Lateral ankle sprains are one of the most common injuries in athletics. Injury to lateral ankle ligaments can result in persistent instability of the ankle joint, known as functional ankle instability (FAI). Two methods of treating FAI are ankle taping and ankle bracing. The purpose of this study was to compare the effects of ankle taping and ankle bracing on ankle joint kinematics and kinetics during a landing task. Methods: Seven individuals with FAI and seven healthy controls performed three landing trials in each of three conditions: control, taped and braced. Ground reaction forces and three-dimensional kinematics were collected simultaneously while participants performed single-leg step-off landing trials from a box with a height of 0.6m. Peak ankle joint angles and moments were calculated using customized software. Results: Individuals with FAI produced significantly smaller inversion moments compared to healthy controls (p = 0.006). Ankle stabilization did not significantly alter ankle joint angles, ranges of motion or moments. Discussion: The present findings suggest that individuals with FAI exhibit unique ankle biomechanics independent of ankle stabilization modality. Future research may seek to investigate the multi-joint biomechanical adaptations associated with ankle stabilization in individuals with FAI compared to healthy controls.

The paper presents design and manufacturing process of an individualized ankle orthosis using additive manufacturing technologies and reverse engineering. Conventional processes of manufacturing of orthoses are expensive and time consuming Thus, an alternative method was proposed. The patient’s leg was 3D scanned and the orthosis was designed using a CAD system. It was then manufactured using the Fused Deposition Modelling technology, assembled and fully tested. Positive results were obtained.

Opis:
Total ankle replacement (TAR) is a relatively new concept and is becoming more popular for treatment of ankle arthritis and fractures. Because of the high costs and difficulties of experimental studies, the developments of TAR prostheses are progressing very slowly. For this reason, the medical imaging techniques such as CT, and MR have become more and more useful. The finite element method (FEM) is a widely used technique to estimate the mechanical behaviors of materials and structures in engineering applications. FEM has also been increasingly applied to biomechanical analyses of human bones, tissues and organs, thanks to the development of both the computing capabilities and the medical imaging techniques. 3-D finite element models of the human foot and ankle from reconstruction of MR and CT images have been investigated by some authors. In this study, data of geometries (used in modeling) of a normal and a prosthetic foot and ankle were obtained from a 3D reconstruction of CT images. The segmentation software, MIMICS was used to generate the 3D images of the bony structures, soft tissues and components of prosthesis of normal and prosthetic ankle-foot complex. Except the spaces between the adjacent surface of the phalanges fused, metatarsals, cuneiforms, cuboid, navicular, talus and calcaneus bones, soft tissues and components of prosthesis were independently developed to form foot and ankle complex. SOLIDWORKS program was used to form the boundary surfaces of all model components and then the solid models were obtained from these boundary surfaces. Finite element analyses software, ABAQUS was used to perform the numerical stress analyses of these models for balanced standing position. Plantar pressure and von Mises stress distributions of the normal and prosthetic ankles were compared with each other. There was a peak pressure increase at the 4th metatarsal, first metatarsal and talus bones and a decrease at the intermediate cuneiform and calcaneus bones, in prosthetic ankle-foot complex compared to normal one. The predicted plantar pressures and von Misses stress distributions for a normal foot were consistent with other FE models given in the literature. The present study is aimed to open new approaches for the development of ankle prosthesis.

Background: Osteoarthritis, which causes pain, deformities and instability of the rearfoot, is the most common indication for ankle arthrodesis. The aim of this study was to present the influence of the ankle arthrodesis on the performance of this joint in the three planes during walking at natural velocity.Materials and methods: Fourteen patients, age from 34 to 66 years (mean: 54 years), with ankle arthrodesis participated in the testing procedures. Angular changes of the ankle in the three planes of movement during particular gait phases and spatiotemporal gait parameters were evaluated by a three–dimensional motion analysis system, Vicon®. The study was performed at the Biokinetics Laboratory of the Academy of Physical Education in Kraków. The results of locomotion analysis were compared to a control group, consisting of thirty healthy persons aged 40–65 years.Results: In the saggital plane feet joints worked symmetrically during all gait phases, but there were angular changes of plantar and dorsal flexion, which were different from biomechanical norm. In frontal plane, feet were in abduction during almost all gait phases. In the transversal plane, feet were in internal rotation. Significant differences in spatiotemporal gait factors were observed between the two groups.Conclusions: Ankle arthrodesis leads to significant changes in motion pattern of the ankle joints.

