The main task of this publication is to present the psychological care and psycho-education system in the Polish Police. The aim of the mental health care system is to provide help and psychological support to police officers and police employees, and to relieve their tensions caused by negative external factors, called stressors. This help and support is necessary due to the fact that the profession of a police officer is considered a ‘high-risk’ job, one of the most stressful in the world and resulting in many difficult experiences, burdens and problems in the psychological sphere. Persistent mental disorders are often the cause of somatic diseases. Police officers on duty are confronted with many difficult situations, stressful events and they are exposed to numerous challenging and risky situations, which in consequence are the cause of the professional stress in officers. This, in turn, leads to psychological tensions and various ailments, somatic and mental illnesses. The article presents a number of factors that occur in the external service that are sources of work stress. Reference has also been made to the fact that a very common source of this stress is an inappropriate management process within the Police. The reason for numerous tensions are often conflicts between superiors and subordinates. However, the key to the publication is the description of the functioning of the mentioned system. The history of its creation is the first step in its presentation here. The organisational structure of the system, the relatively full range of tasks assigned to officers, the organisation of their work, as well as the catalogue of events requiring their intervention and the forms of psychological assistance have all been presented. The paper also focuses on the psycho-education system — the one carried out directly by police psychologists, and this performed in a broader sense, called the psychological education which is carried out by the lecturers and management of the Police. As part of this psychological education process, future and current managers are equipped with a wide range of psychological knowledge. It is necessary for managers at various levels to be the first to provide the necessary psychological support and ‘first aid’ to their subordinates in cases of occupational stress and psychological load caused by service conditions. The conclusion shows that a well-functioning system of psychological care and psycho-education has a positive impact on the mental condition of police officers and employees, on their level of satisfaction with police service, and indirectly on the quality and effectiveness of the police work.

Opis:
Use of direct coercion is a very peculiar situation, as it requires the use of force against another human being. Licence to take such action must be governed by the law, in the case of psychiatric care stipulated in the provisions of Mental Health Protection Act. Under this act, doctors and nurses are authorised to use such means as restraint, seclusion, and/or administration of relevant drugs under force to a patient displaying aggressive or auto-aggressive behaviour. Similar behaviours, however, can also be presented by special education students, some of who were once patients of psychiatric hospitals. Educators in schools are not allowed or authorised to use physical force against a student. What should a teacher do in a situation of aggression on the part of a student to protect their own and other people’s safety? Is it permissible in certain situations to restrain a student? Perhaps in some clearly defined cases teachers have an obligation to use force against a student with a disability? This article is an attempt to answer these questions.
Zastosowanie przymusu bezpośredniego to sytuacja szczególna, wymagająca użycia siły wobec drugiego człowieka. Możliwość podjęcia podobnych działań musi być regulowana przez określone akty prawne. W kontekście rzeczywistości psychiatrycznej należy wskazać przede wszystkim na zapisy ustawy o ochronie zdrowia psychicznego. Zwrócono w niej uwagę, że lekarz i pielęgniarka mogą podjąć wobec agresywnego lub autoagresywnego pacjenta takie działania, jak unieruchomienie, przytrzymanie, izolacja i przymusowe podanie leków. Jednak niebezpieczne zachowania mogą prezentować również uczniowie szkół specjalnych, których część była niegdyś pacjentami szpitali psychiatrycznych. Nie zmienia to faktu, że na terenie szkół pedagodzy nie mają formalnego prawa do zastosowania przymusu bezpośredniego wobec ucznia. Co zatem mogą zrobić, kiedy podopieczny stosuje siłę? W jaki sposób nauczyciel powinien postąpić w przypadku agresji ucznia, by chronić bezpieczeństwo własne oraz innych osób? Czy w pewnych sytuacjach wolno mu przytrzymać podopiecznego albo go unieruchomić? Czy w wybranych przypadkach pedagog ma obowiązek użycia siły wobec niepełnosprawnego ucznia? W artykule podjęta zostanie próba udzielenia odpowiedzi na powyższe pytania.

