There is no doubt that vitamin D plays a crucial role in the maintenance of musculoskeletal system. But the function of this ancient molecule presumably ranges far beyond hormone-like regulation, as it could be generated by simple unicellular organisms. First, we are going to discuss the role of vitamin D as a global regulator of homeostasis from a historical perspective, but later we will focus on current views and its relevance to human physiology and pathology. Three milestones are defining the impact of vitamin D on science and humanity. Firstly, discovery that vitamin D is the cure for rickets, brought us supplementation programs and rapid irradiation of this devastating disease. Secondly, detail description of photoproduction of vitamin D, its subsequent metabolism and interaction with vitamin D receptor VDR, provided mechanistic background for future discoveries. Finally, recent large epidemiological studies provided indirect, but strong evidence that optimal level of vitamin D in serum has beneficial effects on our health and protects us from multiple diseases, including cancer. Furthermore, existence of alternative pathways of vitamin D metabolism and multiple intracellular targets broadens our understanding of its physiological activities and offers new and very promising tools for prophylactics and treatment of many diseases of civilization. Although vitamin D (and its derivatives) should not be regarded as a cure-all for every human disease, its beneficial effects on the human health have to be taken under consideration.

Opis:
Homocysteine (Hcy), a sulfur amino acid, is the only direct precursor for l-methionine synthesis through a reaction that requires vitamin B12, representing a connection with "one-carbon" units metabolism. Hcy catabolism requires vitamin B6 and as a consequence, alteration in folic acid and B vitamins status impairs Hcy biotransformation. Numerous studies have indicated that Hcy is an independent risk factor for cardio- and cerebrovascular diseases. In the last decade, several clinical trials have investigated the possible correlation between the use of folic acid and vitamins B6 and B12 for lowering Hcy plasma concentration and the reduced risk of stroke or its recurrence. This review is aimed to present some aspects of Hcy biochemistry, as well as the mechanisms through which it exerts the toxic effects on the vascular endothelium. We also discuss the results of some of the clinical trials developed to investigate the beneficial effects of vitamin therapy in the prevention and management of stroke.

Opis:
Long-term exposure to cadmium (Cd) leads to the development of a number of conditions associated with liver and kidney damage, reproductive and cardiovascular disorders, in addition to visual impairment, blindness and hearing loss, among others. Cadmium has been classified as a human carcinogen by the International Agency for Research on Cancer. The toxicity of Cd is related to its pro-oxidant properties and the associated increase in oxidative stress. Antioxidant ingredients may be helpful in preventing the adverse effects of Cd. The effect of well-known antioxidant vitamins (E, C, A and β-carotene) in the prevention of Cd-induced toxicity is presented in this study. Numerous studies in animal models have shown that the effects of vitamins: E, C, A, and β-carotene were effective in reducing Cd concentrations in organs and tissues and reduced Cd-induced changes in liver, kidney, and reproductive, circulatory, nervous, immune, and respiratory systems. In contrast, the limited number of human studies does not allow to accurately determine the role of these nutrients in reducing Cd-induced toxicity, indicating the need for further studies clarifying the role of antioxidant vitamins in reducing Cd-induced toxicity. However, it seems reasonable to promote the consumption of natural food products that are sources of antioxidant vitamins in groups of people with occupational and environmental exposure to Cd.

Introduction: Vitamin D deficiency is a worldwide public health problem. The objective of this survey was to assess the undergraduate students’ vitamin D status and knowledge about this vitamin. Materials and methods: An online multi-choice survey was designed and launched in Northern Poland (Gdańsk region). The first part of the survey assessed diet, supplementation, UV radiation exposure (UVE) and general health of respondents. The second part was a vitamin D knowledge test (vitamin D optimal level, deficiency-related diseases). 1766 student volunteers responded to the survey: 369 male and 1397 female. Data was divided according to the respondents’ sex and university affiliation. Appropriate parametric or non-parametric statistical tests were used with statistical significance set at p<0.05. Results: Regular consumption of vitamin D-rich food was high, except for fish (only 18; 22%). High number of participants did not declare any type of supplementation (43; 44%) and only occasionally were exposed to UV (77%; 80%). The most frequently recognized disease linked to vitamin D was osteoporosis. Medical University students obtained higher test scores (4,55), however this did not correlate with healthy vitamin D habits. Conclusion: Undergraduate medical and non-medical students have unsatisfactory vitamin D status and poor understanding of its function and impact on health, which implies the need for changes in the educational program.

