Opis:
Nursing aides are particularly susceptible to manual handling injuries because they have the primary responsibility for heavy lifting. The aim of this study was to determine why a specific group of nursing aides have the highest manual handling injury rate in their hospital. The study investigated the adequacy of the manual handling training program, knowledge of manual handling, mechanical aid availability, and use of mechanical aids. Results indicated that the nursing aides’ manual handling knowledge was adequate but that they rarely used mechanical aids. This lack of use of aids was in part due to an over-reliance on their own strength and abilities. This may have been due to a lack of suitable mechanical aids on the wards or a lack of familiarity with the available aids. This study suggests that neither training alone nor the purchase of equipment alone is likely to resolve manual handling problems.

Opis:
Working as a doctor, nurse, and midwife is associated with great responsibility for the health and life of patients. It is a source of many burdens, which result in marked, chronic stress leading to professional burnout and related consequences affecting all spheres of life. The research aimed to analyze healthcare workers’ preferred stress-coping strategies. The study encompassed 134 healthcare workers practicing the profession of doctor, nurse, and midwife, working in the cities of Lublin in the period from May 2022 to February 2023. We employed a self-made questionnaire and the Stress Coping Inventory (Mini-COPE). The PS IMAGO 9.0 program was used for the calculations and analysis of the results. The strategies of coping with stress most often used by doctors, nurses, and midwives were based on active coping and planning. The three surveyed groups of healthcare workers differ statistically significantly in using the denial strategy. Nurses are more likely than physicians to use denial as a coping strategy. There were no statistically significant differences in the remaining strategies of coping with stress. Healthcare workers in stressful situations often choose active ways of dealing with them, especially active coping and planning. An important factor that strengthens the choice of active strategies for coping with stress seems to be training aimed at improving the professional qualifications of healthcare workers.

Occupational health practitioners deal with the questions of confidentiality, relation between the right to work and the right to health, between individual freedom and the risks other can run. An audit of pre-employment health assessment of health care workers, in the United Kingdom and in France, discussed the efficiency of pre-employment screening. Screening tests and medical examinations should not be used as a pretext to avoid implementing effective preventive measures. The pre-employment examination has mostly to be used for education of the future employees and for collection of baseline data. Appropriate procedures such as developing preventive policies, health promotion, and control of hazards in the work environment are efficient for the promotion of equal employment rights for women, older workers, or people with disabilities.

The terminally ill children have been perceived as the patients having special needs who require special care. Most of them have been oncologically or neurologically sick children, also those with different types of congenital physical abnormalities and metabolic diseases. In addition to the Paediatric Department, the terminally ill children have also been provided with care by health care professionals and mobile hospic and some children have been located in the Children's Home in the palliative care sector since birth. The priority objective of the health and nursing care is to ensure an adequate ethics not only to a child but also to its parents (relatives) and implement palliative care focused on life quality and eliminate suffering in a terminally ill child.

Objectives: To elucidate the association between metabolic syndrome (MetS) and disability due to low back pain (LBP) among care workers. Material and Methods: This cross-sectional study enrolled 656 care workers having experienced LBP in the year prior to the year of this study. The Roland-Morris Disability questionnaire (RDQ) and self-reported questionnaires regarding LBP, fear of movement, depressive symptom, psychosocial factors, intensity of pain, and duration of pain were administered, and a medical examination was performed. Metabolic syndrome was defined according to the international definition agreed in 2009. Out of the 656 care workers, we included 316 care workers (response rate: 48.2%) who had fully completed the questionnaires as the study sample (males: 13.6%, median age = 51 years old, range: 35–74 years old). To examine the association between MetS and the level of disability due to LBP, we used the Poisson regression analysis and estimated crude and adjusted prevalence ratios (PR). Results: Out of the 316 care workers, 52 (16.5%) were diagnosed as having MetS. Metabolic syndrome was significantly associated with the RDQ score (adjusted PR: 1.57, 95% confidence interval (CI): 1.17–2.11) after adjusting for covariates, such as age, sex, fear of movement, job demands, social support, intensity of pain, and duration of pain. Conclusions: This study showed that MetS was independently associated with disability due to LBP among care workers. A multidisciplinary intervention taking MetS into consideration may be an effective way to reduce disability due to LBP in people with both LBP and MetS. Opis:
Italy is one of the most important destination countries for Romanians. At the same time, the Italian care sector relies mainly on migrant labour, most of whom are Romanian women. Historically, Italy is considered one of the landmark countries for the southern or Mediterranean welfare state, characterised by its fragmented labour market, underdeveloped social protection system, informal economy and unpaid care work, usually done by the women in the family. Italy has one of the highest rates in Europe of both the elderly population and life expectancy at birth. In the last 20 years, the care work was gradually redistributed to migrant care workers, most of them women from former socialist countries, who often live in the household where they work. Migration from Eastern Europe, particularly Romania, has been facilitated, on the one hand, by rising unemployment and low-paid job opportunities in migrants’ countries of origin in the context of the deindustrialisation of state industry and, on the other, by the Italian elderly public-support system which is based on cash benefits granted to the family which can be redistributed to employ migrant care workers. In this paper we analyse three specific types of care work migration from Romania to Italy and the main challenges which they face, taking into account the specifics of the work and the type of migration chosen. The methodology is qualitative, based on 20 semi-structured online interviews with Romanian care workers and two interviews with stakeholders.

