The study is aimed to assess the role of angiogenesis in the pathogenesis of external genital endometriosis. This study was conducted in the gynecological clinics of the Clinical Military Medicine Center of the Southern region of Ukraine (Odessa, Ukraine). There were examined 215 patients of reproductive age (30,2±0,9 years). These patients were distributed in 2 clinical groups: 94 patients with the external genital endometriosis and 81 with adenomyosis. The control group was presented by 40 healthy females of the same age (29,7±0,5 years). The clinical examination was conducted accordingly to the clinical guidelines recommended by the PHMU orders № 620 and 676 There were 14 (14,9%) patients of I group with IV stage of the external genital endometriosis, 33 (35,1%) – with III stage and 47 (50,0%) with the mild manifestations of the disease. The infertility occurs amongst 27.7% of the patients. In the second group the majority of patients have I-II stage of adenomyosis (54.3%). The concentration of VEGF in the peritoneal fluid was highly variable (23.1±1.3 ng/ml) and was correlated with the severity of endometriosis lesions (r=0,61 p<0,05)

Introduction and aim. Endometriosis is a complex condition in which endometrium, tissue that resembles the uterine lining, develops outside the uterus. It is considered to be a chronic, estrogen-dependent, inflammatory gynecological disorder having multi-factorial origins. This review paper aims to consolidate recent information on ethnic differences, endometriosis risks, and the disease’s etiology in the global context. Material and methods. A systematic search was performed using a variety of international electronic databases, including “PubMed” and “DOAJ”, using the terms endometriosis, endometriosis and infertility, endometriosis and cancer, and treatment of endometriosis. Analysis of the literature. Endometriosis can appear anywhere in the body, including the umbilicus, the cecum and ileum of the digestive tract, the breast, the lungs, and the genitourinary organs. It is typically clinically asymptomatic with no obvious clinical manifestation and expensive treatment, which makes the diagnosis late. There is a complex interplay between socioeconomic status, family history, societal beliefs and laws, personal habits, reproductive and gynaecological conditions, and environmental influences in the development of endometriosis. Conclusion. Women with endometriosis should be given more attention, and specific resources in the healthcare system should be utilized to provide more efficient multidisciplinary healthcare and treatment.

The study presented a case of a 32-year old female patient admitted to the department of surgery with symptoms of abdominal pain and retention of gas and stool. The abdominal X-ray examination showed signs of intestinal obstruction. Abdominal CT suggested the possibility of a cancerous lesion located in the colon. The patient was qualified for surgical intervention. An anterior rectal resection was performed. The histopathological examination revealed the presence of colorectal endometriosis. After two months the patient was subject to gastrointestinal continuity restoration. Endometriosis is a rare cause of intestinal obstruction, considering patients hospitalized in the department of general surgery.

Aim: The aim of the study was to determine the frequency of the malignant endometrioma transformation in the structures of the abdominal cavity organs and the pelvis. Material and methods: Making use of the hospital database, we have analyzed histopathological results of patients with ovarian carcinoma operated on between 2004 and 2012 at the Oncology Center in Bydgoszcz. In the case of 69 patients meeting the Sampson criteria, endometriosis and ovarian carcinoma were confirmed in one tissue sample. Following a thorough analysis of operation reports and histopathological results, we have observed the presence of carcinoma texture and endometriosis in: the adnexa, rectovaginal septum, sigmoid colon and the rectum, abdominal integuments, urinary bladder. Results: Endometriosis related to the texture of the carcinoma was present in the adnexa of 30 patients (44.7%), in the rectovaginal septum – in 25 (37.3%), in abdominal integuments – in 7 (10.4%), in the sigmoid colon or rectum – in 3 (4.5%) and within the urinary bladder – in 2 (3%); in the case of 2 patients, endometriosis was focused in several sites. Summary: Differences between patients with ovarian carcinomas, either associated with endometriosis or not – concerning the degree of clinical advancement, histopathological type and prognosis – induce one to search for new methods and new schemes of known therapies.
Cel: Celem badania było określenie częstotliwości transformacji złośliwej endometrioma w strukturach narządowych jamy brzusznej i miednicy. Materiał i metody: Korzystając ze szpitalnej bazy danych, przeanalizowaliśmy wyniki histopatologiczne pacjentek z rakiem jajnika, które były operowane między 2004 a 2012 rokiem w Centrum Onkologii w Bydgoszczy. U 69 chorych spełniających kryteria Sampsona potwierdziliśmy endometriozę i raka jajnika w jednym preparacie tkankowym. Po dokładnej analizie protokołów operacyjnych oraz wyników histopatologicznych ustaliliśmy obecność utkania raka i endometriozy w: przydatkach, przegrodzie odbytniczo-pochwowej, esicy i odbytnicy, powłokach brzusznych, pęcherzu moczowym. Wyniki: Endometrioza związana z utkaniem raka występowała w przydatkach u 30 pacjentek (44,7%), w przegrodzie odbytniczo-pochwowej – u 25 (37,3%), w powłokach jamy brzusznej – u 7 (10,4%), w esicy lub odbytnicy – u 3 (4,5%), a w obrębie pęcherza moczowego – u 2 (3%); u 2 chorych ogniska endometriozy występowały w kilku lokalizacjach. Podsumowanie: Różnice między pacjentkami z rakami jajnika związanymi i niezwiązanymi z endometriozą – dotyczące stopnia zaawansowania klinicznego, typu histopatologicznego oraz rokowania – skłaniają do poszukiwania nowych metod lub nowych schematów znanych terapii.

