Opis:
Small scale mining continues to contribute significantly to the growth of Ghana's economy. However, the sector poses serious dangers to human health and the environment. Ground failures resulting from poorly supported stopes have led to injuries and fatalities in recent times. Dust and fumes from drilling and blasting of ore present health threats due to poor ventilation. Four prominent small scale underground mines were studied to identify the safety issues associated with small scale underground mining in Ghana. It is recognized that small scale underground mining in Ghana is inundated with unsafe acts and conditions including stope collapse, improper choice of working tools, absence of personal protective equipment and land degradation. Inadequate monitoring of the operations and lack of regulatory enforcement by the Minerals Commission of Ghana are major contributing factors to the environmental, safety and national security issues of the operations.

Formalna Analiza Ryzyka (FSA) jest skuteczną metodą mającą na celu zwiększenie bezpieczeństwa morskiego. Szczególnie zwraca uwagę na ochronę życia, zdrowia, środowiska morskiego i mienia, wykorzystując analizę ryzyka, metody jego kontroli i ocenę kosztów. Celem FSA jest wspieranie procesu zarządzania bezpieczeństwem, standaryzacji bezpieczeństwa oraz ustalanie polityki działań na rzecz bezpieczeństwa. Metoda ma charakter ogólny i może być wykorzystana, jako narzędzie wspomagające podejmowanie decyzji w zakresie tworzenia nowych regulacji prawnych, projektowania, klasyfikacji, budowy i eksploatacji statków. Metoda nadaje się do analizy pojedynczego statku lub typu statków, akwenów, np.: tankowców, zatok, portów, itp. W artykule przeanalizowano dokumenty międzynarodowe dotyczące FSA.
A Formal Safety Assessment (FSA) is an effective method that is aimed to increase maritime safety. Particularly draws attention to the protection of life, health, property and the marine environment, using risk analysis, methods of its control and the assessment of costs. The main purpose of FSA is to support the process of security management, the standardization of safety and determining safety action policy. The method is general and can be used as a tool to assist decision making in the field of the creation of new legal regulations, classification, design, construction and operation of ships. Method is suitable for the analysis of a single vessel or type of vessel, such as: oil tankers, gas tankers or different areas, for example bays, ports, jetties, etc. The article analyses the international instruments relating to the FSA.

W artykule przedstawiono jedyną w Polsce i Europie placówkę naukowo-badawczą – Kopalnię Doświadczalną „Barbara” posiadającą poligon doświadczalny umożliwiający wykonywanie badań urządzeń, sprzętu, materiałów i procedur postępowania w rzeczywistych warunkach dołowych. KD „Barbara” działa od 1925 r. stojąc na straży bezpieczeństwa w górnictwie, a także w innych gałęziach przemysłu. Realizuje również badania certyfikujące w ramach Jednostki Notyfikowanej Głównego Instytutu Górnictwa (GIG) dla następujących dyrektyw: 2013/29/UE, 2014/28/UE i 2014/34/UE.
In this article, a presentation is made of the only research and testing facility in Poland and indeed Europe – „Barbara” Experimental Mine – possessing an underground proving ground enabling the conduct of tests on: devices, equipment, materials and procedures in actual underground conditions. „Barbara” Experimental Mine has been working since 1925 in upholding the safety of mining as well as other branches of industry. It also carries out certification tests under the auspices of the Notification Body of the Central Mining Institute (GIG) for the following directives: 2013/29/UE, 2014/28/UE and 2014/34/UE.

There is presented a practically well grounded approach concerned with the identification, evaluation, prediction and optimization of reliability, availability and safety of technical systems related to their operation processes. The main emphasis of this approach is on multi-state systems composed of ageing components and changing during the operation processes their structures and their components reliability and safety characteristics. There are proposed the convenient tools for analyzing these systems in the form of semi-markov modeling the systems’ operation processes and multistate modeling the systems’ reliability. There are described theoretical results of the proposed approach to reliability and safety analysis of multistate systems with degrading components in their operation processes and the possibility of their practical applications to the reliability and safety analysis and optimization of the complex technical port and maritime transportation systems.

