Opis:: Basic safety structures of multi-state systems of components with degrading safety states related to their variable operation conditions are defined. For these systems the conditional and unconditional multi-state safety functions are determined. A semi-markov process for the considered systems operation modelling is applied. Further, the paper offers an approach to the solution of a practically important problem of linking the multi-state systems safety models and the systems operation processes models. Theoretical definitions and results are illustrated by the example of their application in the safety and risk evaluation of the Stena Baltica ferry operating at the Baltic Sea. The ferry transportation system has been considered in varying in time operation conditions. The system safety structure and its components safety functions were changing in variable operation conditions.

Opis:: Basic notions of the ageing multistate systems safety analysis are introduced. The system components and the system multistate safety functions are defined. The mean values and variances of the multistate systems lifetimes in the safety state subsets and the mean values of their lifetimes in the particular safety states are defined. The multi-state system risk function and the moment of exceeding by the system the critical safety state are introduced. The exemplary safety structures of the multistate systems with ageing components are defined and their safety functions are determined. As a particular case, the safety functions of the considered multistate systems composed of components having exponential safety functions are determined. Applications of the proposed multistate system safety models to the evaluation and prediction of the safty characteristics of the oil piping transportation system is presented as well.

Opis:: Industrial large-scale urea production creates a number of hazards due to equipment and machine failures as well as service errors. Failures can cause material, environmental and human losses. The work presents the assessment of safety hazards and failure effects for selected emergency scenarios of Mitsui - Toatsu's urea synthesis reactor. A fragment of the process implemented in the synthesis node was covered by HAZOP (Hazard and Operability Study), and the critical points, from the security point of view, were identified. Several emergency scenarios for various types of failures and various effects are presented. The analyzed contingency scenarios include: blocking the valve on the outflow from the reactor and leaks caused by corrosion resulting from the maintenance of improper technological parameters of the process. The work suggested the need for new security measures and modernization of the installation by using the safety function for the selected critical point. The hazards analyzed and the contingency scenarios developed are essential for risk assessment and its possible reduction.

Opis:: There are presented general safety analytical models of complex multistate technical systems related to their operation processes. They are the integrated general models of complex technical systems, linking their multistate safety models and their operation processes models and considering variable at the different operation states safety structures and their components safety parameters. The conditional safety functions at the system particular operation states and independent of the system particular operation states the unconditional safety function and the risk function of the complex technical systems are defined. These joint models of the safety and the variable in time system operation processes are constructed for multistate series, parallel, “m out of n”, consecutive “m out of n: F”, series-parallel, parallel-series, series-“m out of k”, “mi out of li”-series, series-consecutive “m out of k: F” and consecutive “mi out of li: F”-series systems. The joint models are applied to determining safety characteristics of these systems related to their varying in time safety structures and their components safety characteristics. Under the assumption that the considered systems are exponential, the unconditional safety functions of these systems are determined. The proposed models and methods are applied to the safety analysis, evaluation and prediction of the one subsystem of the port grain transportation system related to varying in time their operation processes, structures and components safety parameters.

Opis:: Starting from consideration that urban intersections are sites with promise for safety and operational improvements, the paper describes the steps taken to develop a crash predictive model for estimating the safety performance of urban unsignalized intersections located in Palermo, Italy. The focus is on unsignalized four-legged one-way intersections widespread in Italian downtowns. The sample considered in the study consist of 92 intersections in Palermo, Italy. For the study were collected crashes occurred in the sites during the years 2006-2012, geometric design and functional characteristics and traffic flow. Results showed that data were overdispersed and NB1 distributed. In order to account for the correlation within responses Generalized Estimating Equations (GEE) were used under different working correlation matrices.

Opis:: In the reliability investigation of large-scale systems, the problem of the complexity of their safety functions arises. This problem may be approximately solved by assuming that the number of system components tends to infinity and finding the limit safety function of the system. The problem of finding the limit reliability function of large-scale systems is well known for basic two-state systems (Gnedenko for series and parallel systems [1], Smirnov for “k out of n” systems [8]). The problem of possible limit reliability functions for series-parallel and parallel-series is also well recognized [3]. The solution for two-state series-“k out of n” systems, while k/n→0 and also while k/n→1 can be found in [5] and [6]. The paper is concerned with mathematical methods in multistate asymptotic approach to series-“k out of n” systems safety and shows the methods of establishing its possible limit safety functions, while k/n→ λ, 0˂ λ˂ 1.

