Objectives: The Centers for Disease Control and Prevention (CDC) as well as the World Health Organization (WHO) state that adequate hand hygiene maintained by medical personnel is an indispensable prerequisite for controlling nosocomial infections. The recommendations of CDC and WHO emphasize the obligation to wash hands after each contact with a patient, after the exposure to a potentially infectious material or upon the contact with objects surrounding the patient. Materials and Methods: The study was performed by quasi-observation among the group of 188 medical staff members (nurses and physicians) working in three selected hospitals of the Łódź Province. The procedure of hand washing/disinfection performed directly after the patient contact according to the recommendations of CDC and WHO was observed. The results were subject to statistical analysis (p < 0.05). Results: During 1544 h of observations, 4101 activities requiring hand washing were recorded. The medical personnel followed the hand hygiene procedures after the patient contact in 26.4% of the situations that require hygiene according to the guidelines. The level of observance of the hand washing procedures depended significantly on the type of performed activity, profession, degree of workload, index of activity, and time of duty hours. The mean time of hand washing after patient contact was 9.2 s for physicians and 6.7 s for nurses. Conclusion: Both the level of observance of hygienic procedures after the contact with patients as well as the time of hand washing are insufficient. There is an urgent need to work out educational programs on maintaining proper hand hygiene for medical personnel.

Introduction: To minimize the risk of cross-infection between the patient and the medical staff, it is necessary to use individual protective measures such as gloves. According to the recommendations of the Centers for Disease Control and Prevention (CDC) and the World Health Organization (WHO), protective gloves should always be used upon contact with blood, mucosa, injured skin or other potentially infectious material. Materials and Methods: The aim of the study was to evaluate, through quasi-observation, the use of protective gloves by medical staff according to the guidelines issued by the CDC and WHO. The results were subject to statistical analysis (p < 0.05). Results: During 1544 hours of observations, 3498 situations were recorded in which wearing protective gloves is demanded from the medical staff. The overall percentage of the observance of using gloves was 50%. The use of gloves depended significantly on the type of ward, profession, performed activity, number of situations that require wearing gloves during the observation unit and the real workload. During the entire study, as many as 718 contacts with patients were observed in which the same gloves were used several times. Conclusion: Wearing disposable protective gloves by the medical staff is insufficient.

Introduction: According to the Centers for Disease Control and Prevention (CDC) as well as the World Health Organization (WHO) recommendations, medical staff are obliged to decontaminate the skin of the hands before every single patient contact. Materials and Methods: The study was performed by quasi-observation among the group of 188 medical staff (nurses and physicians) working in three selected hospitals of the Łódź province. The procedure of hand washing and disinfection performed directly before the patient contact according to the CDC and WHO recommendations were observed. The results was subject to statistical analysis (p < 0.05). Results: During 1544 hours of observation 4101 activities requiring hand washing were recorded. The medical staff obeyed the hand washing procedure before the patient contact only in 5.2% of the situations. There was no activity observed before which hand hygiene was maintained in 100% of cases. Observance of hand hygiene depended signifi cantly on the type of the performed activity, the professional group, and the workload index. A decrease in percentage observance of hand hygiene according to the time of the day was found to be of statistical signifi cance. The mean time of hand washing was 8.5 s for physicians and 6.6 s for nurses. Conclusion: The level of observance of hand washing procedures among the medical staff prior to the patient contact appears to be alarmingly below the expectations.

Objectives More and more countries introduce a total ban on smoking tobacco in public places. The aim of this work was to evaluate the effectiveness of “The Act of 8 April 2010 on amendment of the act on protection of health against the consequences of consumption of tobacco and tobacco products and act on National Sanitary Inspectorate” and assess the frequency of complying with the smoking bans by the students of the Medical University of Lodz, Poland. Material and Methods Between 2007–2011, at the Social Medicine Institute of the Medical University of Lodz, a study using random survey was carried out involving students who were starting their studies at the Health Department of the Medical University of Lodz. The analysis of the collected material showed that 1038 people reported being smokers at the time of the study. Among that group, 530 students were included in the study prior to, and 508 after the introduction of the amendment. In order to verify their compliance with the smoking ban, the respondents were asked whether they smoked only in designated areas or wherever they wanted to. Results The ratio of people claiming they smoked anywhere they wanted to, disregarding the smoking ban, was 60% (N = 318) and after the amendment had been introduced, this ratio was 62.2% (N = 316), it increased by 2.2 percentage points. The observed difference was statistically irrelevant (Chi² = 0.530, p > 0.05). Conclusions The Act ”On amendment of the act on protection of health against the consequences of consumption of tobacco and tobacco products and Act on National Sanitary Inspectorate” in Poland did not result in the expected changes in the frequency of complying with the smoking ban by the 1st year students.