A quiet revolution underway is leading to changes in healthcare for many countries of the world. The sentinels of health have always been physicians; the medical model, representing the research, education, and experience of medical doctors, has worked well for treating acute conditions of injury and physical illness and been very successful in reducing disease-producing morbidity and mortality. But pressing contemporary issues, such as spiraling healthcare costs, population aging and the need to manage chronic conditions, and recognition that mental health care is necessary for societies to be physically healthy, are forcing a reevaluation of existing conceptualizations of care. In response, physicians are increasingly working in integrated practices with other specialists to broaden care coverage to include social and behavioral conditions empirically demonstrated to influence medical care outcomes. Aiding the development of these new, more inclusive models of healthcare are advances in communication technologies. Practitioners are now using synchronous and asynchronous forms of communication to deliver physical and mental health services beyond the boundaries of traditional brick-and-mortar practices, into communities where clients live and work. This brief review of the potential of telehealth to address disparities in healthcare has two purposes: 1) examine an innovative model of comprehensive, integrated biopsychosocial services useful in single- and multiple-location practices; and, 2) identify challenges of using synchronous, virtual technologies for client-centered, mental health care service provision in rural, underserved areas.

Objectives: The association between depressive symptoms and psycho‑organisational work environment has been established in the literature. Some studies have evaluated depressive symptoms in healthcare workers, but little research has been carried out among nurse managers. The aim of the study is to evaluate the depressive symptoms prevalence among nurse managers' population and work environment factors. Material and Methods: A descriptive correlational research design was used. Data were collected from 296 nurse managers in five hospitals in the eastern area of France between 2007 and 2008. Health outcomes were evaluated by measuring depressive symptoms (CES-D scale), the exposure data by assessing psycho‑organisational work environment with effort-reward imbalance-model of Siegrist. Multiple logistic regressions were used to describe the strength of the association between depressive symptoms and effort-reward imbalance adjusted for personal and occupational characteristics of the nurse managers. Results: Among the nurse managers, a third had depressive symptoms, and 18% presented an effort-reward imbalance (ratio: ≥ 1). A significant association was found between depressive symptoms and effort-reward imbalance (OR = 10.81, 95% CI: 5.1-23, p < $10^{-3}$), and with esteem as a reward (OR = 3.21, 95% CI: 1.6-6.3, p < $10^{-2}$). Conclusion: In view of the hierarchical situation of nurse managers and their primary roles in hospitals, it is necessaryto take prevention measures to improve their work environment and health.

The aim of the present study was to systematically review the association of comorbid mental disorders with indirect health care costs in patients with coronary artery disease (CAD). A comprehensive database search was conducted for studies investigating persons with CAD and comorbid mental disorders (Medline, EMBASE, PsycINFO, Psyndex, EconLit, IBSS). All studies were included, which allowed for a comparison of indirect health care costs between CAD patients with comorbid mental disorders and CAD patients without mental disorders. The literature search revealed 4962 potentially relevant studies, out of which 13 primary studies met the inclusion criteria. Depression was investigated most often (N = 10), followed by anxiety disorders (N = 3) and any mental disorder not further specifi ed (N = 3). All studies focused on return to work as indirect cost outcome. CAD patients with depression showed diminished odds for return to work, compared to CAD patients without depression (OR = 0.37; 95% CI: 0.27-0.51). The fi ndings for comorbid anxiety and any mental disorder were inconsistent. Indirect health care costs were exclusively assessed by a patient self-report (N = 13). There is strong evidence for diminished odds of return to work in CAD patients with comorbid depression, highlighting the need for integrated CAD and depression care. With regard to other comorbid mental disorders, however, the evidence is sparse and inconclusive.

