Most of the pathological changes in the spine begin with the physiological loss of intervertebral disc function. Discopathy of the lumbar spine leads to a significant deterioration in the quality of life, which is why the therapeutic team strives is to improve patients quality of life through actions aimed at reducing spinal dysfunction to an extent that allows for human functioning. The research aim was to determine the quality of life of patients who had been treat- ed as a result of discopathy of the lumbar spine and to establish the relationship between the experience of negative feelings and a subjective assessment of the quality of life with factors resulting frompatient socio-demographics. The study included 110 patients treated for lumbar discopathy. A diagnostic survey method was employed as a research technique, with the use of a proprietary survey questionnaire. Statistic analysis of the results was performedusing the statistical package PQStat v1.6.6. Subjective evaluation of patients quality of life in 66 persons (60%) was at an average level, only 6 (5.45%) respondents rated the quality of their lives as being higher. Those living in towns of up to 50,000 residents rated their quality of life higher (6.21), with the lowest level being reported by those living in the countryside (4.95). Age did not influence significantly the quality of life, but older people most often experienced pain (p=0.4). Analysis of variance did not show statistically significant differences between marital status and respondent quality of life (p = 0.53) but did show significant differences between education levels and the incidence of negative feelings such as fear, anxiety or depression. Socio-demographic variables: such as gender, age and marital status do not significantly differentiate the quality of life of the respondents. There is a relationship between experiencing negative feel ings and educational levels. The lower the education level obtained, the more often negative feelings are experienced. The experiencing of negative feelings and disease duration are variables that reduce the quality of life of respondents. ------------------------------------------------------------------------------------------------------------------------------------

Although randomized trials have proven the benefit of revascularization with the use of carotid endarterectomy (CEA) or stenting (CAS) for appropriate patients, health care purchasers increasingly look beyond clinical outcome toward measures of health-related quality of life (HRQoL) in apportioning limited resources. 36 patients after TIA participated in this study. One group (n=18) had undergone CEA, whereas the second group (n=18) had undergone CAS. Both groups were matched according to age and sex. The Short Form 36 (SF-36) was used to assess the differences in patient-perceived HRQoL in two groups of patients who had suffered minor cerebral ischemic events. No significant difference in health profile between the CEA and CAS was detected for the eight SF-36 domains. However, the CEA group rated a significantly improved change in general health after one year compared with the CAS group (p<0.001). A greater proportion of the CEA group comparing to the CAS group thought their treatment had been successful and that their health had been improved (p<0.001). Both groups shared the same level of anxiety over potential future cerebral ischemic events (p=0.3). Patients' perception of HRQoL measured by the SF-36 domains was almost identical between the CEA and CAS apart from a small but significant improvement in self-reported overall health in the CEA group after one year follow up. HRQoL outcome measures may be of value in future clinical trials of cerebral revascularization to compare the effectiveness of carotid revascularization with the particular method of operation.

The aim of the study was to answer the question as to whether transcranial direct current stimulation (tDCS) is more effective in reducing burnout syndrome in nurses than the commonly used individual psychotherapy. The study included 40 nurses from various health care facilities located in the Lesser Poland and Podkarpackie Voivodeship suffering from burnout syndrome. They were assigned to the experimental group while 20 to the control group. Two different therapy models were used: the experimental group (A) included 20 of the nurses treated with the use of transcranial direct current stimulation (tDCS), four times a week for 8 weeks, in sessions that lasted initially 10, then 15, 20, 25 and finally 30 minutes and the control group (B) included 20 of the nurses treated with individual psychotherapy employed for 8 weeks, once a week for 30 minutes. To evaluate the results we used: screening with a clinical interview, the Mini–Mental State Examination (MMSE), the Beck Depression Inventory and a Polish adaptation of the Italian questionnaire created by Massimo Sentinello (LBQ). Treatment was provided. The transcranial direct current stimulation (tDCS) used in Group A is more effective in reducing many symptoms than is the case with individual psychotherapy. Comparing the intragroup effects, it was found that the tDCS employed in Group A significantly reduced the intensity of depressive symptoms among the surveyed nurses [F (1.38) = 57.62; p <0.001; η2 = 0.603] while the individual psychotherapy used in Group B failed to produce a statistically significant effect [F (1.38) = 1.794; p = 0.188; η2 = 0.045]. These include a reduction of depression, a reduction in chronic psychophysical exhaustion, vegetative problem reduction, and the improvement of nurse-patient relations. Neurotherapy with the use of transcranial direct current stimulation (tDCS) is more effective in reducing burnout syndrome than the commonly used individual psychotherapy. It also helps to return these nurses to full professional activity. The studies presented above recommend the use of new neurotechnologies in therapy as a result of their usefulness and non-invasive character.