The paper objectives are to present the methods and tools useful in the statistical identifying the unknown parameters of the components reliability and safety of complex industrial systems and to apply them in the maritime industry. There are presented statistical methods of estimating the unknown intensities of departure from the reliability state subsets of the exponential distribution of the component lifetimes of the multistate systems operating in various operation states. The goodness-of-fit method applied to testing the hypotheses concerned with the exponential form of the multistate reliability function of the particular components of the complex technical system in variable operations conditions is suggested. An application of these tools to reliability characteristics of a ferry operating at the Baltic Sea waters is presented as well.

Opis:
Accidents, breakdowns or incidents threatening both human life and the environment are inherent parts in operating in the offshore sector. Regular monitoring and analysis of all kinds of events are important to isolate the most common and dangerous ones, to be able to prevent them in the future. The article summarizes the sources containing information regarding accidents occurred in the maritime offshore sector. Due to their widespread and easy access, the article is focused on electronically published databases and safety reports concerning the risk and dangers related to the implementation of marine exploration – mining projects. Based on these data, the authors made analyses, presented statistics and proposed ways to minimize threats in the offshore sector of the marine industry.

A new approach is proposed for safety investigations of complex multistate systems. These systems have dependent components, called critical infrastructures, with variable operating conditions. The safety function of the critical infrastructure system is defined and determined for an exemplary “m out of l” critical infrastructure. In the fully-developed model, it is assumed that system components have multistate exponential safety functions with interdependent departure rates from subsets of safety states. A critical infrastructure safety model is adopted for an oil pipeline transportation system operating in a maritime port.

Basic concepts of the safety analysis of ageing multistate systems are introduced. The system components and the system multistate safety functions are defined. The mean values and variances of the multistate systems’ lifespans in the safety state subsets and the mean values of their lifespans in the particular safety states are defined. The multistate system risk function and the moment of exceeding the critical safety state are introduced. A series safety structure and a parallel-series safety structure of the multistate systems with ageing components are defined and their safety functions are determined. The multistate system safety models are applied to the prediction of safety characteristics of a maritime ferry.

This project was conducted with a multicultural construction company in Hawaii, USA. The job duties performed included drywall and carpentry work. The following objectives were selected for this project: (a) fire prevention training and inspection of first aid equipment; (b) blood-borne pathogen training and risk evaluation; (c) ergonomic and risk evaluation intervention program; (d) electrical safety training and inspection program; (e) slips, trips, and falls safety training; (f) stress assessment and Personal Profile System; (g) safety and health program survey; (h) improving employee relations and morale by emphasizing spirituality; and (i) computer-assisted biofeedback stress management training. Results of the project indicated that observed safety hazards, reported injuries, and levels of perceived stress were reduced for the majority of the population.

The main objective of this paper is to present recently developed, the general safety analytical models of complex multistate technical systems related to their operation processes and to apply them practically to critical infrastructures. To realize this goal, the integrated model of critical infrastructure safety related to its operation process is proposed. The basic safety characteristics of this model are presented as the very practically significant. Furthermore, the unconditional safety functions of systems with different safety structures are determined under assumption that their safety functions are exponential. In case of the critical infrastructure safety analysis, its safety function and risk function which graph corresponds to the fragility curve, its mean lifetime up to the exceeding a critical safety state, the moment when its risk function value exceeds the acceptable safety level, the component and critical infrastructure intensities of ageing/degradation and the coefficients of operation impact on component and critical infrastructure intensities of ageing are defined.

First, basic notions of the multistate system safety analysis are introduced, i.e. the multistate components and the multistate system, the multistate system component safety function, the multistate system safety and the multistate system risk function are defined. Moreover, the multistate system component and the multistate system main safety characteristics, i.e. their mean values of the lifetimes and in the safety state subsets and in the particular safety states and standard deviations and the moment when the system risk function exceeds a fixed permitted level are determined. Furthermore, there are constructed safety models of the multi-state homogeneous and non-homogeneous series, parallel, “m out of n”, consecutive “m out of n: F”, series-parallel, parallel-series, series-“m out of n”, “m out of n”-series, series-consecutive “m out of n: F” and consecutive “m out of n: F”-series systems and their safety functions are determined. Moreover, a very often met in practiceseries system composed of multistate subsystems identical with the considered earlier multistate systems is considered and its safety function is determined.