Due to late diagnosis, ovarian cancer is the most deadly gynecologic malignancy. Inflammation is one of the risk factors of ovarian cancer and the inflammatory response implicates all stages of tumorigenesis. The purpose of this study was to analyze the concentration of molecules, which can take part in malignant processes. We analyzed patients with ovarian cancer, with endometriosis and healthy controls. Thirty-seven analytes were measured in serum using BioPlex Pro Human Inflammation Panel. We were able to detect 28 of the proteins among the studied groups. We found a significant increase in 22 of the tested molecules (BAFF, Chitinase3-like 1, IFN-alpha2, IFN-beta, IFN-gamma, IFN-lambda2, IFN-lambda1, gp130, IL-2, IL-12 (p40), IL-11, IL-32, IL-35, MMP3, Osteocalcin, Pantraxin-3, sCD163, TNFRSF8, sIL-6Ralpha, STNF-R1, STNF-R2, and TSLP) in the ovarian cancer group in comparison to the healthy controls. Two of them (IL-20, MMP1) did not show significant differences between groups. Moreover, we identified decreased concentrations of APRIL and osteopontin in ovarian cancer vs. healthy controls. While this study is a preliminary report, we hope this will encourage a further use of multiplex analysis in ovarian cancer biomarker research.

Objective: The objective of this study was to demonstrate the histological diversity of endometriosis-associated ovarian carcinoma. Material and methods: Using record linkage techniques, we have conducted histopathological assessment in patients with endometriosis-associated ovarian cancer who received surgical treatment between 2004–2010 in the Oncology Centre in Bydgoszcz. Endometriosis and cervical carcinoma were confirmed in one tissue specimen in 62 patients. Results: Endometriosis seemed to promote the development of specific histologic types of ovarian cancer. Of 62 subjects, 53.22% (33/62) had endometrioid adenocarcinoma, 25.8% (16/62) had clear-cell carcinoma, 19.35% (12/62) had serous adenocarcinoma, and 1.61% (1/62) had mucinous adenocarcinoma. Our results support the hypothesis that coexistent endometriosis is more often associated with endometrioid and clear-cell carcinomas than other histologic subtypes as well as that clear-cell variant is the most common cancer developing in the abdominal wall scar from a previous laparotomy. Among patients with endometriosis-associated ovarian carcinoma in the abdominal scar, three (75%) had clear-cell carcinoma and one had papillary serous adenocarcinoma. Conclusions: According to Sampson’s criteria, endometrioid carcinoma of the ovary was the most common histological diagnosis in patients with endometriosis-associated ovarian carcinoma. The clear-cell variant of cancer was the most common type in patients with endometriosis-associated ovarian carcinoma in the abdominal scar after laparotomy.
Cel: Celem pracy jest prezentacja zróżnicowania histopatologicznego raków jajnika współistniejących z endometriozą. Materiał i metody: Korzystając ze szpitalnej bazy danych, przeanalizowaliśmy wyniki histopatologiczne pacjentek z rakiem jajnika, które były operowane między 2004 a 2010 rokiem w Centrum Onkologii w Bydgoszczy. U 62 chorych potwierdziliśmy endometriozę i raka jajnika w jednym preparacie tkankowym. Wyniki: Wydaje się, iż endometrioza wpływa na rozwój specyficznych typów histopatologicznych raków jajnika. Spośród 62 pacjentek 53,22% (33 przypadki) miało raka endometrioidalnego, 25,8% (16) – raka jasnokomórkowego, 19,35% (12) – raka surowiczego, a 1,61% (1) – raka śluzowego. Nasze wyniki potwierdzają tezę, że endometrioza może promować rozwój określonych typów histopatologicznych raka jajnika. Odnotowaliśmy częstsze występowanie raka endometrioidalnego i jasnokomórkowego niż innych podtypów raka jajnika, a także przewagę raka jasnokomórkowego wśród raków powstałych w bliźnie po laparotomii. W grupie pacjentek z rakiem w bliźnie trzy (75%) miały raka jasnokomórkowego, a jedna – surowiczego. Podsumowanie: Jeśli przyjąć klasyczne kryteria Sampsona, typ endometrialny był najczęstszym rozpoznaniem histopatologicznym u pacjentek z rakiem jajnika, któremu towarzyszyła endometrioza. Typ jasnokomórkowy okazał się zaś najczęstszym rozpoznaniem wśród chorych z rakiem, któremu towarzyszyła endometrioza w bliźnie po laparotomii.