Opis:
Multiple Sclerosis (MS) is a chronic inflammatory disease affecting the central nervous system through the destruction of sheaths (myelin), disseminated in space and time. The aim of our work is to study the neurocognitive concept and mood disorders in patients with MS. The study includes in this study 75 cases of MS, collected at the Neurology (B) and Neurogenetics Department of Rabat Ibn Sina Hospital. The study was performed on MS patients using different scales that included: a self-questionnaire (socio-demographic, clinical), Hospital Anxiety and Depression scale (HADS), Rosenberg's Self-Esteem scale (RSE) and Expanded Disability Status Scale (EDSS). The mean age for the onset of the disease was 30-years, with a sex ratio of 3.4. The main clinical signs are: 69.33% and 70.67% of patients had respectively anxious and depressive disorder, which was associated with low self-esteem in 89% of them. The distribution according to the evolutionary forms of MS is 65% Relapsing Remitting (RR), 29% secondary progressive (SP) and 5% Primary Progressive (PP). As for the assessment of disability using EDSS, we note an average 4.07±1.829. That results of our study concord with other studies on the same age category. It confirms the predominance of women. They are followed by the appearance of psychological disorders such as depression anxiety and cognitive disorders. Remitting MS is the most common when compared to other forms. The anxiety and depression score was strongly correlated with the degree of self-esteem and disability.

The aim of the study was a comparison of two groups of women who suffered from breast cancer in the past, selected on the basis of the time elapsed since treatment completion (<7 and >7 years). The specific areas of interest were the quality of life, sense of control, and level of depression. Results from 37 women were analysed. The average age in the research sample was 57. The study was conducted with the use of EORTC QLQ-C30, MHLC, and Depression Assessment Questionnaire. Women in whose case the time elapsed since the completion of anticancer treatment is no more than 7 years have a more negative assessment of the quality of life aspects, that is physical and social functioning, in comparison to persons in whose case the adaptation time to changes, following cancer and treatment, is longer (more than 7 years). The studied persons with a shorter period elapsed since the completion of anticancer therapy hold stronger beliefs concerning their own influence on their health in comparison to women who have not undergone oncological treatment for at least 7 years. The author’s own research highlights the trend pertaining to the differences in the intensity of experiencing guilt and anxiety between the groups. However, it requires further research involving a larger group of studied patients. Despite the progress in the diagnostics and treatment of breast cancer, patients with this type of cancer still experience long-term subjective difficulties in areas making up their physical, mental, and social well-being. The research proves the differences in functioning of women depending on the time that has elapsed since the completion of anticancer treatment.

This paper discussed a case study of a 15-year-old girl who had had a hemorrhagic stroke, caused by arteriovenous malformation. A brain arteriovenous malformation is a defect in the vascular system, a tangle of abnormal blood vessels connecting arteries and veins in the brain. The patient attended regularly neuropsychological rehabilitation and several various therapeutic methods were applied. A neuropsychological diagnosis was performed before and after therapy. The first assessment showed some cognitive disturbances: difficulties in the organization of memorized material, difficulties in maintaining attention and deficits in abstract thinking. Furthermore there were observed emotional problems: depression and high perfectionism. The patient participated in individual therapy lasting one year. Neuropsychological rehabilitation took into consideration the personal psychological resources and cognitive and emotional difficulties. As a result, there was observed an improvement in consequent cognitive functions: organization of memorized material, working memory, abstract thinking and verbal fluency. The emotional state was stable. It is important that the therapy should be holistic and includes all aspects of the patient’s functioning, which may have an influence on the final result of rehabilitation. One of the most important aims of the therapy psychologist was an acceptance of the disease. The cognitive tasks can also include psychological and therapeutic exercises.

Alexithymia is an emotion regulation problem that may be related to suicidality, especially in patients with SUD. The study aimed at estimating the prevalence of alexithymia in Moroccan psychoactive substances users, and to establish the links between alexithymia and suicidal ideation and behaviors, anxiety and depression in drug-dependent patients. We also aimed to elucidate if alexithymia predicts suicidal thoughts and behaviors in these consumers. The study included 451 consecutively admitted patients with substance use. Consumers were examined with the Toronto alexithymia scale (TAS-20), State and Trait Anxiety Inventory (STAI- Y), Beck Depression Inventory (BDI) and The Columbia-Suicide Severity Rating Scale (C-SSRS). Among substance-dependent individuals, 72.7% was identified as a group with alexithymia. Current age and age at first substance use were lower in the alexithymic group (p<0.01). The mean STAI-Y state and C-SSRS scores were higher in the group with alexithymia (p<0.01). There was a statistically significant difference between the groups in terms of mean scores for anxiety traits (p<0.05) and no significant difference in terms of mean scores for depression. All of the scores correlated significantly with. Addicted patients with alexithymia could be targeted at preventing the onset of suicidal thoughts and behaviour. The present study also evokes that whenever an emotional regulation disorder is observed, the possibility of anxio-depressive symptomatology and suicidality (ideation and behavior) needs to be assessed.

