Introduction and aim. This study compared the efficacy of core stabilization (CSE) and trunk balance exercises (TBE) with flexibility training on pain-related disability (PRD), psychological status (PS) and fear avoidance belief (FAB) in patients with non-specific chronic low back pain (NSCLBP). Material and methods. Twenty-eight (28) participants diagnosed of NSCLBP were randomly assigned into CSE, TBE, and control groups (CG). Participants in CSE (n=10); TBE (n=8) and CG groups (n=10) received core stabilization exercise, trunk balance exercise and back care advice respectively. All participants received flexibility training in addition to treatment in their respective groups. Assessment of outcomes were done at baseline, end of 4th and 8th week. Results. There was significant improvement in all outcomes in the CSE, TBE and CG at 8 weeks; PRD (p=0.005, p=0.008, p=0.005), PS: depression (p=0.005, p=0.008, p=0.007); anxiety (p=0.005, p=0.007) and FAB about work (p=0.005, p=0.007, p=0.005); about physical activity (p=0.005, p=0.018, p=0.006). Comparison of outcomes between CSE and TBE groups showed no significant difference (p>0.05). Conclusion. Both CSE and TBE with flexibility training are effective in improving PRD, PS and FAB of patients with NSCLBP.

The current spread of SARS-CoV-2 indicates a long-term fight against the widespread and exponential increase in morbidity and mortality across the globe. A variety of non-pharmacological strategies to mitigate and suppress virus transmission have been investigated and introduced. Currently, emerging studies focus mostly on the management of hospital-treated patients in the acute phase of the disease, including the legitimacy of using physiotherapeutic procedures. However, current literature lacks guidelines for rehabilitation related to maintaining continuity and universality of the therapy after the end of the acute phase of the disease and discharge from hospital. The authors suggest implementing an immediate rehabilitation program in post-infection patients as data from previous epidemics of respiratory-related viral diseases shows that COVID-19 survivors should be expected to have impaired lung ventilation function, and reduced exercise tolerance and muscular weakness, and prolonged return to work and participation. It should be assumed that only the introduction of immediate recommendations for the implementation of rehabilitation procedures based on simple and well-known tests, as well as their obligatory regime, can contribute to the reduction of respiratory disability leading, in a short time, to infections recurrence and, in the long run, to a lower quality of life and socioeconomic burden on the population. This article presents a respiratory rehabilitation program for COVID-19 survivors, recommended by the Polish Society of Physiotherapy. This program was approved by the Polish Minister of Health and implemented as a pilot program at the Hospital of the Ministry of the Interior and Administration in Głuchołazy, Poland. Psychological distress is a multidimensional concern affecting patients’ ability to cope with cancer, its physical symptoms, and treatments. This study examined the effect of an exercise program and a group psychotherapy program on the quality of life of Greek cancer patients. The sample consisted of 39 cancer patients (10 males and 29 females), assigned randomly in three groups of 13 patients each group, that is, a control, an exercise and a psychotherapy group. The duration of the training program for the individuals of the exercise group was 10 weeks at a frequency of two sessions per week, 60 minutes each session. The patients of the psychotherapy group received a 10 weeks’ supportive–expressive group therapy, at a frequency of one time per week, of 90 minutes each meeting. The quality of life SF-36 questionnaire was administered to examine the short-term effect of both programs prior and after intervention on quality of life. Control group individuals did not participate in any program and they just filled in the SF-36 questionnaire prior and after intervention. Improvement in “vitality” (p = .006) and mental health subscale (p = .011) was statistically significant between pre and post measures in the supportive therapy group. All other domains exhibit no significant changes. In the exercise group, physical functioning, role functioning and emotional role values were also improved but not to the point to generate statistically significant results. The findings of the present study support the positive impact of psychotherapy intervention on vitality and mental health component of patients with cancer, followed in less extent by the beneficial effect of the exercise program.

