Chronic heart failure (CHF) is a complex disease process connected with cardiovascular system as well as other organs and skeletal muscles. In connection with the above, cardiac rehabilitation, consisting of exercise training and diet supplementation, aims at recovery of physical, social and psychic function and removing risk factors influencing the occurrence of circulatory system diseases. Evidence has shown that exercise training in CHF patients, both aerobic and resistance, can increase peak oxygen consumption and exercise capacity, improve NYHA (New York Heart Association) functional class, reduce mortality and improve the quality of life. Evidence suggests that most improvement is due to the effects of training on the peripheral circulation and skeletal muscle, rather than on the heart itself. Exercise training can improve skeletal muscle metabolism, increase blood flow within the active skeletal muscles, increase capillary density, promote the synthesis and release of nitric oxide, improve angiogenesis, and decrease oxidative stress. Physical effort reduces sympathetic arousal and increases parasympathetic arousal, thus reducing cardiac dysrythmia and ischemia. Mitochondria start working harder, as the demand for energy is higher and electron flow provides energy in the form of ATP. Studies have consistently demonstrated that exercise training is safe and has no deleterious effect on central haemodynamics, left ventricular remodeling, systolic or diastolic function, or myocardial metabolism. Taking several supplements that have documented roles in medical therapy, including vitamins B, C and E, coenzyme Q10, alpha-lipoic acid, chromium, omega-3 polyunsaturated fatty acids, L-carnitine, and quercetin, has beneficiary effect on many diseases, including CHF. In our experience, 19 patients with CHF who undertook resistance (weight) training and food supplementation, returned to their normal activities after 4 months, without any complaints.

Opis:
Leptospirosis is a widespread although recently neglected zoonosis recognized worldwide. The disease seems to be underestimated, especially in countries located in the temperate climatic zone. The presented article concerns the main characteristics of leptospirosis and describes formerly known and recently observed environmental, occupational and recreational risk factors significant in the spreading and pathogenesis of the disease. The aspects of epidemiology significant in the temperate climatic zone are emphasized. The majority of cited articles present cases of the disease reported from Europe or North America. Climatic changes (warming) and extreme weather events such as floods are potential risk factors of leptospirosis. Also, some socio-economic phenomena, such as the intensive migration of people resulting in the transfer of the infections acquired in tropical countries, or worsening of economic status in the cities, increase the probability of disease. Apart from the danger connected with rodents, which are the main vectors of leptospires, occurrence of the disease in dogs and cats can generate a higher risk of infection for humans. Infections may also be acquired during various types of agricultural work and during recreational activities, such as swimming. The results of recent investigations show that ticks are also potential vectors of leptospires. The more frequent emergence of leptospirosis in countries located in the temperate climatic zone emphasize the need to verify knowledge related to the risk of its appearance, and to consider this disease during diagnostic processes.

During combine harvesting of 5 various cereal crops (rye, barley, oats, buckwheat, corn) 24 samples of grain and 24 samples of settled grain dust were collected on farms located in the Lublin province of eastern Poland. The samples were examined for the concentration of total microfungi, Fusarium species, deoxynivalenol (DON), nivalenol (NIV), and ochratoxin A (OTA). Microfungi able to grow on malt agar were present in 79.2% of grain samples and in 91.7% of grain dust samples in the concentrations of 1.0-801.3 × 103 cfu/g and 1.5-12440.0 × 103 cfu/g, respectively. The concentration of microfungi in grain dust samples was signifi cantly greater than in grain samples (p<0.01). Fusarium strains were isolated from 54.2% of grain samples and from 58.3% of grain dust samples in the concentrations of 0.1-375.0 × 103 cfu/g and 4.0-7,700.0 × 103 cfu/g, respectively. They were found in all samples of grain and grain dust from rye, barley and corn, but only in 0-16.7% of samples of grain and grain dust from oats and buckwheat. DON was found in 79.2% of grain samples and in 100% of grain dust samples in the concentrations of 0.001-0.18 μg/g and 0.006-0.283 μg/g, respectively. NIV was detected in 62.5% of grain samples and in 94.4% of grain dust samples in the concentrations of 0.004-0.502 μg/g and 0.005-0.339 μg/g, respectively. OTA was detected in 58.3% of grain samples and in 91.7% of grain dust samples in the concentrations of 0.00039- 0.00195 μg/g and 0.00036-0.00285 μg/g, respectively. The concentrations of DON, total fusariotoxins (DON + NIV) and OTA were signifi cantly greater in grain dust samples than in grain samples (p<0.05, p<0.05, and p<0.001, respectively). The concentration of Fusarium poae in the samples of rye grain and dust was signifi cantly correlated with the concentrations of DON (p<0.05), NIV (p<0.01), and total fusariotoxins (p<0.05). Similarly, the concentration of Fusarium culmorum in the samples of barley grain and dust was signifi cantly correlated with the concentration of total fusariotoxins (p<0.05). A signifi cant correlation was also found between the concentration of total fungi grown on malt agar and the concentration of OTA (p<0.05). In conclusion, although the concentration of DON, NIV and OTA in the samples of grain dust collected from 5 various cereals on farms in eastern Poland was not large, the persistent presence of these mycotoxins in over 90% of examined samples poses a potential health risk of chronic respiratory intoxication for exposed grain farmers.

A total of 123 water samples were examined in parallel by culture and semi-nested PCR for the presence of Legionella. They comprised: 35 samples of hot water distributed by the urban municipal water supply system (MWSS) taken in institutions, 45 samples of hot water distributed by urban MWSS taken in dwellings, 27 samples of cold water distributed by rural MWSS taken in dwellings, and 16 samples of cold well water taken in rural areas. The greatest frequency of the isolation of Legionella by culture (88.6%) was recorded in the samples of hot water from the urban institutions, having been greater compared to all other sources (p<0.001). The frequency of Legionella isolation from hot water in urban dwellings (28.9%) was significantly greater compared to the combined value (2.3%) for cold water from rural MWSS and wells (p<0.001). Strains belonging to Legionella pneumophila serogroups 2-14 predominated in the examined samples, while strains of L. pneumophila serogroup 1 and strains of Legionella spp. (other than L. pneumophila) were 3-fold less numerous. The rates of positive findings in the semi-nested PCR (stage 2) were greater than culture isolations in all kinds of samples, except for urban institutions. The correlation between the culture and PCR results was positive for samples of hot water from urban MWSS (p<0.01), but not for samples of cold water from rural MWSS and wells (p>0.5). A significant correlation was found between rates of PCRpositive results and numbers of Legionella pneumophila serogroups 2-14 strains, but not for other Legionella serogroups or species. In conclusion, our results support the opinion that though PCR cannot be a substitute for the isolation of Legionella by culture, it could be regarded as an useful complementary method.

The aim of the present study was: – to compare methods for concentration and isolation of Legionella DNA from water; – to examine the efficacy of various modifications of PCR test (PCR, semi-nested PCR, and real-time PCR) for the detection of known numbers of Legionella pneumophila in water samples artificially contaminated with the strain of this bacterium and in randomly selected samples of environmental water, in parallel with examination by culture. It was found that filtration is much more effective than centrifugation for the concentration of DNA in water samples, and that the Qiamp DNA Mini-Kit is the most efficient for isolation of Legionella DNA from water. The semi-nested PCR and real-time PCR proved to be the most sensitive methods for detection of Legionella DNA in water samples. Both PCR modifications showed a high correlation with recovery of Legionella by culture (p<0.01), while no correlation occurred between the results of one-stage PCR and culture (p>0.1).