Purpose: The objective of the study is to evaluate the degree of balance disorders in patients with surgical treatment of ankle fractures with the use of stabilometric examinations.Methods: The subjects in the study were 21 patients with ankle fractures treated surgically, within one year of the procedure. The control group were 20 healthy subjects. The balance was evaluated with the use of force platform in standing posture in both single and double limb stance. The analysed parameters were the transition area of the centre of feet pressure (COP), the length of the COP path and the COP velocity. The range of movement in the ankle joint and the intensification of pain were also measured.Results: In the balance evaluation in double limb stance, there were no statistically significant differences. A significant difference was found in the attempts of single limb-stance An average value of COP transition area in the study group was 261.2 mm2 in single stance (on the right operated limb) and in the control group – 93.2 mm2, so the difference was statistically significant (p=0,0096). The presence of pain, the presence or the removal of anastomosis had no significant influence on the balance of the studied subjects. Also a significant correlation between the balance of the subjects and their age was found.Conclusions: The balance in single limb stance after an instable ankle fracture within one year of the surgical procedure is significantly poorer in comparison with healthy subjects. Elderly persons have significantly poorer balance control.

Introduction and objective. Common to the talocrural and subtalar joints, the calcaneofibular ligament forms with the anterior bundle a mechanical coupling factor for ankle stabilization. Adhering to the deep surface of the fibular sheath, doubled in depth by the lateral talocalcaneal ligament, it is solicited in isolated varus and its lesion is involved in subtalar instability. The therapeutic strategy for chronic ankle instability is currently based on the anatomical reconstruction of the ligaments, particularly by arthroscopy, hence the objective of this study, which aims to highlight the great variability of the calcaneofibular ligament. Materials and method. Our work is based on the dissection of 24 ankles. The number of beams, the dimensions at the neutral point and after power-up, the relationships with the surrounding anatomical structures as well as the points of insertion of the CFL are studied. Results. The calcaneofibular ligament was monofascicular in 91.6% of cases and bifascicular in 8.4%. Its average length in a neutral position was 26.85 ± 3.46 mm, and its width was 5.33 ± 1.45 mm. In dorsiflexion, the length was 27.10 ± 0.05 mm, and in forced inversion, it was 26.98 ± 0.07 mm. The distance between its fibular insertion and the apex of the lateral malleolus was 6.2 ± 0.9 mm. Conclusions. Injuries of the calcaneofibular ligament are common in lateral ankle sprains, which can often get complicated by chronic instability. The macroscopic and morphometric knowledge of this ligament can allow a more accurate anatomical reconstruction and a more accurate choice of graft.

Ankle arthrodesis is still the gold standard in the treatment of end-stage degeneration in the ankle joint. The anatomical surgical approach, correct foot alignment and stable fixation are the factors determining the positive effect of surgery. The study presents the technique of ankle arthrodesis by modified Mann’s technique which meets the above-mentioned criteria with the economical use of implants for its stabilization.

Dobry wynik leczenia zależy od dobrego pod względem anatomicznym dostępu do stawu, prawidłowej korekcji zniekształcenia oraz stabilnego zespolenia. Praca prezentuje technikę artrodezy stawu skokowo-goleniowego zmodyfikowanym sposobem Manna, która spełnia te kryteria, a dodatkowo nie wymaga stosowania drogich implantów.

This study developed an ankle rehabilitation device for post-stroke patients. First, the research models and dynamic equations of the device are addressed. Second, the Sliding Mode Controller for the ankle rehabilitation device is designed, and the device's response is simulated on the software MATLAB. Third, the ankle rehabilitation device is successfully manufactured from aluminum and uses linear actuators to emulate dorsiflexion and plantarflexion exercises for humans. The advantages of the device are a simple design, low cost, and mounts onto rehabilitative equipment. The device can operate fast through experiments, has a foot drive mechanism overshoot of 0°, and a maximum angle error of 1°. Moreover, the rehabilitation robot can operate consistently and is comfortable for stroke patients to use. Finally, we will fully develop the device and proceed to clinical implementation.

