In this article I am focused on the functioning of the transnational intergenerational care system. This is dynamic,as it is bound up with the life cycle of the transnational family, and on the one hand, denotes practices associatedwith any assistance parents provide to their migrant children and on the other – in the event of elderly peoplebeing faced with health and basic living problems – with the phenomenon of migrants caring for their parentsin old age. The transnational system of care also incorporates the involvement (or lack of involvement, as faras this triggers consequences that are of relevance here) of relatively immobile people, for example the siblingsof migrants who provide (or not, as the case may be) domestic support for their elderly parents. In this articleI adopt the thesis that migrants who function in different care regimes change not only their own but also theirparents’ attitudes towards elderly care.

Polish culture is strongly linked to both nuclear family and family networks, which are believed to safeguard stability and sense of security for individuals and communities. The migrations of young Polish men and women, which often scope to entire families, significantly alter the fundamentals of the above guarantees. The migration of Polish youth changes social expectations, possibilities of a family existing in an unchanged form, as well as provisions of care to those who need it, primarily children and elderly members of the kinship structure. For families affected by temporary migrations and experiencing increasingly settlement-oriented mobility, being “on the move” becomes “a way of life,” in which periods of “togetherness” are intertwined with much longer phases of separation. The practices and strategies employed by migrants in the hopes of overcoming the aforementioned challenges require thorough analysis. Therefore, a main goal of our article is to focus on multi-dimensional consequences of migration that pertain to the changes of ties and relationships in families, as well as the organization of child and elderly care.

Italy is one of the most important destination countries for Romanians. At the same time, the Italian care sector relies mainly on migrant labour, most of whom are Romanian women. Historically, Italy is considered one of the landmark countries for the southern or Mediterranean welfare state, characterised by its fragmented labour market, underdeveloped social protection system, informal economy and unpaid care work, usually done by the women in the family. Italy has one of the highest rates in Europe of both the elderly population and life expectancy at birth. In the last 20 years, the care work was gradually redistributed to migrant care workers, most of them women from former socialist countries, who often live in the household where they work. Migration from Eastern Europe, particularly Romania, has been facilitated, on the one hand, by rising unemployment and low-paid job opportunities in migrants’ countries of origin in the context of the deindustrialisation of state industry and, on the other, by the Italian elderly public-support system which is based on cash benefits granted to the family which can be redistributed to employ migrant care workers. In this paper we analyse three specific types of care work migration from Romania to Italy and the main challenges which they face, taking into account the specifics of the work and the type of migration chosen. The methodology is qualitative, based on 20 semi-structured online interviews with Romanian care workers and two interviews with stakeholders.

Providing informal care to adults, especially elderly people, may affect many aspects of caregivers’ life, such as: physical and mental health, financial situation, social contacts, etc. Supporting dependent seniors is associated to a higher level of stress, burden and depression as well as higher mortality. The main purpose of this paper is to analyse the relationship between caregiving for adults and the subjective quality of life among Poles aged 50–69. We took into account not only the fact of providing care to adult people, but also its beginning, continuation and ending between waves. We assumed that subjective quality of life may be expressed by two variables: one describing life satisfaction, and the second one – loneliness. We used the panel subsample from the Generation and Gender Surveys (GGS) carried out in Poland in 2010/2011 and in 2014. We found a negative effect of stopping caregiving between waves on wellbeing of women-carers, which may be related to the loss of a close person. Moreover, providing care for a longer period of time increases loneliness, which confirms that providing support to others may lead to isolation and smaller social networks.

Objective. We investigated the usefulness of a team-based risk assessment method in patient transfer situations in municipal care homes for the elderly. Methods. Evaluation of risk assessment and action plans carried out in 2009. Focus group interviews with care givers and one-to-one interviews with managers and occupational therapists. Results. The evaluation showed that action plans and interventions were developed for each resident with identified risk connected with movement/transfer in daily life. Twenty-six patients (28%) of a total number of 94 were assigned to the "no risk" category regarding movement/transfer situations in daily life. The other 68 patients (72%) required further interventions, which were documented in action plans. The interviews indicated that the approach of team-based risk assessment and action plans was perceived as a functional participatory method aiming to improve daily life and work. Conclusion. Systematic team-based risk assessment and action plans benefit staff as well as residents at care homes for the elderly.