Opis:
Italians are one of the fastest aging societies in the world. The percentage of people over 60 years old is over 20%. There is also a systematic grow of the number of people who are over 80 years old. Many of them suffer from various medical conditions and need help in everyday life activities. The Italian social help system is traditionally based on family solidarity. Relatives (mostly women) are expected to carry the burden of caring for dependent members of a family. State structures are not prepared for replacing or at least supporting families in all these tasks. Families unable to fulfill this role have been searching for alternative solutions. One of the most popular one is to employ a personal attendant. Due to the lack of interest of the Italian workers for this kind of work, the gap is being filled by female migrants. The aim of the article is to present the work of foreign female caregivers and domestic workers in a broader context that includes the transformation of paid domestic service in recent decades and the ongoing social and demographic changes.

Objectives: The study presents data concerning occupational exposures among the staff of 5 hospitals in the Małopolska province in 2008–2012, taking into account the frequency and circumstances of exposure formation, occupational groups of hospital workers, as well as diversification of the reported rates in subsequent years between the hospitals and in each of them. An additional objective of the analysis was to assess the practical usefulness of the reported data for planning and evaluation of the effectiveness of procedures serving to minimize the risk of healthcare workers’ exposure to pathogens transmitted through blood. Material and Methods: Data were derived from occupational exposure registries kept by 5 hospitals of varying sizes and operational profiles from the Małopolska province from the years 2008–2012. Results: Seven hundred and seventy-five cases of exposure were found in a group of 3165 potentially exposed workers in the analyzed period. Most cases were observed in nurses (68%) and these were mainly various types of needlestick injuries (78%). Exposure rates with respect to all workers ranged from 2.6% to 8.3% in individual hospitals, but the differences in their values registered in the hospitals in subsequent years did not bear any statistical significance, in a way similar to the rates calculated separately for each occupational group. Conclusions: There was no upward or downward trend in the number of reported cases of exposure to bloodborne pathogens in the studied period in any of the hospitals. Statistically significant differences in the percentages of exposures were reported between individual hospitals in some years of the analyzed period, which confirms the need for registries in individual units in order to plan and evaluate the effectiveness of preventative measures.

Opis:
Objectives: The risk of transmission of Mycobacterium tuberculosis from patients with tuberculosis to health care workers (HCWs) has been well documented but little is known about the prevalence of latent tuberculosis infection (LTBI) in Iranian HCWs. The aim of this study was to determine the prevalence of LTBI among HCWs by using IFNgamma-release assay and compare the results with those of tuberculin skin test (TST). Methods: Two hundred HCWs were evaluated with both TST and QuantiFERON-TB Gold In Tube test (QFT-GIT). The obtained data were analyzed by SPSS v.16 Software. Results: The participants were 73 males and 127 females with the mean age of 34.36±8.26 years. TST was positive in 105 cases (52.5%) and the QFT results were positive in 17 cases (8.5%). There was poor agreement between the two tests (53%, к = 0.115). Induration diameter of TST ≥ 10 mm and working duration ≥ 10 years were independent predictors for positive QFT (p = 0.004). Conclusions: Due to the fact that BCG vaccination has been administered routinely to all HCWs in Iran, specifi c tests should be introduced for high risk groups. QFT thus seems to be more effective for LTBI diagnosis than TST among HCWs with BCG immunization history.

Mimo licznych rekomendacji eksperckich stan zaszczepienia pracowników medycznych pozostaje niski. Influenza is one of the most common respiratory diseases affecting people of all age groups all over the world. Seasonal influenza leads to substantial morbidity and mortality on a global scale. Vaccines are undeniably one of the most important health advances of the past century, however, managing influenza in working populations remains a difficult issue. Vaccination of health care workers (HCW) is an efficient way to reduce the risk of occupational infection and to prevent nosocomial transmission to vulnerable patients. Despite this, achieving high immunization rates among those professionals is a challenge. Knowledge and attitudes of healthcare providers have significant impact on the frequency with which vaccines are offered and accepted, but many HCWs are poorly equipped to make informed recommendations about vaccine merits and risks. Principal reasons for vaccination are the willing not to be infected and avoiding transmission to patients and the family. The main reasons for refusing is lack of time, a feeling of invulnerability to vaccination, conviction of not being at risk, of being too young or in good health. Misconceptions about influenza vaccine efficacy, like adverse effects, and fear of contracting illness from the vaccine are significantly associated with noncompliance with vaccination. Therefore, strategies to increase awareness of the importance of recommending influenza immunization among health professionals are required. Med Pr 2013;64(1):119–129

