordability not only enhances access and use of health services but also cushions people against adverse financial risks associated with catastrophic health care spending. Design/methodology/approach – The study used the Kenya Household Health Expenditure and Utilization Survey (KHHEUS) 2013 data, and employed the two-part model estimation approach. The sample size considered in the estimation was 6,961 with the unit of analysis being an individual household member. Findings – The overall results of this study showed that having a health insurance cover did not exclude one from using OOP payments. However, the probability of such spending was low for people with insurance compared to those without insurance. Additionally, if any cash was paid for any health care service, having insurance did not have a significant effect on the level of that spending. Research implications/limitations – The findings of this study imply that uptake of health insurance does not fully cushion people from both using cash payments when seeking health services and ramifications of catastrophic health care spending. The study however, encountered challenges of unavailability of more recent data in the KHHEUS series. In addition, the sample size was relatively small to the population after data cleaning. Originality/value/contribution – Potential effect of health insurance on OOP health care payments had not been explored in Kenya. As such, this study filled this gap. In addition, the two-part model estimation technique was also employed with the latest household health survey data.

BackgroundWhen assessing physical activity (PA), particular attention should be paid to medical university students who are taught to be health care professionals (HCPs) responsible for maintaining health in humans. However, different studies have shown that HCPs exhibit the same unhealthy behaviors as the general population. This study analyzed PA among medical university students of different faculties and their adherence to current PA recommendations.Material and MethodsData from 216 medical university students of physiotherapy, dietetics and pharmacy, including males (N = 44) and females (N = 172), the mean age of 22.3±1.8 years, were collected. The International Physical Activity Questionnaire in its long form (IPAQ-LF) was used to assess and classify PA behaviors. The results were analyzed in accordance with World Health Organization recommendations regarding PA.ResultsOver 60% of all the students were classified as active during all-day activity. However, while analyzing PA in different domains, the same shares of all the students were still insufficiently active during leisure time, and so they did not meet the recommendation of >75 min/week of vigorous PA, >150 min/week of moderate PA or an equivalent combination. All the students self-reported PA mainly in the work and transport domains. Generally, physiotherapy students were the most active and performed PA with higher intensity.ConclusionsThis study revealed a low level of leisure time PA among the students, and no habit of regular PA. Some changes in medical education should be suggested to include physical education as a long-term subject in medical school curricula. Future research is needed to investigate the exercise barriers that students perceive, which can guide future interventions aimed at improving their PA, and thereby impact on the quality of health care which they will provide.

Social enterprises (SEs) are organizations that pursue a purpose of general interest through the carrying out of economic activities on a steady and permanent basis. Despite the legal, economic and social differences among the Member States (MSs) of the European Union, SEs display at least two similar characteristics. Firstly, SEs are capable of combining entrepreneurial activities together with the pursuit of a social mission. Secondly, SEs largely deliver health care services both independently and in close partnership with public authorities, either through public procurement or by means of direct co-operation agreements. In this perspective, modern welfare systems rely heavily on a significant proportion of health care services and benefits provided by SEs. In addition, in some MSs, such as Italy, SEs are supported by enabling legal frameworks, which also include some important tax benefits. Against this background, the article aims to shed light on the legal aspects and the role of SEs in the delivery of health care services as well as their contribution to ensuring citizens/patients their fundamental right to health.

Health care is a key sector of every economy and is of grate medical, so- cial, and economical importance to all citizens. It is also one of the most diverse sectors in the world, especially in the aspect of financing. Some countries offer “free” care, covered by the national budget, while others have implemented various forms of privet financing, like privet healthcare insurances. In Poland most medical services are funded by the State and ideas of privatisation are in progress. Despite that it has been estimated that 30% of overall healthcare expenditures are covered by households. This rate is among the highest in Europe, which suggests low efficiency of health care sector. Analysis of magnitude and structure of health care expenditures according to financing sources can give essential data for sector’s efficiency evaluation. The main goal of this paper is to carry a spatiotemporal analysis of healthcare expenditures and their structure in OECD countries in years 2000-2007. Spatial differentiation of expenditures according to their sources and their dynamic of changes will be researched in chosen countries, including Poland. Three dimensional (time–space structure) analysis will be performed using panel shift-share analysis based on Berzeg model