Chociaż władze rosyjskie uważają społeczeństwo obywatelskie za zagrożenie dla ich rządów, to społeczne organizacje pozarządowe (NGO) są oceniane pozytywnie przez instytucje państwowe na wszystkich poziomach władzy. W przeciwieństwie do organizacji broniących praw człowieka czy też aktywistów politycznych społecznie ukierunkowane organizacje trzeciego sektora raczej nie są krytykowane przez maszynę propagandową, lecz wypełniają obecnie kluczową rolę we wdrażaniu polityki społecznej w Rosji. Analizując trajektorie relacji z władzą 18 organizacji pozarządowych z Petersburga, zajmujących się opieką nad osobami starszymi, artykuł omawia trzy typy relacji między NGO a instytucjami państwa: NGO wspierające politykę państwa poprzez bycie częścią tej polityki; NGO uzupełniające polityki państwa w obszarach, gdzie państwo nie potrafi odpowiednio rozwiązać problemów; oraz NGO zastępujące państwo, a w niektórych przypadkach sprawujące władzę nad lokalnymi państwowymi instytucjami społecznymi.
Although the Russian regime considers civil society to be a threat to its authority, social NGOs are positively viewed by state institutions at all levels. Unlike human rights organisations and political activists, socially oriented third sector organisations are not typically criticised by the propaganda machine. On contrary, these NGOs currently play a crucial role in social policy implementation in Russia. By analysing the trajectories of state-third sector relations of 18 Saint-Petersburg-based non-governmental elderly care organisations, the article characterises three ways in which NGOs approach relationships with the state: NGOs which support state policy by being part of it, NGOs which supplement state policies in spheres the state fails to address appropriately, and NGOs which substitute for the state, in some cases exercising power over local state social services.

This paper is a reaction to a number of critical articles in Gothenburg’s media about privatized care services for elderly, which were published in the last year. Home services for elderly are seldom studied separately. Nonetheless they deserve researchers’ attention since the Swedish authority perceive home services as the best solution for older people according to Ädelreform of 1992, secured by the Social Service Act of 1980: 620 (Socialtjänstlag 1980: 620), 19§ and 20§ and its following adjustments that did not changed the first law in this regard (e.g., Socialtjänstlag 1997: 313; 1998: 384), and since especially knowledge about differences between public and private agencies providing home care services for the elderly is severely lacking (Öppna jämförelser: vård och omsorg om äldre, 2012). Hence, in this paper I ask: What have the public and private home care providers in common? What differences can be observed in their understanding of home care services for elderly and working methods? What lessons can be learned from the implementation of the state delegation of home care services for elderly to the municipal authorities? The purpose of this study is to compare and evaluate the public and private home care services for elderly given economic limitations after delegating them to municipality in the Gothenburg Region. The additional aim is to make politicians conscious about this development. The theoretical model of delegation and decentralization by Cristiano Castelfranchi and Rino Falcone (1998) and the Resource Dependency Theory by Pfeffer and Salancik (1978) constitute the theoretical reference frame. The study is based on an analysis of state regulation, policy documents and semi-structured interviews with the chief responsible for public and private home care services for elderly at the municipal level. This study reveals that the delegation of care for elderly to the municipalities faced some serious problems not be solved until 2013 and surprisingly that these problems are especially seen where the recipients of such care don’t have a choice on their service provider. The lesson drawn from the research is that if politicians or other authorities take away the right from people to make their own decisions about their own lives this inevitably results in dissatisfaction and subsequent reforms.
Celem artykułu jest porównanie i ocena publicznych i prywatnych domowych usług opiekuńczych dla ludzi starszych w regionie Goeteborg w Szwecji po przekazaniu przez władze centralne odpowiedzialności za te usługi na poziom samorządowy. Artykuł oparty jest na analizie legislacji, dokumentów urzędowych oraz wywiadów z kierownikami instytucji samorządowych odpowiedzialnymi za usługi opiekuńcze dla ludzi starszych w miejscu zamieszkania.

