Hazard is a safety condition that could cause or be a part of unsafe aircraft operation. It also has an impact on aviation safety related to aircraft systems as well as services. Safety risk is a part of so-called safety management and is the predicted probability and severity of the outcomes of a hazard. Aircraft utilized by airlines and cargo operators which have MTOM >5700 kg are defined by authors as a large aircraft and have to be treated in their systems reliability analysis separately from small aircraft. Despite the fact that both types of aircraft categories events are included in the same databases - European Coordination Centre for Aviation Incident Reporting Systems (ECCAIRS) and Aircraft Continuing Airworthiness Monitoring (ACAM), due to different predominant systems failures could not be considered in the same way. The authors have performed processing of the data contained in available databases analysing large aircraft airframe systems faults, which were assigned to the specific ATA chapters. The most frequently occurring defects of the aircraft systems were identified. The goal of this article is to present a method of the current reliability of large aircraft systems assessment. Based on it, the authors are proposing a way of safety risk estimation and prediction of the safety levels for the next two years. The results of this analysis may support the decisions of supervisory authorities in the areas where security threats are most important. They can also help aircraft operators with identification of the aircraft systems, which require special attention.

Let us consider a level crossing as a technical object with a certain level of safety. In the paper we propose a new method based on the theory of geometric programming. It allows to solve the problem of minimizing common risk of the object safety violation in a simple analytic form due to the choice of object protection parameter set. There is given a numerical example illustrating the calculated scheme of the method.

Safety level analysis is included in the paper in context of SWIM system in air traffic management. The example is given. It examines the limitations and drawbacks of current ICT systems used for air traffic management. Analyzing the development of communication systems for the management of general air traffic, it can be concluded that the development of the terrestrial segment of the exchange of information between the parties relating to the air traffic will fluctuate towards a solution based on a service-oriented architecture SOA. This architecture will be the basis for the implementation of the concept of an information exchange system SWIM.

In this paper, we introduce a new risk evaluation model for evaluating the navigation safety zone for an entire ship route. This model considers a new algorithm to determine the navigational safety zone in real-time, and also takes the navigation officers’ perception while navigating a ship into consideration. The risk quantification has been developed using a questionnaire and incorporated into the new model. A simulation was carried out for the Osaka bay area in order to verify the usefulness of the proposed model. A new approach was employed to monitor the level of navigation safety along a ship route. The entire ship route is divided into small sections as a gridded matrix. The level of navigation safety can be quantified by means of a safety index on the basis of the ship’s navigation data within a specified distance range. The results show that the comparison between risks identified for different sections across the entire ship route is easy, which helps determine the navigational safety zone quickly. This model is expected to be able to serve as a new tool for managing safety throughout an entire ship route area in real-time in order to support the port safety authority or vessel traffic service center.

This article addresses some functional safety assessment procedures with cybersecurity aspects in critical industrial installations with regard to the functional safety requirements specified in standards IEC 61508 and IEC 61511. The functional safety management includes hazard identification, risk analysis and assessment, specification of overall safety requirements and definition of safety functions. Based on the risk assessment results, the safety integrity level (SIL) is determined for consecutive safety functions. These functions are implemented within the industrial control system (ICS) and/or the distributed control system (DCS) that consists of the basic process control system (BPCS) and/or the safety instrumented system (SIS). The determination of the required SIL related to the required risk mitigation is based on the semi-quantitative evaluation method. Verification of the SIL for the considered architectures o fthe BPCS and/or the SIS is supported by probabilistic models with appropriate data and model parameters including cybersecurity related aspects. The proposed approach is illustrated on the example of critical industrial installations.

A Safety Integrity Level (SIL) is a measure of performance required for a safety instrumented function. The IEC 61508/61511 standards define four safety integrity levels, SIL1 to SIL4, where SIL4 is the level with the most stringent requirements. For each safety integrity level there are many design requirements, including requirements for the probability of failure on demand (PFD). Verification of the required failure probabilisty is usually based on a quantitative analysis. In this paper we argue that such an approach is better replaced by a semi-quantitative approach. The approach acknowledges that the PFD requirement for a safety function cannot be adequately verified only by reference to an assigned probability number. There is a need for seeing beyond the probability number. The key aspect to include is related to uncertainty. Such an aspect is of ten ignored in verification of a safety integrity level. The offshore oil and gas industry is the starting point, but the discussion is to large extent general.