Introduction. The prevalence of endometriosis among reproductive age women is 7–17%; however, these figures reach 20–50% in patients suffering from infertility. Matrix metalloproteinases (MMPs) activity is thought to be particularly essential in the early phases of endometriosis development. Any changes in the equilibrium between MMPs activity and their tissue inhibitors (TIMPs) could be potentially harmful, promoting endometriosis development. The aim of this study was to investigate whether the MMP-2, MMP-9, TIMP-1 or TGF-B2 expression in eutopic endometrium from women with early endometriosis differ when compared with healthy subjects. The results were referred to the serum progesterone levels. Materials and method. Endometrial biopsy was taken from 42 patients (18 in the study group, 22 in thecontrol group) at the time of hysteroscopy for routine histology and for RT-PCR procedures. Comparison of the quantity of gene products was performed with a programme for densitometry and compared to GADPH product, which was a reference value. Results. The obtained results did not reveal any statistical difference in endometrial expression of MMP-2, MMp-9, TIMP-1, and TGF-β2 or serum progesterone level between women with endometriosis and without visible signs of this illness. Conclusion. Despite the lack of statistical differences, it was observed that both examined metalloproteinases expressed a tendency to higher gene expression in the eutopic endometrium of women with endometriosis. However, both TIMP-1 and TGF-β2 expressions had the same tendency – higher values in endometriosis patients.

The endometrium of women with endometriosis has a different expression of cytokines, angiogenic and hormonal factors compared to healthy women. Endometriosis is a disease occurring mostly in women at a reproductive age with a frequency of 7–10%. Despite many studies and hypotheses on the pathogenesis of the disease, marker specific to endometriosis has not yet been detected. Thus, laparoscopy remains the gold standard in the diagnosis, particularly in the case of peritoneal form that cannot be visualized by means of an ultrasound test. Other forms of endometriosis – ovarian (chocolate cysts) and deep infiltrating – can be identified in an ultrasound test. Scientists are currently searching for markers that in a specific combination would ensure maximum sensitivity and specificity of the non-invasive detection of endometriosis, even at early stages. The article presents selected factors, such as interleukin 8, vascular endothelial growth factor, platelet-derived growth factor, nerve growth factor, detected in the endometrium, which can be a potential diagnostic target. Diagnoses made at the level of endometrium would facilitate the identification of endometriosis and would significantly reduce costs associated with the necessity to conduct laparoscopy.
Endometrium pacjentek z endometriozą charakteryzuje się inną ekspresją cytokin, czynników angiogennych i hormonalnych niż u kobiet zdrowych. Endometrioza występuje głównie u kobiet w wieku reprodukcyjnym, z częstością około 7–10%. Mimo wielu badań i hipotez dotyczących patogenezy nie wykryto dotychczas swoistego markera dla endometriozy. Złotym standardem diagnostycznym, zwłaszcza w przypadku postaci otrzewnowej, której nie udaje się uwidocznić w badaniu ultrasonograficznym, pozostaje laparoskopia. Inne formy endometriozy – jajnikową (torbiele czekoladowe) i głęboko naciekającą – można rozpoznać w badaniu ultrasonograficznym. Obecnie poszukuje się markerów, które w określonej kombinacji zapewniłyby maksymalną czułość i swoistość nieinwazyjnego wykrywania endometriozy, nawet we wczesnych stadiach. Czułość oznaczeń wykonywanych z krwi nie jest niestety satysfakcjonująca w odniesieniu do otrzewnowej postaci choroby. W artykule zaprezentowano potencjalne cele diagnostyczne – wybrane czynniki wykrywane w endometrium, czyli interleukinę 8, czynnik wzrostu śródbłonka naczyniowego, płytkopochodny czynnik wzrostu i czynnik wzrostu nerwów. Pobranie endometrium do badań jest łatwe i może być wykonywane ambulatoryjnie. Diagnozowanie z poziomu endometrium ułatwiłoby rozpoznawanie endometriozy i znacząco zmniejszyło koszty związane z koniecznością przeprowadzania laparoskopii.

