Opis:
This article aims at presenting an emerging, yeast-related, health threat. Candida auris (Metschnikowiaceae, Candida/Clavispora clade), is an emerging fungus, that presents a serious global health threat. It causes wounds, ears, respiratory and urinary tracts infections. In some patients, these yeasts can enter the bloodstream and spread throughout the body, causing serious systemic infections. C.auris is often resistant to commonly used antifungal drugs, such as amphotericin B, polyenes or echinocandins, making infections difficult to treat. C. auris persists in the environment for months, and persistent environmental contamination, contaminated medical equipment and other fomites, are believed to play a role in nosocomial C. auris transmission. Specialized laboratory methods (RT-PCR, MALDI-TOF MS), are needed to accurately identify C. auris yeast. Echinocandins are currently recommended as first-line therapy in adults and in infants (children above 2 months of age). For neonates amphotericin B deoxycholate is recommended. It is suspected, that C. auris may also be pathogenic to animals.

The incidence of Candida spp. infections has increased in recent years due to the widespread use of broad-spectrum antibiotics and the growing numbers of immunocompromised individuals. Despite the predominance of Candida albicans, non-albicans Candida species, such as Candida glabrata, Candida tropicalis, Candida guilliermondii, Candida dubliniensis, Candida parapsilosis, and Candida krusei are emerging as both, colonizers and pathogens, that can cause superficial and systemic infections. Currently, there are various methods for identifying yeasts from clinical samples. Conventional methods for identification of Candida species are based on morphological and physiological attributes. Although phenotypic identification methods are cost effective and practical procedures for routine discrimination of isolates in the clinical microbiology laboratory, the accurate identification of all isolates. Moreover, given the variability in the phenotypic characteristics and other irregularities in the taxonomy of the genus Candida, identification based on traditional methods has become less reliable. Important progress has been made with various studies on the phenotypic characteristics of these species using commercially available identification systems. The aim of this review is to summarize the methods available for identification of Candida species. The first part of the article presents the characteristics of phenotypic methods and quick identification techniques, including systems based on the use of chromogenic media.

Mycotic infections are an increasing diagnostic and therapeutic problem in pathology of the alimentary tract. The work shows the results of research on the large intestine candidosis. The following invasive factors were isolated: Candida albicans, C. guilliermondi, C. pseudotropicalis, C. tropicalis. Rectoscopic views were described. In severe C. albicans infections HIV test were performed. Four patients were HIV positive.

It is known that fungi representing different genera and species can cause organ-limited or systemic infections after disrupting of the natural defense mechanisms in a human organism. The treatment of mycoses still encounters considerable difficulties. Therefore, in vitro assessment of the susceptibility of fungal strains to the antifungal agents now in use and to new drugs is needed more urgently than ever before. It should be emphasized that we treat the fungal susceptibility to antifungal drugs as a quantitative feature of the strain examined. The aim of the presently reported study was the evaluation of the antimycotic action of two azole compounds – miconazole and itraconazole (Janssen) against 205 Candida strains isolated from the various biological specimens of two groups of patients – hospitalized (group 1) and examined in outpatient clinic (group 2); differentiation of species and codes of these strains; analysis of dose-response curves and parameters of polygons of the azoles minimal inhibitory concentrations (MIC). The susceptibility to miconazole and itraconazole was estimated with the agar diffusion test on 3% Sabouraud’s agar – the method developed in our laboratory, using several different concentrations of the drug, which made the plotting of dose-response curves possible. The lowest concentration inhibiting the growth of fungal strain (MIC) was calculated using a transformed equation of rectilinear regression according to Kadłubowski. Species and fungal codes of isolated strains were evaluated according to the guidelines worked out in our department with the use of different media and biochemical tests (bioMérieux). Among 89 strains isolated from the hospitalized patients, six species of the genus Candida genus were found; one strain belonged to Trichosporon cutaneum species. The most frequently encountered species was Candida albicans (73%), which significantly dominated over C. tropicalis, C. parapsilosis, C. glabrata, C. lipolytica and C. famata. All strains from the second group of patients belonged to C. albicans species. In all C. albicans strains from both groups of patients, the most frequent assimilation code (2576174) was found. The miconazole MIC values for Candida strains isolated from the group 1 were characterized by a wide range of variation, from 0.0247mg/l to 6.826 mg/l, from group 2 – 0.0277 to 0.719 mg/l. The itraconazole MIC values were 0.011 to 2.813 mg/l, and 0.0103 to 0.718 mg/l, respectively. The analysis of mean values (x) of miconazole and itraconazole MICs and other parameters allowed us to find that the strains isolated from the patients of group 1 were significantly less susceptible to both drugs in comparison with the strains of the group 2 patients. Also, C. albicans strains from this group of patients had a significantly lower (x) MIC in comparison to the mean values for the most of Candida species isolated from the hospitalized patients (P<0.001). In conclusion, we have found that the most Candida strains from both groups of patients were susceptible to the examined antifungal agents. The strains isolated from the outpatient clinic patients were generally more susceptible especially to itraconazole in comparison with strains from hospitalized patients.

