Opis:
The objective of this study was to describe spatial and temporal parameters during gait in pregnant women, and to compare it with women in post-partum and with a control group. Methods: To investigate alteration in natural locomotion, we used an electronic walkway (GAITRite system). Fifty-eight pregnant women (four last months of pregnancy), nine post-partum women and twenty-three healthy nulligravidae women participated in this study. The women performed the motor task at three different speeds: preferred, fast and slow. Spatial and temporal parameters for pregnant and non-pregnant were compared. Results: In pregnant women, gait speed, step length and cadence were reduced. Consequently, cycle time was longer. The gait cycle was modified by an increase of stance phase and a decrease of swing phase. As a result, an increase of double support and a decrease of single support phases were observed. Step width increased by 15%. Conclusions: The pattern of gait displayed significant modifications during pregnancy as compared to nulliparous women. These changes favour a more stable and safe gait. After childbirth, women kept some characteristics of gait in pregnancy during 8 months.

Badania biomechaniki chodu są cennym narzędziem przy projektowaniu i testowaniu obuwia oraz zaopatrzenia ortopedycznego. Zbudowany w IPS Oddział Kraków system do analizy chodu pozwala na wideogrametryczne określenie ruchu kończyn i ciała człowieka w trójwymiarowej przestrzeni.
The study of biomechanics of gait is a valuable tool for designing and testing of footwear and orthopedic devices. Built in IPS Krakow Branch system for gait analysis allows to determine via videogrammetry the movements of the limbs and the human body in three-dimensional space. The system includes treadmill, a set of two cameras at a predetermined distance, a set of passive markers and a laptop with Diopser program developed in the Institute. The program allows the determination of geometrical parameters between user-defined points. It is possible to track up to 20 markers placed on a person or object and calculate the length and angles of any geometries. The input is a set of two time-synchronized recordings, the output of the program is a spreadsheet file containing the selected measurements. Important factors that affect the correct designation of markers’ positions are resolution and recording, markers’ color and object’s illumination. The paper presents the capabilities of Diopser system, the way the measurements are made, interpretation of results obtained for test recordings and identifies areas of potential applications of the system.

Opis:
Purpose: Medial longitudinal arch (MLA) strengthening has been considered an important part of successful flatfoot treatment. But, to date, the biomechanical loading behavior of the medial arch in flatfoot has not been evaluated. This study aimed to evaluate the MLA moment, MLA deformation angle, foot kinematics and ground reaction forces (GRF) in both normal foot and flatfoot groups. Methods: Each participant’s foot was classified according to arch type using foot prints and radiographs. Twenty-eight non-obese adults (13 flatfeet and 15 normal feet) were involved. The biomechanics data were collected in a 3D motion analysis laboratory. The MLA biomechanics were calculated. Hindfoot and forefoot kinematics were also analyzed. Results: The flatfoot group had a significantly greater peak eversion MLA moment ( p = 0.005) and a smaller peak MLA deformation angle ( p < 0.05) during specific subphases. The peak of hindfoot plantarflexion ( p < 0.05) and internal rotation ( p < 0.05) and the peak of forefoot abduction ( p < 0.05) in the specific subphases were greater in the flatfoot group. The flatfoot group also had significantly smaller peak vertical GRF ( p < 0.05) during late stance and larger peak medial GRF ( p < 0.05) during mid stance. Conclusions: This study found a significantly greater eversion deforming force acting at the MLA structure, greater hindfoot and forefoot motion, less MLA flexibility and abnormal GRF in a flatfoot group during walking, which reflected the deficit of foot function in a flatfoot group.

