Introduction. Colorectal cancer may be benign or malignant. According to the World Health Organization and CDC, it is the second most common cancer worldwide, after lung cancer. The mortality of colorectal cancer has been dropping for more than 20 years due to the improvements in screening techniques and treatments. Aim. The aim of this article is to discuss the role of new biomarkers for the diagnosis and treatment of colon cancer. Material and methods. This article is a review done in regards to discuss the role of new biomarkers for the diagnosis and treatment of colon cancer. Analysis of the literature. A review is discussed the role of new biomarkers for the diagnosis and treatment of colon cancer using current literature. Conclusion. The screening tests based on diagnostic new biomarkers may cause faster detection of cancer and risk factors, and provide prognostic information in order to adjust individual therapy.

Cancers are among the most feared diseases of modern civilization. In Poland, colorectal cancer is one of the tumors with the worst prognosis. The ability to cure is primarily dependent on the stage of the disease at the time of diagnosis. The aim of the study was evaluate antioxidant response in patients with colorectal carcinoma. Material and methods. Twenty patients (14 men and 6 women) aged 61.9± 11.1 years with colorectal cancer were included in the study. Twenty healthy subjects (4 men and 16 women) aged 64 ± 15.3 years formed the control group. The erythrocyte activities of antioxidant enzymes, superoxide dismutase (SOD), and glutathione peroxidase (GPx), Results. A significant increase of GPx, and SOD (p < 0.05) were seen in patients compared to healthy controls. Conclusion. The results indicate that the tested antioxidant enzyme activity of glutathione peroxidase and superoxide dismutase is increased in patients diagnosed with colorectal cancer compared to the control group.

The study presented two cases of synchronous occurrence of colon and appendiceal adenocarcinoma. Both patients required surgical intervention, due to acute peritonitis during the course of acute appendicitis. In case of one patient we performed abdominal CT confirming the presence of sigmoid cancer. The patient was subjected to appendectomy and Hartmann’s operation. The second patient underwent an appendectomy, and colonoscopy performed two months later revealed the presence of rectal adenocarcinoma. The patient was subjected to low anterior rectal resection. The histopathological results considering both patients revealed the presence of synchronous colon and appendiceal adenocarcinoma.

Detection of mutations in families with a hereditary predisposition to colon cancer gives an opportunity to precisely define the high-risk group. 36 patients operated on for colon cancer, with familiar prevalence of this malignancy, were investigated using the DNA microarrays method with the potential detection of 170 mutations in MLH1, MSH2, MSH6, CHEK2, and NOD2 genes. In microarrays analysis of DNA in 9 patients (25% of the investigated group), 6 different mutations were found. The effectiveness of genetic screening using the microarray method is comparable to the effectiveness of other, much more expensive and time-consuming methods.

Dendritic cells are heterogeneous population of the leukocytes and most potent APC in activation of naive T lymphocytes. Therefore the DCs generated in vitro are under research for their application in anti-tumor immunotherapy. The aim of the study was generation of the immature dendritic cells from peripheral blood monocytes collected from colorectal cancer patients and comparison of their ability to endocytosis, cytokine production and immunophenotype to DCs generated from healthy donors. Material and methods. 16 adenocarcinoma stage II patients were included in the study. Dendritic cells were generated in the presence of rhGM-CSF and IL-4. PBMC were isolated from the blood of patients and 16 healthy donors - control group. Immunophenotype, ability of endocytosis of Dextran- FITC as well as intracellular IL-12 expression of the generated dendritic cells was measured using flow cytometry. The cytokines (IL-6, IL-10, IL-12p70, IFN-γ) concentration in the supernatants of DCs culture was measured by ELISA. Results. The percentage of the immature dendritic cells and expression of CD206 and CD209 antigens was significantly higher in patients group (p <0.05 and p <0.001 respectively). Significantly (p <0.001) higher expression of the antigens which initiate the Th2 immune response (CD80-/CD86 + and B7-H2 + / CD209 +) was in the patients group. There were no differences in endocytosis ability and the cytokines (IL-6, IL-10, IL-12p70, IFN-γ) concentration between investigated groups. Conclusions. High immature markers expression on the generated dendritic cells together with identical endocytosis ability in patients group is advantageous in antitumor autologous cells immunotherapy planning. However there is one troubling fact - high expression of markers, which may induce tolerance to particular antigen. It seems to be more reasonable to use the autologous DCs in the antitumor immunotherapy, especially due to the incompatibility in allogenic cells in the context of HLA complex.

Introduction:The presence of tumor budding, i.e.,single cancer cells or a nest of poorly differentiated cells at the front of tumor invasion appears to be a new histopathological indicator of increased aggressiveness of colorectal carcinoma. Purpose: The aim of this work was a retrospective evaluation of the invasion front (tumorbudding, vascular invasion,and lymphocytic infiltration) in postoperative biopsies of patients with colorectal carcinoma and analysis of the 5-year survival. Materialsand methods:The study was based on the material received after surgical treatment of 164 patients with colon cancer. Tissue was obtained directly following tumor resection, fixed in 10% formaldehyde and embedded in paraffin blocks using a routine method by melting with paraffin at a temperature of 56º C. These samples were then routinely stained with haematoxylin and eosin and underwent a histopathological evaluation, with particular attention being paid to the invasion front of the tumor. The immunohistochemical expression of cytokeratin 20 was also evaluated using anti-human CK20 monoclonal antibody (clone Ks.20.8, Dako, Poland). Results: Tumor budding was found in 124 out of 164 patients. Statistical analysis showed a correlation between the presence of tumor budding TB and depth of invasion (pT), lymph node metastasis, distant metastasis, lymphocytic infiltration,and vascular invasion. The cumulative five-year survival correlated with the lack of tumor budding and vascular invasion, as well as a decrease in lymphocyticinfiltration.