Vitamin D plays important, pleiotropic role in the maintenance of global homeostasis. Its influence goes far beyond the regulation of calcium and phosphorus balance, as diverse activities of vitamin D and its natural metabolites assure proper functioning of major human organs, including skin. Recently, we reviewed the current understanding of vitamin D impact on human health from historical perspective (Wierzbicka et al. (2014) The renaissance of vitamin D. Acta Biochim Pol 61: 679-686). This article focuses on its functions in the skin. The skin and its appendages, creates a platform connecting and protecting internal organs against, usually harmful, external environment. It uppermost layer - epidermis in order to maintain a protective barrier undergoes a constant exchange of cornified keratinocytes layer. Its disturbance leads to development of serious skin disorders including psoriasis, vitiligo, atopic dermatitis and skin cancer. All of those dermatopathologies have a huge impact on modern societies, affecting not only the physical, but also mental state of patients as well as their social status. Furthermore, multiple human systemic diseases (autoimmune, blood and digestive diseases) have skin manifestation, thus "condition of the skin" often reflects the condition and pathological changes within the internal organs. In humans, the skin is the natural source of vitamin D, which is produced locally from 7-dehydrocholesterol in photoreaction induced by ultraviolet B (UVB) radiation from the sun. It is also well established, that the process of proliferation and differentiation of keratinocytes is tightly regulated by calcium and the active form of vitamin D (1,25(OH)2D3). Thus, the skin physiology is inseparably connected with vitamin D production and activity. Unfortunately, UVB, which is required for vitamin D production, is also known as the main cause of a skin cancer, including melanoma. Here, we are going to review benefits of vitamin D and its analogues in the maintenance of epidermal barrier and its potential use in the treatment of common skin diseases.

Chromium (Cr) is a very common element. It occurs in 2 oxidation states, Cr(III) and Cr(VI). Although Cr(III) is not considered an element essential for mammals, it raises lots of controversy due to its role in the body. While Cr(III) action should be considered an effect of pharmacological action, Cr(VI) is included in the first group of carcinogens for humans. Moreover, it induces numerous pathological changes in the respiratory, urinary, reproductive and digestive systems. In addition, Cr(VI) is used in many industry branches, causing millions of workers all over the world to be exposed to Cr(VI) compounds. A considerable number of the occupationally exposed individuals are in favor of a deep analysis of the mechanisms of Cr(VI) action and a search for a way to reduce its negative impact on the human body. Numerous reactive oxygen species inducing oxidative stress and causing various damage are produced during Cr(VI) reduction in the cells. A good balance between antioxidants and pro-oxidants can reduce Cr(VI)-induced damage. The influence of vitamins and microelements on the adverse Cr(VI) effects has no systematic research results summary. Therefore, this work focuses on the role of dietary antioxidants such as vitamins and microelements in the prevention of Cr(VI) adverse health effects. Numerous studies have revealed a protective influence of vitamins (mainly vitamins E and C) as well as microelements (especially selenium) on the reduction of Cr(VI)-induced adverse changes. A potential protective effect of these ingredients may be useful in occupational groups that are particularly exposed to Cr(VI). However, more research in this area is required.

