Objectives Local anesthetics are some of the most common drugs used in dentistry and about 6 million people daily around the world are subjected to their effect in procedures performed by a similar number of dentists who, in turn, are usually not aware of the fact of being allergic to this group of medications. Assuming that dentists are also patients, and that in their daily practice the contact with the allergen is very frequent, it is essential to assess the incidence of allergy to lidocaine in general dental practitioners. Material and Methods The authors evaluated a group of 100 general dental practitioners in whom adverse reactions similar to anaphylaxis occurred after local anesthesia. The study included individuals who, in their thorough medical history, had experienced episodes of such reactions regarding the skin, airways, gastrointestinal tract and other areas. Results The relations between type I hypersensitivity and certain symptoms, time from exposure to their appearance, as well as time from the last episode were investigated. Allergy to lidocaine was detected in 17 subjects – type I hypersensitivity was diagnosed in 13 cases (skin prick test – 7; intradermic test – 6) and 4 subjects had IgE-independent allergy (patch test). In the group where type I hypersensitivity was detected, urticaria, angioedema and rhinitis were indicated as inclusion criteria more often than among other subjects. According to the results, the occurrence of angioedema increased the risk of detection of type I hypersensitivity 68.8 times, and 1 year longer period from the last episode decreased this risk by circa 55%. Conclusions The most important, from the clinical practice point of view, was to show the relation between the medical history indicating the anaphylactic nature of the lidocaine intolerance, and an allergy confirmed by skin tests. Introduction: CRS is a complex systemic disease affecting more than 10% of the population. There are two main types of CRS phenotypes: CRSwNP and CRSsNP. In the Caucasian population, the prevalence of inflammation markers typical of the Th1 profile is observed in CRSsNP, whereas Th2 and Th17 in CRSwNP. Th2 inflammation is observed in the CRSwNP phenotype with concomitant allergies, asthma or hypersensitivity to NSAIDs. Objectives: The aim of the study was to evaluate, based on the authors’ own material, whether allergies, asthma or hypersensitivity to NSAIDs were a risk factor for the development of a specific CRS phenotype. An attempt was also made to investigate the influence of comorbidities on the extent of sinus endoscopic procedures, which depended on the severity of inflammation. Methods: In the years 2006–2015, ESS was performed on 2217 patients with different CRS phenotypes. Patients with an allergy, bronchial asthma and hypersensitivity to NSAIDs were subjected to analysis. Results: Based on logistic regression, it was found that among the mentioned comorbidities, only asthma (P < 0.0001) and hypersensitivity to NSAIDs (P = 0.0007) significantly affect the occurrence of the phenotype with polyps, whereas the impact of allergies is statistically insignificant (P = 0.1909). The relationship between the type of ESS and CRS phenotypes is statistically significant (P < 0.0001). Conclusions: Bronchial asthma and hypersensitivity to NSAIDs have a statistically significant effect on the occurrence of the CRSwNP phenotype. This effect was not observed in allergies. The impact of allergies, asthma and hypersensitivity on the phenotype was observed in the group of patients subjected to the most extensive surgery (ESS 4).

ObjectivesAmbient air pollution is related to adverse respiratory effects. Because of a popular habit of recreational physical activity, the effects of exposure to increased levels of air pollution attract increasing attention. It remains unclear whether the allergy status has an impact on acute responses to air pollution exposure during brief exercise in young adults. The aim of the study was to determine if acute respiratory responses to ambient air pollution during physical exercise differ between young subjects with and without a history of upper respiratory allergy.Material and MethodsOverall, 41 young males with (N = 15, 36.6%) and without allergy (N = 26, 63.4%) performed short moderate-intensive cycle-ergometer sessions in winter air pollution exposure conditions. Associations were analyzed between environmental conditions and acute physiological changes in spirometry, fractioned exhaled nitric oxide (FeNO) levels, blood pressure and pulse oximetry.ResultsNo associations between air pollution concentrations and changes in forced vital capacity, forced expiratory volume in 1 s and the Tiffeneau index were found. In the subjects without allergy, the increased air pollution concentrations recorded during exercise were associated with a post-exercise increase/a smaller decrease in FeNO (SO2: Spearman’s ρ = 0.44, NOx: ρ = 0.51, and particulate matter [PM] levels – PM10: ρ = 0.51, PM2.5: ρ = 0.52). This effect was not observed in the subjects with allergy.ConclusionsUpper-respiratory allergy may be a modifying factor in human response to air pollution during exercise. Exposure to air pollution during brief moderate-intensive exercise did not have any acute negative impact on respiratory and cardiovascular function in young males. However, in the case of FeNO, subclinical post-exercise changes related to air pollution were observed in volunteers without allergy.

