Opis:
No in-depth research in the area of risk management of the human factor in organizational processes (in particular operational ones) is without doubt an important issue in effective risk management, particularly in units managed with critical infrastructure. The absence of systemic solutions, models, algorithms for predicting employees' risky behavior causes management's ignorance, not recognizing problems, a lack of forward-looking perspective and focus on nothing but the current issues of the organization. This article attempts to explain the need for dynamic assessment of the risks of human error in management terms because only such assessment can increase resilience to crises in companies with critical infrastructure. In the following study, the concept of ‘the risk of a human factor’ will be used interchangeably with the term ‘the risks associated with employees’ errors’ that are generated by behavioral risk and the risk of latent skills gaps.
Brak pogłębionych badań w obszarze zarządzania ryzykiem czynnika ludzkiego w procesach organizacyjnych (w szczególności operacyjnych) jest bez wątpienia istotnym problemem w efektywnym zarządzaniu ryzykiem szczególnie w jednostkach zarządzających infrastrukturą krytyczną. Brak rozwiązań systemowych, modeli, algorytmów predykcji ryzykownych zachowań pracowników powoduje ignorancję kierownictwa, niedostrzeganie problemów, brak myślenia perspektywicznego i koncentrację na bieżących problemach organizacji. W niniejszym artykule podjęto próbę wyjaśnienia konieczności dynamicznej oceny ryzyka, związanego z błędami ludzkimi w ujęciu zarządczym, gdyż tylko taki charakter oceny może zwiększać odporność na zdarzenia kryzysowe w przedsiębiorstwach z infrastrukturą krytyczną. W pracy będzie stosowane pojęcie: ryzyko czynnika ludzkiego zamiennie z pojęciem ryzyka związanego z błędami pracowników, generowanymi przez ryzyko behawioralne i ryzyko utajonych luk kompetencyjnych.

The documentation of human factor influence on the scenario development in maritime accidents compared with expert methods is commonly used as a basis in the process of setting up safety regulations and instructions. The new accidents and near misses show the necessity for further studies in determining the human factor influence on both risk acceptance criteria and development of risk control options for the manoeuvers in restricted waters. The paper presents the model of human error probability proposed for the assessment of ship masters and marine pilots' error decision and its influence on the risk of port manoeuvres.

The paper presents a new approach to the evaluation of human factor (HF) influence on the risk of maritime accidents. This approach is based on a formal human–ship–environment system definition including the relationships between humans, technology, environment, and organization. Social and organizational factors have already been included in the International Maritime Organization regulations; however, the individual factors still need investigation. A combination of psychological studies and technical operations of sea-going ships has given a huge opportunity to use HF assessment in rule-making processes. The main scientific goal of the research presented in the paper was the development of a method to assess the influence of HF on the risk of maneuvering accidents in restricted waters. This method is based on research within the area of technical ship operation and the results of the psychological profile of operators. The proposed model is based on a quantitative HF model developed by an authorized psychologist comprising personality traits, vulnerability to stress, and risk approach. The investigations were carried out with a group of 32 experienced ship masters performing a complex maneuvering task on the Full Mission Ship Handling Simulator. The multidimensional dependencies between variables of the psychological profile and the risk of an accident resulting from maneuvering errors were implemented into the Sugeno fuzzy model. The developed model allows risk assessment to be conducted that depends on the selected personality profile features. These features can be measured using psychological questionnaires, and then the risk of an accident due to maneuvering error can be calculated for a captain or marine pilot in order to improve human resource management.

This article focuses on issues related to risk assessment when maneuvering a loaded bulk carrier in close proximity to a vessel performing underwater work at the time. It is based on a detailed analysis of an incident that took place in the Gulf of Gdansk. The write-up explains real turns of events, conditions and factors that contributed to the incident, but also its consequences are explained. Some other aspects of this article focuses on, are the processes of examination of the direct causes of the incident and identification not compliance with regulations, requirements, or procedures that help to find out the human, technical, and organizational errors. The authors of this text indicate the safety guards that have failed, give the reasons for their failure and, where it was possible, point out the safety guards that should or must be established. The article does not take into account theoretical models for the described accidents, but only practical aspects, human errors and applicable local and international laws and regulations. Particular attention was devoted to the analysis of human errors made by officers maneuvering the surface vessel in the close vicinity of divers performing underwater works.

A paradigm shift is presently underway in the shipping industry promising safer, greener and more efficient ship traffic. In this article, we will look at some of the accidents from conventional shipping and see if they could have been avoided with autonomous ship technology. A hypothesis of increased safety is often brought forward, and we know from various studies that the number of maritime accidents that involves what is called “human error” ranges from some 60‐90 percent. If we replace the human with automation, can we then reduce the number of accidents? On the other hand, is there a possibility for new types of accidents to appear? What about the accidents that are today averted by the crew? This paper will present a method to assess these different aspects of the risk scenarios in light of the specific capabilities and constraints of autonomous ships.