Background. Antioxidant vitamins such as alpha-tocopherol (vitamin E), ascorbic acid (vitamin C), and beta-carotene (provitamin A), play a significant role in protecting the body from an excess of free radicals. The vitamin content of the diet is thus very important. Objective. To assess whether the dietary intake of vitamins A, C and E is adequate in a selected group of adolescents living in Warsaw. Material and methods. The mean vitamin consumption was estimated using three-day dietary records whilst vitamin contents of selected foodstuffs were obtained from Food Composition Tables. Both were related to Polish nutrition standards applied to adolescents; i.e. EAR (Estimated Average Requirement) for vitamins A and C and AI (Adequate Intake) for vitamin E. Results. Dietary vitamin C in adolescents and vitamin E in girls were around 18-25% lower than that recommended. Diets that were deficient in vitamins C and E were recorded in 47% to 67% adolescents. Conclusions. It is therefore necessary to increase the consumption of vegetables and fruit which provide a valuable dietary source for these vitamins.
Wprowadzenie. Witaminy antyoksydacyjne – alfa-tokoferol (witamina E), kwas askorbinowy (witamina C) i beta-karoten (prowitamina A) pełnią istotną rolę w ochronie organizmu przed wolnymi rodnikami, dlatego odpowiednia zawartość tych witamin w diecie ma istotne znaczenie. Cel. Celem badań była ocena poziomu spożycia witamin A, C i E z produktami spożywczymi w wybranej grupie młodzieży zamieszkałej na terenie Warszawy. Materiał i metody. Średnie spożycie witamin oszacowano na podstawie badań przeprowadzonych metodą trzydniowego bieżącego notowania spożycia produktów spożywczych oraz przy zastosowaniu tabel wartości odżywczej produktów spożywczych i odniesiono do polskich norm dla młodzieży ustalonych na poziomie średniego zapotrzebowania (EAR) dla witamin A i C oraz na poziomie wystarczającego spożycia (AI) dla witaminy E. Wyniki. Zawartość witaminy C w diecie młodzieży oraz witaminy E w diecie dziewcząt była o około 18-25% niższa od zalecanej. Diety ubogie w witaminę C i E spożywało około 47-67% młodych osób. Wnioski. Istnieje potrzeba zwiększenia spożycia warzyw i owoców, jako ważnych źródeł tych witamin w diecie.

Opis:
Vitamin D is a compound responsible for maintaining mineral homeostasis. It protects against calcium and phosphate deficiency through the effects on the intestine, kidney, parathyroid gland and bone. All mechanisms that help maintain mineral homeostasis of the body are regulated by the vitamin D hormonal form - calcitriol. Synthesis of vitamin D starts in the skin as a non-enzymatic process, which begins during exposure to sunlight, when the absorption of ultraviolet B (UVB) radiation results in convertion of 7-dehydrocholesterol, a metabolite of cholesterol that is stored in the skin, to precholecalciferol (previtamin-D3) that is immediately converted into cholecalciferol (vitamin D3). After the skin synthesis cholecalciferol is transported to the liver where it undergoes hydroxylation, what results in formation of calcidiol (25(OH) D3). The second metabolic process takes place in the kidney, where calcidiol undergoes hydroxylation at the C-1 position to the hormonal, the most active metabolite - 1,25-dihydroxyvitamin D (calcitriol). Vitamin D deficiency may result in bone diseases, such as rickets in children and osteomalacia and osteoporosis in adults. Symptoms of osteomalacia affect mainly the skeletal system and are similar to that observed in rickets. It concerns thoracic kyphosis, pelvis deformities and also the varus knee. Osteoporosis is another condition that is related to abnormalities of mineral homeostasis. It is characterized by the progressive loss of bone mass, impaired bone microarchitecture, and consequently increased fragility and susceptibility to fracture. For the last several years other, non-classic actions of vitamin D3 have been discussed. It was engendered by the discovery of vitamin D3 receptor (VDR) in the most of body tissues and cells. Hence, there are many hypotheses which suggest the inverse relationship between vitamin D status and various diseases, such as cancer, autoimmune diseases, diabetes mellitus and others.
Witamina D jest związkiem chemicznym odpowiedzialnym za utrzymanie homeostazy mineralnej organizmu. Poprzez wpływ na jelita, nerki, przytarczyce i kości zapobiega niedoborowi wapnia i fosforanów. Aktywną formą witaminy D3 o właściwościach hormonalnych jest kalcytriol, który odpowiada za utrzymanie homeostazy mineralnej organizmu. Pierwszy etap syntezy witaminy D zachodzi w skórze, pod wpływem ekspozycji na światło słoneczne (UVB). Polega on na nieenzymatycznej przemianie 7-dehydrocholesterolu do prowitaminy D (pre-D3), która natychmiast ulega przekształceniu do cholekalcyferolu (witaminy D3). Wyprodukowana w skórze witamina D3 jest następnie transportowana do wątroby, gdzie ulega hydroksylacji, w wyniku której powstaje kalcydiol (25(OH)D3). Drugi proces metaboliczny zachodzi w nerkach, gdzie kalcydiol ulega hydroksylacji w pozycji C-1 do hormonalnie najbardziej aktywnego metabolitu witaminy D - 1,25-dihydroksywitaminy D (kalcytriol). Niedobór witaminy D może prowadzić do chorób kości, takich jak krzywica u dzieci oraz osteomalacja i osteoporoza u osób dorosłych. Objawy osteomalacji dotyczą głównie układu kostnego i są zbliżone do tych obserwowanych w krzywicy. Są to m.in.: kifoza piersiowa, deformacje miednicy i szpotawość kolan. Osteoporoza to choroba, która także związane jest z zaburzeniem homeostazy mineralnej. Charakteryzuje się postępującą utratą masy kostnej, uszkodzeniami mikroarchitektury kości, i w konsekwencji zwiększoną ich kruchością i podatnością na złamania. Na przestrzeni ostatnich lat pojawiły się doniesienia dotyczące innych, nieklasycznych działań witaminy D3. Stwierdzono obecność receptora witaminy D (VDR) w wielu tkankach organizmu, które nie biorą udziału w utrzymaniu homeostazy wapniowo-fosforanowej. Świadczy to o wielokierunkowym działaniu tego związku. Pojawiło się wiele hipotez sugerujących związek między niedoborem witaminy D a występowaniem wielu różnych chorób, takich jak: nowotwory, choroby autoimmunologiczne, cukrzyca i inne.

