Background. Dry Thai traditional tobacco (Nicotiana Tabacum L.) production involves a unique process: (a) picking tobacco leaves, (b) curing tobacco leaves, (c) removing stems of tobacco leaves, cutting leaves and putting on a bamboo rack, (d) drying in the sun, reversing a rack, spraying a tobacco extract to adjust the tobacco’s color, storing dried tobacco and packaging. These processes may lead to adverse health effects caused by dermal absorption of nicotine such as Green Tobacco Sickness (GTS). Objectives. The aim of this study was to determine the correlation between GTS resulting from dry Thai traditional tobacco production and salivary cotinine levels among Thai traditional tobacco farmers in Nan Province, Thailand. Materials and Methods. A prospective cohort study was conducted with 20 tobacco farmers and 20 non-tobacco farmers in Praputtabath Sub-District and Phatow Sub-District. The participants were randomly selected and interviewed using in person questionnaires with bi-weekly follow-up for 14 weeks. During each contact, the cotinine concentration was measured by NicAlertTM Saliva strip tests (NCTS). Descriptive statistics and Spearman’s correlation (Spearman’s rho) was used to examine the relationship between the variables at both 0.01 and 0.05 significant probability levels. Results. This study indicated that GTS from dry tobacco production has the potential to be considered a common occupational disease. This study demonstrated the usefulness of salivary cotinine level measurements by NCTS. The levels were well correlated with farmers who were employed in the dry Thai tobacco production industry. Salivary cotinine levels were also significantly correlated with the prevalence of GTS in the group of tobacco farmers at any given time within a crop season. However, the production process of dry Thai traditional tobacco is different from that evaluated in our previous studies where GTS and salivary cotinine level were correlated in workers working in humid conditions. Conclusions. The long-term effects of such exposure should be investigated and health education programs with health risk exposure studies to increase awareness amongst farmers is recommended.

Global Adult Tobacco Survey (GATS) Project in PolandSocial and economical consequences of using tobacco products are commonly known. This is an issue of a global scale and requires systematic monitoring. The need of in depth examination of this issue concerns also Poland. In 2008 Polish Ministry of Health joined Global Adult Tobacco Survey (GATS). This survey is conducted under Bloomberg Global Initiative to Reduce Tobacco Use. In Poland GATS is being coordinated by World Health Organization. Three institutions were selected to conduct the survey: Maria Sklodowska-Curie Oncology Center (Centrum Onkologii – Instytut im. M. Skłodowskiej-Curie), Pentor Research International and Warsaw Medical University (Warszawski Uniwersytet Medyczny). GATS survey is national, representative survey of households conducted amongst adult population. Globally standardized protocol is being used to implement the survey in Poland. The protocol has been adapted to the Polish needs and reality.The main aim of the project is to assess the usage of tobacco products and examination of the size of the problem, exposure to the tobacco smoke and perception, attitudes and knowledge regarding tobacco products use as well issues concerning taxes on tobacco products. Information gained during GATS will support tobacco control policy. Experience gained during the survey implementation will, in the future, allow building systematic monitoring system of tobacco use and other health behaviors in our country.

The purpose of this work is to analyze the regulatory environment for tobacco use, with particular reference to changes made in the last 10 years. An attempt was made to answer the question of the extent to which the regulatory environment has a real impact on the achievement of the objective set out in the existing legal provisions, which is to protect the health and rights of the persons concerned. An analytical method was used in the work to assess the application of legal principles. A method of analyzing the current line of jurisprudence was also used. In the process of final evaluation of public utilities, certain functional methods were used. The assessment was subjected to Polish law, within the scope of the currently binding Act on Health Protection Against the Consequences of Using Tobacco and Tobacco Products of 1995. The study states that the provisions of this Act are only a formal guarantee of protection. The liability arising from the application of its standard is illusory because the procedure for imposing penalties is ineffective. The pursuit of any property claims related to violation of the rights of related entities, as envisaged in this Act, is complicated. This does not apply to a case which is not affected by the decision.

Ponadto, przedstawione zostały konieczne zabezpieczenia obiektów magazynowych oraz procedury bezpieczeństwa podczas wykonywania wybranych czynności logistycznych.
The problems presented in this article concern the requirements that must be met during the implementation of logistics processes related to the delivery, storage and distribution of tobacco raw materials and products. The most important procedures that must be followed to provide the security of the products were pointed out. These procedures concern primarily the requirements related to the use of means of transport, the acceptance, storage and shipping of raw tobacco and tobacco products. Also presented are necessary protection procedures for storehouses as well as safety procedures while performing the selected logistic operations.

