    Opis:
    Przeprowadzono badanie dla 33 przypadków leczenia stereotaktycznego w obszarze głowy. Dla każdego przypadku w systemie planowania leczenia Eclipse v.11 został stworzony plan leczenia z wykorzystaniem dynamicznej techniki VMAT (Volumetric Arc Therapy) [1]. Plany radioterapeutyczne realizowano na akceleratorze TrueBeam (Varian Medical System Palo Alto, CA, USA), który wyposażony jest w kolimator wielolistkowy MLC 120 HD [2]. Analizie poddano m.in.: wielkość napromienianych zmian, Indeks Paddicka czy wskaźnik gradientu dawki (Gradient Index) [3]. Dokonano również statystycznej analizy wzajemnych zależności między badanymi parametrami z wykorzystaniem testu korelacji Rho-Spearmana [4]. Przeprowadzona analiza wykazała, że pomiędzy wybranymi badanymi wielkościami występują wzajemne istotne statystycznie korelacje. Taki wynik może być pomocny dla fizyka, sugerując, które parametry mogą być silniej związane z wielkością targetu, pozwalając zaznaczyć potrzebę większego skupienia na wybranych aspektach procesu planowania leczenia.
    The study was performed for 33 cases of head stereotactic treatment. For each case, a treatment plan was created using the dynamic VMAT (Volumetric Arc Therapy) technique [1]. Radiotherapy plans were calculated in Eclipse TPS (Treatment Planning System) for TrueBeam accelerator (Varian Medical System, Palo Alto, CA, USA), which is equipped with the MLC 120 HD multi-leaf collimator [2]. For the analysis the following factors were chosen: the size of the irradiated targets, the Paddick Index and the Gradient Index. Additionally statistical analysis of the relationships between the studied parameters was also made using the Rho-Spearman correlation test. The analysis showed that there are mutual statistically significant correlations between the selected quantities tested. Such a result may be helpful for the physicist, suggesting which parameters may be more stronger related to the size of the target, allowing to emphasize the need for greater focus on selected aspects of the treatment planning process.

    Radioterapia stereotaktyczna i radiochirurgia wymagają dostarczenia wysokiej dawki promieniowania do bardzo małego obszaru napromieniania w jednej lub kilku frakcjach. Założenia takiego leczenia wymagają od fizyka planującego leczenia zwrócenia szczególnej uwagi na analizę gradientu spadku dawki wokół targetu. W niniejszej pracy przedstawiono propozycję ilościowej analizy gradientu spadku dawki opartej na zbudowanym modelu zależności promienia równoważnego od wielkości napromienianej zmiany. Model wykazał liniową zależność parametrów. Na jego podstawie możliwe staje się opracowanie wewnętrznych poziomów referencyjnych dotyczących gradientu spadku dawki w planie leczenia. Niniejsza praca jest równocześnie próbą oraz propozycją przedstawienia wewnętrznej analizy gradientu spadku dawki w planach leczeniach dotyczących stereotaksji oraz radiochirurgii okolicy głowy.
    Stereotactic radiotherapy (SRT) and radiosurgery (SRS) require a high dose of radiation to be delivered to a very small area in one or more fractions. Assumptions of such treatment oblique the medical physicist to pay special attention to analyse the dose distributiom inside and outside the target. In this article there is a proposal for quantitative analysis of the dose decrease gradient based on the built dependence model of the equivalent radius and the size of the irradiated volume. The model showed a linear relationship of considered parameters. On its basis it becomes possible to establish internal reference levels for the dose gradient in treatment plan. This work is the proposal to apply the dose gradient analysis for assessment of SRT and SRS treatment plans in the head area.

    The article is dedicated to pointing out and discussing the challenges which are encountered by epidemiological supervisory authorities in the face of globalization. In the first part, an analysis of the International Health Regulations (IHR), which create the basis of the global epidemiological supervision, is presented. In the light of these regulations, the decision instruments necessary to estimate the risk in the case of infectious diseases will be determined. In the second part, the author discusses an extremely interesting problem connected with the so-called spatial epidemiology, which deals with mapping of the location and intensity of diseases. Determining the appropriate repositories, using information based on unofficial reports and activities of electronic media and community portals in it, will be important for the conducted analysis. It is significant, because more and more often the opinion that progress in fighting epidemics can be made through using the so-called cognitive surplus appearing on the basis of crowdsourcing in the decision making process is winning more and more popularity.

    Opis:
    Prospects for the Development of Epidemiological Supervision: Some Remarks Inspired by Disease Mapping The article is dedicated to pointing out and discussing the challenges which appear for epidemiological supervisory authorities in the face of globalisation. The research area determined in this way has necessitated the papers division in three parts. The first part offers analysis of the International Health Regulations (IHR) which are the basis of the global epidemiological supervision system. The decision instruments necessary to estimate the risk in case of infectious diseases will be determined in the light of these regulations. The deliberations will also touch upon the issue, interesting from many points of view, of a unilateral mixed act in case of which the international aspects are accompanied by issuing legally binding national decisions. These problems are well illustrated by the solutions contained in Article 32b(2) and Article 33 of the Act of 5 December 2008 on the Prevention and Control of Human Infections and Infectious Diseases. In the second part, the analysis will cover two aspects of health administration, i.e. social and cognitive agility. In this context, the problems of limited rationality will be presented. The third part of the publication, features an extremely interesting problem related to the so-called spatial epidemiology dealing with mapping of the location and intensity of diseases. Determining appropriate repositories, using the information based on unofficial reports and activities of electronic media and community portals in it, will be important for the analysis conducted. It is significant because increasingly frequent are views that the progress in epidemics control can be achieved through using the so-called cognitive surplus appearing on the basis of crowdsourcing in the decision-making process.