Vitamin D is essential for the proper functioning of the human body. There is also evidence of its strong association with fertility problems in women. This review aims to evaluate the relationship between vitamin D and diseases affecting women’s fertility (polycystic ovarian syndrome (PCOS), uterine leiomyomas and endometriosis), and in vitro fertilization (IVF) outcome. A systematic review of the literature was conducted in Scopus and PubMed for relevant English language publications since 1989. Vitamin D influences the functioning of the reproductive system in women and has been associated with PCOS, uterine leiomyomas, endometriosis and in vitro fertilization (IVF) outcome. However, further studies on larger groups of patients are needed to establish what role vitamin D plays in the treatment of female infertility.

After uterine cancer, ovarian cancer is the second most common female reproductive tract malignancy. In Poland, in 2015, there were 3735 new cases and 2738 deaths from ovarian cancer. Over 80% of all cases are diagnosed in women aged over 50, whereas only 6% – before 35 years of age. Most cases are diagnosed, when clinical symptoms have already appeared, and the disease is at least locally advanced. The case presented below describes a very young 28-year-old woman with a tumour in the pelvis, ascites and very high Ca 125 and HE 4 levels. The clinical symptoms as well as laboratory and imaging tests were suggestive of epithelial ovarian cancer that required urgent surgery due to progression of the disease. The surgery posed a significant risk of complications, but owing to the deterioration of the patient’s condition it was considered necessary. The patient was born with a severe, complex heart defect, had undergone two cardiac surgeries, and had a pacemaker (first VVIR, then DDD). She was also intensively treated for iatrogenic HCV infection and hypothyroidism. A multidisciplinary team (a gynecologist, cardiologist and anesthetist) were behind the decision to carry out a major gynecological operation which ended with success. For the past 6 years, the patient has been under constant oncological follow-up and enjoys a good health.

Opis:
Ovarian clear cell carcinoma belongs to rather rare ovarian neoplasms and is believed to be a separate disease with a specific pathogenetic mechanism. It is diagnosed in women younger than 60 years of age, usually in early stages (FIGO I–II in 60% of cases). The National Comprehensive Cancer Network recommends surgical staging in early cancers and cytoreduction with adjuvant platinum- or taxane-based chemotherapy in later stages. Clear cell carcinoma of the ovary is characterized by worse response to chemotherapy and high carboplatin resistance. The mechanisms of its chemoresistance are unknown. The response to first-line chemotherapy is observed in only 11–27% of cases (vs. 73–81% for high grade carcinoma). Progression during chemotherapy is noted in 52% of patients (vs. 29% for high grade carcinoma). A relapse is associated with poor prognosis since it usually means considerable chemoresistance. Mean overall survival amounts to 25.3 months (vs. 42 months in epithelial ovarian carcinoma). Due to poor treatment outcomes in advanced stages of ovarian clear cell carcinoma, molecular targeted therapies are being developed. Research is being conducted in three directions: to develop new cytostatics, angiogenesis inhibitors and immunomodulators. Moreover, combinations of these drugs are also being investigated. It occurs that the future treatment is a therapy targeted to specifically impaired cell proliferation mechanisms in ovarian clear cell carcinoma. Moreover, the importance of adjuvant radiotherapy in stages FIGO IC–II must be emphasized.
Rak jasnokomórkowy należy do grupy rzadziej występujących nowotworów jajnika i jest uważany za oddzielną chorobę ze specyficznym mechanizmem patogenetycznym. Raka tego rozpoznaje się u kobiet poniżej 60. roku życia, zwykle w niskim stopniu zaawansowania (60% FIGO I–II). National Comprehensive Cancer Network rekomenduje we wczesnych stadiach zaawansowania chirurgiczny staging, a w wyższych stopniach – operację cytoredukcyjną i uzupełniającą chemioterapię opartą na platynie lub taksanach. Rak jasnokomórkowy jajnika charakteryzuje się gorszą odpowiedzią na chemioterapię i wysokim odsetkiem oporności na karboplatynę. Mechanizmy chemiooporności nie są znane. Odpowiedź na chemioterapię pierwszego rzutu występuje jedynie w 11–27% przypadków (vs 73–81% dla wysoko zróżnicowanych raków). Progresję w czasie chemioterapii odnotowuje się u 52% pacjentek (vs 29% dla wysoko zróżnicowanych raków). Wznowa raka jasnokomórkowego jajnika wiąże się ze złym rokowaniem, ponieważ zwykle oznacza dużą oporność na chemioterapię. Czas całkowitego przeżycia wynosi średnio 25,3 miesiąca, w porównaniu z 42 miesiącami w przypadku nabłonkowych raków jajnika. Ze względu na złe wyniki leczenia chorych w zaawansowanych stadiach raka jasnokomórkowego trwają poszukiwania terapii ukierunkowanej molekularnie. Badania są prowadzone w trzech kierunkach: nowe cytostatyki, leki hamujące angiogenezę, leki immunomodulujące; bada się też kombinacje wymienionych grup leków. Wydaje się, że przyszłością leczenia jest terapia ukierunkowana na mechanizmy proliferacji komórek swoiście zaburzonych w raku jasnokomórkowym jajnika. Należy podkreślić ważną rolę radioterapii w leczeniu pacjentek w stopniu FIGO IC–II.

