The aim of the study was to analyze indications and results of the first one thousand liver transplantations at Chair and Clinic of General, Transplantation and Liver Surgery, Medical University of Warsaw.Material and methods. Data from 1000 transplantations (944 patients) performed at Chair and Clinic of General, Transplantation and Liver Surgery between 1994 and 2011 were analyzed retrospectively. These included 943 first transplantations and 55 retransplantations and 2 re-retransplantations. Frequency of particular indications for first transplantation and retransplantations was established. Perioperative mortality was defined as death within 30 days after the transplantation. Kaplan-Meier survival analysis was used to estimate 5-year patient and graft survival.Results. The most common indications for first transplantation included: liver failure caused by hepatitis C infection (27.8%) and hepatitis B infection (18%) and alcoholic liver disease (17.7%). Early (< 6 months) and late (> 6 months) retransplantations were dominated by hepatic artery thrombosis (54.3%) and recurrence of the underlying disease (45%). Perioperative mortality rate was 8.9% for first transplantations and 34.5% for retransplantations. Five-year patient and graft survival rate was 74.3% and 71%, respectively, after first transplantations and 54.7% and 52.9%, respectively, after retransplantations.Conclusions. Development of liver transplantation program provided more than 1000 transplantations and excellent long-term results. Liver failure caused by hepatitis C and B infections remains the most common cause of liver transplantation and structure of other indications is consistent with European data.

Unfortunately, there is no such thing as an organic replacement lens to be implanted after cataract surgery. That is why intraocular lens (IOL) must work with the eye’s natural physiology not only to mimic the biological functions of the crystalline lens, but also to maximize patient outcomes. While preparing a patient for surgery, we should take into account some factors which can determine the outcomes, such as: the IOL design, which will ensure stability and predictable refractive effect, a high biocompatibility with the tissues of the eye, and the protective effect to the eye. Properly selected lens can ensure restoration of functional vision, image quality, the best possible contrast or the reduction of spherical aberration, which all testify to the cataract surgery success.

Opis:
Orbital infection occurs when pathogens pass from an infected sinus into the orbit. The inflammation is spreading onto the orbital structures from the surrounding tissues mainly from ethmoidal sinuses due to its specific anatomical conditions. The orbital complications are: preseptal cellulitis, orbital cellulitis, subperiosteal abscess, orbital abscess and intracranial infections. The diagnosis of subperiosteal orbital abscess is based mainly on clinical presentation. The symptoms depend on which sinus is affected. Orbital symptoms include periorbital swelling, proptosis, ophthalmoplegia, chemosis, and optic nerve compression. Computer tomography is mandatory. Early diagnosis and appropriate treatment can prevent from permanent vision loss and intracranial complication. The study discusses the course of the disease of 25-year-old woman who was admitted to Emergency Department due to inflammatory eyelid oedema and proptosis for 3 days. No symptoms of sinusitis were noted. The CT scans established diagnosis – subperiosteal orbital abscess. The patient was treated with broad spectrum antibiotics and functional endoscopic sinus surgery. Her treatment and recovery were uneventful. In follow-up nighter ophthalmological nor otorhinolaryngological complains were noted.

Opis:
Technological advancement, availability and common use of diagnostic imaging slowly but consistently leads to a change in its nature from being additional studies to becoming the basis for diagnostic process and treatment planning especially in complex cases that require surgical treatment. CT angiography study presentation using the illusion called “Pepper’s Ghost” in comparison to a three-dimensional printout and regular CT scan has been made to point out the relevance of research and implementation of new technologies in the diagnosis and surgical planning. Our image, despite being far from ideal and being just an illusion of a hologram, was more appealing and detailed to surgeons in comparison to a printed 3D model and standard CT angiography displayed with Osirix software. In order to change the existing methods of presenting imaging studies, it is advisable to use latest technologies, and among those currently available, ones based on virtual or mixed reality deserve special attention.