Objectives Non-adherence and non-compliance to pharmaceutical treatment is one of the most common causes of not effective management of patients suffering from ischemic heart disease (IHD). It is crucial to understand the reasons behind it but studies on this subject performed in the Polish population are still lacking. Material and Methods The 329 patients (160 male and 169 female) diagnosed with IHD who reported for follow-up appointments are examined. The following standardized questionnaires were used: Treatment Satisfaction with Medicines Questionnaire (SATMED-Q) and Adherence to Refills and Medication Scale (ARMS), which evaluates the patient’s compliance and adherence level, respectively. Results Patients with IHD showed moderate compliance with pharmacological recommendations and average satisfaction with treatment. Anemia, drugs side effects, and SATMED-Q total score were significant predictors of the overall ARMS score in the univariate analysis, whereas the male gender and satisfaction with treatment improves this results. In multivariate analysis, significant predictors of lower adherence included family history of IHD, anemia and drugs side effects, while higher education and SATMED-Q overall score increased adherence. Conclusions Treatment satisfaction is a significant predictor of increased overall treatment adherence as well as adherence in terms of drug intake and drug and prescription refills. Raising patient awareness should be an important goal of future educational activities.

ObjectivesThis study aimed to explore the self-care strategies undertaken by Polish nurses, and more specifically: to assess the participants’ self-care strategies; to check self-medication patterns in the study group; and to analyze compliance with medical recommendations regarding pharmacotherapy.Material and MethodsA quantitative and cross-sectional survey was conducted to examine the phenomenon of interest. The study employed a questionnaire survey with an independently designed questionnaire as a research tool. A total of 446 registered nurses taking part in different professional training courses for nurses in January–June 2018 who agreed to participate were included in the study.ResultsThe findings of this study reveal generally poor self-care strategies and compliance with medical recommendations among Polish nurses. The most frequent health behaviors among the surveyed nurses were caring for personal hygiene and a healthy diet. About 31% of the nurses confirmed supplements use. The most common reasons for taking supplements concerned the prevention of vitamin and mineral deficiencies (77.5%), and boosting of the immune system (49.3%). The most popular supplements included packs of vitamins (57.2%), single vitamin D3 (33.3%) and magnesium (31.2%). The vast majority of participants (79.8%) took some kind of over-the-counter drugs (OTCs) in the last 6 months, most often painkillers and flu medications, relying most frequently on the information included on the drug leaflets. The analysis showed a statistically significant relationship between declared OTC use and age, marital status, years of professional experience and economic status. Overall, 26.9% of the study participants declared the use of both supplements and OTCs, while 16.4% of the participants used neither supplements nor OTCs.ConclusionsSelf-care strategies undertaken by nurses should be seen as an essential factor in their positive therapeutic relationship with patients. The ageing nursing workforce should make all of us increasingly aware that their self-care needs will increase as well.

