Objectives This study has researched the significance of histologically raised findings and lung dust analyses in the context of claiming the recognition of and thus compensation for an asbestos-associated occupational disease. Material and Methods For this approach, all findings from the German Mesothelioma Register in 2015 that included lung dust analyses were evaluated and were compared with information on asbestos fiber exposure at work based on fiber years, and with the results of radiological findings. Results For 68 insured persons, recognition of an asbestos-induced lung disease according to Section 4104 of the German Ordinance on Occupational Diseases (Berufskrankheitenverordnung – BKV) could be recommended solely on the basis of the histological examinations of lung tissues and complementary lung dust analyses. Neither did the calculation of the cumulative asbestos dust exposure at work yield 25 fiber years, nor could bridge findings (e.g., plaques) be identified. In addition, the autopsies of 12 patients revealed plaques that had not been diagnosed during radiological examinations. These results show that – irrespective of the prescribed working techniques and radiological diagnosis – pathological/anatomical and histological diagnostics are often the only way for the insureds to demonstrate the causal connection between asbestos and their disease. Even after long intervals of up to 40 years post last exposure, the asbestos fibers would still be easily detectable in the lung tissues evaluated. Conclusions Whenever suitable tissue is available, it should be examined for mild asbestosis with the aid of a lung dust analysis. Otherwise there is a risk that an occupational disease is wrongfully rejected. In the context of health insurance, the lung dust analysis and the resulting proof of the presence of asbestosis often constitute one option of providing evidence of an occupational disease. Lung cancer is associated with one of the highest mortality rates among malignant tumors. It is the main death cause of men and women in Poland where over 15 000 men and 7 000 women die with this diagnosis annually. 1.7 million people die every year due to lung cancer in the world. Two main types of lung cancer are small cell lung cancer (SCLC) and non-small cell lung cancer (NSCLC). The history of lung cancer treatment begins with surgical approach followed by addition of chemotherapy and radiotherapy, which were used either separately or in combination depending on the stage of the lung cancer. Many somatic mutations were identified and molecularly targeted therapies could have been established. One of the oldest and the best known group of molecularly targeted drugs used in lung cancer treatment are tyrosine kinase inhibitors (TKIs) of epidermal growth factor receptor (EGFR). First EGFR-TKI was gefitinib, which has been examined in clinical trials before erlotinib, afatinib, dacomitinib and osimertinib. EGFR-TKIs increased overall survival (OS) with significantly less side effects when compared to standard chemotherapy. Another group of molecularly targeted drugs are anaplastic lymphoma (ALK) kinase inhibitors such as crizotinib, alectinib and ceritinib. Another innovation which was introduced in NSCLC treatment was immunotherapy. Its effect is based on modification of immune system leading to activation of cytotoxic T lymphocytes (CTL). Currently nivolumab and pembrolizumab (anti-PD1 antibodies) as well as atezoliumab (anti-PD-L1 antibody) are being used in NSCLC patients. The discovery of innovative therapies for NSCLC patients resulted in significant extension of patients' life expectancy while minimizing the side effects of such therapy. Moreover, the quality of patients’ life was significantly improved. However, important problems still remain to be solved: overcoming the resistance in the course of molecularly targeted therapies and the lack of predictive factors that determine the effectiveness of immunotherapy.

Opis:
Objectives Radon concentrations for 31 Polish underground tourist routes were analyzed. The equivalent dose to the lung, the effective dose and the relative risk were calculated for employees of the analyzed routes on the grounds of information on radon concentrations, work time, etc. Material and Methods The relative risk for lung cancers was calculated using the Biological Effects of Ionizing Radiation (BEIR) VI Committee model. Equivalent doses to the lungs of workers were determined using the coefficients calculated by the Kendall and Smith. The conversion coefficient proposed by the International Atomic Energy Agency (IAEA) in the report No. 33 was used for estimating the effective doses. Results In 13 routes, the effective dose was found to be above 1 mSv/year, and in 3 routes, it exceeded 6 mSv/year. For 5 routes, the equivalent dose to lungs was higher than 100 mSv/year, and in 1 case it was as high as 490 mSv/year. In 22.6% of underground workplaces the risk of developing lung cancer among employees was about 2 times higher than that for the general population, and for 1 tourist route it was about 5 times higher. The geometric mean of the relative risk of lung cancer for all workers of underground tourist routes was 1.73 (95% confidence interval (CI): 1.6–1.87). Routes were divided into: caves, mines, post-military underground constructions and urban underground constructions. Conclusions The difference between levels of the relative risk of developing lung cancer for all types of underground tourist routes was not found to be significant. If we include the professional group of the employees of underground tourist routes into the group of occupational exposure, the number of persons who are included in the Category A due to occupational exposure may increase by about 3/4. The professional group of the employees of underground tourist routes should be monitored for their exposure to radon. Int J Occup Med Environ Health 2017;30(5):687–694