Introduction: Knee osteoarthritis is a common disease, particularly in the elderly population. Degenerative disorders of the cartilage cause pain and stiffness in the knee joint. As a result, patients frequently experience limitation in their daily activities. Purpose: To compare the effects of exercise, acupuncture, and magnet therapy on symptoms and physical function among middle-aged and elderly people with knee osteoarthritis. Materials and methods: We carried out a parallel three-arm randomized clinical trial in an outpatient clinic of the Department of Physical Medicine and Rehabilitation at a University Hospital. In total, 60 female patients of age ≥ 40 years with chronic knee osteoarthritis were randomly allocated to one of three groups of isometric exercise, acupuncture, and magnet therapy. The primary outcome measure was subjective pain rated according to the Visual Analog Scale. We also used the Western Ontario McMaster Universities Osteoarthritis Index for assessing and monitoring the symptoms of knee osteoarthritis in order to evaluate the efficacy of the interventions. An isometric exercise program was prescribed, consisting of three sessions per week for six weeks. Participants in the acupuncture group received three treatment sessions per week for six weeks, each lasting for 30 minutes. For pulsed electro-magnetic field therapy, a total of three 30-minute sessions per week for a period of 6 weeks were administered to each patient. Results: There were no statistically significant differences between the study groups in terms of the outcome measures (all p values > 0.05). However, within-group analyses showed that there were significant differences regarding the clinical outcomes in each group (all p values < 0.05). Conclusion: All three therapeutic modalities are effective and could be used as the first line of treatment for knee osteoarthritis. Moreover, it should be noted that the favorable outcomes remained at least two months after treatment concluded.
Wstęp: Choroba zwyrodnieniowa stawu kolanowego jest powszechnym schorzeniem, zwłaszcza u osób w podeszłym wieku. Zaburzenia zwyrodnieniowe chrząstki wywołują ból i sztywnienie stawu kolanowego. W rezultacie, pacjenci często napotykają ograniczenia w codziennych czynnościach. Cel: Porównanie wpływu ćwiczeń, akupunktury i magnetoterapii na objawy i funkcje fizyczne wśród osób w średnim wieku i u osób starszych z chorobą zwyrodnieniową stawu kolanowego. Materiały i metody: Przeprowadziliśmy randomizowane badanie kliniczne w trzech grupach równoległych w poradni Oddziału Medycyny Fizykalnej i Rehabilitacji Szpitala Uniwersyteckiego. W sumie, 60 pacjentek w wieku 40 lat, z przewlekłą chorobą zwyrodnieniową stawu kolanowego, przydzielono losowo do jednej z trzech grup: ćwiczeń izometrycznych, akupunktury i magnetoterapii. Głównym kryterium oceny był subiektywny ból, oceniany według wizualnej skali analogowej (ang. Visual Analog Scale). Do oceny i monitorowania objawów choroby zwyrodnieniowej stawu kolanowego oraz w celu oszacowania skuteczności zabiegów, użyliśmy również indeksu WOMAC (ang. the Western Ontario McMaster Universities Osteoarthritis Index). Zalecono program ćwiczeń izometrycznych, złożony z trzech sesji tygodniowo, przez okres 6 tygodni. Uczestniczki grupy z akupunkturą, otrzymały trzy 30-minutowe sesje terapeutyczne na tydzień, przez okres 6 tygodni. W terapii pulsacyjnego pola elektro-magnetycznego, zastosowano u każdej pacjentki w sumie trzy 30-minutowe sesje na tydzień przez okres 6 tygodni. Wyniki: Nie było różnic istotnych statystycznie pomiędzy grupami badawczymi pod względem wyników pomiaru (wszystkie wartości p > 0.05). Jednakże, analizy wewnątrzgrupowe wykazały, że były istotne różnice dotyczące wyników klinicznych w każdej z grup (wszystkie wartości p < 0.05). Wnioski: Wszystkie trzy techniki terapeutyczne są efektywne i mogłyby być użyte jako pierwsza linia leczenia choroby zwyrodnieniowej stawu kolanowego. Ponadto, warto zauważyć, że pozytywne efekty utrzymywały się przez co najmniej dwa miesiące po zakończeniu leczenia.

In this article, authors wish to share their experiences in work with the patients after cardio-thoracic surgery. Consecutive stages of early hospital rehabilitation and applied methods of therapeutic conduct have been described in detail. There are two main stages in cardio-surgical physiotherapy. Stage I – pre-operation preparation, stage II – early rehabilitation consisting of two periods: the first one run in an intensive care unit (ICU), the second one in a surgical unit. Along with the description of physiotherapeutic techniques, the most frequently occurring problems a therapist can face as well as the ways of managing them have been presented. The authors present certain dissimilarities in surgical rehabilitation of persons with different kinds of interventions on heart: CABG, plasty or the exchange of heart valve (aortic, mitral, more seldom tricuspid), the heart transplantation, operations of chest aortic aneurysm and abdominal aortic aneurysm, plasty of cervical arteries and corrective operations of cardiac defects. In the first stage the significance of patient’s complex preparation for the operation highlighting the importance of the patient’s education, which guarantees better postsurgical rehabilitation, is emphasized. In the second stage standard physiotherapeutic methods need to be aided by modern equipment: BIPAP Vision or Cough Assist. It has been observed that education, life quality assessment and support groups for patients after cardio-surgery have positively contibuted to rehabilitation process.
W niniejszym artykule autorzy dzielą się swoimi doświadczeniami związanymi z pracą z chorymi poddawanymi operacjom kardiochirurgicznym. W szczegółowy sposób przedstawione są stosowane metody postępowania na poszczególnych etapach wczesnej rehabilitacji szpitalnej. Wyróżniono, w ramach fizjoterapii kardiochirurgicznej, dwa główne etapy. Etap I, to przygotowanie przedoperacyjne, etap II – rehabilitacja wczesna, składająca się z dwóch okresów: pierwszego prowadzonego na oddziale intensywnej terapii, oraz drugiego – na oddziale pooperacyjnym. Wraz z opisem stosowanych technik fizjoterapeutycznych wskazano najczęściej występujące problemy, z którymi terapeuta może się spotkać oraz przedstawiono sposoby radzenia sobie z nimi. Zaprezentowano również pewne odmienności w rehabilitacji pooperacyjnej u osób z różnymi rodzajami zabiegów kardiochirurgicznych, tj: zabiegów pomostowania tętnic wieńcowych, plastyki lub wymiany zastawek serca (aortalnej, mitralnej, rzadziej trójdzielnej), transplantacji serca, operacji tętniaków aorty piersiowej i brzusznej, plastyki tętnic szyjnych i korekcji wad wrodzonych serca. W ramach etapu I wskazano znaczenie kompleksowego przygotowania pacjentów przed operacją, podkreślając ważne miejsce edukacji chorego, która gwarantuje lepsze pooperacyjne usprawnianie, szczególnie ważne w trudnych pierwszych dobach po zabiegu. W drugim etapie rehabilitacji zaznaczono potrzebę wspomagania standardowych metod fizjoterapii stosowaniem nowoczesnej aparatury: BIPAP Vision oraz Cough Assist. Zwrócono uwagę na pozytywne efekty włączenia w proces rehabilitacyjny – edukacji, pomiaru jakości życia i tworzenia grup wsparcia dla chorych po operacjach na sercu.