Anaphylaxis is a life-threatening condition characterized by a rapid onset, requiring immediate intervention. It occurs through immunologic or non-immunologic mechanisms and can be triggered by various factors such as foods, medications or insect venoms. The spectrum of symptoms is broad, with skin, respiratory, and gastrointestinal symptoms being most commonly observed. Diagnosis relies on clinical criteria, with the primary treatment involving the intramuscular administration of adrenaline. Prevention involves avoiding triggers and if avoidance is not possible, desensitization should be considered. Deaths from anaphylactic shock are rare and often result from delayed or improper medical assistance and the absence of witnesses to summon help. It is estimated that around 1% of anaphylactic shock cases result in death. Deaths from intentional attempts to induce anaphylactic reactions through suicide are rare. Postmortem diagnosis presents a challenge due to the lack of specific changes, therefore it is essential to thoroughly assess the circumstances of death, medical history and exclude other potential causes. Identifying factors that may trigger an anaphylactic reaction, such as ingested foods or contact with allergens is crucial. A rapid and accurate diagnosis is key to avoid errors resulting from post-mortem processes. Histopathological examinations and the measurement of biochemical markers, such as tryptase and IgE levels, may be helpful in determining the cause of death.
Anafilaksja to stan zagrażający życiu, charakteryzujący się szybkim przebiegiem i wymagający natychmiastowej interwencji. Zachodzi zarówno w mechanizmie immunologicznym, jak i nieimmunologicznym. Wywoływana jest przez różnorodne czynniki, takie jak pokarmy, leki czy jady owadów. Spektrum objawów jest bardzo szerokie; najczęściej obserwuje się objawy skórne, ze strony układu oddechowego oraz pokarmowego. Diagnostyka opiera się na kryteriach klinicznych, a głównym elementem leczenia jest domięśniowe podanie adrenaliny. Prewencja polega na unikaniu wyzwalaczy, a gdy nie jest to możliwe, należy rozważyć przeprowadzenie desensytyzacji. Zgony spowodowane wstrząsem anafilaktycznym są rzadkie i często wynikają z opóźnionej lub niewłaściwej pomocy medycznej oraz braku świadków, którzy mogliby wezwać pomoc. Szacuje się, że około 1% przypadków wstrząsu anafilaktycznego kończy się zgonem. Rzadko dochodzi do zgonu w wyniku samobójczych prób poprzez celowe wywołanie reakcji anafilaktycznej. Diagnostyka pośmiertna stanowi wyzwanie ze względu na brak specyficznych zmian, dlatego należy dokładnie ocenić okoliczności zgonu, historię medyczną oraz wykluczyć inne potencjalne przyczyny. Istotne jest zidentyfikowanie czynników mogących wywołać reakcję anafilaktyczną, takich jak spożyte pokarmy czy kontakt z alergenami. Kluczowa jest szybka i dokładna diagnostyka w celu uniknięcia błędów wynikających z procesów rozkładu ciała po śmierci. Badania histopatologiczne oraz oznaczenie markerów biochemicznych, takich jak poziomy tryptazy i IgE, mogą być pomocne w ustaleniu przyczyny zgonu.

Objectives On average about 10% of parents report hypersensitivity to at least 1 drug in their children. After diagnosis process a few of these reactions are being confirmed as drug hypersensitivity reactions. The aim of the study was to assess the real-life prevalence of drug hypersensitivity in children based on drug provocation tests. Material and Methods The authors included 113 children, aged 4–18 years, referred to Pediatrics and Allergy Clinic in Łódź, Poland, due to incidence of adverse reaction during treatment. Medical history regarding allergies to drugs was taken in accordance to the form developed by the United States Food and Drug Administration Adverse Event Reporting System. Skin prick tests, intradermal test and drug provocation test were performed in all patients. Results In all 113 patients suspected of drug allergy, after all diagnostic procedures, the authors proved IgE-mediated allergy to β-lactams, nonsteroid anti-inflammatory drugs, local anesthetics in 19 patients (16.8%). Previous history of allergy was a risk factor for drug allergy in studied patients (p = 0.001). The most frequent symptoms of allergy were urticaria and erythematous papular rash. Conclusions Drug allergy is a difficult problem in the practice of a doctor and is difficult to diagnose, especially in the pediatric population. It seems that too often isolated symptoms reported during infection or disease are taken as a symptom of drug allergy, and not as a symptom resulting from the course of the disease.