Biomechanical analyses seek to improve understanding of the mechanisms of knee injury and to find ways to reduce knee injury incidence. Many clinical biomechanics researchers use a standard kinematic adopted from Newington Children’s Hospital. Biomechanical studies include the knee joint configurations, where joint architecture immutably constrains and guides movement outcomes. Investigators will default to reporting resultant joint deformation without considering the influence of joint architectural configurations on kinematic responses. The purpose of this study was to develop a new joint angular kinematic method that accounts for influence of dynamic joint architectural configuration on deformation values. Twenty subjects performed unloaded dynamic flexion/ extension and 45° cutting maneuver. The knee deformation angles obtained with the new method proposed were compared with the values that obtained using the standard method. One way repeated measurement ANOVA’s was used to compare knee deformation angles values from the standard method that uses a static trial and the new method that uses a dynamic trial. The proposed method distinguishes between dynamic joint architectural configuration and joint deformation. Loaded standard abduction/adduction (β) and rotation (γ) angles were 3.4 ±1.8° and 11.2 ±3.8°, respectively. Using the new method, the β and γ angles decrease to 1.5 ±1.4° (<0.05) and 7.1 ±1.8° (<0.05) during cutting. The new method accounted for dynamic joint architectural configuration produced loaded β and γ angles that had smaller magnitudes than the standard method, suggesting that previous studies may have overestimated β and γ angles. Injury management strategies could be influenced by a consideration for dynamic joint architectural configuration. Such a consideration could influence ligament repair strategies. Future studies should account for dynamic configuration when establishing the influence of joint deformation on graft design and appropriate isometry values during reconstruction.

The reported prevalence of periampullary duodenal diverticula varies between 9 and 32.8%. The aim of the study was to evaluate the prevalence of periampullary diverticula in the studied population and establish whether their presence influence the risk of choledocholithiasis and the risk of Endoscopic Retrograde Cholangio Pancreatography (ERCP) related complications. Material and methods. The study group of 3788 patients who underwent ERCP between 1996 and 2016at the 2nd Department of General Surgery Jagiellonian University Medical College in Kraków were analyzed. The group comprised of 2464 women (mean age 61.7 years) and 1324 men (mean age 61.8 years). The patients were divided into two groups. Group A included patients in whom there were no periampullary diverticula detected. Group B included patients in whom the opening of the bile duct was in the vicinity of a duodenal diverticulum. Results. There were 3332 patients included in group A (2154 women and 1178 men) and 456 patients in group B (310 women and 146 men). The prevalence of periampullary duodenal diverticula in the analyzed group was 12.8%. The presence of stones or biliary sludge was diagnosed in 1542 patients (47.6%) in group A and 290 patients (68.1%) in group B. Recurrence of choledocholithiasis occurred in 4.5% of patients (70/1542) in group A and 10.3% of patients (30/290) in group B. Complications occurred in a total of 76 patients in group A (2.3%) and 22 patients in group B (4.8%). Conclusions. The presence of choledocholithiasis and the risk of ERCP related complications are significantly higher in the group with duodenal diverticula.

Purpose: Rectal cancer is one of the most common malignancies of the gastrointestinal tract. The gold standard method is surgical resection. The approach to rectal cancer is still controversial. Nowadays, robotic approach gains popularity in comparison to traditional laparoscopy. However, there is lack of studies assessing rectal resections with primary anastomosis. Methods: We performed a systematic review and meta-analysis according to the PRISMA guidelines. The primary outcomes of interest were morbidity and short-term complications. Results: An initial reference search yielded 1250 articles. Finally, we chose six studies covering 1580 patients that we included in the quantitative analysis. In our study, we demonstrated that laparoscopic and robotic surgery are non-inferior to one another in terms of morbidity (RR=1.1 95% CI: 0.89-1.39), major complication rate (RR=1.01, 95% CI: 0.60-1.69) or in length of hospitalization (MD=0,15 95% CI: -0.60−0.90). The latter has slight advantage in quality of mesorectal excision (RD = -0.19, 95% CI: -0.35 − -0.03. I2=69%) and anastomotic leakage rate (OR=2.25, 95% CI: 1.23-4.09, I2=0%). Conclusion: In certain cases Robotic Surgery provide better quality of resected specimen and lower leakage ratio, nevertheless due to heterogeneity the results are uncertain. There is substantial need for large randomized controlled studies.