Ankle sprains are common soft-tissue injuries that are often treated in emergency departments. These injuries can have significant consequences for the patient, including long-term morbidity and loss of productivity. The objective of this study was to examine the direct and indirect health resource utilization associated with ankle sprains. 296 adult patients with acute ankle sprains participated in the study in Kingston, Ontario, Canada. Data were collected using a one-month productivity questionnaire. Overall, 11% (95% CI, 8-15%) of the participants visited a physician following the initial emergency department visit. Almost all (95%; 95% CI, 92-97%) of the participants used medications or supportive treatments and 55% (95% CI, 50-61%) reported taking time off from work, school, or housework. The use of unpaid assistance was indicated by 56% (95% CI, 50-62%). Findings from this analysis highlight the significant patient-related and health care system burden of acute ankle sprains.

Opis:
Purpose: The curvature morphology of the articulating surfaces determines the physiological movement pattern. We quantitatively examined the curvature morphology of the tibiotalar articulating surfaces and specified their geometric contact patterns. Methods: Geometrically equivalent cartographic nets were marked on the talar and tibial articulating surfaces of true-to-scale moldings of 20 human ankle joints (intervals of 5 mm) to relate corresponding articulating units of the surfaces. The corresponding contours of the net lines were compared, and the incongruity of articulating surfaces could thus be quantified locally. Results: All tibial sagittal net lines represented circular arcs. Along the sagittal talar net lines, the curvature radii increased medially from anterior to posterior but decreased laterally. Each net line could be approximated by three circular arcs. Examining these three parts of the talar net lines, the anterior sagittal curvature radii increased from medial to lateral, whereas the posterior radii decreased. The tibial and talar transversal net lines were congruent. The articulation surfaces showed a transversal contact line in every dorsal/plantar joint position. The degree of local congruity was solely ascertained by the incongruity of the corresponding sagittal net lines. The maximal degrees of congruity were found laterally for dorsal flexion, laterally/centrally for neutral joint position, and centrally/medially for plantar flexion. Conclusions: By the transversal line contact, the contact area is broadened over the articulating surfaces from lateral to medial. In dorsal flexion, compressive loads are mainly transferred by lateral/anterior zones and in plantar flexion by medial/posterior zones of the articulating surfaces. Reconstruction of the transversal contact line is essential.

Chronic ankle instability (CAI) is a common sequence of ankle sprain. Conservative treatment of CAI is currently accepted as a primary mean of managing the condition and preventing of recurrence. Objectives: The purpose of the current study was to evaluate the effect of Weight-bearing Exercise for Better Balance (WEBB) program and unilateral balance training exercises on postural control in females with CAI. Methods: Thirty-five females with CAI participated in this study. The participants’ mean values of age, body mass, height, and body mass index were 21.76 ± 1.96 y, 64.82 ± 9.37 kg, 161.85 ± 7.24 cm, and 24.76 ± 3.76 kg/m², respectively. They were randomly assigned into three groups: experimental group A (n = 13) who received WEBB program, experimental group B (n = 12) who received unilateral balance training, and control group C (n = 10) using statistical random tables. Balance parameters were measured for all participants via Biodex Balance System (BBS) before and after four weeks of balance training. They included overall stability index (OASI), antero-posterior stability index (APSI), and medio-lateral stability index (MLSI). Results: There was a significant improvement in the post testing mean values of OASI and APSI, compared with the pre testing values in both experimental groups (A and B). There was no significant difference between the pre and post testing mean values of MLSI in the experimental group (A). Conclusions: Both WEBB program and unilateral balance training have positive effects on postural control in females with CAI.