Purpose: Footwear and equipment worn by military personnel is of importance for them to be able to meet the physical demands specific to their profession daily activities. Aim of the present study was to investigate by means of gait analysis how army-provided footwear and equipment influence the range of motion of hip, knee and ankle joints as well as stride length. Methods: Thirty-two soldiers were subjected to gait analysis on a treadmill by way of video recordings and goniometric measurements. Results: The stride length increased when military shoes are worn. We found no influence on stride length in connection to increased loading. The weight of the shoes represents the decisive factor. Neither shoes nor equipment changed the range of motion of the knee joint.Weight of equipment affected range of motion of the hip joint. The range of motion of the upper and lower ankle joints was mainly influenced by the properties of the shoes. Conclusions: Military footwear and weight of equipment influence stride length and range of motion of joints of the lower extremities in a specific way. Shape of material is the decisive factor.

Opis:
Celem pracy jest określenie wpływu różnych ustawień ortezy typu Walker na zmianę kątów w stawach kończyn dolnych podczas chodu. W badaniach wzięło udział 10 zdrowych osób. Do badań czterech rodzajów chodu -jeden bez ortezy i trzy w różnych ustawieniach ortezy wykorzystano system Vicon.
The aim of the work is to determine the effect of various settings of Walker type orthosis on lower limb kinematics parameters during walking. 10 healthy people participated in the study. The Vicon system was used to test four types of the gait - one without an orthosis and three in different orthosis settings. It has been proven that the way the orthosis is positioned has a significant impact on changing the angles in the knee and hip joints of the lower limb, on which the orthosis is worn. The absence of the orthotic effect on the kinematics of the limb free from orthosis is very beneficial.

Opis:
Purpose: The kinematic analysis of gait during pregnancy provides more information about the anatomical changes and contributes to exercise and rehabilitation prescription. The purposes were to describe and quantify the spatial, temporal and kinematic parameters in the joints of the lower limb during gait at the end of the first, second and third trimesters of pregnancy and in the postpartum period. Methods: A three-dimensional analysis was performed in eleven pregnant women. Repeated measures ANOVA was performed for comparisons between periods. Results: The longitudinal effect of pregnancy was not observed in spatial and temporal parameters. In joint kinematics the effect of pregnancy was observed in all joints for the three planes of motion. The hip joint and pelvis are the structures with more changes, and the results point to an increase in the anterior tilt of the pelvis as the pregnancy progresses, as well as a decrease of the amplitudes of the hip joint. The results suggested that pregnant women need to maintain the stability of the body, and become more efficient in locomotion. Conclusions: In general, the results retrieve the values from the beginning of pregnancy, indicating that the body was self-organized in order to overcome the morphological and physiological changes which women suffer during pregnancy, indicating that they have the ability to adapt depending on the demands, and after the effect of pregnancy is over, they return to values similar to those found in early pregnancy

Opis:
Purpose: Children with cerebral palsy (CP) often use a crouch gait pattern that has disturbances in the knee joint kinematics. Although the length and rate of lengthening of the hamstring musculature have been speculated to be the reason that these disturbances are not adequately dissipated, this relationship has not been adequately explored. The purpose of this exploratory investigation was to use simulations of a musculoskeletal model and Floquet analysis to evaluate how the performance of hamstrings musculature during gait may be related to the knee joint instabilities seen in children with CP. Methods: Children with CP and typically developing (TD) children walked on a treadmill as a motion capture system assessed the knee joint kinematics. Floquet analysis was used to quantify the rate that disturbances present at the knee joint were dissipated, and simulations of a musculoskeletal model were used to estimate the in vivo length and velocity of the hamstrings. Pearson correlation coefficients were calculated to determine if there was a relationship between the rate that the disturbances were dissipated and the performance of the hamstring musculature. Results: The children with CP had hamstrings that lengthened more slowly than TD children, and required more strides to dissipate disturbances in the knee joint kinematics. There was negative correlation between the rate that the hamstrings lengthened and the rate that the knee joint disturbances were dissipated. Conclusions: Our results suggest that the ability of children with CP to dissipate the knee joint disturbances may be related to the inability to properly control the hamstring musculature.

