Opis:
Introduction. Adverse pregnancy outcomes are less common among physically active women, and children born to such mothers are less likely to be at risk for macrosomia, obesity and metabolic diseases in the future. Objectives. The aims of the study were to establish physical activity (PA) patterns among pregnant women in the third trimester, and to determine the attitudes of prenatal care providers to maternal PA during pregnancy. Materials and method. The study was conducted in 2017 using surveys from the Polish Pregnancy-related Assessment Monitoring System program (Pol-PrAMS). The study included 3,451 postpartum women. The Pregnancy Physical Activity Questionnaire was used to investigate their PA. This part of Pol-PrAMS study was completed by 2,744 postpartum women who were subjected to statistical analysis. Results. Sedentary or light physical activity comprised 75% of the overall PA in the third trimester of pregnancy (mean values of energy expenditure: 67 and 93.3 MET-h/week, respectively). Household or caregiving activities accounted for almost 50% of all activities and were the most common PA types (mean energy expenditure: 105 MET-h/week). Restriction of PA in pregnancy was reported by over 60% of the women, most often due to concerns over proper foetal development. Over 85% of prenatal care providers either did not address the issue of PA with the future mothers at all, or recommended PA restriction. Conclusions. Sedentary and light-intensity PA are the two predominant types of physical activity in the third trimester. The most energy-consuming tasks involve household and caregiving activities. Restriction of activity was reported by the majority of the respondents. Prenatal care providers either did not address the matter of PA in pregnancy or recommended PA restriction.

Introduction. An adequate mode of nutrition is among the most important environmental factors affecting the development of Man and maintenance of a good health status. An improper selection of nutrients and irregular consumption of meals may lead to overweight and obesity. Objective. The characteristics of health behaviours of the examined population of schoolchildren, with consideration of nutrition and body weight disorders. A comparison of the opinions of schoolchildren and their parents concerning health behaviours. Development of guidelines for educational programmes carried out in the place of residence of the population of schoolchildren and their parents. Materials and method. The survey covered a randomised group of schoolchildren attending elementary and secondary schools in the Kalisz Province and province of the city of Kalisz. The study was conducted in May and June 2009, in a randomly selected representative group of 1,100 boys and girls from classes V and VI of elementary schools, and 1,100 secondary school adolescents aged 16–19 and their parents. The studies of schoolchildren attending elementary and secondary schools were compared with the all-Polish studies of junior high school adolescents in the school year 2006–2007. Results. The respondents most often consumed 3–4 meals; however, as many as 26% of junior high school adolescents and 27% of secondary school adolescents admitted that they consume only one meal daily. The schoolchildren show inadequate nutritional habits concerning an insufficient consumption of fruits, vegetables and fish, in favour of high calorific meals and sweet snacks and drinks. Parents improperly assess the body weight of their children and perceive them as slimmer, which is not confirmed by the BMI value for age and gender. Conclusions. Systematic monitoring and analysis of changes in the health behaviours of adolescents should be a basis for planning health education and promotion programmes. Educational programmes concerning various aspects of health should be implemented in an organized and complementary way, directed not only at schools, but also at entire families and local communities. Knowledge, beliefs, skills and attitudes towards health acquired during the period of adolescence decide about life style in adulthood.

Opis:
Introduction. The dynamic rise of alcohol consumption in adolescents is a matter of serious concern, requiring frequently updated monitoring. By such means, it is possible to optimise preventative measures for dealing with this problem. Objective. To estimate the magnitude/amount and frequency of alcohol consumed by middle and high school pupils in Poland, including the circumstances when alcohol was first drunk. Materials and Methods. A randomised survey was performed throughout Poland on middle school (junior high school) pupils, (n=9360) in 2009, followed by both middle and high school pupils in 2011 (n=7971). The questionnaire was devised by the Polish Chief Sanitary Inspectorate (GIS). Results. A strikingly sharp increase in alcohol consumption (29%) was observed in subjects between 2009 – 2011. In the latter year, 1 month prior to survey, respectively, 50% and 71% of middle school and high school pupils drank alcohol, and correspondingly, 36% and 63% of these pupils ever became intoxicated/drunk. Conclusions. 1) Adolescent alcohol consumption increases with age and is highest in girls. Monitoring as well as in-depth analysis thus becomes necessary. 2) Systematic monitoring and analysis of changing healthy lifestyle behaviour should be used for taking the necessary corrective action. This should happen concurrently and consist of planned health education programmes, including health promotion.