Opis:: The design of a blasting monitoring and control system and its practical application in a blind gallery named Dongguabang in Guilaizhuang gold mine in China was presented in the paper. Based on reason analysis of blasting accidents occurring in metal mines, a blasting accident model is established in order to explain the process of the accident and its direct reasons. Also, effective approaches for preventing blasting accidents and avoiding casualties are achieved by controlling unsafe behaviour of workers and elimination of the critical "touch". Combined with analysis of safety ergonomics of blasting devices, all these make a joint contribution to providing theoretical instruction and references to establishment and design of blocking functions of blasting monitoring system. A set of blasting monitoring and control system is implemented, and equipped with blocking functions when unsafe behaviour and conditions appear. Tests of the monitoring system are carried out in the Donguabang heading of Guilaizhuang gold mine. It is documented that the blocking functions and the central computer platform run well during the tests. Test results reveal that blasting monitoring and control system and its functions are able to ensure safety and fulfil work requirements in Guilaizhuang gold mine.

Opis:: Road safety modelling on regional level of NUTS 2 in the EU is the complex issue ant authors of this article indicate this in previous publications. During multivariate models development they discovered that it is difficult to make regression model well described all regions, even if they are from one country. In the first step Poisson model of road fatality rate as fatalities per 100 thou of citizens was prepared. In this article was presented the fuzzy regression approach to estimate lover and upper bounds of the modeled value.

Opis:: A dual-channel computer architecture for utilisation in programmable logic controllers is presented. Faults can be detected by novel high-speed comparators with fail-safe operation. The cyclic operating mode of PLCs and a specification-level, graphical programming paradigm based on the interconnection of application-oriented standard software function modules are architecturally supported. Thus, by design, there is no semantic gap between the programming and machine execution levels enabling the safety licensing of application software by an extremely simple, but rigorous method, viz., diverse back translation.

Opis:: W pracy zaprezentowano główne założenia projektu europejskiego SOFIA, podejmującego próbę zbudowania systemu umożliwiającego automatyczne sprowadzenie samolotu na ziemię w przypadku wystąpienia na jego pokładzie działań o charakterze terrorystycznym. In the article main assumptions of the SOFIA VI FP are presented. The SOFIA project is a response to the challenge of development concepts and techniques enabling the aircraft to return automatically and safely to the airport in case of hostile actions on board. The main structure of Flight Reconfiguration Function module was presented as well as some problems, which appeared during the implementation on four experimental platforms, are discussed. Such platforms are: two research simulators and two aircraft. As a conclusion, the test trials program and some remarks about test flights results are provided.

Opis:: W artykule przedstawione zostały metody estymacji nieznanych parametrów wykładniczego rozkładu czasów przebywania elementów systemu w podzbiorach stanów niezawodnościowych. Rozważone zostały różne przypadki przeprowadzonych badań empirycznych, włączając mało liczne realizacje oraz badania niedokończone. Ponadto zaprezentowany został program komputerowy oparty na tych metodach wraz z przykładem zastosowania do identyfikacji niezawodności systemów lin w Stoczni Marynarki Wojennej w Gdyni. Program wyznacza oszacowanie nieznanych warun-kowych intensywności wyjścia z podzbioru stanów niezawodnościowych wykładniczej funkcji niezawodności elementów systemu w różnych stanach eksploatacyjnych, a następnie pozwala na weryfikację hipotezy o wykładniczych dystrybuantach warunkowego czasu przebywania elementów systemu w podzbiorach stanów niezawodnościowych.
This paper presents methods for estimating unknown parameters of the exponential distribution of the system components of conditional lifetime in the reliability state subsets. There are different cases of performed empirical experiments considered, taking into account small number of realisations and non-completed investigations. Moreover, the paper presents a computer program based on these methods with an example of an application for reliability identification of the rope system in Naval Shipyard in Gdynia. The program determines the evaluation of unknown conditional intensities of departure from the reliability state subsets of the system components' exponential reliability function in variable operation states. Finally, the program allows a verification of the hypothesis about the exponential distribution of the system components' conditional lifetime in the reliability states subset.
