Opis:
The study compares chronologically the weeks in which Trichinella spiralis infected meat was consumed. The first symptoms appeared and clinical diagnosis of trichinellosis was made in 37 patients. Several factors were responsible for the delay in final diagnosis of trichinellosis: widely distributed infected meat, contemporary common respiratory viral infections in population with fever and myalgia, diagnostic difficulties in the first index case.

The activity of IgG, IgM and IgE antibodies to somatic antigen of Trichinella spiralis in the sera of patients with trichinellosis at various intervals after infection was examined by means of ELISA. Mathematical analysis of the dose-response curves was used. Elevated level of IgG and IgM antibodies of relatively high avidity and of rather low IgE avidity was documented. Amount or avidity ofIgG antibodies was found to be most useful for the diagnosis of trichinellosis (85% positive results in patients' sera). The isotype of IgM avidity constitutes a better diagnostic value than the amount of it (60% and 35% of positive results, respectively).

Opis:
Selected clinical problems have been discussed in the light of new data on pathogenesis. This has been illustrated by own observations in trichinellosis patients. Value of nail fald capillaroscopy in estimation of spread and intensity of capillary lesions in trichinellosis has been discussed. Changes in eosinophilia have been presented in patients treated with anthelmintics or corticosteroids at the acute stage of symptoms (3rd week). Pathophysiology of gastrointestinal phase of invasion has been discussed and pathomorphology of small intestine biopsies has been presented as observed in trichinellosis patients with diarrhoea. Attention has been drawn to atypical course of trichinellosis, noted with increasing frequency, particularly in persons infected with sylvatic strain of Trichinella spiralis. It is worth stressing that in patients exhibiting an atypical, clinical pattern of trfchinellosis accompanied by low blood eosinophilia, treatment with anthelmintics and corticosteroids is started as a rule with a delay, thus promoting a very intense Trichinella invasion and unfavourable outcome.

In this review the pathomechanisms of human trichinellosis with particular relation to intestinal and muscular invasion are presented. The numerous factors initiated by antigen released by Trichinella, which play a role in the development of pathological process (including immunological, pathomorphological, metabolic and bioelectric disturbances) and short clinical characteristics of the disease are also presented. Pathology of late period of trichninellosis sequelae are discussed.

Since the XIX century human trichinellosis has remained an unsolved problem of public healthcare in Poland. This paper describes the past situation and analyses current changes in the epidemiological pattern of trichinellosis in Poland. Epidemiological data from the last 60 years, point out that the number of human cases as well as the number of deaths caused by trichinellosis has decreased significantly. Up to 90s the main source of Trichinella infection for people was pork. Among other implemented control measures, the introduction of the artificial digestion method in the early 80s to detect trichinellosis in pigs resulted in a shift in the sources of Trichinella infection in humans - pork was replaced with wild boar meat. In the years 1990-1995 the number of outbreaks due to pork consumption was 3.5-times higher than in the years 2000-2005. In the early nineties pork was the source of infection causing about 71% of all outbreaks; in 2000-2005 that number has fallen to only 12%. On the other hand wild boar meat was responsible for 23% of the outbreaks in 1990-1995 and as many as 88% of all outbreaks in the years 2000-2005. Moreover the number of persons infected in the outbreaks significantly decreased. The study of wild animals demonstrated that wild boars in Poland are infected not only with T. spiralis but also with Trichinella britovi. These results and EU recommendations indicate a requirement of determining the Trichinella species which cause infections in outbreaks. In the 3 trichinellosis outbreaks in 2005 the infected meat products were examined with molecular tools. T. spiralis species larvae were the etiological agents of infection in all these outbreaks. The current epidemiological situation of trichinellosis in Poland indicates a need of increasing the awareness of risks related to wild boar meat consumption among the general public. Introducing the artificial digestion method as an obligatory method for wild boar meat examination is also necessary.

Sixty four patients who had passed the acute phase of trichinellosis 3, 4, 5 and 8 months and 1, 2 and 4 years earlier, have been examined with respect to their clinical condition, persistence of possible subjective complaints which could be associated with the acute phase passed, leucocytosis, absolute number of acidophilic granulocytes in blood, indices of immunological response, both humoral (IgG, IgA and IgM) and cellular (percentage of T and B lymphocytes in blood), serotonine level in blood, monoaminooxidase activity in blood serum and passive hemaglutination reaction with trichinellosis antigen. It has been found that the frequency of occurrence and intensity of subjective complaints decreases with time. Changes in humoral and cellular reactivity, and long-lasting persistence of antibodies against the trichinellosis antigen, determined with passive hemaglutination method, have been also observed in particular patients.

Opis:
The influence of Nalcrom (sodium cromoglycate) on the cource of the intestinal phase of trichinellosis in mice was investigated. The animals infected with 200 Trichinella spiralis larvae were treated with Nalcrom between 7-20 or 3-20 days after infection (d.a.i.). The drug was administered in two doses: 0,6 or 1, 7 mg/mouse/day. In the all groups of animals received Nalcrom higher number of mast cells and eosinophilis than in the control groups was observed. These results are the opposite of those obtained with Nalcrom in ulcerative colitis in man.