Opis:
Epidemiological studies in Poland show that tobacco smoking by adolescents at reproductive age is still frequently observed. This concerns both boys and girls. The study was based on all-Polish population studies of health behaviours of adolescents aged 14-24 (Youth Behavioural Polish Survey – YBPS) conducted in 2011, and the Pregnancy-related Assessment Monitoring Survey (PrAMS). More than 12% of pregnant women do not discontinue smoking in association with becoming pregnant and expose the foetus to tobacco smoke, despite being aware of the hazardous effect of smoking on the health of the mother and child. Smoking in pregnancy is mainly observed among mothers with a low education level and those aged under 23. According to the Baker’s Foetal Origins of Adult Health and Diseases Hypothesis, exposure of the foetus to the components of tobacco smoke results in many perturbations in the form of a lower birth weight, prematurity, worse state of neonates after birth, and higher susceptibility to contacting civilization diseases at the age of adulthood. The results of studies confirm some observations. Polish studies clearly confirmed a lower birth weight of babies delivered by mothers who smoke; however, earlier termination of pregnancy and worse status of neonates after birth were not observed. According to the Baker’s hypothesis, a lower birth weight of babies delivered by smoking mothers during the further life cycle exposes the offspring to the risk of contracting civilization diseases. The efforts undertaken by public health authorities should be biased towards education of the population at reproductive age about the hazardous effect of smoking on the health of the foetus and the offspring born. Women at reproductive age should be encouraged to discontinue smoking in association with the planning of pregnancy and in pregnancy.

Opis:
It is recognized that the levels of women’s physical activity during pregnancy has a direct bearing on the method of delivery and health of the newborn. The main objective of the study was investigation the level of physical activity of women during pregnancy according to the Pregnancy Physical Activity Questionnaire (PPAQ). The subjects of this study were n=2852 post-partum women surveyed together with their newborns, representing all obstetric hospital departments throughout Poland. The questionnaires were completed on a single day during the second week of November 2011. The women were also asked about the amount, range and type of physical activity they performed before becoming pregnant. The Pregnancy Physical Activity Questionnaire was used for precisely measuring physical activity according to the standard metabolically equivalent (MET). In addition, comparisons were made between the weight of the infant and newborn status using the APGAR scale with the amount of physical activity performed by the mothers. There were decidedly low physical activity levels observed in pregnant women compared to those before becoming pregnant. Appropriate interventions can therefore now be targeted through remedial action in Poland. It is important to perform intervention studies intended to test this hypothesis and attempt to identify the most appropriate levels for intensity, duration and frequency of physical exercise during pregnancy. The studies should consider the four domains of daily physical activity and utilize tools that reliably measure exposure variables. Such studies would provide valuable information for recommendations about physical activity during pregnancy.

Opis:
Shiga toxin producing Escherichia coli (STEC) are the most virulent diarrhoeagenic E. coli known to date. They can spread with alarming ease via the food chain, as recently demonstrated by the large outbreak of STEC O104:H4 borne by sprouted seeds in 2011, clustered in northern Germany, and subsequently affecting other countries. Indeed, a significant number of infections to verocytotoxin producing Escherichia coli O104:H4 have been reported from the WHO European Region resulting in many cases of bloody diarrhoea and haemolytic uraemic syndrome in Germany, 15 other European countries and North America. Eventually, the European Food Standards Agency, (EFSA), identified the likely source to a single consignment of fenugreek seeds from an Egyptian exporter as being linked to the two outbreaks in Germany and France. The situation was closely monitored by the Chief Sanitary Inspectorate public health authority in Poland where actions undertaken ensured that the public was well informed about the dangers of STEC contamination of food, how to avoid infection, and what to do if infected. Tracing the fenugreek distributors also enabled the identification of suspected batches and their isolation. As a result, there were very few reported cases of STEC infection in Poland. Effective control over such outbreaks is therefore a vital public health task. This should include early detection and rapid identification of the contagion mode, followed by removing the foodstuff(s) from the market, providing consumer advice, and preventing secondary spreading. As a mitigation measure, screening/monitoring those involved in food handling is also warranted to exclude carriers who can be asymptomatic.

