INTRODUCTION: Sinonasal inverted papillomas (IPs) are a major challenge for ENT specialists. In the last decades, the number of endoscopic procedures has increased, while the percentage of external or double approach procedures has decreased. The aim of this study was to evaluate long-term IP treatment outcomes according to the applied surgical method. MATERIAL AND METHODS: the retrospective study included 69 patients (28 women and 41 men) treated during 2002–2012 in the Department of Otolaryngology and Laryngological Oncology of Poznań University of Medical Sciences. Of these patients, 47 were operated endoscopically, 16 received a double procedure and six received an external procedure. RESULTS: The recurrence rate in patients treated endoscopically was 32%, while patients treated by the external/double approach showed 64% (p = 0.03306). The recurrence rate after endoscopic surgery was 75% during 2002–2006, but was 28% during 2007–2012. There was no correlation between recurrence rate and age (p = 0.087686), gender (p = 0.42810), type of symptoms (p = 0.20955), or Krouse staging (p = 0.23658). A significant impact on relapses was the position to Ohngren’s plane (p = 0.004768) and tumor location in the anterior ethmoid (p = 0.00416). DISCUSSION: Endoscopic procedures are an effective method of IP treatment, however reasonable indications and surgeon experience should be taken into consideration.

WSTĘP: Brodawczaki odwrócone masywu szczękowo – sitowego (IP) stanowią duże wyzwanie dla otorynolaryngologów. W ostatnich dekadach obserwowany jest wzrost liczby operacji endoskopowych przy malejącym odsetku zabiegów z dojścia zewnętrznego / podwójnego. Celem pracy była analiza odległych wyników leczenia IP w zależności od zastosowanej metody operacyjnej. MATERIAŁ I METODY: Retrospektywne badanie obejmuje grupę 69 pacjentów (28 kobiet i 41 mężczyzn) leczonych w latach 2002-2012 w Klinice Otolaryngologii i Onkologii Laryngologicznej Uniwersytetu Medycznego w Poznaniu; 47 – endoskopowo; 16 – z dojścia podwójnego i 6 – z dostępu zewnętrznego. WYNIKI: Odsetek wznów u chorych leczonych endoskopowo wynosił 32%; natomiast u chorych leczonych z dostępu podwójnego/zewnętrznego – 64 % (p=0.03306). W latach 2002 – 2006 odestek wznów po operacjach endoskopowych wynosił 75 %; natomiast w latach 2007 – 2012 – 28 %. Odsetek wznów nie zależał od wieku (p=0.087686), płci (p=0.42810), rodzaju objawów (p=0,20955), stopnia rozległości wg Krouse (p=0.23658). Miało istotny wpływ na wznowę położenie zmian względem płaszczyzny Ohngrena (p=0,004768); oraz występowanie guza w sitowiu przednim (p=0,00416). DYSKUSJA: Operacje endoskopowe są efektywną metodą leczenia IP przy uwzględnieniu należytych wskazań i doświadczeniu operatora.

W ostatnich latach zaobserwowano znaczący wzrost częstości występowania brodawczaków odwróconych (IP) w stabilnej populacji. Celem pracy była analiza występowania IP w latach 2002–2012 w odniesieniu do czynników epidemiologicznych oraz rozpoczęcie dyskusji o problemach związanych z ewidencją częstości występowania tych łagodnych guzów, nie ujętych jak dotąd w oficjalnych rejestrach. Badanie objęło grupę 69 chorych leczonych z powodu IP w Klinice Otolaryngologii i Onkologii Laryngologicznej Uniwersytetu Medycznego w Poznaniu. Od 2006 roku obserwujemy znaczący wzrost chorych zgłaszających się do kliniki z powodu IP. Wyodrębniono dwie podgrupy – pierwsza obejmowała 11 chorych leczonych w latach 2002–2006, do drugiej zaliczono 58 chorych leczonych w latach 2007–2012. Grupy te następnie porównano ze sobą. Pomimo znaczącej różnicy w liczbie chorych w dwóch analizowanych okresach, nie stwierdzono istotnych statystycznie różnic związanych z lokalizacją guza (p>0,05), stopniem zaawansowania (p=0,16), czasem trwania objawów (p=0,39), miejscem zamieszkania, paleniem tytoniu (p=0,41). Wiek chorych był jedyną zmienną, która różniła się w sposób istotny statystycznie (48,45 vs. 56,93 lat; p=0,043569); średnia wieku chorych w drugim analizowanym okresie była wyższa.