The aim of this article is to identify problems of the risk assessment of the electric / electronic / programmable electronic (E/E/PE) systems concentrating on the functional safety and security aspects. These aspects should be considered in an integrated way in the system life cycle. The role of functional safety solutions is effective reducing the risk from unacceptable level. The risk is defined as a combination of the probability of occurrence of harm and the severity of that harm. Security is concerned with the protection of assets including the E/E/PE systems or safety instrumented systems (SISs) from potential threats including cyber attacks. This article deals with current challenges and methodological issues of integrating the functional safety and security aspects of the programmable systems’ operation for the control and protection of hazardous industrial systems.

In the article the ProSIL software for computer aided functional safety management is presented. The software consists of three modules for the determination of the required SIL level (ProSILen) and verification of the SIL level (ProSILver). In the ProSIL the calibrated knowledge-based risk graph method for determining the required safety integrity level (SIL) of the safety functions identified in hazard analysis is implemented. The SILs are then verified for safety-related control and protection systems implementing relevant functions with regard to random failures and potential systematic failures. The assessment methods are compatible with international standards IEC 61508 and IEC 61511. Some current challenges and methodological issues concerning knowledge-based functional safety management in life cycle are also discussed.

The functional safety management in life cycle is a complex process starting with identifying hazards and defining safety-related functions (SRFs) with regard to the results of risk assessment oriented at determining the safety integrity level of consecutive functions. Another element of such process is a verification of required SIL for considered architectures of safety-related system that implements given safety function. Due to complexity of the problem, to overcome difficulties in safety-related decision making often under considerable uncertainties, usually without taking into account security aspects, we propose to apply the RIDM methodology oriented on functional safety management of programmable control and protection systems in life cycle taking into some more important risk-related factors identified.

A security level of distributed control and protection system may have a significant impact on the results of functional safety analysis. However, the issue of integrating the safety and security aspects is difficult and usually is neglected during the functional safety analysis. This article presents a method of functional safety analysis which takes into consideration a concept of integrating these two aspects. It is based on proposed classification of communication channels used in the computer system / network and the scope of such system distribution. The functional safety analysis is to be performed at every stage of system lifecycle, but one of the most important parts is defining required safety functions and determining the safety integrity level for them. The integration concept might be taken into account at this stage. The basis of a method proposed is the assumption that the security level is considered as a risk parameter in graphs of functional safety analyses.

Opis:
Railway traffic control systems require extremely high level of operational safety. Due to very high costs of safety failure, this field is subject to the regulation by numerous norms, which describe formal methods of safety level calculation (denoted as Safety Integrity Level - SIL). Such an analysis is tedious and time consuming, especially in case of complex systems. The paper proposes application of UML modeling approach to perform joint analysis of the system architecture and its operation. The approach also uses the Fault Tree Analysis (FTA) and can be used to identify the weakest links in the whole system. The method allows to quickly introduce changes in system architecture or parameters and evaluate the impact on the safety. The proposed approach was successfully applied to the real case of a railway system.
Systemy SRK wymagają szczególnie wysokiego poziomu bezpieczeństwa eksploatacji. Ze względu na bardzo wysokie koszty awarii, dziedzina ta jest przedmiotem podlegającym regulacjom wielu norm opisujących formalne metody obliczania poziomu bezpieczeństwa (oznaczony, jako poziom nienaruszalności bezpieczeństwa - SIL). Analiza prowadząca do wyznaczenia poziomu SIL jest trudna i czasochłonna, zwłaszcza w wypadku bardzo skomplikowanych systemów. Niniejsza praca proponuje aplikację podejścia z wykorzystaniem modelowania UML do przeprowadzania analizy architektury systemu i jego działania. Podejście to korzysta również z analizy drzewa błędów FTA i może być użyte do identyfikacji najsłabszych elementów systemu. Metoda ta pozwala również na szybkie wprowadzanie zmian w architekturze lub parametrach systemu, oraz pozwala obliczyć ich wpływ na bezpieczeństwo. Zaproponowane rozwiązanie zostało z powodzeniem zastosowane w prawdziwym systemie kolejowym.

Authors describe the possibilities of the industrial Ethernet usage in safety-critical applications as a component of safety-related control systems. The main part of the paper summarizes the safety requirements of industrial Ethernet ProfiNet and Ethernet/IP based on ProfiSafe and CIPSafety safety profiles oriented towards identification of communication errors and recommendations of protective mechanisms which are applied in communication protocols. In the practical part the results of performed cryptanalytic attacks on the wireless communication protocol based on the IEEE 802.11 standard are mentioned.