Introduction. Physical activity is one of the basic needs of people throughout their lives. Physical activity prevents many diseases, including cardiovascular diseases, which have the highest mortality rate, both among urban and rural populations in Poland. The average life expectancy in Poland and the European Union is increasing. The proportion of children and adolescents under the age of 20 is decreasing slowly but steadily, while the proportion of older people (aged 65+) in the total population is gradually increasing. According to the forecast of the Central Statistical Office, the percentage of older people increased rapidly after 2010, and will reach 22.3% in 2030. The number of the elderly will increase from 5,134,000 in 2007 to 8,195,000 in 2030. The health situation of Poles is less favourable compared to other European Union states. The most common health risks are: nicotine, drug addiction, alcoholism and obesity. One of the causes of obesity is an unhealthy lifestyle. Lifestyle is a basic determinant of human health. Objective. The aim of this study is to present research results based on a literature review on the importance of physical activity for human health as an element of organizing and managing the lifestyle of urban and rural populations. Conclusions. Comprehensive measures should be taken to increase public health awareness and promote health education, especially among school-age children and youths, and to provide appropriate sports infrastructure in particularly neglected rural areas.

Introduction and objective. Many studies have indicated numerous nutrition mistakes among school-aged children and adolescents in both urban and rural environments. The aim of this study was to assess the nutritional habits of the Polish population, consisting of 7,974 individuals aged 12–17, from rural and urban environments, as well as to identify environmental variations of these habits and to verify the existing information on the incorrect nutrition of school-age children and adolescents. Materials and method. The research covered a group of 7,974 respondents – school-age adolescents with a similar age structure (12–17 years). The study on subjects from secondary school grades 1 – 3 was conducted in randomly selected schools from 2 random Polish provinces; 5 counties were randomly selected, followed by a choice of 2 communes: one rural and one urban. The research technique was a self-designed survey questionnaire. The obtained results were subjected to statistical analysis using the Pearson Chi 2 and V Cramer test. Results. The research revealed environment-based differences in subjects’ nutrition. Breakfast was consumed daily by a statistically significantly fewer subjects from the rural environment (36.31%) than from the urban areas (51.32%); second breakfast was consumed by an insignificantly smaller proportion of respondents from the urban environment (40.00%) than from the rural one (46.00%); dinner was eaten daily by 86.00% of urban subjects and 82.00% of rural respondents; afternoon tea and supper were eaten rarely by respondents from both environments. The diet of respondents was dominated by anti-health behaviours. Conclusions. Most of the respondents displayed incorrect nutritional behaviours. Nutritional mistakes occurred among respondents from both rural and urban environments, with the predominance of the rural areas.

Introduction and objective. As landowners occupied with agricultural production comprise a sizeable part of the populations in mid- and western European countries, it seemed reasonable to assess the organization of health care systems concerning farmers and their families in Poland and Germany. Both countries have similar geographical conditions and rural environments. It so happens that in Poland the principles of the system of agricultural insurance (KRUS) is based on the experiences of Germany and France. State of knowledge. Basically, both in Poland and Germany, the agricultural health insurance companies provide the same insurance cover as other health insurance companies. In both countries, under certain conditions, in the case of illness, the insured farmers receive instead of sickness benefit operational assistance and home help. In spite of the similarities that characterize both administrations, many particular differences are to be noted, e.g. the farmers’ social insurance in Poland is subject to only one ministry, in contrast to Germany where two ministries are responsible for farmers’ social insurance. In Poland, KRUS is a monopolistic organization, whereas in Germany, nine similar independent structures fulfil the task of a health insurance company. Needless to say, many more funds are available for prevention, treatment and rehabilitation in Germany than in Poland, due to obvious differences in the overall national income.