Introduction. Exercises have been shown to relieve symptoms in non-specific chronic low back pain (NSCLBP) patients. Aim. This study compared the effects of cognitive behavioural therapy (CBT) and core stabilization exercises (CSE) on pain-related disability, psychological status and sleep disturbance in patients with NSCLBP. Material and methods. This randomized controlled trial involved a total of thirty-seven (37) participants. They were randomly allotted into three groups [CBT (11), CSE (14) and control (12)]. The intervention was done once per week for duration of 60 minutes for the CBT group, 30 minutes for CSE group and 10 minutes for the control group twice per week for 8 consecutive weeks. Assessment of outcome was done at baseline, 4 weeks and 8 weeks. Data were analyzed using statistical package for social science version 25 at alpha level of less than 0.05. Results. The results of this study showed that there was significant improvement in the level of pain-related disability (p= 0.001), level of anxiety (p =0.001), depression (p = 0.01, p = 0.001, p =0.001) and sleep disturbance (p = 0.001) in all the groups (CBT, CSE, control) post treatment. Conclusion. CBT and CSE are both effective in the treatment of pain-related disability, sleep disturbance, and psychological status of NSCLBP patients.

Background Comprehensive cardiac rehabilitation aims to restore pathophysiological and psychosocial consequences of myocardial infarction (MI). The aim of the study was to assess how exercise-only-based cardiac rehabilitation (ECR) influences the attitude to the therapy (ATT), to the aims in life and professional work (AAL) amongst men and women after MI. Material and Methods The study comprised 44 post-MI patients: 28 men and 16 women, mean age 58±10 years old, referred to ECR. Patients underwent 24 interval cycle ergometer trainings 3 times a week. At the beginning and after the training program (TP) each patient underwent exercise stress test (EST) and was scored to ATT and ALL based on the Psychological Effects of Rehabilitation Score Scale (PERSS) according to Tylka and Makowska. The analysis covered: 1) EST findings: maximal workload and test duration (min), 2) ATT and AAL based on PERSS, 3) resuming professional work. Results Exercise capacity improved significantly after TP. Attitude to the aims in life and professional work significantly increased in the whole group (4.4±2.8 vs. 5.1±2.4, p < 0.01) and separately in men (4.5±2.9 vs. 5.1±2.5, p < 0.05) and women (4.3±2.6 vs. 5.0±2.0, p < 0.05). Attitude to the therapy did not change significantly in the whole group (5.6±2.8 vs. 6.0±2.8) and in men (5.9±2.9 vs. 6.0±2.9), but increased significantly in women (5.0±2.5 vs. 6.1±2.7, p < 0.05). Professional work was resumed, averagely by 86.4% of all patients (85.7% men and 87.5% women). Conclusions Physical training beneficially influenced post-MI men’s and women’s attitude to the aims in life, professional work and attitude to the therapy in women. Med Pr. 2019;70(1):1–7

Objectives The purpose of the current study was to explore the effects of a mobile health (mHealth) intervention based on the Persuasive System Design (PSD) model on health-related outcomes among office workers. Material and Methods The authors conducted a trial that consisted of a 4-week baseline and an 8-week intervention period by reference to 23 office workers in a private research company. The mHealth application was developed to improve these workers’ daily step count, decrease their sedentary time, and increase their sleep duration in accordance with the PSD model. The app features included at least 1 principal factor from each of the 4 main categories of the PSD model (primary task support, dialogue support, system credibility support, and social support). The objective health-related variables were measured using a smartwatch (Fitbit Luxe) that was synchronized with the application using the Fitbit Web Application Programming Interface. Subjects used the app, which included self-monitoring, personalized messages, education, and a competition system for users, during the intervention period. Results Sedentary time exhibited a significant decrease (a median reduction of 14 min/day, p < 0.05) during the intervention period. No significant differences in daily step count and sleep duration were observed between the baseline and intervention periods. Conclusions This study suggests that the mHealth intervention based on the PSD model was useful for reducing sedentary time among office workers. Given that many previous studies on this topic have not been based on any theories, future studies should investigate the impact of structured selection behavior change theories on health-related outcomes among office workers.
