Opis:
Photodynamic therapy is a treatment that uses a combination of light-absorbing photosensitizers and dissolved oxygen to kill cancer. One specific limitation of photodynamic therapy is that the visible light used for photosensitizer excitation has a short tissue penetration depth of several millimeters. This limits the application of photodynamic therapy to surface cancers in the absence of a technique to illuminate deeper tissue. Efforts to extend tissue depth to which photodynamic therapy can be applied have been attempted with use of up-conversion and persistent-luminescent nanoparticles that absorb near infrared light and emit visible light for photosensitizer excitation, yet an initial excitation with an external light source is still required. More recently, systems employing chemiluminescence as an excitation energy source designed to bypass the use of external light have been developed and investigated as potential agents that could overcome the problem of achieving photodynamic therapy in deep tissue. We wish to provide an overview of several systems that have been recently reported that employ both radiative and non-radiative chemiluminescent energy transfer for photosensitizer excitation that have been developed in the hope of achieving “dark” photodynamic therapy. This article reviews several of these important new developments in the design of photodynamic therapeutic systems that utilize chemiluminescence.

Introduction. Arterial damage associated with musculoskeletal injuries at room conditions is very common. Aim. It is worth remembering that examining a patient who has suffered an accident with a broken bone or traumatic joint dislocation may be accompanied by vascular damage. A dislocation or fracture often masks damage to the artery. Description of the case. In this paper, a 20-year-old patient being a passenger suffered upper right limb trauma in a car accident. Conclusion. The effects of vascular injuries appear only after a few hours after setting the fracture. It is not known then whether the artery was damaged at the same time with the fracture, whether the injury occurred during the adjustment, or whether the ischemia is finally the result of the pressure exerted by too tight plaster casts.

Introduction. The development of perianal fistulas are a risk factor in colonic and rectal disease. Perianal CD treatment requires a combination of surgical and therapeutic treatments aimed to prevent septic complications, reduce fistula discharge and ultimately heal fistulas. Aim. The purpose of the study was to evaluate the efficacy and safety of biosimilar IFX (CT-P13) and adalimumab in active fistulizing perianal Crohn’s disease (CD) in patients from the Subcarpathian Region (South-Eastern Poland). Material and methods. Thirty patients with CD with perianal fistulas naïve to anti-TNF therapy were enrolled (13 females/ 17males) ranging from 18 to 64 years of age. Twenty-one were treated with biosimilar infliximab (CT-P13), nine were treated with adalimumab (ADA). The treated patients had ileal CD (4), ileo-colonic CD (13) or colonic CD (13). All of them received standard immunosuppression with no additional steroid therapy. Response was evaluated at week 16 and 40 after the first CT-P13 dose, and 16 and 40 weeks after the first ADA dose. Remission was defined as the complete closure of all fistulas and partial response as a reduction (≥50%) in the number of draining fistulas. Results. Treatment outcomes with CT-P13 and ADA were both effective and similar in the percentage of patients with perianal fistula improvement, perianal fistula remission, no effect or observed adverse events. Conclusion. In patients with active fistulizing CD, both CT-P13 and ADA were effective and safe, however a slight superiority of CT-P13 was visible

Introduction. The fraction of exhaled nitric oxide (FeNO) is used as a non-invasive biomarker that reflects inflammation in the airways. It is so versatile that it used to control asthma severity as well as to monitor response to treatment. However, the exact cut-off point of the nitric oxide level which allows one to make a precise diagnosis of asthma is unclear. Aim. To examine the possibility of using advanced statistical methods such as receiver operating characteristic for the analysis of FeNO concentrations for improving the diagnosis of asthma. Materials and methods. Receiver operating characteristic (ROC) was used for analyzing results to determine levels of nitric oxide which may be a prognostic indicator of asthma. The studied group consisted of 111 children including 69 asthmatic patients, and 42 age- and sex-matched healthy subjects. Measurement of exhaled nitric oxide was conducted in all subjects included in this study. Results. FeNO level was higher in asthmatic patients. The analysis of results showed that the cut-off point for the FeNO concentration is 11.5 ppb. Sensitivity and specificity with the FeNO level allowed us to determine a value of the diagnostic variable of FeNO concentration of 14.0 ppb. A comparison of FeNO level and sex of the subjects showed there is no correlation between these parameters of patients. Conclusions. Currently, the FeNO measurement provides complementary d