Investigation was devoted determination if cancer tissues of lungs are mineralized or not. Moreover author like find answer for question: if the mineralization of tissue may lead to formation of cancer tumors or cancer tissues producers of substances necessary for lung mineralization? Maybe both phenomenon are possible? Investigation were performed using classic mineralogical methods on thin sections of cancer tissues done with the use of microtom. Obtained results confirmed the presence of trace mineralization of cancer tissues. It is represented by two types i.e. secret and visible mineralization. Both type are probably factors creating deformations of DNA leading to formation of cancer. This hypothesis have to be confirmed by experimental investigation.

The association between the lung cancer and indoor radon exposure in Russian population was investigated. The average indoor radon concentration for each region was estimated using the annual reports issued by the Saint-Petersburg Ramzaev Research Institute of Radiation Hygiene for the period 2008–2013. The average standardized lung cancer mortalities among males and females were estimated using the reports of the Moscow Hertzen Cancer Research Institute for the period 2008–2012. The relative risk (RR) was estimated as a ratio between the average mortality within seven exposure intervals and background mortality. The slope factors of linear dependence between the indoor radon exposure and lung cancer RR are 0.026 (–0.11÷0.17) and 0.83 (0.52–1.12) per radon concentration 100 Bq/m3 for males and females, respectively (with 90% confidence interval). The obtained results can be explained by the confounding effect of tobacco smoking. Significant excess risk of lung cancer in female population can be associated with radon exposure and low prevalence of smoking.

Opis:
Transthoracic lung ultrasound is ultrasonographic assessment of the lungs, parietal and visceral pleura and the space between these two membranes. It is a part of chest ultrasound examination which also includes assessment of the ribs, intercostal spaces, diaphragm, heart and mediastinum. Lung ultrasound as a diagnostic tool which has been known for 30 years, but its advantages and disadvantages are not commonly known and therefore it is not routinely used by clinicians. In paediatrics, lung ultrasound has become more popular since the first decade of the 21st century. The main advantage of this method is no exposure to ionizing radiation. Transthoracic lung ultrasound can be performed in practically any conditions and repeated several times. What is more, its accessibility, reliability, accuracy and low cost compared to other methods, makes lung ultrasound the great tool for lung assessment. It can be used both for initial diagnosis and monitoring of treatment (bedside examinations are possible). In this paper, we describe the technique of lung ultrasound examination, equipment requirements, normal lung ultrasound appearance, disadvantages of the examination and lung ultrasound findings in the most common paediatric lung diseases, such as: pneumonia, pleural fluid, atelectasis, pneumothorax, bronchiolitis, pulmonary oedema and pulmonary embolism.
Przezklatkowe ultrasonograficzne badanie płuc to ultrasonograficzna ocena płuc, opłucnej ściennej, opłucnej płucnej i przestrzeni między tymi błonami. Jest częścią ultrasonograficznego badania klatki piersiowej, w którym dodatkowo ocenia się struktury powierzchowne ograniczające jamę klatki piersiowej, przeponę, śródpiersie i serce. Przezklatkowa ultrasonografia płuc jako metoda diagnostyczna chorób płuc stosowana jest od ponad 30 lat, jednakże jej zalety i ograniczenia nie są powszechnie znane i nie uzyskały jeszcze pełnej akceptacji klinicystów. Od pierwszej dekady XXI wieku badanie to staje się coraz powszechniej wykorzystywaną metodą diagnostyki obrazowej w pediatrii. Do tej pory nie udokumentowano działań niepożądanych, co więcej – jest to metoda pozbawiona szkodliwego promieniowania jonizującego. Przezklatkowe ultrasonograficzne badanie płuc można wykonać praktycznie w każdych warunkach i wielokrotnie powtarzać. Z uwagi na jego dostępność, wiarygodność i dokładność w porównaniu z metodami uznanymi za referencyjne, a także niski koszt wydaje się najlepszym badaniem służącym do oceny płuc. Może być ono wykorzystywane zarówno w procesie wstępnej diagnostyki, jak i w monitorowaniu przebiegu leczenia (możliwość wykonania badań przyłóżkowo). W artykule oprócz zagadnień dotyczących techniki badania ultrasonograficznego, wymagań sprzętowych, prawidłowego ultrasonograficznego obrazu płuc i ograniczeń badania omówiono jego rolę w diagnostyce chorób płuc u dzieci, takich jak: zapalenie płuc, płyn w jamie opłucnej, niedodma, odma opłucnowa, zapalenie oskrzelików, obrzęk płuc i zatorowość płucna.