The Chronicles of Narnia has an established position in the canon of children’s literature. However, what on the surface is a fairy tale involving adventures and magic; with children, kings, talking beasts, and wood spirits as main protagonists; is, in fact, a set of stories deeply rooted in Christian and chivalric traditions, containing elements of beast fable and morality tale. The story, according to Madeline L’Engle, depending on the reader's cultural knowledge and experience, may be understood on various levels, from the literal one of an adventure story for children, through the moral and allegorical levels, eventually reaching the anagogical level. While reading The Chronicles, one is able to notice various references to other written works, interwoven into the text, with the Bible, chivalric romances and beast fables being the most prominent sources of intertextual allusions. In The Last Battle Lewis attempts to answer John Donne’s question, “What if this present were the world’s last night?" (Holy Sonnet XIII) and presents a comprehensive image of Narnian apocalypse and life after death in Aslan’s country. The following paper will present the most noteworthy intertextual references in the final volume of The Narniad.

„Etyka codzienności – czy można być dobrym po prostu?" to praca wyrastająca z refleksji nad kondycją współczesnej debaty etycznej, jak również nad praktyką moralności oraz polityką moralności, które obserwujemy i których doświadczamy w życiu codziennym na początku XXI wieku. U podłoża prezentowanych badań spoczywa przekonanie, że etyczny wymiar rzeczywistości, w której żyjemy i którą tworzymy, pozostaje głęboko spleciony z codzienną formą i praktyką naszego życia. Jednocześnie stanowisko to nie musi, nie powinno i nie oznacza rezygnacji z ideałów, które narodziły się w łonie rozmaitych kultur i rozumów tę codzienność współkształtujących. W tym sensie jest to także zapis poszukiwań ścieżki pomiędzy konsumpcjonistycznym nihilizmem a fundamentalistycznym uniwersalizmem, dwiema ideologiami współczesnego świata, które w nim dominują i wiodą go w przeciwstawnych kierunkach. Praca broni tezy, że każda moralność jest projekcją formy życia, w tym języka formę tę tworzącego oraz praktyki owe życie ucieleśniającej. W tym kontekście podstawowym celem badawczym staje się konstrukcja narracji pozwalającej ugruntować przekonania moralne w faktycznym miejscu ich pochodzenia – a więc codzienności – w sposób, który jednocześnie eliminuje typowe kompleksy subiektywności i lokalności. Perspektywa ta pozwala z kolei na nawiązanie dialogu pomiędzy autentycznymi wartościami obecnymi w życiu jednostki z tyranią systemów etycznych roszczących pretensje do głębi i uniwersalności. Jest to więc także próba ucieczki od przemocy symbolicznej dominujących dyskursów ideologicznych oraz przywrócenia moralności do jej zwyczajnego użycia w zgodzie z Wittgensteinowskim postulatem powrotu do codzienności. Wśród pytań badawczych, które organizują całość namysłu, najistotniejsze dotykają surowej praktyki życia, a więc związków lokalnej formy życia z lokalnie obowiązującą moralnością. Jak formy życia wyznaczają reguły gier w moralność? Jaki związek zachodzi między dzisiejszą formą życia konsumpcjonistycznego społeczeństwa i jego zglobalizowanej rzeczywistości, a funkcjonującymi współcześnie grami w moralność? Jakim sposobem w nurcie codziennej praktyki, w mechanizmie społecznej tresury, wyłania się mistyczny wymiar etyki? W jaki sposób dokonuje się ewolucja świadomości, która przekracza nie tylko wartość życia jednostkowego, ale i samą siebie w autodestrukcyjnym akcie zakwestionowania kryterium mechanizmu doboru naturalnego, stworzenia wartości wyższej niż przetrwanie gatunku? Czy arbitralność, egoizm i subiektywność przekonań są właściwymi dla rozważań nad moralnością kategoriami analitycznymi? W jaki sposób fantazja i ideologia penetrują sferę refleksji moralnej, a w szczególności czy możliwy jest spójny ogląd moralności, który wolny jest od pojęć wolności, godności i celowości? Czy uwzględnienie cierpienia Innego jest niezbywalnym elementem wszelkiej możliwej moralności? A jeśli tak, to kim ów