Abnormal vitamin K status was documented in patients with chronic kidney diseases (CKD), especially those undergoing hemodialysis. The data related to patients undergoing peritoneal dialysis (PD) are contradictory. Therefore, in the present study we aimed to evaluate vitamin K status in patients with CKD who are treated with continuous ambulatory PD. Twenty-eight patients entered into the study. Dialysis vintage ranged from 3 to 89 months. Vitamin K status was assessed in all subjects using undercarboxylated prothrombin measurement (PIVKA-II). In addition, total protein and albumin levels, total cholesterol, LDL cholesterol, triglyceride, calcium, urea and creatinine concentrations were determined. PIVKA-II concentrations were abnormal in 13 (46.4 %) subjects. BMI values, both total and LDL cholesterol concentrations were significantly higher in patients with than those without vitamin K deficiency. Moreover, PIVKA II levels correlated with BMI values (r = 0.441, p < 0.019), LDL cholesterol (r = 0.434, p < 0.021) and creatinine (r = 0.406, p < 0.032) concentrations. However, through the use of logistic regression analysis and multiple regression analysis, no clinical factor was documented to be the independent risk factor of vitamin K deficiency. In conclusion, vitamin K deficiency is a frequent condition in peritoneally dialyzed patients. Assessment of vitamin K status should become a standard procedure in this group of patients.

Introduction. Reports continue to show that a significant association exists between serum vitamin D level and metabolic syndrome (MS)-associated inflammation. However, information on the serum levels of vitamin D and alterations in inflammation in different vitamin D status is presently lacking. Aim. To determine the serum levels of vitamin D and TNF-α, and assess their possible relationship with gender in individuals with MS. Material and methods. Sixty adults with MS and 40 controls were enrolled into this case-control study. Serum vitamin D and TNF-α levels were measured and participants stratified into different vitamin D status. Results. None of the participants had vitamin D deficiency and the mean vitamin D level was similar in MS compared with the controls. However, TNF-α level was significantly higher in MS compared with the controls. Serum vitamin D level had significant inverse correlation with serum TNF-α level in MS. Also vitamin D level was significantly lower while TNF-α level was significantly higher in female-MS compared with the male-MS. Conclusion. Adults with MS have elevated TNF-α level which appears to be associated with the serum level of vitamin D. Also, females with MS have low vitamin D level and this may exacerbate the MS-associated inflammation in them.