Objectives This study aims to characterize personal attitudes and knowledge of a sample of Italian occupational physicians (OPhs) towards immunization practice in the case of healthcare workers (HCWs). Material and Methods A total of 90 OPhs (42.2% of males, 57.8% of females, mean age of 50.1±8.3 years old) compiled a structured questionnaire through a telephonic interview. They were asked about the official Italian recommendations for HCWs, their general knowledge of vaccine practice, their propensity towards vaccines (both in general and about specific immunizations), their risk perception about the vaccine-preventable infectious diseases. Eventually, a regression analysis was performed in order to identify factors predictive for vaccine propensity. Results Only 12 out of 90 subjects correctly identified all the 7 recommended immunizations. The hepatitis B virus (HBV) vaccine was correctly identified by 95.6% of the sample, and was also associated with the more positive attitude and the more accurate risk perception. Influenza vaccine had the lowest acceptance (75.9%). Eventually, pertussis, measles, parotitis and varicella vaccines were insufficiently recognized as recommended ones (all cases < 50% of the sample). General knowledge of vaccine and knowledge of official recommendations were significantly correlated with the attitude towards immunization practice (r = 0.259, p = 0.014 and r = 0.438, p < 0.0001). In the regression analysis general knowledge (unstandardized coefficient (B) = 0.300, 95% confidence interval (CI): 0.090–0.510, p = 0.006) and risk perception (B = 0.579, 95% CI: 0.155–1.003, p = 0.008) were significant predictors of the propensity to vaccinate. Conclusions Vaccinations gaps in HCWs may found their roots in OPhs incomplete knowledge of evidence-based recommendations. Specific training programs and formations courses should then be planned. Omówiono aktualną definicję i diagnostykę krztuśca. U osób szczepionych przed kilku laty przebieg kliniczny choroby może być znacznie złagodzony i bardzo niecharakterystyczny - bez typowego kaszlu i wymiotów. Krztusiec należy uwzględniać w diagnostyce różnicowej kaszlu utrzymującego się ponad 14 dni. Poprawa sytuacji epidemiologicznej wymaga, poza uodpornieniem niemowląt, powszechnych i regularnych szczepień przypominających młodzieży i dorosłych. W polskim Programie Szczepień Ochronnych na 2013 rok wprowadzono zalecenie szczepienia pracowników oddziałów noworodkowych i dziecięcych. Wydaje się, że szczepienia przypominające pracowników ochrony zdrowia potrójną szczepionką przeciwko błonicy, tężcowi i krztuścowi o zmniejszonej dawce antygenów (Diphteria, Tetanus and acellular Pertussis vaccine - dTpa) - zwłaszcza mających kontakt z niemowlętami, dziećmi i osobami starszymi - mogą być najbardziej skutecznym sposobem zmniejszenia ryzyka transmisji krztuśca w placówkach ochrony zdrowia. Med. Pr. 2013;64(5):731–739
We discuss the changing epidemiological situation of pertussis observed in recent years, with a focus on the shift of cases from young children to older age groups, teenagers and adults. Whooping cough may affect healthcare workers who belong to a high-risk group and cause hospital infections. We present a case report of pertussis in a nurse and the recommended prophylactic measures in healthcare workers. The current definition and diagnosis of pertussis is also discussed. The clinical course of pertussis can be significantly alleviated and highly non-specific, with no typical coughing and vomiting in people vaccinated against whooping cough a few years earlier. Pertussis should be considered in the differential diagnosis of cough lasting more than fourteen days. Improvement of the epidemiological situation requires, besides immunization of infants, regular and universal booster immunization for adolescents and adults. Vaccinations for health care workers of neonatal and pediatric wards are recommended in the National Program of Immunization for 2013. It seems that booster vaccination of health care workers with a triple vaccine against diphtheria, tetanus and acellular pertussis (dTpa) of the reduced quantity of antigens, particularly of health workers caring for infants, children and the elderly, may be the most effective way to reduce the risk of pertussis transmission in the health care environment.
This article aims at discussing ethical issues which should underlie a good relationship between a physician and a patient. Reflections are based on the theses described in the The Charter for Health Care Workers developed on the initiative of the Pontifical Council for the Pastoral Care of Health Care Workers and approved by the Congregation for the Doctrine of the Faith. The paper attempts to present the physician's attitudes towards the pressing problems of prenatal period of human life as well as adulthood and the elderly. Ethical issues, such as the inviolable right to life and utmost respect for human life, seem to be valid and justifiable in times of commercialization and fast development of medical services, and thus need further thorough analysis.

Pracownicy ochrony zdrowia w Polsce mają możliwość rozpoznania krzywdzenia dziecka i obowiązek zgłaszania swoich podejrzeń. Niniejszy artykuł przedstawia wyniki badania przeprowadzonego wśród pracujących w Polsce lekarzy, pielęgniarek, ratowników medycznych i położnych.