The main goal of this paper is to present contractual relations between Polish tobacco producer’s groups and their purchasers (contract seasons: 2010/2011, 2011/2012, 2012/2013). The hypothesis of the research is based on the statement: “horizontal (agricultural producers’ groups) and vertical (contracts) integration in agriculture lowers farmers’ income risk and reduces transaction costs, which strengthens farmers’ position in the negotiation process with their purchasers”. In verifying the hypothesis, the author conducted empirical research among Polish tobacco producers’ groups. The data were collected in the period between January and May 2013. The theoretical background for the research is built on the new institutional economics, contract theory and transaction cost theory. Results provide the supposition that contracts and cooperation within producer’s groups lower farmers’ income risk, but in the case of contractual arrangements not so signifi cantly. It is caused by following factors: contracts are signed for one season only; they do not fulfill all commitments in contracts and they do not provide financial sanctions, neither for farmers nor for purchasers/fi rst processors. Existing contracts between the tobacco industry and tobacco producers’ groups or tobacco growers can be categorised as production contracts in agriculture. Also individual farmer’s transaction costs of contracts are lower due to cooperation than tobacco producer’s groups. Therefore one can state that quasi-hierarchical structures in the tobacco sector improve efficiency of member’s farms.
Głównym celem artykułu jest analiza procesu kontraktacji wśród grup producentów tytoniu w Polsce w sezonach 2010/2011, 2011/2012 i 2012/2013. Przyjęta hipoteza brzmi integracja pozioma (grupy producenckie) oraz pionowa (kontraktacja) przyczyniają się do zmniejszenie ryzyka i kosztów transakcyjnych oraz wzmocnienia pozycji negocjacyjnej polskich plantatorów tytoniu. Dla jej weryfikacji przeprowadzono między styczniem i majem 2013 roku badania ankietowe wśród czterech grup producentów tytoniu w Polsce. Tło teoretyczne dla przeprowadzonych badań stanowiła nowa ekonomia instytucjonalna. Wyniki empiryczne pozwalają wnioskować, że dzięki kontraktacji zmniejsza się ryzyko dochodowe plantatorów (członków grup producenckich), choć w przypadku kontraktów z pierwszymi przetwórcami nie tak znacząco. Jest to spowodowane m.in. tym, że kontrakty zawierane są tylko na jeden sezon produkcyjny, a także brakiem egzekwowania wszystkich postanowień zawartych w umowach oraz brakiem sankcji finansowych zarówno dla odbiorców/pierwszych przetwórców, jak i rolników. Umowy kontraktacji między przemysłem tytoniowym a rolnikami mogą być zakwalifikowane jako kontrakty produkcyjne. Ponadto indywidualne koszty transakcyjne plantatorów tytoniu ulegają zmniejszeniu dzięki kooperacji w ramach grup producenckich. Dlatego można stwierdzić, że te quasi-hierarchiczne struktury w sektorze tytoniowym poprawiają efektywność członkowskich gospodarstw rolnych.

Heated tobacco products (HTPs) are a form of nicotine delivery intended to provide an alternative to traditional cigarettes. The aim of this systematic review was to present the current state of knowledge on HTPs with an emphasis on the potential impact of HTP use on human health. During the preparation of this systematic review, the literature on HTPs available within Medline/PubMed, EMBASE, CINAHL, ScienceDirect, and Google Scholar was retrieved and examined. In the final review, 97 research papers were included. The authors specifically assessed the construction and operation of HTPs, as well as the chemical composition of HTP tobacco sticks and the generated aerosol, based on evidence from experimental animal and cellular studies, and human-based studies.Heated tobacco products were found to generate lower concentrations of chemical compounds compared to traditional cigarettes, except for water, propylene glycol, glycerol, and acetol. The nicotine levels delivered to the aerosol by HTPs were 70–80% as those of conventional combustion. The results of in vitro and in vivo assessments of HTP aerosols revealed reduced toxicity, but these were mainly based on studies sponsored by the tobacco industry. Independent human-based studies indicated that there was a potentially harmful impact of the active and passive HTP smoking on human health. Currently, a large body of knowledge on HTP exposures and health effects is provided by the tobacco industry (52% of identified studies). Based on the available evidence, HTPs produce lower levels of toxic chemicals, compared to conventional cigarettes, but they are still not risk-free. Objectives Exposure to environmental tobacco smoke (ETS) constitutes a threat to the health of many people. In order to diminish ETS exposure, countries (including Poland) implemented legal restrictions of smoking in public places and worksites. Currently more attention is also paid to reduce overall and residential ETS exposure by voluntary smoke-free home policy adoption. The aim of current analysis was to evaluate the prevalence and determinants of implementing smoking bans at place of residence among economically active males and females in Poland. Material and Methods Data from cross-sectional, household study – Global Adult Tobacco Survey (GATS 2009–2010) were analyzed. The logistic regression model was applied for appropriate calculations. Results Out of 3696 studied subjects only 37.1% adopted total smoking ban within the home. Decreased likelihood of adopting total smoking bans was associated with current smoker status, low education attainment, lack of awareness on adverse health consequences of ETS, low level of support for tobacco control policies, and cohabitation with a smoker in both genders. Having smoke-free homes was also linked with age in women, place of residence and work smoking policy in indoor areas in men. Conclusions Targeted activities to encourage adopting voluntary smoke-free rules among groups least likely to implement 100% smoking bans in the home and activities to decrease social acceptance of smoking in the presence of nonsmokers, children, pregnant woman are urgently needed.