Background. The health benefits arising from antioxidant vitamins A, C and E are well recognised and their recommended dietary intake for the general population have been established. However, there is still a need for assessing antioxidant vitamin intake in different population groups. Objective. To assess intake of antioxidant vitamins: A, C, E and β-carotene, and to identify their major sources in the diets of healthy subjects. Material and methods. The study group consisted of 182 adults; both men and women from polish population. Antioxidant vitamin dietary intake was assessed by individual 3-day records. Data were analysed using updated “Polish Food Composition Tables” and ‘Dieta 5’ Software. Results. The average daily intake of antioxidant vitamins was: 1076 mg for vitamin A (including 46% of retinol and 55% of β-carotene), 107 mg for vitamin C and 9 mg for vitamin E. Higher dietary intake of these vitamins was observed in men compared to women. Conclusions. The average intake of antioxidant vitamins was found to be in recommended range, however, significant differences were observed between the lowest and the highest intake. They were related to differences in the consumption of food products recognized as major sources of vitamins A, C, E and β-carotene in study population.
Wprowadzenie. W dobie nieustannego narażenia organizmu na działanie wolnych rodników, prawidłowe działanie układu antyoksydacyjnego, a tym samym dostarczanie wraz z dietą witamin oksydacyjnych w ilościach odpowiadających zapotrzebowaniu, odgrywa kluczową rolę w zapobieganiu licznych chorób cywilizacyjnych. Cel badań. Celem pracy była ocena spożycia witamin antyoksydacyjnych, ze szczególnym uwzględnieniem witamin A, C i E oraz β-karotenu, a także wskazanie ich źródeł w diecie badanych. Materiał i metody. Badania przeprowadzono w grupie 182 dorosłych osób. Oceny spożycia witamin dokonano w oparciu o trzydniowe zapisy spożycia. Dane analizowano przy użyciu programu komputerowego „Dieta 5” oraz Tabel składu i wartości odżywczej żywności. Wyniki. Średnie dzienne spożycie witamin antyoksydacyjnych wyniosło: dla witaminy A - 1076 μg, (z czego 46% z retinolu a 55% z β-karotenu), dla witaminy C - 107 mg, oraz dla witaminy E - 9 mg, przy wyższych poziomach spożycia u mężczyzn niż u kobiet. Wnioski. Średnie spożycie witamin antyoksydacyjnych odpowiadało zaleceniom, jednak sszczegółowa analiza wyników wykazała istnienie dużych rozbieżności między niskim a wysokim poziomem spożycia tych witamin, związanych z istotnymi różnicami w zakresie konsumpcji produktów będących głównymi źródłami witamin A, C i E oraz β-karotenu.

Objectives Aim of this project is determination of the correlation between the level of vitamin D in blood serum and duration of pregnancy in population in central Poland. Material and Methods 25-Hydroxyvitamin D (25(OH)D) level was determined in blood serum, using enzyme-linked immunosorbent assay (ELISA). Standardized history of each patient was recorded. The history included: general medical history, data regarding the course of pregnancy and information about health-related behavior that could influence vitamin D concentration. Two hundred-and-one Caucasian women at childbirth were qualified into the study. The study group was divided into 2 parts: 100 patients who had a spontaneous premature birth and 101 patients who had birth at full term. Results Vitamin D deficiency (< 30 ng/ml) was very common for both groups (69.6% of patients in the premature group and 72% – in the control group). Patients who had a premature birth had severe vitamin D deficiency (less than 10 ng/ml) more often than in the control group (34% vs. 14.2%, p = 0.001). Severe vitamin D deficiency increased the risk of premature birth but the association was not statistically significant in the multivariate regression model (odds ratio (OR) = 2.47, 95% confidence interval (CI): 0.86–7.15, p = 0.094). Conclusions Severe vitamin D deficiency (< 10 ng/ml) may be the factor increasing the risk of preterm birth. Int J Occup Med Environ Health 2017;30(6):933–941