Inny miałby być? Książka poświęcona jest także analizie pojawiających się w toku tego namysłu sprzężeń i próbie zrozumienia dzisiejszych form życia w perspektywie problemów moralnych trapiących współczesność. Zwieńczeniem tej wędrówki staje się refleksja nad kondycją człowieka współczesnego. W tle tych dociekań spoczywa bowiem poczucie głębokiego dysonansu, jaki zachodzi między abstrakcyjną sferą moralności deklarowanej a codziennymi praktykami, między intencją lub pozorem intencji a jej codziennym przełożeniem, także więc między fantazją moralną, ideologią moralną i realnością moralną. Struktura pracy oparta jest o trzy rozdziały, z których pierwszy poświęcony jest rekonstrukcji filozofii moralności Ludwiga Wittgensteina, drugi naświetleniu wybranych aspektów socjologii moralności, w tym zwłaszcza koncepcji z obszaru socjologii Pierre'a Bourdieu, trzeci zaś syntezie rozważań poprzedzających, wyznaczeniu nowych narzędzi opisu oraz narracji, jak również analizie praktycznych aspektów funkcjonowania systemów przekonań moralnych w świecie dzisiejszym. Rozdział pierwszy zawiera swego rodzaju filozoficzne preliminaria ukierunkowane na zwięzłe nakreślenie i wyjaśnienie tych aspektów myśli Wittgensteina, które będą odgrywać rolę kluczową w namyśle nad etyką. W szczególności więc wprowadzone zostają w nim centralne dla całej pracy kategorie gier językowych, widzenia jako oraz form życia, stanowiące zarazem swoistą konceptualizację Wittgensteinowskich rozważań dotyczących antropologii, socjologii i historii kultury. Zasygnalizowany zostaje gramatyczny związek świadomości z wszelką możliwą etyką w kontekście dyskusji nad możliwym rozumieniem arbitralności moralności oraz inwariantów etycznych. Wyjaśnienie konsubstancjalności języka oraz ludzkich oglądów i przekonań moralnych pozwala na ukazanie komplementarnej roli codzienności oraz świadomości dla rozwijanej koncepcji etycznej. Rozdział drugi opisuje etykę codzienności z perspektywy socjologicznej, formułując szeroki wachlarz ujęć wobec niej tak afirmatywnych, jak i krytycznych. Dzięki sprzężeniu tych wielu spojrzeń wypracowana zostaje z kolei perspektywa pozwalającą sproblematyzować rozważane zagadnienia w wymiarze kategorii celowości, godności oraz wolności. Ostatnia z tych kategorii otwiera także szerokie pole refleksji nad rolą, determinizmem oraz swobodą samych modeli poznawczych. Ostatecznie kluczowym etapem tej części rozważań jest zastosowanie socjologii refleksyjnej Pierre'a Bourdieu oraz jego koncepcji gier społecznych do sformułowania koncepcji kapitału moralnego oraz dalszych badań nad etyką codzienności. W szczególności pozwala to wyjaśnić, jak w kontekście społeczno-politycznym moralność funkcjonuje jako tytułowy towar, oręż oraz dystynkcja. Ważnym wątkiem jest także ujęcie polityki małych spraw Jeffreya Goldfarba jako dyskursu właściwego etyce codzienności. Część ostatnia zawiera zarówno podsumowanie, wnioski z dociekań wcześniejszych, jak i autonomiczne rozważania zmierzające do pełniejszej konceptualizacji etyki codzienności oraz określenia miejsca koncepcji formy życia w praktyce współczesnych nauk społecznych. Kolejno rozważane są praktyczno-teoretyczne warunki możliwości etyki codzienności oraz mechanizmy, dzięki którym manifestuje się jej autentycznie i immanentnie społeczny oraz codzienny charakter. Wyniki te stanowią asumpt do dalszych badań nad teorią względności moralności. Następnie zarysowane zostają wybrane aspekty interferencji etyki i polityki, kluczowe dla zrozumienia fenomenu moralności, w tym wpływ zasłon fantazji i ideologii na samo polityczne funkcjonowanie moralności. W obszarze refleksji nad ekonomią moralności wskazuje się na odległość moralną, odległość lojalnościową oraz rachunek uznania jako trzy komponenty wyznaczające wektor działania moralnego. Końcowa partia tekstu zawiera próbę rozpoznania osiowych cech współczesnych form życia, które stanowią nie tylko o kształcie współczesnej moralności, ale i świata: tyranii indywidualizmu, interpasywności, a następnie wynikającej z nich atrofii zaufania, napędzającej z kolei mechanizmy właściwe dla utowarowienia moralności oraz rozwoju form hipermoralności.