The aim of the study was to assess the extent of depressive symptoms and anxiety and to assess the quality of life of mothers taking care of children with moderate to severe mental retardation in relation to the presence of children’s behavioural disorders and psychopathological symptoms. Material and method: Seventy-five mothers taking care of mentally retarded children were included in the study. The mean age of mothers was 42.95 (SD=8.47) and the mean age of children was 14.7 (SD=6.25). Fifty-two percent of children were irritable, 25% were aggressive, 7% had sexual disorders, 36% of children had depressive symptoms, 10% – anxiety, 10% – somatoform disorders, 13% – sleep disorders, 15% had eating disorders; psychotic symptoms/ odd behaviour were characteristic to 37% of children. Thirty-one percent of children were under control of a psychiatrist. The mental health condition of mothers was tested using Beck Depression Inventory (BDI), State-Trait Anxiety Inventory (STAI) and Hopelessness Scale (HS), whereas the quality of life was tested by the Quality of Life Scale. Next, the test results were statistically analysed. Results: The study revealed a significant correlation between the mental health condition of the mother and behavioural and mental disorders of the child (depression, anxiety, somatoform disorders or psychotic symptoms/ odd behaviour). There was a correlation between the quality of life of the mother and child’s irritability (p=0.006), depressive symptoms (0.0476) and psychotic symptoms/odd behaviour (p=0.0006). Conclusion: There were apparent correlations between the quality of life and the mental health condition of mothers and some child’s behavioural disorders or psychopathological symptoms (irritability, depressive symptoms, psychotic symptoms/odd behaviour).
Celem badania była ocena nasilenia objawów depresyjnych, nasilenia lęku oraz ocena jakości życia matek opiekujących się dziećmi z diagnozą upośledzenia umysłowego w stopniu umiarkowanym i znacznym w zależności od występowania u dzieci zaburzeń zachowania i objawów psychopatologicznych. Materiał i metoda: Populację badaną stanowiło 75 matek opiekujących się dziećmi z rozpoznaniem upośledzenia umysłowego. Średni wiek badanych kobiet wynosił 42,95 roku (SD=8,47); średni wiek dzieci wynosił 14,7 roku (SD=6,25). Drażliwość występowała u 52% dzieci, zachowania agresywne – u 25% podopiecznych, a zaburzenia zachowań seksualnych – u ok. 7%. Objawy depresyjne miało 36% dzieci, lękowe – 10%, somatyzacyjne – 10%, zaburzenia snu – 13%, zaburzenia odżywiania – 15%, objawy psychotyczne/zachowania dziwaczne – 37% dzieci. Pod opieką psychiatry pozostawało 31% dzieci. Stan psychiczny matek oceniano za pomocą Inwentarza Depresji Becka (BDI), Inwentarza Stanu i Cechy Lęku Spielbergera (STAI) i Skali Poczucia Beznadziejności (HS), a jakość życia za pomocą Skali Jakości Życia. Otrzymane wyniki poddano analizie statystycznej. Wyniki: Wykazano istotne związki między stanem psychicznym matki a występowaniem u dziecka zaburzeń zachowania i niektórych zaburzeń psychicznych (depresyjnych, lękowych, somatyzacyjnych, psychotycznych/zachowań dziwacznych). Potwierdzono związek między jakością życia matki a występowaniem u dziecka drażliwości (p=0,006), objawów depresyjnych (0,0476) oraz objawów psychotycznych/zachowań dziwacznych (p=0,0006). Wnioski: Jakość życia i stan psychiczny badanych matek miały wyraźny związek z obecnością u podopiecznego niektórych zaburzeń zachowania i objawów psychopatologicznych (drażliwości, objawów depresyjnych, objawów psychotycznych/zachowań dziwacznych).

Contemporary world events are demonstrating the need to embrace and further develop tele-health options for assessment and delivery of biopsychosocial healthcare services. This is now possible, given advances in communication technologies allowing virtual connections of medical personnel with constituents, as well as necessary, in light of recent challenges posed by infectious conditions and growing needs for travel restrictions, social distancing, and isolation of large portions of populations. Moreover, the opportunity to virtually connect with persons through ubiquitous computer-based and handheld communication devices allows comprehensive care provision to include underserved areas, where restricted, walk-in access to brick-and-mortar establishments has long been recognized as a limiting factor in healthcare. This review examines evolving approaches of tele-healthcare, with a specific focus on telemedicine as a bridge between traditional, in-person approaches to diagnose and treat medical conditions and new healthcare opportunities developing to meet changing societal needs. The three purposes of the review are: discuss background information, with a brief look at policy and procedure guiding applications of tele-techniques in healthcare practices; identify relevant scientific studies to show the breadth of new evidence-based research for telemedicine practices; and, discuss challenges for the further development of telemedicine as healthcare systems in the United States evolve to meet current and projected healthcare needs.