Opis:
Introduction. Despite advances in medical care AKI (acute kidney injury) is associated with high morbidity and mortality. The lack of adequate early renal injury biomarkers is often a problem for an early AKI diagnosis. In recent years, numerous scientific studies have been carried out which reveal new urine and serum markers to assess the period of the kidney injury before revealing its late clinical effects. In most clinical settings, AKI is due to acute renal tubular necrosis which results in protein accumulation in urine. Determination of the concentrations of proteins such as NGAL (neutrophil gelatinase-associated lipocalin) and KIM-1 (kidney injury molecule-1) are of great significance in the diagnosis of AKI. Aim. The purpose of the study was to review the literature about significance of NGAL and KIM-1 proteins for diagnosis of acute kidney injury (AKI) in clinical practice. Materials and method. Introduction. Breast cancer is the most frequent neoplasm among women. That is the reason why scientists all over the world are attempting to improve early detection methods of this particular malignancy. Aim. The most common and most widely used examination methods for screening for and detecting breast cancer is presented herein. Material and methods. This review was performed according to systematic literature search of three major bibliographic databases. Results. Available data suggest that incidence and mortality in high-resource countries has been declining whereas incidence and mortality in low-resource countries has been increasing. Conclusion. The role of a physician is to select the most suitable one for each patient in order to obtain the best result. No matter the method however, between 2005 and 2011, the 5-year relative survival was found to be 89%. This is thought to be due to both the increase in utilization of population-wide screening, as well as advances in treatment.

Introduction. Bladder mullerianosis is a rare and proliferative lesion that contains at least two types of ectopic Mullerian tissue in its wall. Aim. To present case of bladder mullerianosis. Description of case. The text contains a description of a clinical case of a 50-year-old woman admitted to a gynecological ward due to diarrheal symptoms and abdominal pain. In a CT examination of the abdominal cavity with contrast, within the posterior left-sided wall of the bladder a 43x25mm proliferative lesion suggestive of neoplastic character was revealed. Transurethral resection of the lesion (TURB) was performed. Histopathology revealed endosalpingiosis with small endocervical foci. The picture of hyperplasia met the criterion of mullerianosis. Conclusion. Bladder Mullerianosis is a very rare disease that occurs mainly in women of reproductive age. It has very good prognosis. It is important to differentiate the lesion with malignant tumor. The basis for the diagnosis is the histopathological examination of the lesion tissues taken during the surgery.

Magnetic Resonance Imaging functional Magnetic Resonance Imaging
Opis:
Magnetic Nuclear Resonance (MRI) is a non-invasive tissue imaging method. This technique is based on the influence of a strong magnetic field and electromagnetic wave of strictly defined frequency on the nucleus of elements with non-zero spin. The study describes one of the variants of functional MRI, (fMRI), which has become a key technique in brain imaging. This technique has excellent spatial and temporal resolution and involves a changing signal intensity depending on the degree of oxygenation of the blood. Blood oxygenation levels are known to vary in accordance with neural activity and these differences can be used to detect brain activity. This is due to increased demand for energy and oxygen in the area of increased neural activity. The basis of this imaging is the so-called Blood Oxygenation-Level Dependent (BLOD) effect. The aim of this paper is to present the scope of fMRI as a diagnostic method in neurology and in neurosurgery. This paper presents the principles of fMRI, methods of application, research result development, and suggests areas of possible medical applications. The limitations of fMRI as a clinical tool in medical applications will also be addressed. Studies presented in this paper are based on clinical fMRI experience and a literature review.

Photodynamic therapy is a complex process involving the introduction of photosensitizers into the patient’s body and irradiation of them in order to destroy the lesion, and activate the immune system. An important role in photodynamic therapy is played by photobiochemical and physical mechanisms that affect the tumor vessels and lead to the death of the damaged cell. The aim of the study is to determine the effect of photodynamic therapy with the use of Hypericin (Hyp) on the secretion of selected cytokines by colorectal cancer cells. Material and methods. Two colorectal cancer cell lines SW480 and SW620 were used in the study. Cells treated Hypericin were exposed to visible light. Then cell viability was determined by the MTT assay with 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium. Assays were performed for control samples without hypericin and light exposure, with Hyp without light exposure, without Hyp and irradiated with light, and test samples with Hyp and light exposure. Results. In the experiment we reveal, that Hyp- photodynamic activity does not influence the secretion of cytokines. Conclusion. The obtaining results confirming the destructive effect of Hyp- PDT on the colon cancer cells, show a possibility of extending the indication for photodynamic therapy using Hyp, qualification of precancerous changes.

