The paper is the first part of the probabilistic general model of critical infrastructure accident consequences including the process of initiating events, the process of environment threats and the process of environment degradation models. Basic notions concerned with the events initiating dangerous for the environment after the critical infrastructure accident or its loss of safety critical level are introduced. The methods and procedures of estimating the process of initiating events unknown basic parameters and identifying the distributions of its conditional sojourn times at its states are proposed. Under these all assumptions from the constructed model and after its unknown parameters identification, the main characteristics of the process of the initiating events are predicted. Finally, the proposed model and methods are applied to modelling, identification and prediction of the process of initial events generated by the critical infrastructure defined as a ship operating in the Baltic Sea basin.

We have already examined the worldwide trends for outcomes (measured as accidents, errors and events) using data available for large complex technological systems with human involvement. That analysis was a dissection of the basic available, published data on real and measured risks, for trends and inter-comparisons of outcome rates. We found and showed how all the data agreed with the learning theory when the accumulated experience is accounted for. Here, learning includes both positive and negative feedback, directly or indirectly, as a result of prior outcomes or experience gained, in both the organizational and individual contexts. Our purpose here and now is to try to introduce some predictability and insight into the risk or occurrence of these apparently random events. In seeking such a general risk prediction we adopt a fundamental theoretical approach that is and must be testable against the world’s existing data. Comparisons with outcome error data from the world’s commercial airlines, the two shuttle failures, and from nuclear plant operator transient control behaviour, show a reasonable level of accord. The results demonstrate that the risk is dynamic, and that it may be predicted using the MERE learning hypothesis and the minimum failure rate, and can be utilized for predictive risk analysis purposes.

Internal and external hazards analysis methods have been used recently to evaluate operating nuclear power plants (NPP) and to identify the resilience of NPP in case of hazards and potential needs for modification of plant systems and procedures as well as to support design of new plants The external hazards risk contribution has been modeled in many NPP PSA (at least for some external hazards) over the last decade, including events occurring during shutdown and low power operations. Recently, a revision of the German nuclear safety regulations has been successfully completed and these regulations entitled “Safety Requirements for Nuclear Power Plants”, also requiring a detailed investigation of internal and external hazards and combinations of different hazards applying deterministic and probabilistic safety assessment methods.

Assessing the Occurrence Probability of a given sequence of events in a determined order is necessary in many scientific fields. That is the case in the following fields: nucleation and microstructure growth in materials, Narrow-Band process, financial risk analysis, Sequential detection theory, rainfall modelling, in optics to model the sequences of photoelectrons under detection, population biology, software reliability, queuing in network traffic exhibiting long-range dependence behaviour, and DNA sequences and gene time expression modelling. However, the topic has a particular interest in the field of risk, safety and maintenance assessments. The lecture will focus on sequences composed of Double Stochastic Poisson Processes.

This paper presents a probabilistic model of hazard-related interdependence between the operations carried out in the ports of the Baltic Sea region and in their neighborhoods. Each single operation, considered w.r.t. its hazardous aspect, will be defined as a point process consisting of undesired events (emergencies and/or accidents). Thus, the interdependence between these processes can be regarded as interaction between such events. The developed model will specify the impact of hazard related events occurring within one process on the risk of occurrence of such events in the other processes. This model will be a basis for the analysis of inter-process dependencies, including the feedback and cascading effects, as implied by the cause-effect relationships between the events occurring in different processes. Furthermore, it is envisaged to be used for assessing the potential effects of accidents or catastrophic events, and for developing the appropriate prevention measures. The procedures derived from the model will be applied to analyzing the mutual impacts between the processes realized in the oil and container terminals, forecasting negative effects of these impacts along with assessing their costs, and planning preventive actions aimed at avoiding such effects.

This paper is a continuation of [1] which presents a probabilistic model of hazard-related interactions between different operations carried out in a (generic) Baltic Sea Region port area. Each such operation, considering its hazardous aspect, is defined as a series of undesired events (emergencies and/or accidents) occurring at random instants, i.e. as a random process. An event can be primary (occurring by itself) or secondary (caused by another event in the same or another process). The processes interact in the sense that a primary event in one process can cause a cascade of events spanning multiple processes. In [1] the formulas were derived for the cause-effect probabilities expressing the impact of a single event on the occurrence of the ensuing events in the triggered cascade. Also, the formulas for risks of undesired events, using these probabilities, were obtained. As these formulas are complicated and difficult to implement numerically, the need arose to develop a simple tool for computing the considered risks. Such a tool, in the form of an easy-toimplement algorithm, along with an illustrative example is presented in the current work.

This paper is both a summarization and extension of [6] and [7], where a stochastic model of interacting operations carried out in a generic Baltic Sea Region port was proposed and analyzed. Each operation involves a number of possible unwanted events (critical incidents) whose instances occur randomly and can cause instances of other events affecting this or other operations. This can lead to a cause-effect chain of events affecting one or multiple operations. The model presented in [6] is somewhat complex, therefore it was downgraded to a simpler, application-oriented version demonstrated in [7], where an algorithm computing the risks of critical incidents is constructed and then applied to a real-life example. The current paper, apart from presenting a method of computing the risks of critical incidents, occurring by themselves or resulting from the cascade effect, also features a method of root-cause analysis of such incidents. First, the formulas for the root-cause probabilities are derived, where such a probability quantifies the likelihood that a critical incident occurring in step h of a cascade was caused by another incident that initiated this cascade. Second, an algorithm computing the root-cause probabilities, based on the derived formulas, is constructed. This algorithm is illustrated by its application to the example given in [7]. The presented results can be used as a tool for fault propagation analysis and fault diagnosis applied not only to a port environment, but to any complex industrial system.