Background Managers influence the way organization works as well as the functioning of subordinates – in the context of their work life but non-professional functioning as well e.g., attitude towards work-life balance or taking care of health. We focused on the superior-subordinate relation, referring to social power bases theory by Raven. We identified the reasons why subordinates decide to follow their superiors’ orders and determined specific styles of compliance with superiors’ will. Understanding why employees listen to their superiors may be valuable in the context of supporting healthy organizational climate and atmosphere of co-operation or communicating values – for example, as regards taking care of own health. We discussed the results referring to the issue of influencing employees in the context of their health behavior. Material and Methods The research involved 100 Polish employees, aged 28 years old on average, who filled in the Interpersonal Power Inventory by Raven et al. for subordinates in a Polish adaptation by Zaleski. The questionnaire includes 11 subscales referring to power bases. Results Based on the cluster analysis results, we recognized people who complied because of: all kinds of power bases (typical for 46% of the respondents); the respect for superiors’ professionalism (34%); and formal/objective reasons (20%). Conclusions Employees differ in terms of their styles of compliance. Their motives to comply with superiors’ instructions constitute compilations of power bases. The superiors’ awareness of the reasons why their employees decide to follow orders is necessary for successful management. Wstęp Kierownicy mogą wpływać nie tylko na zachowania pracowników związane z wykonywaną pracą, ale także na pozazawodowe aspekty ich funkcjonowania, np. na podejście do kwestii równoważenia życia zawodowego i prywatnego czy zachowań zdrowotnych. W poniższym badaniu autorki skoncentrowały się na relacji przełożony–podwładny, odwołując się do koncepcji podstaw władzy Ravena. Celem było zbadanie, jakim podstawom władzy podporządkowują się polscy pracownicy i ustalenie specyficznych stylów podporządkowywania się woli przełożonego. Wiedza, dlaczego pracownik słucha swojego przełożonego może być cenna w kontekście wspierania zdrowego klimatu organizacyjnego i atmosfery współpracy czy przekazywania pracownikom wartości – np. odnoszących się do dbałości o własne zdrowie. Autorki przeanalizowały wyniki w kontekście wywierania przez przełożonych wpływu na zdrowotne zachowania pracowników. Materiał i metody Badaniem objęto 100 polskich pracowników (średnia wieku: 28 lat), którzy wypełnili polską wersję Inwentarza Wpływu Społecznego (Interpersonal Power Inventory) Ravena i wsp. w wersji dla pracowników. Kwestionariusz zawiera 11 podskal odnoszących się do podstaw władzy. Wyniki Na podstawie analizy skupień wyróżniono osoby podporządkowujące się przełożonemu ze względu na: wszystkie podstawy władzy (typowe dla 46% respondentów); szacunek dla profesjonalizmu przełożonego (34%) oraz formalne/obiektywne przyczyny (20%). Wnioski Pracownicy różnią się w zakresie stylów podporządkowania się woli przełożonych. Przyczyny, dla których się podporządkowują, są kompilacją różnych motywów. Świadomość przełożonych na temat tego, co sprawia, że pracownicy wypełniają ich polecenia, jest warunkiem skutecznego kierowania ludźmi. Opis przypadku dotyczy zapoczątkowanego podczas badań okresowych postępowania profilaktyczno-orzeczniczego, które doprowadziło do wczesnego wykrycia przewlekłej białaczki szpikowej. Szczególną uwagę zwrócono na skrócenie terminu ważności badania okresowego uzasadnione nieprawidłowością w stanie zdrowia, która nie miała bezpośredniego związku z warunkami zawodowymi, oraz na sposób komunikacji i wpływ na pracownika, aby zastosował się do zaleceń lekarskich. Wywnioskowano, że wprawdzie obligatoryjne i cykliczne badania profilaktyczne do celów Kodeksu pracy stwarzają możliwość wczesnego wykrycia nieuświadomionych stanów chorobowych, dając tym samym szansę na poprawę stanu zdrowia pacjentów, ale jednocześnie mogą być traktowane jako ingerencja w prawo do samostanowienia jednostki o własnym życiu i zdrowiu. Uznano, że w ocenie sytuacji kluczowe są postawa lekarza medycyny pracy oraz sposób komunikacji z badanym pracownikiem, które nie tyle zmuszą, ile przekonają – dla dobra pacjenta – do podjęcia diagnostyki lub interwencji prozdrowotnej.
The case report concerns the prophylactic and medical certification procedure initiated during periodic examinations, which led to the early detection of chronic myeloid leukemia. Particular attention was paid to the shortening of the period of validity of the periodic examination, justified by an abnormal health condition that is not directly related to the working conditions, as well as the method of communication and influencing the employee to comply with medical recommendations. The conclusions stated that, although obligatory and periodic preventive examinations for the purposes of the Labor Code create the possibility of early detection of unawereness of disease, thus giving a chance to improve health, but at the same time they can be treated as an interference with the individual’s right to self-determination about his life and health. It was noted that the key factor in assessing the situation would be the attitude of the occupational medicine physician and the manner of his communication with the employee, who would not be forced, but convinced – for his benefit, to undertake diagnostics or health interventions.