"Everyday ethics - can you simply be good?" is a work arising from reflection on the condition of contemporary ethical debate, as well as on the practice of morality and the policy of morality, which we observe and experience in everyday life at the beginning of the 21st century. The presented research is based on the belief that the ethical dimension of the reality in which we live and which we create remains deeply intertwined with the everyday form and practice of our life. At the same time, this position does not have to, should not and does not mean giving up the ideals born in the bosom of various cultures and minds that co-shape this everyday life. In this sense, it is also a record of the search for a path between consumerist nihilism and fundamentalist universalism, two ideologies of the modern world that dominate it and lead it in opposite directions. The work defends the thesis that every morality is a projection of a form of life, including language that creates this form and practices that embody this life. In this context, the basic research goal is the construction of a narrative that allows to ground moral beliefs in their actual place of origin - i.e. everyday life - in a way that at the same time eliminates the typical complexes of subjectivity and locality. This perspective, in turn, allows for establishing a dialogue between the authentic values present in the life of an individual with the tyranny of ethical systems claiming profundity and universality. It is therefore also an attempt to escape from the symbolic violence of the dominant ideological discourses and to restore morality to its ordinary use in accordance with Wittgenstein's postulate to return to everyday life. Among the research questions that organize the whole reflection, the most important ones concern the strict practice of life, i.e. the relationship between the local form of life and the local morality. How do life forms dictate the rules of morality games? What connection is there between the present-day form of consumerist society and its globalized reality, and the morality games that function today? How does the mystical dimension of ethics emerge in the current of everyday practice, in the mechanism of social training? How does consciousness evolve that exceeds not only the value of individual life, but also itself in the self-destructive act of questioning the criterion of the mechanism of natural selection, of creating a value higher than the survival of the species? Are arbitrariness, egoism and the subjectivity of beliefs appropriate analytical categories for considering morality? How do fantasy and ideology penetrate the sphere of moral reflection, and in particular, is it possible to have a coherent view of morality that is free from the notions of freedom, dignity and purposefulness? Is taking into account the suffering of the Other an indispensable part of all possible morality? If so, who would the Other be? The book is also devoted to the analysis of the feedback emerging in the course of this reflection and an attempt to understand today's forms of life from the perspective of moral problems plaguing the present day. The culmination of this journey is reflection on the condition of modern man. In the background of these investigations there is a sense of a deep dissonance between the abstract sphere of declared morality and everyday practices, between the intention or appearance of intention and its everyday translation, also between moral fantasy, moral ideology and moral reality. The structure of the work is based on three chapters, the first of which is devoted to the reconstruction of Ludwig Wittgenstein's philosophy of morality, the second to highlighting selected aspects of the sociology of morality, including in particular Pierre Bourdieu's concept of sociology, and the third to the synthesis of the preceding considerations, the designation of new tools of description and narration as well as the analysis of practical aspects of the functioning of moral belief systems in today's world. The first chapter contains a kind of philosophical preliminaries aimed at briefly delineating