ObjectivesContrary to popular opinion on the preventive properties of vitamin D, results of previous studies have been inconclusive. The aim of this research was to evaluate the associations between the intake of vitamin D and metabolic abnormalities in a representative sample of Polish adults.Material and MethodsWithin the framework of the Multi-Center National Population Health Examination Survey (referred to as WOBASZ), a random sample of 2381 adult residents of Poland (53.8% of whom were women) was examined. All the study subjects were extensively reviewed, including 24-h dietary recall. The intake of vitamin D was assessed on the basis of dietary and supplements reviews. Metabolic abnormalities were evaluated using measurements of waist circumference (WC), blood pressure (BP), serum triglycerides (TG), high-density lipoprotein cholesterol (HDL-C) and fasting glycemia. Metabolic syndrome was defined according to the International Diabetes Federation.ResultsOf all the study participants, about 4.4 % of women and 2.6% of men declared a regular supplementation of vitamin D. Among women, a significant inverse correlation between vitamin D supplementation and the mean systolic BP was found (p < 0.01). A more substantial relationship was noted after dividing the study subjects according to their body mass. Among obese men, there was a significant inverse relationship between vitamin D intake and the mean systolic BP (p < 0.01) and diastolic BP (p < 0.05), as well as a positive correlation with HDL-C (p < 0.05). Among obese women, a negative correlation was found between vitamin D supplementation and the mean systolic BP (p < 0.01) and diastolic BP (p < 0.05), and a positive correlation with TG (p < 0.05). Among non-obese male subjects, a negative correlation between vitamin D intake and WC was observed.ConclusionsThe obtained findings suggest that the correlation between vitamin D intake and metabolic abnormalities may depend on the obesity status. A higher vitamin D intake may reduce BP and increase HDL-C in obese subjects. The positive relationship between vitamin D intake and TG concentration in women needs further investigation.

Introduction and objective. Deficits of vitamin resources constitute a significant public health problem, especially among the elderly population. The aim of the research was to determine the level of vitamin 25 (OH) D and vitamins from group B in the chronically ill elderly in domiciliary care, depending on functional capacity and coexisting diseases. Materials and method. The pilot study included 137 patients staying in long-term domiciliary care. Samples of the participants’ venous blood was obtained for laboratory tests. Centrifuged serum was used to determine the level of the following biochemical parameters: vitamin 25 (OH)D, B12, folic acid and total protein, albumin, triglycerides, total cholesterol and HDL cholesterol. Assessment of the functional status of patients was made by using the Barthel scale. Results. More than 3⁄4 of the patients with functional deficit (according to Barthel’s score 0–85 points) were deficient in vitamin 25 (OH)D, while folic acid values were below the reference values in more than half of the patients. Respondents with lower functional efficiency were characterised by a reduced average value of vitamin 25 (OH)D and folic acid. Conclusions. The studied group of the chronically ill elderly was characterised by a deficiency of vitamin D3 and folic acid. Subjects with a functional impairment deficit show a reduced mean value of vitamin 25 (OH)D and folic acid in the blood serum, compared to the group of patients with higher mobility.

The aim of the study was to evaluate the stability of vitamin C and beta-carotene in the freeze-drying process compared to other thermal drying methods. Tomato puree was selected as a model product for testing the stability of vitamin C and vitamin A in drying processes, due to its properties, such as: appropriate consistency, which was favorable for the analyzes performed, and the presence of the above-mentioned vitamins. Model studies were performed on the stability of vitamin C and betacarotene under the conditions of air-drying at 105°C, drying under reduced pressure at 70°C and freeze-drying on a tomato puree matrix. Results proved that freeze-drying is superior to other drying methods with the lowest losses of both vitamin C (losses of 4%) and beta-carotene (losses of 25%) in tomato purée. In the case of drying at 70°C under reduced pressure, the loss of beta-carotene is 55%, and vitamin C - 78%. In the case of air-drying at 105°C, vitamin C is 100% degraded, and the loss of beta-carotene is 91%. Moreover, it can be concluded that vitamin C in a higher dose has a protective effect on carotenoids.