The aim of this study was to present the effects of the ankle arthrodesis on correct functioning of the knee and hip joints during walking at natural velocity.Materials and methods: The study was conducted in the Biokinetics Laboratory of the Academy of Physical Education in Kraków. Fourteen patients with ankle arthrodesis, aged from 34 to 66 years (mean: 54 years), were enrolled into the study and participated in the testing sessions. Angular changes of the knees and hips in three planes of movement during particular gait phases were evaluated by a three–dimensional motion analysis system: Vicon®. The results of locomotion analysis were compared to a control group that consisted of thirty healthy persons aged 40–60 years.Results: Significant differences in the movement of the knees and hips in the three planes of motion were observed between the two groups. In the frontal plane, knees moved symmetrically, but during all the gait phases, knees were in a significant valgus deformity. In the transversal plane, knees were in a significant external rotation during all gait phases. In the saggital plane, hips moved with limited extension. There was also a significant external rotation, outside the biomechanical normal range, of both hips in the transversal plane.Conclusions: The most significant deviations from the biomechanical normal range were found in the frontal and transversal planes for knees and in the saggital and transversal planes for hips.

Georges Cuvier (1798) established the classical concept of Edentata which included sloths, anteaters, armadillos, aardvarks, and pangolins. With the growing body of comparative morphological data becoming available during the nineteenth century, it was evident that Cuvier's “Edentata” was an artificial group (e.g., Huxley 1872). In his classical textbook, Weber (1904) excluded aardvarks and pangolins from the Edentata and put them in separate orders, Tubulidentata and Pholidota. Later on, fossil taxa were repeatedly added to and removed from Edentata, such as various xenarthran groups, taeniodonts, palaeanodonts, and gondwanatheres, but the South American Xenarthra always was considered as their core group. Even the living order Pholidota has been cited again as ?Edentata incertae sedis many years after Weber’s work (Romer 1966). The validity and extent of a higher taxon Edentata are still in dispute. In this discussion, the Middle Eocene pholidotan Eomanis and the putative xenarthran Eurotamandua from Grube Messel near Darmstadt (Germany) play an important role (Storch 1978, 1981, 2003; Rose and Emry 1993; Gaudin and Branham 1998; Rose 1999). Eomanis krebsi and Eurotamandua joresi have been subject to some discussion regarding their taxonomic distinction. It has been suggested that the only specimen known of Eo. krebsi might actually be a juvenile representative of the senior species E. joresi. A reexamination of the type specimen of Eo. krebsi has yielded some new observations regarding the identity of some of its ankle elements. An element that was previously identified as a navicular, is here reidentified as a partial distal tibia, whereas a partially exposed calcaneus had gone unnoticed. These two elements display several differences in morphology between Eo. krebsi and E. joresi, indicating that these are in fact distinct species.

Opis:
Proposition of compliant foot for bipedal robot is introduced and its properties are investigated. The foot consists of four compliant elements (spring-damper) mounted to four vertices of a rectangular frame. The results of robot gait analysis using Zero Moment Point method are shown. ZMP trajectories for rigid and compliant foot are compared and conclusions are formulated. Foot compliance reduces needed for postural stabilization compensatory movements of the upper part of the body by those simplifying control methods and construction. Obtained results will be applied in real prototype of small humanoidal robot. Robot construction in which proposed foot will be applied is shortly introduced.

Introduction and objective: Diabetic patients are at high risk for peripheral arterial disease (PAD) characterized by symptoms of intermittent claudication or critical limb ischemia. Measurement of ankle-brachial index (ABI) has emerged as the diagnostic tool of choice, because it is relatively simple, non-invasive and inexpensive. It is also an independent marker of increased morbidity and mortality from cardiovascular diseases. The aim of the presented study was to assess the relationship between current glycemic control defined by glycated hemoglobin (HbA1c) level, and quantitative changes in the arteries of the lower limbs in patients with type 2 diabetes. Materials and methods: 175 patients with type 2 diabetes hospitalized in the Diabetology Ward were studied. VENO Doppler and a sphygmomanometer were used to assess blood flow. Results: The average level of HbA1c was assessed at 8.48%. Although the average level of ABI indicator was 1.20 (normal), only 45% of evaluated patients had their individual index within the normal range. Signs of ischemia were found in 17.7% of examined subjects. There was no conclusive correlation between ABI and HbA1c levels. Conclusions: The current level of glycemic control evaluated as HbA1c has no direct impact on the advancement of diabetic angiopathy evaluated as ABI.