The paper features a discussion on selected results of research conducted among 42 migrants working as elderly caregivers. The purpose of the research was to obtain information about the scope of provided care, among others: running the home, medical help, accompanying an elderly person.The studies were of exploratory nature, based on individual in-depth interviews (IDI). The sampling of respondents was done using the snowball method and Respondent Driven Sampling (RDS). The data was analysed using the MAXQDA program.The research problem in the article was to determine the role of immigrants in elderly care. Whether they are complementary or substitutionary to the family. Based on the conducted research, it was established that the duties of elderly caregivers mostly included preparation of meals, cleaning, making laundry and shopping. Foreign workers were responsible for their hygiene, medical care, e.g. giving injections, rehabilitation. Their task also involved spending time with the seniors—taking them for a walk, making conversation, eating meals together or watching TV. It was determined that the scope of duties of an immigrant taking care of an elderly person was mainly affected by two factors: the elderly persons’ health status and work mode. Somewhat different duties applied to caregivers living with the elderly (so-called living-in) than persons renting an apartment and providing hourly care (so-called living-out).
W artykule omówiono wybrane wyniki badań przeprowadzonych wśród 42 cudzoziemców pracujących w Polsce w charakterze opiekunów osób starszych. Celem badania było m.in. uzyskanie wiedzy o zakresie świadczonej opieki odnoszącej się do: prowadzenia domu, pomocy medycznej, towarzyszenia osobie starszej.Prezentowane badania mają charakter eksploracyjny. Polegały na realizacji indywidualnych wywiadów pogłębionych (IDI). W dotarciu do respondentów wykorzystano metodę kuli śniegowej (snowball sampling) oraz Respondent Driven Sampling (RDS). Dane analizowano z wykorzystaniem programu MAXQDA.Problemem badawczym podjętym w artykule jest rola, jaką w opiece senioralnej sprawują imigranci – badania służyły m.in. ustaleniu, czy pełnią oni funkcje komplementarne czy substytucyjne wobec rodzin podopiecznych. Na podstawie przeprowadzonych badań ustalono, że do obowiązków opiekunów seniorów należy najczęściej: przygotowanie posiłków, sprzątanie, pranie oraz robienie zakupów. Pracownicy z zagranicy odpowiadają także za podawanie leków, robienie zastrzyków i rehabilitację podopiecznych. Większość badanych imigrantów przyznała, że ich zadaniem jest też spędzanie czasu z osobą, którą się opiekują – np. pójście na spacer, rozmowa, zjedzenie razem posiłku oraz wspólne oglądanie telewizji.Określono, że na zakres obowiązków imigranta opiekującego się seniorem wpływają przede wszystkim dwa czynniki: stan zdrowia podopiecznego oraz tryb świadczenia pracy. Nieco inne obowiązki miały osoby mieszkające ze swoim podopiecznym (tzw. living-in) niż osoby samodzielnie wynajmujące mieszkanie i świadczące pracę opiekuńczą na godziny (tzw. living-out).

One of the most common reasons for patients reporting to their general practitioners is pain. With increasing age, these conditions become more common in the population and are more severe. In addition to the cause of pain, the clinical picture also includes comorbidities and medications taken for this reason, as well as psychophysiological changes that occur in the body with increasing age. With the aging of society, the problem of older people's pain is becoming a growing problem. The article presents data on the characteristics of pain among seniors, the principles of its treatment and describes the most common problems associated with pain therapy.

Introduction: Family is the basic institution providing informal care to elderly and disabled individuals. Purpose: To present care situations encountered by persons providing home care to an elder, the caregivers’ needs for support and the factors determining their satisfaction with the care they provide. Materials and methods: The study was conducted at the Geriatric Ward of the Hospital of the Ministry of Interior in Bialystok among 105 home caregivers of the elderly. A diagnostics survey was based on an original survey questionnaire and the COPE Index. Functional capacity in the elderly (N=100) was determined based on the Barthel ADL (activities' in daily living) Index and the category of nursing care (category, I-III). The analysis covered also the pressure sore risk assessment (the Norton Scale). Results: The analysis based on the Barthel ADL Index showed that over 30.0% of the elderly individuals were given up to 40 points in the nursing assessment (seriously disabled). Almost all patients – 97.0% qualified to the 3rd category of nursing care. 64.0% of the elderly had an increased risk of pressure sore development (the Norton Scale). The problems connected with providing home care listed by caregivers included the lack of time, disability of the elder and locomotion difficulties. The analysis based on the Cope Index showed a positive relationship between the caregiver’s age (r=0.216), education (r=0.196), the distance from the elder’s home (r=0.216), the quality of received support (r=0.554), and the satisfaction in the care provided. Conclusions: Caregivers reported the need for different forms of support. Higher education, longer distance from home and the quality of received support were significantly associated with higher satisfaction with caregiving.