Introduction and objective. Data on the possible role of peritoneal fluid free radical-mediated oxidative damage in the pathogenesis of endometriosis still remains inconsistent. The aim of the study was to determine iron metabolism markers and their influence on oxidative stress parameters in the peritoneal fluid of women with endometriosis. Materials and method. 110 women with endometriosis and 119 patients with benign ovarian cysts were included in the study. All visible peritoneal fluid was aspirated during laparoscopy from the anterior and posterior cul-de-sacs. under direct vision to avoid blood contamination. Haemoglobin, iron, total oxidative status, and total antioxidant status were measured using standard colourimetric kits. Results. Haemoglobin, iron levels, as well as total oxidative status values were significantly higher, whereas total antioxidant status values were significantly lower in the peritoneal fluid of patients with endometriosis, in comparison to the reference groups. No differences were observed in peritoneal fluid concentrations of all parameters measured in relation to the phase of the menstrual cycle. Conclusions. Peritoneal fluid of women with endometriosis is characterized by disrupted iron metabolism. This is most likely related to an increased number of erythrocytes in the peritoneal cavity of endometriotic women, which leads to a higher concentration of haemoglobin in this environment. Impaired iron homeostasis may have a significant influence on the pathophysiology of peritoneal endometriosis by the direct impact of haemoglobin derivatives and/or formation of the pro-inflammatory and pro-oxidative environment. Peritoneal cavity oxidative stress occurs predominantly in women in advanced stages of the disease.

Introduction and aim. Endometriosis is a common inflammatory disease affecting 6-10% of women of reproductive age. It is defined as the growth of endometrial-like tissue outside the uterine cavity. Dysmenorrhea, pelvic pain, dyspareunia and infertility are the main symptoms of endometriosis patients. Endometriosis treatment methods can be broadly divided into surgical and pharmacological. Currently, hormonal drugs are often used for women with endometriosis to relieve bothersome symptoms. The aim of this article is to review new publications presenting the effectiveness as well as side effects of the use of progestins and combined oral contraceptives in the hormonal treatment of endometriosis. Material and methods. A review of the literature regarding progestins and combined oral contraceptives in the treatment of endometriosis was performed using the PubMed database. In the end, 67 articles were included in this review. Analysis of the literature. Progestins and combined oral contraceptives significantly reduce dysmenorrhea, dyspareunia and pelvic pain in women with endometriosis. However, there is a risk of potential side effects, which should be taken into account when choosing a therapy for each patient individually. Conclusion. Endometriosis is a chronic disease that has a significant impact on the health-related quality of life of patients. When choosing a treatment, many aspects should be considered, primarily the patient’s preferences, drug tolerance and safety. Further drug research is needed to determine the most effective treatment for endometriosis.
