Opis:
Background Musculoskeletal disorders mainly affecting the spine constitute one of the major social problems. Frequently, they constitute the cause of prolonged absence at work. Working in a fire brigade is specific because of the physical strain, working time under adverse conditions or stress related to rescue operations. Material and Methods The study group comprised 61 firefighters, 60 men (98.36%) and 1 woman (1.64%). The age of examined firefighters ranged 20–56 years old. The average age of the examined persons was 33.77 years old and the average working service in a fire brigade was 9.59 years. The following research instruments were applied as the study methods: own survey, the Numerical Rating Scale, the International Physical Activity Questionnaire (IPAQ) – long version and the Oswestry Disability Index (ODI) questionnaire. The Pearson correlation coefficient at significance level of p < 0.05 was used for determining the dependencies among the variables. Results Knowledge of ergonomic lifting and handling principles concerning heavy loads was reported by 84% of the group, the remaining 16% declared ignorance of such rules. The dependence of pain on the length of service of the examined was insignificant and amounted to 0.281 Pearson’s r. The results of the ODI questionnaire did not significantly change statistically with age. Conclusions The IPAQ confirmed the high level of physical activity of the study population. Even though the study group was small, the results of ODI showed a minor degree of disability. Med Pr 2018;69(4):365–373

Opis:
Chronic lower limb ischemia significantly impairs the everyday functioning of the patient as it progresses. Patients complain of the inability to walk longer distances, climb stairs or walk briskly. The aim of this study was to assess the influence of chronic lower limb ischemia on the long-term results measured using the Walking Impairment Questionnaire (WIQ), as well as to establish a relationship between our results and selected variables, i.e. BMI (Body Mass Index), ICD (Intermittent Claudication Index), ACD (Absolute Claudication Index) and gender. Materials and Methods. The research group consisted of 50 individuals diagnosed with lower limb ischemia stage IIb according to the Fontaine classification. All of the patients experienced intermittent claudication after walking a distance of less than 200 m. The mean age was 64,4±6,0 years for men (n=38) and 62,7±6,9 years for women (n=12). BMI for both sexes was 27,1±4,1 kg/m². The study used the WIQ questionnaire, which consists of 21 items and evaluates walking speed, walking distance and stair climbing. In order to obtain information concerning gender, age and BMI of the patients, an original survey was created and distributed. In addition, the patients were evaluated using the treadmill stress test, which helps to determine the severity of claudication among patients. Results. Our research showed no correlation between the WIQ score and the gender of the patient. A statistically significant correlation was found between ICD, ACD and WIQ scores (rho=0,760, rho=0,770). No relationship was observed between the BMI and WIQ score (p=0,612). Scores in the individual WIQ domains strongly correlated with the total score obtained in the questionnaire. Conclusions. The WIQ questionnaire proved to be a reliable tool for assessing motor function and disorders in patients with chronic lower limb ischemia. The results of the treadmill stress test complied with the score of the questionnaire. There was no correlation between the WIQ score and gender, as well as with the BMI of the patients.

A number of epidemiological studies conducted over the last decade indicate a relationship between specific pathogen infections and the development of atherosclerosis, although no pathogenetic pathways connecting these two have been determined. Recent reports support the role of heat shock proteins (HSPs) in atherogenesis. The HSPs are also believed to be a link between the infection and the development of atherosclerotic lesions. The aims of study: Immunohistochemical evaluation of carotid artery segments to show the relationship between the presence of heat shock proteins and the serum levels of anti-hsp60 antibodies. An attempt to demonstrate a relationship between an expression of chronic C. pneumoniae and CMV antigens. Material and Methods: The study included 41 patients qualified for carotid artery endarterectomy and 18 healthy volunteers of corresponding age. Levels of anti- hsp60, anti-C. pneumoniae IgA and IgG, anti-CMV IgG antibodies as well as hsCRP were determined. Results: The mean serum levels of anti-hsp60 antibodies were higher in patients with advanced atherosclerosis as compared to healthy volunteers (55.3±64.1 vs 32.8±29.8; p < 0.05). There was a strong correlation between anti-hsp60 antibodies and the expression of hsp60 in carotid arterial wall, as confirmed by immunohistochemical evaluation. The study group showed statistically significant higher levels of hsCRP. Furthermore, statistically significant higher serum levels of anti-C. pneumoniae IgG and IgA as well as anti-CMV IgG antibodies were found in the study group as compared to controls. No correlation was shown between the markers of chronic infection induced by the tested pathogens and serum levels of anti-HSP and hsCRP. Conclusions: Higher protein expression in vascular walls is closely correlated with the level of anti-hsp60. At the same time, no significant relationship between anti-hsp60 antibodies and serological markers of infection was observed, which may only indicate an indirect role of infection in the assessment of breaking the immunological tolerance against autologous HSPs.