Considering the increased demographic aging and system changes in the welfare system in Poland, home care became one of the most important forms of taking care of disabled or elderly persons. Although the country is rooted in the idea of a nanny state, based on large institutions and providing minimum income, for a long time now family has been considered as the basic provider of informal care to the elderly. The aim of the present study is to give some insight into the resources of adult children providing care to their elderly, disabled parents. In order to acquire a comprehensive perception of the phenomenon, the authors chose an interpretavist paradigm and applied a phenomenographic method. 21 participants, aged 41–65, who were caregivers of their parents, took part in semi-structured interviews. Two main research problems were formulated: (1) How do the participants perceive their resources as caregivers of their elderly, disabled parents? (2) What kind of support related to the care of their parents do they consider essential? Results: The participants distinguish two main kinds of resources: internal (e.g. values, passions, feelings for parents, economic situation) and external (e.g. parents, family, friends, professionals, medical equipment and centers they have access to). The narratives of the participants also indicate a number of needs they experience. Implications for practice are discussed.

This work conducted a category product evaluation and user demand system through the bibliometrics method, product evaluation analysis, focus group interviews and questionnaires. Based on the system, we designed smart clothing from the aspects of the clothing structure, hardware, software program and intelligent terminal platform to meet the needs of disabled elderly people and their caregivers in multiple scenarios. According to the test results of the smart clothing, the average error of temperature and humidity monitoring is 0.20℃ and 2.88%RH. The time of putting on-taking off clothing in 6 representative daily scenarios was reduced by 51.67%. The daily body checking times, uncomfortable behaviours, and the anxiety of caregivers was decreased by 42.31%, 28.52% and 74.37%. Compared with ordinary clothing, six of the eight comfort performances are basically the same, and two are slightly worse.

Gminy jako najmniejsze jednostki samorządu terytorialnego w Polsce są odpowiedzialne za działania z zakresu pomocy społecznej. Rosnąca liczba osób starszych może wpływać na poziom wydatków związanych z zapewnieniem im opieki i stanowić coraz większy problem dla samorządu lokalnego, zwłaszcza dla gmin o niższym poziomie rozwoju społeczno‑ekonomicznego. W ostatnich latach zaobserwowano stosunkowo wysoki wzrost kwoty świadczeń z tytułu usług opiekuńczych i specjalistycznych usług opiekuńczych. Rodzi się zatem pytanie: „Czy można zaobserwować istotną zależność między udziałem osób starszych w populacji a stopniem obciążenia finansowego gmin z tytułu świadczenia usług opiekuńczych?”. Celem artykułu jest wskazanie tych województw, w których gminy są najbardziej obciążone świadczeniem usług opiekuńczych i sprawdzenie, czy ma to związek z obserwowaną w nich sytuacją demograficzną. Dane poddane analizie statystycznej pochodziły z ogólnodostępnych źródeł: sprawozdań Ministerstwa Rodziny, Pracy i Polityki Społecznej oraz opracowań Głównego Urzędu Statystycznego. Określono aktualną sytuację poszczególnych województw w zakresie starzenia się społeczeństwa oraz obciążenia świadczeniami na usługi opiekuńcze. Tylko w przypadku połowy województw stwierdzono, że poziom (niski, średni lub wysoki) obciążenia przedmiotowymi świadczeniami odpowiada takiemu samemu poziomowi udziału osób starszych w populacji, co świadczyć może o związku kwoty świadczeń na usługi opiekuńcze z sytuacją demograficzną danego województwa.
Local governments as the smallest units of territorial self‑government in Poland are responsible for providing care services. The growing number of the elderly can influence the expenditures level connected with ensuring proper care for them, and it may constitute a growing problem for local government, especially for districts with a lower level of socio‑economic development. In recent years, a quite high increase in benefits amount due to care services and specialist care services has been observed. Therefore, a question arises whether a significant dependence can be observed between the share of the elderly in the population and the degree of local governments’ financial burden due to providing care services. The aim of this paper is to indicate voivodeships where local governments bear the biggest burden of providing care services and to verify whether it is connected with their demographic situation. The data which were subject to the static analysis conducted came from public sources: the report of the Ministry of Family, Labour and Social Policy of the Republic of Poland and elaborations of the Central Statistic Office. The situation of particular voivodeships has been defined in the scope of society ageing and burdening local government with care services. Only in the case of half of voivodeships it has been proven that the level (low, medium or high) of burden of the said services corresponds to the same level of the number of the elderly in their population, which can confirm that the amount of care services is connected with a demographic situation of a given voivodeship.