It is imperative that pathogenic agents are identified accurately to the species level. The implementation of automatic systems for the identification of Candida yeasts is associated with a large number of tests performed by a given unit, which certainly has a further impact on the price of a single test and the use of devices. Hence, automatic systems are widely used in high-throughput laboratories, e.g. in commercial laboratories with a wide range of activities. Veterinary clinics with their own microbiological unit usually use classical identification methods or semi-automatic systems based on biochemical tests for independent interpretation of the results. Nevertheless, the increased number of invasive procedures, inherent in prolonged hospitalization of animals, and the use of broad-spectrum antibiotic therapy, leads to the conclusion that using automatic fungal identification systems in veterinary laboratories is only a matter of time, and their implementation may leave the classical techniques obsolete. This article provides an overview and discussion of automatic systems as well as new, rapid methods of diagnosing Candida fungi. Particular attention has been focused on Vitek YBC, Vitek 2 ID-YST, Abbot Quantum II, and Microbial ID systems.

Opis:
A rapid and accurate identification of the Candida is crucial for clinical treatment of local and systemic candidiasis. Premature diagnosis of invasive fungal infections is problematic because most clinical signs are non-specific and cultures are often negative or become positive too late for the initiation of effective antifungal therapy. Therefore, studies have been performed to improve molecular techniques for the diagnosis of candidiasis. Today, molecular strategies, such as PCR and non-PCR based methods are used to complement conventional laboratory approach. They provide accurate results in much short time of 1.5–3 h. Given the high accuracy and time saving with molecular typing techniques it is likely that most of these methods could improve routine clinical laboratory identification of Candida yeast species. However, further studies are needed for standardization of such procedures. This article presents an overview and discussion on molecular identification methods in the context of their application for the diagnosis of Candida fungi. Particular attention has been focused on the advantages and limitations of the indicated methods and the possibilities of their implementation for routine use in clinical laboratories.

The objective of the study was the analysis of selected parameters of humoral and cellular immunity in patients with Trichomonas tenax and fungi existing concurrently in the oral cavity ontocenosis. The study included 37 patients (age range 26-70 years, x=54.5±4.l) diagnosed with trichomonosomycosis. Standard methods were used to determine the total level of protein and its individual fractions, leukocytosis and sedimentation rate. Serum levels of immunoglobulins IgA, IgM and IgG, and the level of IgA immunoglobulin in saliva (S-IgA) were determined with the immunodilTussion method using NOR-Partigen and LC-Partigen (Behring) platelets. To define blastic and miotic indices (IB and IM), rosette tests ER, EAR and EACR as well as phytohemagglutinin-induced (PHA) blastic transformation test were employed. In 21.1% of patients, a decrease in total protein level (on the average 6.85±0.23 g/dl), and in 11.1% in globulins of y fraction (18.5±1.18%) was observed, in 40% of cases the leukocytes count was higher (10080±690.7), and in all patients the sedimentation rate was accelerated (77.9±12.2 mm). The level of class A immunoglobulins (368.7±49,0 mg%) was in 29.2% above norm, and of IgG class (1182.8±92.9 mg%) was in 13.6% below norm and in 13.6% above norm. In case of S-IgA (8.15±0.63 mg%), the level was below norm in all patients. Also in all patients a decreased lymphocytes T count (value ER - 37.3±1.04%) and deficiency in their function (indices: IB 22.5±2.98%, IM 2.3±0.84%) could be observed, whereas lymphocytes B count was increased (values EAR - 18.3±0.7%; values EACR -22.1-0.77). These data indicate that in cases of oral trichmonosomycosis there occurs a low level of secretory immunoglobulin class A in saliva and the number of lymphocytes T, deficient in function, is decreased.