and explaining those aspects of Wittgenstein's thought that will play a key role in considering ethics. In particular, therefore, it introduces the categories of language games, vision as and forms of life, central to the whole work, which at the same time constitute a peculiar conceptualization of Wittgenstein's considerations on anthropology, sociology and the history of culture. The grammatical connection of consciousness with all possible ethics is signaled in the context of the discussion on the possible understanding of the arbitrariness of morality and ethical invariants. Explaining the consubstantiality of language and human views and moral beliefs allows us to show the complementary role of everyday life and awareness for the developed ethical concept. The second chapter describes the ethics of everyday life from a sociological perspective, formulating a wide range of approaches to it, both affirmative and critical. Thanks to the coupling of these many views, a perspective is developed that allows to problematize the issues under consideration in terms of the categories of purpose, dignity and freedom. The last of these categories also opens up a wide field of reflection on the role, determinism and freedom of the cognitive models themselves. Ultimately, the key stage of this part of the discussion is the application of Pierre Bourdieu's reflective sociology and his concept of social games to formulate the concept of moral capital and further research into the ethics of everyday life. In particular, it helps to explain how, in the socio-political context, morality functions as the title commodity, weapon and distinction. An important theme is also the approach to Jeffrey Goldfarb's policy of small matters as a discourse appropriate to the ethics of everyday life. The last part contains both a summary, conclusions from previous investigations, as well as autonomous considerations aimed at a more complete conceptualization of everyday ethics and determining the place of the concept of a form of life in the practice of contemporary social sciences. The practical and theoretical conditions of the possibilities of everyday ethics as well as the mechanisms by which its authentically and immanently social and everyday character are manifested are consecutively considered. These results are a stimulus for further research into the theory of relativity of morality. Then, selected aspects of the interference of ethics and politics are outlined, crucial for understanding the phenomenon of morality, including the influence of the veils of fantasy and ideology on the very political functioning of morality. In the area of reflection on the economy of morality, moral distance, loyalty distance and the calculation of recognition are indicated as three components that define the vector of moral action. The final part of the text attempts to identify the pivotal features of contemporary life forms, which shape not only the shape of contemporary morality, but also the world: the tyranny of individualism, interpassivity, and then the resulting atrophy of trust, which in turn drives the mechanisms specific to the commodification of morality and the development of forms of hypermorality.

Praca rozpoznaje mechanizmy konwersji kapitałów właściwe wybranym grom społecznym, co z kolei dostarcza objaśnień procesu komodyfikacji moralności. Wnioski przedstawiające strukturę relacji pomiędzy kapitałem finansowym i moralnym wykorzystane zostają do analizy dwóch praktycznych przykładów: koncepcji MDG i CSR. Praca wprowadza użyteczny sposób mówienia o zjawiskach z pogranicza ekonomii i etyki, co pozwala formułować dalsze problemy badawcze.
The paper recognizes mechanisms of capital conversion among given social games, which allows in turn to explain the process of commodification of morality. Conclusions depicting the structure of relations between the financial and moral capitals are used to analyze two practical examples: the concepts of MDGs and CSR. The paper proposes a useful way of addressing phenomena on the borderline of economics and ethics, which allows to formulate further research problems.