Opis:
Ballet is an art that puts extreme demands on the dancer’s musculoskeletal system and therefore significantly affects motor behavior of the dancers. The aim of our research was to compare plantar pressure distribution during stance phase of gait between a group of professional ballet dancers and non-dancers. Thirteen professional dancers (5 men, 8 women; mean age of 24.1 ± 3.8 years) and 13 nondancers (5 men, 8 women; mean age of 26.1 ± 5.3 years) participated in this study. Foot pressure analysis during gait was collected using a 2 m pressure plate. The participants were instructed to walk across the platform at a self-selected pace barefoot. Three gait cycles were necessary for the data analysis. The results revealed higher ( p < 0.05) pressure peaks in medial edge of forefoot during gait for dancers in comparison with nondancers. Furthermore, differences in total foot loading and foot loading duration of rearfoot was higher ( p < 0.05) in dancers as well. We can attribute these differences to long-term and intensive dancing exercises that can change the dancer’s gait stereotype.

Purpose: Patient recovery after a surgical procedure depends, among other factors, on the amount of the body weight with which patient loads lower limb. Research studies report different results of the degree of body weight with which lower limb is loaded during three-point crutch gait. The aim of this study was to evaluate the level of the ground reaction forces (GRF) during crutch gait used by patients after total hip arthroplasty (THA) in the first week after discharge from the orthopaedic units. Methods: Ten female patients diagnosed with primary unilateral coxarthrosis participated in a single measurement session. In order to record kinematic and dynamic variables of this gait pattern motion analysis system was used together with two force plates. The static test of body weight distribution between lower limbs was performed on a dual-top stabilometric plate. Results: The average peak values of loading on the operated (O) limb during mid stance and terminal stance of three-point crutch gait were 64.6% and 64.3% of body weight (BW), respectively, whereas in the case of the nonoperated (NO) limb 103.5%BW and 108.8%BW, respectively. The maximum loads on the crutches were significantly higher (by 9%BW) on the NO side as compared to the O side ( p < 0.05). During the static test, average values of body weight distribution on the O and NO limb were 36%BW and 64%BW, respectively. Conclusions: The patients showed surprisingly similar level of loading on the O limb. The weight bearing on the O limb was lower during static trial than during three-point crutch gait.

Opis:
Stroke-related hemiplegia is an important factor influencing parameters of gait. So far, limited papers have assessed temporo-spatial capabilities and their correlations with gait parameters in the early post-stroke stage. This pilot study evaluated the temporospatial parameters of gait and assessed the maximal isometric and isokinetic torque production of the plantar flexor and dorsiflexor muscles. Methods: 15 patients with lower limb spasticity and 15 healthy controls were included. Stroke severity was assessed using the Modified Ashworth Scale and the Barthel Index. Gait cadence, gait speed, and gait cycle were assessed using inertial sensors during a Timed Up and Go test. Maximal isometric and isokinetic torque production of the ankle plantar flexor and dorsiflexor muscles were assessed using an isokinetic dynamometer device. Results: Post-stroke patients had statistically significantly lower gait cadence than healthy participants (17%, p < 0.05). Statistically significantly lower values of vertical acceleration were also noted during a sit-to-stand movement task (42%, p < 0.05). Plantar flexion torque of the affected limb was significantly different during isometric (63%, p ≤ 0.01) and isokinetic work for 30o /s (49%, p = 0.04), 60o /s (58%, p = 0.01) and 20 °/s (53%, p = 0.01). Dorsiflexor muscles’ torque production was significantly different in isometric activity (38%, p = 0.04). A statistically significant positive correlation occurred between the absolute peak torque of the dorsiflexor muscles in both static and speed phases of gait (Rs = 0.65, p = 0.04). Conclusions: Despite the low intensity of spasticity and early phase after stroke, differences in the muscle torque production and temporo-spatial parameters, as well as the correlations between them, were noticeable.