The paper presents a study of the category of hypersensitivity based on Polish parenting websites and a discourse on children and child rearing. Using discourse analysis methods and findings from new childhood studies, the author explores the current aporias regarding sensitivity. She analyses social narratives entailed in them, including those concerning child education and upbringing, related to modern biopolitics and various risk discourses according to which the responsibility for the proper control and development of the child’s sensitivity lies within the parent-expert. Based on the studied material, a thesis is formed that connects the discourse on hypersensitivity with modern, also neoliberal, thinking, pointing to the post-moralistic implications of this relationship – possible outcomes of treating child development as a strictly individualistic process. The article also shows mechanisms of depriving children of subjectivity through discursive child management, in which the child is no longer a person and becomes merely a representative of certain characteristics of a specific group of population recognized as hypersensitive.

Seven populations of N. alata, 4 populations of N. sanderae and several hundred plants belonging to three generations of the hybrid (N. tabacum cv. TB-566 tetra × N. alata) × N. alata were tested for resistance to TSWV. Tested plants were artificial inoculated with TSWV under greenhouse conditions. Out of 11 accessions of N. alata and N. sanderae 7 were found to be composed entirely of plants hypersensitive to TSWV. In the remaining four accessions there were from 16 to 25% of TSWV susceptible plants. The hybrids (N. tabacum cv. TB-566 tetra × N. alata) × N. alata, regardless of their cytogenetical status, showed hypersensitive response to inoculation with TSWV...

Cel. Sprawdzenie związków zachodzących między funkcjonalnością wychowawczą rodziny generacyjnej, hipersensytywnością, resilience-cechą a trzema wymiarami przystosowania w pracy: kapitałem społecznym w miejscu pracy, stresem zawodowym i wypaleniem zawodowym i wygenerowanie modelu przyczynowych powiązań między tymi zmiennymi. Metody. Sondaż przeprowadzony drogą elektroniczną z wykorzystaniem standaryzowanych narzędzi pomiaru. Respondentami były osoby ze stażem pracy minimum pięciu lat, zatrudnione w obecnym miejscu pracy dłużej niż rok. Próba liczyła 622 osoby. Weryfikacja hipotez była wykonana z użyciem współczynnika korelacji Pearsona oraz analizy ścieżek. Wyniki. Analizy doprowadziły do wygenerowania dobrze dopasowanej do danych empirycznych struktury powiązań między zmiennymi modelu. W strukturze tej status zmiennych egzogenicznych przypisano funkcjonalności rodziny, nadwrażliwości emocjonalnej i wrażliwości estetycznej. Wszystkie te zmienne ujawniły istotne związki ze zdolnością pozytywnej adaptacji (resilience-cechą). Zdolność pozytywnej adaptacji okazała się powiązana bezpośrednio lub pośrednio z wymiarami przystosowania zawodowego. Wnioski. Badania potwierdziły status zdolności pozytywnej adaptacji (resilience-cechy) jako czynnika istotnego dla kształtowania się w ontogenezie względnej odporności na procesy zaburzające mechanizmy samoregulacyjne, w tym na powstawanie syndromu wypalenia zawodowego.
Aim. To examine the relationships between the functionality of child-rearing of the family of origin, with regard to hypersensitivity, resilience traits, and three dimensions of adjustment at work: social capital at work, occupational stress, and burnout, and to generate a model of causal relationships between these variables. Method. A survey was conducted electronically using standardized measurement tools. The respondents were individuals with a minimum of 5 years of work experience and employed for more than 1 year at their current job. The sample ultimately consisted of 622 people. Hypothesis verification was performed using Pearson’s correlation coefficient and path analysis. Results. The analyses led to the generation of a well-fitting structure of relationships between the variables of the model and the empirical data. In this structure, the status of exogenous variables was assigned to family functionality, emotional hypersensitivity, and aesthetic sensitivity. All these variables revealed significant relationships with the ability of positive adaptation (the resilience-trait). Positive adaptability appeared to be directly or indirectly related to dimensions of occupational adjustment. Conclusion. The study confirmed the status of the ability of positive adaptation (the resilience-trait) as a factor important for the formation in the ontogenesis of relative resistance to processes that disrupt self-regulation mechanisms, including the formation of a professional burnout syndrome.