Introduction and aim. Photodynamic therapy with hypericin (HYP-PDT) is gaining importance as a potential treatment method for malignant neoplasms. The following study investigated whether HYP-PDT influences the secretion of certain factors of colon cancer cells in vitro. Material and methods. Two colon cancer cell lines were used in this experiment: SW480 and SW620. The cells were properly prepared and then treated with photodynamic therapy with hypericin at concentrations of 0.25 µM and 0.5 µM and irradiation at the doses of 1 J/cm2 , 5 J/cm2 and 10 J/cm2 . After using HYP-PDT, changes in the concentrations of four factors: GM-CSF, MIF, VCAM-1 and ICAM-1 were analyzed in the test tubes. Results. In the case of SW480 cells: a notable decrease in GM-CSF, MIF, VCAM-1 and ICAM-1 secretion was noted after HYP-PDT. In the case of SW620 cells, after HYP-PDT: no effect on GM-CSF secretion was noted; it inhibited the secretion of VCAM-1 and MIF and increased the secretion of ICAM-1. Conclusion. Photodynamic therapy with hypericin shows immunomodulatory potential in an in vitro cell line experiments. This may indicate its possible ability to modify the course of malignant tumors and therefore requires further research.

Introduction. The proper negative margins (R0) breadth in the breast - conserving surgery for invasive breast cancer (IBC) and ductal carcinoma in situ (DCIS) is very important. The presence positive surgical margins (R1) is associated with the necessity of reoperation. It delays the adjuvant therapy and psychologically burdens the patient. The re-operation increases the costs of treatment. The introduction of mammography (MMG) increased detection of DCIS by 20%. With the increase in malignancy, cancer detection decreases in MMG, inversely in MRI groving. Effective preoperative and intraoperative diagnosis aims to reduce the number of R1 resections. Aim. The size of the tumor next to its biology, determines the clinical course of the tumor. The accurate analysis of imaging tests is important. Material and methods. This analysis was performed using a systematic literature search. Results. Adequate surgical margins in breast cancer surgery for breast cancer have been reviewed. It is important to know if the cancer is multifocal and what the extent of the tumor is. Conclusion. The adequacy of margins is important for adjusting the volume of excision. It is avoiding unnecessary resection of healthy breast tissue. It is essential for a good cosmetic result and the local recurrence rate. The combination of breast MRI with conventional breast imaging resulted in the lover rate of the R1 resectios and the lower rate of the re-operation.

Currently, new and easier ways of analyzing pharmaceutical drug forms and drug delivery mechanisms are being sought. Magnetic resonance imaging (MRI) is a non-invasive imaging technique that images drug forms such as tablets, liquids and topicals and drug form behavior in living organisms on both the tissue and cellular scale. The advantages of MRI include noninvasiveness, variable sample capacity and ease of transfer of phantom results to in vitro and in vivo studies. This review concerns the usefulness of clinical MRI that cannot be understated as this technique provides non-invasive and non-destructive insight into the properties of drug delivery systems. The research discussed here concerns the use of magnetic resonance, spectroscopy and chromatography to investigate selected pharmaceuticals and covers work of selecting drugs and antibodies for modification by synthesis for evaluation by MRI. Modifications have been aimed at improving therapeutic efficacy, delivery, and MRI. Modification conditions such as (pH, concentration, temperature, and the influence of other components present in the solutions) will be discussed to understand drug delivery system improvements and the reliability and repeatability of the results obtained. We hope to explore and expand the scope of pharmaceutical imaging with MRI for application in clinical medicine.

Opis:
Over the past few decades, magnetic resonance imaging (MRI) has proven to be extremely successful in medical applications. More recently, the biomedical applications of MRI have been gaining more use in the field of clinical pharmacy. In 1977, perfluorocarbon compounds (PFC), which form emulsions that can carry drugs, were analyzed by 19F MRI and emulsified PFC compounds have been investigated as potential blood substitutes since the early 1960s and now a wide variety of PFC compounds are currently available as 19F MRI biomarkers. Molecules with 19F substituents are particularly attractive for use in drug tracking by 19F MRI due to 100% 19F abundance, high 19F MRI sensitivity (0.83 relative to 1H MRI) and an impressively large chemical shift range (400 ppm). Another benefit in the use of 19F MRI is a zero background signal in biological samples due to lack of endogenous fluorine. Therefore, drugs containing fluorine atom have potential for 19F MRI imaging drug delivery to tissue. This article will review recent developments in the use of 19F MRI in imaging drug delivery to tissue and individual cells.