The paper recognizes mechanisms of capital conversion among given social games, which allows in turn to explain the process of commodification of morality. Conclusions depicting the structure of relations between the financial and moral capitals are used to analyse two practical examples: the concepts of SDG and CSR. The paper proposes a useful way of addressing phenomena on the borderline of economics and ethics, which allows to formulate further research problems.

Po pierwsze, dzięki kunsztowi syntezy, struktura narracyjna wykorzystana przez Autorkę doskonale ukazuje ideologiczny rdzeń współczesnego dyskursu (neo)liberalnej opozycji. Zarazem dzięki jej talentowi erystycznemu, sam ładunek perswazyjny tekstu wart jest uznania i pogłębionego wglądu. Niniejsza publikacja stawia sobie za cel rekonstrukcję bazy narracyjnej Zmierzchu demokracji oraz ekspozycję jej aparatu perswazyjnego. Następnie owo studium przypadku wykorzystane zostaje do rozpoznania szerzej funkcjonujących schematów ideologicznych towarzyszących działaniom politycznym, u którego podłoża spoczywa resentyment, gorycz i tęsknota odsuniętych od władzy (neo)liberałów.
Twilight of Democracy by Anne Applebaum is a unique book for at least two reasons. Firstly, thanks to the artistry of synthesis, the narrative structure used by the author perfectly exhibits the ideological core of the contemporary discourse of (neo)liberal opposition. At the same time, thanks to her eristic talent, the very persuasive load of the text is worth recognition and in-depth insight. This publication aims to reconstruct the narrative base of the Twilight of Democracy and to reveal its persuasive apparatus. Then this case study is used to identify the more widely functioning ideological patterns associated with political actions, which are based on resentment, bitterness and the longing of (neo)liberals who were left out of power.

Dynamiczny rozwój transportu jest istotnym czynnikiem rozwoju gospodarczego i znaczącym źródłem uciążliwości do- tyczących głównie negatywnego wpływu na środowisko. Jedną z metod pozwalających na poprawę sytuacji w tym obszarze jest racjonalne planowanie rozwoju systemu transportowego. W artykule omówiono problematykę modelowania rozwoju systemu transportowego. Scharakteryzowano problemy środowiskowe, które są brane pod uwagę przy modelowaniu rozwoju systemu transportowego. Ponadto omówiono koncepcję modelu rozwoju proekologicznego systemu transportowego, będącego rozwinięciem modelu proekologicznego systemu stanowiącego efekt prac nad projektem EMITRANSYS. Wskazano jego elementy oraz zastosowanie.
Dynamic development of transport is important factor of economic development and significant source of nuisance regarding mainly negative impact on environment. One of methods to improve situation is rational planning of transport system development. Article discusses problem of transport system development modelling. Environmental problems, which are taken into account were characterized. Concept of model for pro-ecological transport system development, which is an extension of model constituting effect of work on EMITRANSYS project, was discussed. Its elements and application were indicated.

Introduction. Positron Emission Tomography (PET) is a non-invasive diagnostic tool that provides tomographic images and measures quantitative parameters of cell viability and metabolic activity of tissues. The most important used tracer in PET is an 18-F-Fluorodeoxy-glucose (FDG), the glucose molecule of which is labeled with a radiotracer and allows measurement and mapping of tissue glucose uptake. There are many studies in PET/CT which rely on some mammalian species, and recently on fish. Objective. The aim of this survey by using FDG-PET/CT are to optimize and determine FDG uptake in fish, in this case, common carp (Cyprinus carpio) using three treatments: Basic fish meal, Vita Pulvis and Probiotics s with two replications were used. Sphere Volume of interest (VOI) were drawn hand for the all organs, and standard uptake value (SUV) means were calculated. Results. The SUV mean for glucose uptake in the liver and gastrointestinal tract of fish were more similar to those of humans than rats or mice. SUV mean in fish fed by Probiotics ss in the major organ were less than those fed by Vita Pulvis and basic fish meal. The results present the opportunity to focus on studies of metabolism and screening for the effects of nutrients on body development.