Identifying anxio-depressive disorders, evaluating their prevalence and determining the different factors that can intervene in their hap- pening to the patients suffering from hemodialysis. Transversal descriptive and analytical study over 70 patients, real- ized at the dialysis unit of the Idrissi regional hospital’s department of medicine at Kenitra, Morocco, over a period of three months starting from 27 March to 26 June 2015. The data were acquired from a survey and psychiatric evaluation scales: The scale HADS (Hospital Anxiety and Depression Scale ) to evaluate anxiety and depression and the scale of « Big Five French Inventory to evaluate the treats of personality» (BF-Fr). Average age was from 54,66±15,96 years. The seniority of the hemodialysis was on average 5,20±3,23 years. The studied traits of personality show a decrease regard to the average score of dimen- sion O (overture of sense) and of the dimension E score (extraversion) versus an evaluation of the dimension N score (Neu ro ticism). The prevalence of anxio-depressive troubles were as the following; 74,29% presented an anxious state (suspected or proven) and 70% had a depressive state (suspected or proven). The anxious troubles were correlated to age and to N and O characters of personality traits BFI-Fr. the depressive troubles were correlated to N charac- ters traits of personality BFI-Fr, and the anxious troubles were pos- itively correlated to depressive troubles. The prevalence of anxio-depressive troubles among hemodialysis subjects was elevated with repercussions on the personality. The involvement of a psychologist in the processes of taking charge is obligatory.

The research focuses on the questions of if nurses working in various health care facilities present different abilities for emotional adaptation to the work environment, if they differ in feeling job related burnout, and what stress coping strategies they tend to employ. The Questionnaire of Emotional Intelligence (INTE), Link Burnout Questionnaire (LBQ), and the Coping Inventory for Stressful Situations (CISS) were administered. The study comprised 104 nurses from various health care facilities located within 7 voivodships (Polish provincial administrative divisions). This included three groups of different types of work: general care, specialized medical care, and units providing care for patients at a high risk of dying. A statistical analysis was performed with IBM SPSS Statistic 24. A regression analysis of the interface between emotional intelligence and stress-coping strategies showed that a high degree of emotional intelligence fostered a strategy of task-avoidance coping, and diminished the tendency to concentrate on emotions during stressful situations, it also minimized the propensity of the individual to reduce stress by distracting themselves with other activities. However, the mediation analysis revealed that the impact of emotional intelligence decreased after the role of occupational burnout was taken into consideration. This finding suggests that it is rather the occupational burnout that limits the possibility to apply effective stress coping strategies and that stress management does not reduce the risk of burnout. The study indicated the significant role of emotional intelligence: as a higher level of this form of intelligence corresponded with a lower feeling of burnout in all its dimensions. The mediation analysis revealed that it is the occurrence of burnout that plays the most significant role in adjustment to the work environment. This finding suggests that the prevention of burnout is the most effective factor enabling productive work, and the maintenance of psychological wellbeing despite the many job stressors which may occur in the workplace. It creates the need to clarify its links with depression in order to concentrate on a really effective treatment.

SUMMARY The purpose of this study was to determine the quality of life of a patient with persistent complications of SARS-CoV-2 infection requiring urgens surgical intervention – endarterectomy with emergency angioplasty of an inflamed, bleeding Right Internal Carotid Artery (RICA) by direct access via Right Common Carotid Artery (RCCA), known as Transcarotid Artery Revascularization (TCAR). A patient in her 60s was infected with the SARS-CoV-2 virus and contracted COVID-19 in March 2022, as confirmed by RT PCR antigen test. The infection was followed by short- and long-term complications, many of which can be linked to COVID. These include significant weakness persisting for months after the illness, rapid weight loss of 25 kg, sleep disturbances, chronic fatigue, severe dizziness, onset of diabetes, decrease dimmunity with increased periodontal inflammation (including formation of a periapical abscess of a molar tooth) and secondary suppuration of the submandibular lymphnodes, one of which lying jacent to the right internal carotidartery (RICA). This accumulation of symptoms led the patient to seek medical and neuropsychological help. Test ingusing the Beck Depression Inventory (BDI) confirmed depression, with vegetative disorders being the most predominant. Eight months after undergoing COVID-19, the patient suffered a Transient Ischemic Attack (TIA). The accumulation of diseases (diabetes mellitus, stage III hypertension and TIA) had a dramatic impact on the patient's health, including life-threatening conditions.A vascular surgeon consulted the patient advised immediate surgical treatment: carotid endarterectomy. The urgency of the situation was exacerbated by bleeding during the operation from the operated, secondarily inflamed wall of the RICA (lying adjacent to the suppurated submandibular node). This prompted an emergency decision for an endovascular procedure: the implantation of a stent covered with water-proof material (peripheral stent graft). This was made by a direct access via puncture of the common carotid artery (RCCA) below the endarterectomy level (TCAR). The SF-36 questionnaire was chosen to measure health-related quality of life (HRQOL).The SF-36 results are presented in such a way that higher scores correspond to fewer complaints, indicating better health and higher quality of life. Before revascularization, the patient's HRQOL was found to be lower, which was related to the negative impact of long COVID, while after the procedure, the quality of life gradually improved in subsequent surveys. A significant difference was found in physical function, with a mean score of 66.0 (p<0.001) compared to a score of 94.9 (±9.4) for 100 age-matched health subjects. A similar result was found in the physical role (p < 0.001). The patient's overall quality of life score was 331.0 compared to a score of 578.0 (±111.9) for age-matched normal healthy people. Better quality of life in patients with long COVID is an important therapeutic goal that can be achieved through comprehensive, multispecialty treatment for both physical and psychological conditions.