A complex model of mechanically ventilated ARDS lungs is proposed in the paper. This analogue is based on a combination of four components that describe breathing mechanics: morphology, mechanical properties of surfactant, tissue and chest wall characteristics. Physical-mathematical formulas attained from experimental data have been translated into their electrical equivalents and implemented in MultiSim software. To examine the adequacy of the forward model to the properties and behaviour of mechanically ventilated lungs in patients with ARDS symptoms, several computer simulations have been performed and reported in the paper. Inhomogeneous characteristics observed in the physical properties of ARDS lungs were mapped in a multi-lobe model and the measured outputs were compared with the data from physiological reports. In this way clinicians and scientists can obtain the knowledge on the moment of airway zone reopening/closure expressed as a function of pressure, volume or even time. In the paper, these trends were assessed for inhomogeneous distributions (proper for ARDS) of surfactant properties and airway geometry in consecutive lung lobes. The proposed model enables monitoring of temporal alveolar dynamics in successive lobes as well as those occurring at a higher level of lung structure organization, i.e. in a point P0 which can be used for collection of respiratory data during indirect management of recruitment/de-recruitment processes in ARDS lungs. The complex model and synthetic data generated for various parametrization scenarios make possible prospective studies on designing an indirect mode of alveolar zone management, i.e. with a minimized risk of repeated alveolar recruitment/de-recruitment and mechanical overstraining of lung tissues.

Introduction. Thiram, a fungicides, is widely used on seeds and as foliar agent on turf, vegetables and fruit. It is also used in the rubber industry as a vulcanization accelerator. When absorbed through the respiratory system, it is rapidly metabolised to dimethylthiocarbamate and carbon disulphide, causing noxious effects. A brief review is presented of the literature, centering on the interesting case of a 45-year-old woman admitted to the hospital suffering from acute respiratory failure. Results. Computer tomography in angiographic option (angio-CT) showed an extensive, irregular area of ground glass in both upper lobes and apical segments of the lower lobes of the lungs. A significant enlargement of both atria was also described. There was no improvement after cardiac treatment and patient was transferred to the pulmonary department where she was succesfully treated with systemic glucocortycosteroids. The patient remains under the supervision of the pulmonary out-patient department.

Lung cancer is the most common fatal cancer disease in the world. A characteristic feature of lung cancer is genetic diversity. In the overwhelming majority of cases, smoking is the most important etiopathogenic factor. Lung cancer is a cancer with a very bad prognosis regarding long-term survival. The risk of lung cancer depends primarily on active or passive exposure to the carcinogenic components of tobacco smoke. According to available data, the development of lung cancer in addition to active and passive smoking is directly affected by environmental pollution such as smog and fumes, ionizing radiation, mycotoxins and long-term exposure to asbestos (occupational exposure). Research on the pharmacoprevention of lung cancer began over 30 years ago. The first nutrient that the researchers said could inhibit the development of lung cancer was beta carotene. Unfortunately, long-term regular supplementation with high doses of antioxidant in the form of beta-carotene brought the opposite effect. An increase in the incidence of lung cancer was found in people who received beta carotene in the form of a synthetic food supplement. The other component tested was N-acetylcysteine. It is a sulfur compound and a powerful antioxidant that supports the synthesis of glutathione and cysteine, with destructive effects on carcinogenic substances. N-acetyl-cysteine, used in the form of NAC adduct and epigallocatechin-3-gallate, showed efficacy in inhibiting the development of lung cancer only in animal models. In the pharmacoprevention of lung cancer, the use of vitamin E was also tested in the form of tocotrienol and tocopherol. The following work also shows the existence of a high concentration correlation which belongs to the steroid hormone, mainly estrogen, in the blood and the development of lung cancer in women. An increased risk of lung cancer has been observed in women undergoing long-term hormone replacement therapy. The results show that 2-methoxyestradiol, the endogenous metabolite of 17ß-estradiol, shows positive results that inhibit the growth of lung cancer cell lines. The aim of the work was to present the correlation between tobacco abuse and passive smoking and lung cancer, pharmacoprevention of lung cancer and the association of elevated estrogen concentration in women with an increased risk of lung cancer.