The breast cancer is the most common cancer in women, both in Poland and in the world. Consequences entail a disruption in the physical, psychological and social functioning. The aim of the study was to assess the acceptance of illness by patients treated for breast cancer in the early postoperative period. Material and methods. The research was conducted on the group of 100 consecutive patients aged 32‑80 years (median 56 years) who underwent surgery for breast cancer in the Centre of Oncology in Bydgoszcz w 2014 roku. 68 of women had mastectomy, 32 of women had conservative surgery. Polling was conducted in the early period after surgery. The original questionnaire containing closed questions the scale of acceptance of the disease (AIS) as well as mental adaptation to cancer (Mini-Mac) was used in the study. Results. 38% of patients had high acceptance of the disease, 48% averageand 14% had low acceptance. Patients after conservative surgery had a higher average values for the mental strategies to cope with the disease, for the fighting spirit (23.1), helplessness and hopelessness (13.5), positive revaluation (23), the patients had a lower average (16.5) in the strategy to absorb anxiety. Patients after conservative surgery had a higher average for constructive style (2.6) but lower for destructive style (1.5). High level of mental coping with the disease was observed in 53%of patients with constructive style and 4% of patients with destructive style. While, a low level of mental coping with the dosease was observed in 5% of patients with constructive style and 46% of patients with destructive style. Conclusions. Almost half of women after mastectomy or conservative surgery had an average acceptance of the disease. The disease was accepted best by educated women living in the cities, whitecollar workers with a good economic situation. The following factors were affected the better management of the disease, in order: age, education, current occupation and economic situation, while the type of surgery did not affect better management. More than half of women, regardless of the type of surgery reflected the high level of constructive style.

The study presented three cases of patients diagnosed with adrenal tumors subject to surgical intervention during the past 6 months in our Department. The patients presented with radiological diagnostic difficulties, as to the character and location of the primary tumor.The aim of the study was to demonstrate differences between radiological examination results and the factual situation observed during the adrenalectomy. In all the presented cases patients' were subject to laparoscopic intervention. In two cases conversion to open surgery was necessary. The histopathological results of the surgically removed samples were as follows: leiomyoma, myoperycytoma and pheochromocytoma. In selected cases imaging examinations might be of limited value, especially when determining the character and location of the primary lesion of the adrenal gland.

W pracy przedstawiono zaimplementowane sterowanie admitancyjne i impedancyjne w systemie robotycznym, który może być wykorzystany w zabiegach chirurgicznych w środowisku laboratoryjnym. System ten składa się z robota KUKA LWR oraz konsoli haptycznej Force Dimension Omega 7. W ramach prowadzonych prac badawczych przeprowadzono szereg badań eksperymentalnych weryfikujących zastosowane algorytmy sterowania.
This paper presents implemented admittance and impedance control in a robotic system that can be used in surgical procedures in a laboratory environment. This system consisted of a KUKA LWR robot and Force Dimension Omega 7 haptic console. As part of the research, a number of experimental studies were carried out to verify the control algorithms used.

Opis:
Abdominal tuberculosis is a common problem for clinicians in the tropical world and may manifest with varying clinical scenarios. Intestinal tuberculosis could have intestinal ulcers, strictures, hypertrophic lesions like polyps and may be complicated by perforation, bleeding, and intestinal obstruction. Crohn’s disease is an important differential of intestinal tuberculosis which is closely mimics intestinal tuberculosis in clinical, endoscopic, radiological and histological presentation. Crohn’s disease is known to have a fistulising variant. We report the case of 23 year old lady who had disseminated tuberculosis with intestinal involvement and seemed to improve on anti-tubercular therapy (ATT) but present with intestinal obstruction in the third month of ATT. Surgical exploration revealed clumping of bowel loops with multiple ileo-ileal fistulae. The case is presented because of the presence of entero-enteric fistulae and also because it demonstrated that intestinal tuberculosis may need surgical intervention even after initial improvement because of complications like intestinal obstruction.