Introduction: During the COVID-19 pandemic, workload and physical and mental exhaustion became more extensive, especially among health care workers (HCWs). Material and Methods: This study involved 50 people aged 25 to 64, of whom 37 were women and 13 men. These HCWs were physiotherapists (48%), nurses (30%) and physicians (22%), all professionally active during the COVID-19 pandemic. The research was conducted between June and October 2020, using an on-line survey. The COPE, LBQ and ISI questionnaires were used. Results: Analysis of occupational burnout (LBQ) and its various aspects showed that the Deterioration of relations with clients (patients) was significantly higher among those with up to five years and between 15 and 24 years of experience than in the group with 25–35 years of experience. Sense of professional ineffectiveness and Disappointment were significantly higher (p %lt; 0.05) in the group with the most work experience. 30% of the medical workers surveyed obtained a result indicating subliminal insomnia (ISI) and 28% – clinical insomnia. Comparison of COPE and LBQ scores showed that Psychophysical exhaustion (LBQ) correlated negatively with Religious coping (p = 0.032). Deterioration of relations with clients (patients) (LBQ) correlated negatively with such strategies (COPE) as Active coping (p = 0.048), Planning (p=0.006), Positive reinterpretation and growth (p < 0.001). Sense of professional ineffectiveness (LBQ) correlated negatively with Acceptance strategies (p = 0.017) and Behavioral disengagement (p = 0.021). Disappointment correlated positively with the Positive reinterpretation and growth strategy (p = 0.024). Conclusions: The COVID-19 pandemic had an impact on the development of burnout amongst the HCWs surveyed, especially that related to psychophysical exhaustion, a sense of lack of effectiveness and a sense of disappointment. The sleep cycle was affected. Cognitive-behavioral oriented workshops are recommended when facing burnout.
Wprowadzenie: W trakcie trwania pandemii COVID-19 obciążenie pracą oraz fizyczne i psychiczne wyczerpanie były coraz większe, szczególnie wśród pracowników ochrony zdrowia (POZ). Materiał i metody: W badaniu wzięło udział 50 osób w wieku od 25 do 64 lat – 37 kobiet i 13 mężczyzn. Byli to: fizjoterapeuci (48%), pielęgniarki (30%), lekarze (22%), aktywni zawodowo w czasie pandemii COVID-19. Badanie przeprowadzono w okresie od czerwca do października 2020 r. w formie ankiety on-line. Wykorzystano kwestionariusze COPE, LBQ i ISI. Wyniki: Analiza wypalenia zawodowego (LBQ) i jego poszczególnych aspektów wykazała, że Brak zaangażowania w relacje z klientem (pacjentem) był istotnie wyższy w grupie ze stażem pracy do 5 lat oraz 15–24 lat niż w grupie ze stażem 25–35 lat. Poczucie braku skuteczności zawodowej i Rozczarowanie były istotnie wyższe (p %lt; 0.05) w grupie z najwyższym stażem pracy. 30% ankietowanych pracowników medycznych uzyskało wynik ISI wskazujący na obecność bezsenności podprogowej, a 28% bezsenności klinicznej. Analiza korelacji pomiędzy wynikami COPE a LBQ wykazała ujemną korelację pomiędzy Wyczerpaniem psychofizycznym (LBQ) a Zwrotem ku religii (p = 0,032). Brak zaangażowania w relacje z klientem (LBQ) ujemnie korelował z takimi strategiami (COPE) jak: Aktywne radzenie sobie (p = 0,048), Planowanie (p = 0,006), Pozytywne przewartościowanie i rozwój (p < 0.001). Poczucie braku skuteczności zawodowej (LBQ) było ujemnie skorelowane ze strategiami Akceptacji (p = 0,017) i Brakiem zaangażowania (p = 0,021). Rozczarowanie dodatnio korelowało ze strategią Pozytywne przewartościowanie i rozwój (p = 0,024). Wnioski: Pandemia COVID-19 miała wpływ na rozwój wypalenia zawodowego badanych pracowników ochrony zdrowia, zwłaszcza wypalenia związanego z wyczerpaniem psychofizycznym, poczuciem braku skuteczności i poczuciem rozczarowania. Wpłynęło to na cykl snu. W obliczu wypalenia sugeruje się warsztaty poznawczo-behawioralne.