Introduction: Chest pain is one of the symptoms of lung cancer. Chest pain disrupts patient’s functioning in somatic and psychic area. Purpose: Whether the existence of chest pain affects the level of perioperative anxiety in lung cancer patients. Is there a relationship between everyday functioning due to the chest pain and the level of perioperative anxiety? Is there a relation between anxiety associated with pain and gender? What is the cause of anxiety in this group of the patiens? Methodology: The study was conducted among 150 patients with lung cancer before the scheduled surgery. Data was collected with the use of questionnaire assessment of perioperative anxiety level in patients with lung cancer. Results: The chest pain before the surgery was confirmed by 63 (42%) patients, in case of 87 (58%) patients it was not identified. Chest pain was in case of 48% women and 36% men. 50 (33,3%) research participants who experienced chest pain and 35 patients (23,3%) without this symptom declared experiencing perioperative anxiety. In patients with lung cancer, the correlation: between chest pain and perioperative anxiety was (Z = -4.67; p< .001); between the difficult daily functioning of pain and perioperative anxiety was (Z = -4.72; p< .001); between gender and perioperative anxiety associated with pain was (Z = -3.24; p = 001). Patients afraid of: pain (37,3%), breathing problems (24,0%), physical disability (16,0%), eating problems (12,0%) sleep disorders (9,3%), nothing (1,3%). Conclusions: Patients with chest pain exhibited significantly higher of perioperative anxiety levels than non-symptomatic patients, as well as higher anxiety levels due to deteriorated daily functioning caused by the disorder. Women had a higher level of anxiety associated with pain than men before the surgery. Patients with lung cancer were most afraid of pain in the perioperative period.

Non-small cell lung cancer is one of the most commonly diagnosed cancer with a very high mortality rate. Trying to understand the mechanisms underlying the progression of this type of cancer, it is necessary to evaluate the changes occurring at molecular level in cancer cells. Besides the widely studied signaling pathways and genes which are dysregulated in NSCLC, there is a large group of non-coding RNAs involved in cancer pathogenesis. Those RNAs are tissue specific heterogeneous class of RNAs that play many functions in physiological condition in cells, nevertheless current data has shown that lncRNAs are also functional in different types of cancer. Moreover, it has been suggest that lncRNAs are involved in cancer progression by controlling key signaling pathways involved in diverse types of tumors. Notch signaling is one of those pathways, very often deregulated in NSCLC. Therefore in this review I summarized recent outcomes according the importance of lncRNAs in regulation of Notch pathway in the pathogenesis of NSCLC.

Lung cancer is associated with an increased risk of venous thromboembolism, including pulmonary embolism. In some situations, clinical deterioration in patients with lung cancer accompanied by elevated pressure in the right ventricle, usually measured by commonly used echocardiography, may support the diagnosis of pulmonary embolism. However, there are many other causes of increased pressure in the right ventricle in such patients, for example: progression of cancer, pre-existing lung diseases, surgical resection of pulmonary tissue, pnemotoxicity of radiotherapy or concomitant diseases of the left heart. The article presents 2 clinical cases of patients with lung cancer, in which elevated pressure in the right ventricle was resulted from other causes and accompanied the progression of cancer, despite the clinical picture suggesting a pulmonary embolism. Increased pressure in the right ventricle and usually associated pulmonary hypertension, significantly worsen already poor prognosis of patients with lung cancer. The differential diagnosis should, therefore, take into account the whole clinical picture, excluding venous thromboembolism as an important cause of pulmonary hypertension, but also take into account other potential factors to be able to make the right diagnosis and implement optimal treatment as early as possible.
