Introduction. Oroantral communication (OAC) between the maxillary sinus and the oral cavity is an infrequent post-surgical complication occurring most commonly after extraction of posterior maxillary teeth. Aim. To present the characteristics of OAC and predisposing factors as well as evaluate postoperative pharmacological therapy and complications in patients with an OAC. Material and methods. In this retrospective study, medical records of 63 patients with diagnosed OAC between 2011 and 2018 were analyzed. Results. The most frequent causes for tooth extraction leading to an OAC were periodontitis (n=34; 54%), carious destruction of the tooth (n=14; 22.2%), and tooth impaction (n=10; 16%). First molars (n=28; 44.4%), second molars (n=14; 22.2%) and third molars (n= 13; 20.6%) were the most frequently related teeth to OACs. The majority of OACs appeared in the fourth (n=22; 35%) and third (n=20; 31.7%) decades of life. Conclusion. OACs are rarely seen on an everyday basis by general practitioners; however, if left untreated, they may lead to further serious complications. Proper postoperative precautions must be taken in order to prevent further complications, and thus the evaluation of predisposing factors is of great importance.

Introduction. The prophylactic extraction of asymptomatic wisdom teeth is defined as the surgical removal of wisdom teeth in the absence of local disease. Early extraction of asymptomatic third molars is considered beneficial to patients to prevent the risk of future pathology, and to minimize operative and postoperative risks. The second concept is watchful monitoring of asymptomatic wisdom teeth, adhering to specific indicators for their extraction. Aim. The aim of this paper is to present and evaluate the indications and effects of prophylactic extraction of asymptomatic impacted third molars in adolescents and adults, compared with their retention and watchful monitoring. Material and methods. This study is based on analysis of literature. Conclusion. There exist clear indications for the extraction of third molars which are associated with pathology. Prophylactic extractions of asymptomatic impacted third molars should be performed only before 20 years of age. In older age, asymptomatic third molars should be retained and watchfully monitored, and removed only in cases of evident clinical or radiological symptoms

Opis:
Ehlers-Danlos syndrome (EDS) is a heterogeneous group of heritable connective tissue disorders. The actual classification recognizes 13 subtypes of the syndrome. The classic type is the most common and in 90% of cases is caused by mutations in the COL5A1 and COL5A2 genes responsible for the synthesis of type V collagen. In other cases mutations in the COL1A1 gene encoding type I collagen are responsible for this disease. Classic EDS is characterized by skin hyperextensibility, poor wound healing and joint hypermobility. The clinical manifestation is dominated by symptoms from the musculoskeletal system, cardiovascular system, gastrointestinal tract and nervous system. Patients with Ehlers-Danlos syndrome also have a higher prevalence of obstructive sleep apnoea syndrome. The article presents the case of a 3-month-old boy with a classic type of the syndrome with tonic-clonic seizures, apnoea, fever of unknown cause, delayed psychomotor development with hypotonia, gastroesophageal reflux, fragility of blood vessels and easy bruising.
Zespół Ehlersa i Danlosa to grupa chorób uwarunkowanych genetycznie, w których dochodzi do syntezy nieprawidłowych form kolagenu. Wyróżniamy 13 postaci klinicznych zespołu, z których postać klasyczna należy do najczęstszych i w 90% spowodowana jest mutacjami w genach COL5A1 i COL5A2 odpowiedzialnych za syntezę prokolagenu typu V. W pozostałych przypadkach odpowiedzialną za wystąpienie tej choroby jest mutacja w genie COL1A1 kodującym prokolagen typu I. Postać klasyczną zespołu Ehlersa i Danlosa charakteryzuje nadmierna ruchomość stawów i rozciągliwość skóry oraz słabe gojenie się ran. W obrazie klinicznym dominują objawy ze strony układu mięśniowo-szkieletowego, sercowo-naczyniowego, przewodu pokarmowego oraz układu nerwowego. U pacjentów z zespołem Ehlersa i Danlosa stwierdzono również zwiększoną częstość występowania zespołu obturacyjnego bezdechu. Artykuł prezentuje przypadek 3-miesięcznego chłopca z klasyczną postać zespołu, u którego obserwowano napady toniczno- kloniczne, bezdechy ze spadkami saturacji, gorączki o nieustalonej etiologii, opóźnienie rozwoju ruchowego z uogólnioną wiotkością, refluks żołądkowo-przełykowy, kruchość naczyń krwionośnych i skłonność do siniaczeń.