ObjectivesUniformed services such as police employees are exposed to acute and chronic stressful events at work that may lead to tobacco use. This study aimed to assess the prevalence of cigarette smoking, e-cigarette use and heated tobacco use among police employees in Poland, and to investigate personal characteristics associated with tobacco or e-cigarette use.Material and MethodsThis cross-sectional study was carried out in June–July 2020 on a randomly selected sample of 8789 police employees from the Mazowieckie Province, Poland.ResultsCompleted questionnaires were obtained from 5082 police employees (79.2% being police officers) with an overall response rate of 57.8%. Smoking ≥100 cigarettes or similar amounts of other tobacco products was declared by 54.6% of the respondents, with significant differences (p < 0.001) between males (56.8%) and females (50.3%). Daily cigarette smoking was declared by 19.5% of the respondents, and 13.4% were occasional cigarette smokers. Daily e-cigarette use was declared by 3.1% of the respondents, and 3.2% were occasional e-cigarette users. Daily heated tobacco use was declared by 2.6% of the respondents, and 2.9% were occasional heated tobacco users. Higher odds of occasional cigarette smoking were observed among men compared to women (OR = 1.254, 95% CI: 1.009–1.558), and among the participants aged 20–29 years (OR = 7.982, 95% CI: 3.066–20.775) or 30–44 years (OR = 3.730, 95% CI: 1.44–9.599) vs. those aged ≥60 years. Higher odds of occasional e-cigarette use were observed among the participants aged 20–29 years (OR = 4.554, 95% CI: 1.213–17.101) vs. those aged 60 years. Police employees with office-based work had lower odds of daily cigarette smoking vs. those with fieldwork (OR = 0.726, 95% CI: 0.55–0.946). Police officers had higher odds of daily heated tobacco use compared to civil workers (OR = 3.362, 95% CI: 1.325–8.534). Conclusions The authors observed a marked proportion of police employees who declared occasional tobacco or e-cigarette use, which may indicate the common social smoking phenomenon in this occupational group.

Tobacco smoking is a major avoidable single cause of premature mortality in Poland. Almost one in three Polish males do not live to 65 years of age, and almost half of this premature mortality can be traced back to the much higher smoking prevalence in Poland than in Western Europe – every third Polish male and every fourth Polish female smokes daily. However, the current health situation in Poland is much better than two decades ago when the country entered a period of political and economic upheaval. In the early 1990s, the state of health of the Polish population was catastrophic and its tobacco consumption levels the highest in the world. In the early 1990s, the probability of a 15-year-old Polish boy living to the age of 60 was not just twice lower than in Western Europe, but also lower than in China or India. The health policy of limiting the health consequences of smoking conducted by the European Union and, in the last two decades, by the Polish parliament and government, helped to stop this health catastrophe. In Poland, cigarette consumption has decreased by 30% since 1990, as did lung cancer mortality among males. Despite this progress, tobacco smoking remains the most serious health problem in Poland. Therefore, comprehensive tobacco control policy should not only be continued, but expanded and accelerated. The EU Tobacco Products Directive proposes a package of actions for reducing tobacco-related health harm in Europe. The Directive proposal is rational, science-and-evidence based, and grounded on the best practice examples from other countries. Both the Polish tobacco control law and the WHO Framework Convention on Tobacco Control (FCTC), ratified by Poland in 2006, oblige our country to support tobacco control, including all the initiatives taken by the European Union.

The first one included countless research initiatives, due to which it was possible to introduce new, resistant varieties of tobacco, whereas the second one focused on agrotechnical methods of combating diseases, and was based mainly on preventive measures.