Currently, liver disease is widespread and the awareness of these diseases is low. Early symptoms of liver disease do not necessarily indicate problems with this organ and patients are usually informed of their problems when the stage of the disease is already advanced. Invasive biopsies are the clinical diagnostic method most commonly used in the evaluation of liver disease. A biopsy is associated with a high risk of false results and additional complications. Finding new non-invasive imaging methods has led to the discovery of a new method called Magnetic Resonance Elastography (MRE). This technique allows one to evaluate the mechanical properties of tissues and to distinguish between pathological states. Testing using this technique can be performed on a conventional magnetic resonance system by using few additional components and properly prepared software. Studies have shown that there is a strong correlation between MRE-measured liver stiffness and the degree of fibrosis. MRE is also useful in characterizing liver tumors. Studies show that this technique is highly credible in both health volunteers and patients with liver fibrosis. MRE has tremendous diagnostic potential. The described technique is not currently widely used and has the 127-132potential to serve as a safe and accurate alternative in clinical diagnostics in the future.

Introduction. Stroke is a common vascular disease encountered in the work of a practitioner. Despite this, his differentiation is difficult. This is due to a variety of diseases that can be a “mask” for stroke. These are neurological diseases other than vascular (brain tumors, epilepsy), head injuries and a number of metabolic disorders (fluctuations in glucose, electrolytes). Aim. This work aims to approximate the differentiation of this common disease entity. Case reports show both clinical view and diagnostic difficulties. Description of the cases. Case 1. The patient 59 years old treated for bipolar disorder, hypertension and alcohol abuse. Case 2. The patient 68 years old treated for type 2 diabetes, hypertension, ischemic heart disease, cholecystolithiasis – cholecystectomy. Conclusion. Stroke is a significant clinical and social problem both in Poland and in the world. Despite this, its diagnostics and especially differentiation is difficult. When analyzing the case of each patient with suspected sudden CNS vascular disease, all of them should be remembered. It should also be borne in mind that differential diagnosis cannot delay specific treatment for stroke

Opis:
Despite continuous scientific and technological advances in MR imaging, MR perfusion methods have not yet been widely deployed for routine clinical diagnostics. This is especially true for ASL (arterial spin labelling) methods used to evaluate cerebral perfusion. This method does not require a contrast agent, as new discoveries about gadolinium accumulation in the cerebellum and brain nucleus appear to be a valuable asset and provide the opportunity to be more widely deployed in clinical practice. The aim of this paper is to present the historical determinants of the development of MR perfusion techniques, the disadvantages and advantages and possible clinical applications and prospects of ASL development. Both historical articles published on MR in the 1990s and current research between 2006-2016 have been reviewed. The authors present in the work the MR perfusion method focusing on issues related to arterial spin labeling (ASL). Historically CASL (continuous ASL) and PCSL (pulsed ASL) techniques have been described and the pseudocontinuous ASL (pseudocontinuous ASL) 3D technique presents its technical and methodological considerations, advantages and disadvantages over previous methods. The methods of test protocol optimization and accompanying artifacts, as well as possible clinical applications and development perspectives, have been described.

Continuous scientific research and the increasing saturation of the medical market in Poland implements the possibilities of using advanced MR techniques including MRS in everyday practice. This method, which has so far been used primarily for research purposes, can bring measurable benefits to patients not only in terms of clarifying diagnosis and narrowing differential diagnosis, but also monitoring the course of various diseases and their treatment. Here we present the basic principles of performing and interpreting spectroscopic spectra and possible clinical applications and development prospects of MRS. The literature reviewed both Polish and foreign articles both historically and in the past 10 years. The paper presents methodological issues related to the proper performance of magnetic resonance spectroscopy (MRS) and spectral composition and the role of major metabolites, as well as current clinical applications and directions of MRS development.

Opis:
Introduction. Cytisine, Cytisinum (C11H14N2O), is an organic chemical compound. Cytisine is heterotricyclic compound that is the toxic principle in Laburnum seeds and is found in many members of the Fabaceae (legume, pea or bean) family. Aim. The aim of the study is to measure the influence of water on the form of drug in the magnetic field 1.5 Tesla. Material and methods. For this purpose, magnetic resonance imaging tests were performed to check the solubility of pure cytisine, Desmoxan tablets and Tabex capsules. Results. From a pharmacological point of view, both Desmoxan tablets and Tabex capsules should exert the same effect on the human body, this is due to the identical content of the active substance, in this case cytisine (1.5 mg). Conclusion. The differences in the results obtained may be related to additional excipients that contain medications, but it is believed that they should not have a negative impact on the action of the active substances.