Aim: The goal of this work was to present our experiences and results of treatment of gastric tumors using the per oral specimen extraction (POSE) technique. Material and methods: A retrospective analysis a group of patients treated with laparoscopic stomach wedge resection of gastrointestinal stromal tumor (GIST). During that time 50 patients underwent laparoscopy due to the suspicion of GIST. In 12 patients resected material was removed endoscopically per os (POSE). In the remaining 38 subjects it was evacuated through minilaparotomy. Mean age of patients treated using POSE technique was 65.6 years (48-81 years). There were 9 women and 3 men in this group Results: Mean time of the POSE procedure was 92.5 min (40-160 min). Size of removed tumors ranged from 14 mm to 40 mm (mean: 25 mm). The mean length of hospital stay was 3.2 days (2 to 8 days) for patients treated with POSE. One patient (8.3%) required longer hospitalization (8 days) due to the presence of a fluid collection at the site of gastric suture. This patient was treated conservatively. One patient (8.3%) was diagnosed with surgical site infection (navel wound after an optical trocar). Histopathological examination confirmed radical excision in all of our patients (R0). Conclusions: It seems that the POSE technique is the next stage of development of minimally invasive surgery and may constitute a link in evolution of natural orifice translumenal surgery techniques. Removal of excised material through oral cavity is an attractive, effective, and safe method despite its many limitations.

Complexes formed between clay and soil organic matter are important for carbon sequestration and for soil physical quality. Here, we use samples of loessial soil from South-East Poland to explore the phenomenon of complexing in loess. Soil samples were collected from a single catchment 8 years after the introduction of strip tillage and their compositions were characterized by traditional methods. Complexing was characterized in terms of the content of non-complexed clay which was estimated in two ways: firstly, by measurement of the content of readily-dispersible clay (which was assumed to be the non-complexed clay); and secondly, by calculation using algorithms that had been developed and evaluated previously. The calculations were based on the concept that, at carbon saturation, the clay/organic carbon mass ratio is equal to n. The calculations were done with a range of values of n. It was assumed that the correct value of n was that which gave the greatest coefficient of correlation between the measured values of clay dispersion and the predicted values of non-complexed clay. For the loess used, the optimum value was n = 14.

The effects of different tillage systems on the properties and microbial diversity of an agricultural soil was investigated. In doing so, soil physical, chemical and biological properties were analysed in 2013-2015, on a long-term field experiment on a loamy sand at the IUNG-PIB Experimental Station in Grabów, Poland. Winter wheat was grown under two tillage treatments: conventional tillage using a mouldboard plough and traditional soil tillage equipment, and reduced tillage based on soil crushing-loosening equipment and a rigid-tine cultivator. Chopped wheat straw was used as a mulch on both treatments. Reduced tillage resulted in increased water content throughout the whole soil profile, in comparison with conventional tillage. Under reduced tillage, the content of readily dispersible clay was also reduced, and, therefore, soil stability was increased in the toplayers, compared with conventional tillage. In addition, the beneficial effects of reduced tillage were reflected in higher soil microbial activity as measured with dehydrogenases and hydrolysis of fluorescein diacetate, compared with conventional tillage. Moreover, the polimerase chain reaction – denaturing gradient gel electrophoresis analysis showed that soil under reduced tillage had greater diversity of microbial communities, compared with conventionally-tilled soil. Finally, reduced tillage increased organic matter content, stability in water and microbial diversity in the top layer of the soil.

Introduction; p to 300 million people have the body mass index (BMI) greater than 30 kg/m2. Obesity is the cause of many serious diseases, such as type 2 diabetes, hypertension, and non-alcoholic fatty liver disease (NAFLD). Bariatric surgery is the only effective method of achieving weight loss in patients with morbid obesity. Objectives: The aim of the study was to assess the impact of bariatric surgery on non-alcoholic fatty liver disease in patients operated on due to morbid obesity. Material and Methods: We included 20 patients who were qualified for bariatric procedures based on BMI > 40 kg/ m2 or BMI > 35kg/m2 with the presence of comorbidities. The average body weight in the group was 143.85kg, with an average BMI of 49.16kg/m2. Before the procedure, we evaluated the severity of non-alcoholic fatty liver disease in each patient using the Sheriff-Saadeh ultrasound scale. We also evaluated the levels of liver enzymes. Follow-up evaluation was performed twelve months after surgery. Results: Twelve months after surgery, the average weight was 102.34 kg. The mean %WL was 33.01%, %EWL was 58.8%, and %EBMIL was 61.37%. All patients showed remission of fatty liver disease. Liver damage, evaluated with ultrasound imaging, decreased from an average of 1.85 on the Sheriff-Saadeh scale, before surgery, to 0.15 twelve months after surgery (p < 0.001). As regards liver enzymes, the level of alanine aminotransferase decreased from 64.5 (U/l) to 27.95 (U/l) (p < 0.001), and the level of aspartate aminotransferase decreased from 54.4 (U/l) to 27.2 (U/l). Conclusions: Bariatric procedures not only lead to a significant and lasting weight loss, but they also contribute to the reduction of fatty liver disease and improve liver function.