Objectives This study aimed at assessment of the perceived barriers and motivators to smoking cessation among socially-disadvantaged populations in Poland. It is hypothesized that different factors can be considered depending on the level of smoking addiction. Therefore, a comparison between light and heavy smokers was performed. Material and Methods Data collected during the second wave of a cross-sectional study carried out in the Piotrkowski District in October 2016 – February 2017 among 1668 socio-economically disadvantaged persons constituted the source of information for the present study. Barriers and motivators to smoking cessation among daily smokers were identified via face-to face interviews. Results About one-third of the studied population admitted to being current daily smokers, almost 75% of whom were heavy smokers. The most common barriers to quitting smoking were related to difficulties in quitting (62%), the lack of willingness to quit (56%), as well as addiction and withdrawal symptoms (craving cigarettes [65%], habit [56%], stress and mood swings [55%]). A significantly higher proportion of such barriers was noted among heavy smokers compared to light smokers (p < 0.05). The following motivations to quit were pointed out by the respondents: available pharmacotherapy (47%), access to a free-of-charge cessation clinic (40%), and encouragement and support provided by their doctor (30%), with no differences between various levels of smoking addiction (p > 0.05). Conclusions Developing effective interventions targeted at unique deprived populations requires understanding the barriers and motivators to quitting smoking. Social support and financial issues, including free-of-charge pharmacotherapy and cessation clinics, as well as doctor’s encouragement and support, are crucial for successful smoking cessation in this vulnerable population. This article reviews the placing in Tale’s text history of three records of The Tale on the Origin of Tobacco from the collection entitled Pandok stored in Saint Petersburg archives which were not analyzed earlier: BAN, Lukyanov collection, No. 107 (XVIII c.); NLR, collection of Kolobov, No. 334 (XVIII c.), and IRLI, Severodvinsk collection, No. 29 (XIX c.). The author demonstrates that the Lukyanov record and the Kolobov record belong to type I of the Tale’s main revision by the Institute of Russian Literature, and the record of the Severodvinsk collection combined the patterns of type I and type II. Textual analysis helped to identify the records of type I and type II ex-amined earlier by the author which were most similar to the text of three Tale’s records analyzed in this article.

Green Tobacco Sickness (GTS) has been one of the unexplored areas of occupational health safety. Green Tobacco Sickness is caused by the absorption of nicotine through the skin from wet tobacco plants who have direct contact with tobacco plants during cultivation and harvesting. The present review was carried out to discuss the etiology, symptoms, diagnosis, treatment and prevention of GTS. It is caused by the absorption of nicotine through the skin while the workers are engaged in handling the uncured tobacco leaves. The symptoms include nausea, vomiting, pallor, dizziness, headaches, increased perspiration, chills, abdominal pain, diarrhea, increased salivation, prostration, weakness, breathlessness, and occasional lowering of blood pressure. The use of personal protective equipment like water‑resistant clothing, chemical‑resistant gloves, plastic aprons, and rain‑suits with boots should be used by the tobacco farmers to prevent its occurrence. An international‑level awareness campaign has to be taken up and more stringent workers safety regulations have to be formulated.

Objectives Expanding the information on exposure to environmental tobacco smoke (ETS) at home and its associates is of great public health importance. The aim of the current analysis was to evaluate associates of exposure to environmental tobacco smoke among economically active male and female adults in Poland in their place of residence. Material and Methods Data on the representative sample of 7840 adults from the Global Adult Tobacco Survey (GATS) carried out in Poland in the years 2009 and 2010 were applied. The Global Adult Tobacco Survey is a nationally representative household study. The logistic regression model was used for relevant calculations. Results The exposure to environmental tobacco smoke in the place of living affected 59% of studied subjects. Out of non-smokers 42% of males and 46% females were exposed to the ETS in the at home. Increased risk of residential ETS exposure was associated with low education attainment, lack of awareness on adverse health consequences of second hand smoke (SHS), low level of support for tobacco control policies, living with a smoker. One of the factors associated with the ETS exposure was also the approval for smoking at home of both genders. The residential ETS exposure risk was the highest among males (odds ratio (OR) = 7.1, 95% confidence interval (CI): 6.1–13.8, p < 0.001) and females (OR = 8.1, 95% CI 6.5–11.8, p < 0.001) who declared that smoking was allowed in their place of residence compared to respondents who implemented smoking bans at their place of residence. Conclusions Campaigns to decrease social acceptance of smoking and encourage adopting voluntary smoke-free rules at home might decrease the ETS exposure and reduce related risks to the health of the Polish population. Educational interventions to warn about adverse health effects of the ETS should be broadly implemented particularly in high risk subpopulations.