Introduction: Expression of Epo, a glycoprotein secreted by the fetal liver and the adult kidney in response to cellular hypoxia and its receptor have been described in human solid tumors, such as colon and breast cancer. Purpose: Since activation of Epo-EpoR signaling pathway in erythroid progenitor and precursor cells leads to promotion of proliferation and differentiation or prevention of programmed cell death through Bcl-xl and Bcl-2 it was of interest to investigate expression of Epo, EpoR, apoptosis regulator – Bax and marker of proliferating cells - Ki-67 and assess correlation between them, with regard to clinicopathological variables of colorectal cancer. Materials and methods: The correlations between expression of Epo, EpoR, Bax and Ki-67 in colorectal cancer were analyzed in regard to patient age, sex, primary localization, histopathological type, grading, staging and lymph node invasion. Statistical analyses were performed by using the Spearman rank correlation test applying a significance level of p<0,05. Results: Correlation between Bax and EpoR is positive and statistically significant at all groups of patients except group pT1+pT2. Positive correlation between Bax and Epo is statistically significant at following groups of patients: all patients, age 60, age >60, male, female , primary localization in rectum, primary localization in colon, adenocarcinoma, G2, G3. Statistical analysis revealed no significant correlations between expression of neither Ki-67 with Epo nor Ki-67 with EpoR in all groups of patients. Conclusions: Epo seems to be a pleiotropic cytokine, which can exert its biological effect on several cell types, including neoplastic cells. The effect of Epo-EpoR signaling can differ in various cells and conditions.

Trametes gibbosa, also referred to as lumpy bracket, is a fungus belonging to the Polyporaceae with natural bioactive compounds that have aroused great interest for their potential benefits in human health. This pilot study aimed to investigate the anti-cancer effect of Trametes gibbosa extract against the HCT-116 cell line (human colorectal cancer cells). The extract was obtained by macerating dry biomass with 3 solvents: acetone, isopropyl alcohol and DCM, isolating established anticancer agents –polysaccharide complexes (PSK and PSP) from the extract, and then being dissolved. Cellular toxicity was assessed by trypan blue staining, determining the effect of 1- and 24-hour continuous exposure to the extract of varying concentrations: 100 µl/mL, 200 µl/mL, 500 µl/mL, 1000 µl/ mL. Results have demonstrated a cytotoxic effect in time- and dose-dependent manners. The proposed cellular mechanisms behind the results are most likely apoptosis followed by secondary necrotic cell death, as the literature suggests, although that remains unclear and requires further experiments. Within the course of consecutive studies, it can be established whether the extract or the compounds contained may hold promise for clinical use, e.g., for adjuvant therapy.

Introduction and objective. Young green barley is the most valuable source of nutrients and bioactive substances. It has a broad spectrum of health-promoting properties such as antioxidant, anti-inflammatory, hypoglycaemic, anti-depressant, anti-atherosclerotic and anticancer. The presented study is an attempt to extend this knowledge with particular emphasis on the possibility of using green barley in colon cancer prevention. Materials and method. Extracts were prepared on the basis of two commercial products: ground dried barley grass (YGB INT) and powder of young green barley juice (YGB GW). Their influence on colon epithelial cells (CCD841 CoN) viability and proliferation were analyzed by LDH and MTT assays. Anticancer properties of extracts were screened on colon cancer cell lines (LS180, HT-29) by MTT and BrdU assays. Changes in cells morphology induced by extracts were investigated after May-Grünwald-Giemsa staining. Results. Tested extracts were not toxic against CCD841 CoN and did not affected their proliferation or morphology (LDH test, MTT test, microscopy observation). The MTT revealed that extracts significantly inhibited proliferation of colon cancer cells in a dose-dependent manner. Results of BrdU test confirmed antiproliferative properties of extracts, but opposite to MTT test, indicated YGB GW as a better anticancer agent. Light microscopy observation proved the data obtained from both MTT and BrdU tests and additionally suggested the ability of the extracts to induce necrosis in LS180 and HT-29 cells. Conclusions. The study demonstrated that YGB extracts specifically inhibit proliferation of colon cancer cells without any undesirable effect on colon epithelial cells. Obtained results will provide a rationele for the future development of dietary supplements which could be beneficial in colon cancer chemoprevention.

Introduction: Colorectal cancer (CRC) is the third most common malignancy in men and the second most common in women. The disease constitutes a significant civilization and social problem. The aim: The aim of the study is to assess the sudy group’s awareness and knowledge about CRC, as well as about its diagnostics and treatment. Material and methods: An online questionaire form was distributed in the study group regarding issues related to CRC, and followed by statistical analysis and interpretation of the obtained survey results. Results: After analysis, we found that a significant percentage of the surveyed sample group had basic knowledge and awareness in the area of CRC, whereas about half of the respondents did not consider themselves sufficiently informed about the disease. Conclusions: Considering the scale of the problem posed by CRC, it is necessary to undertake broader action to promote knowledge about this disease and to carry out this type of research on a larger and more socioeconomically diverse population.

Introduction and objective: The risk of development colon cancer (CC) is increased significantly among patients with the type 2 diabetes (T2DM). A mechanism responsible for the higher prevalence of CC among diabetic patients may be associated with the immunity system. The aim of this study is to point out the differences in the immunity state in terms of interleukin 2 level among patients with T2DM suffering from CC, and patients without these diseases Material and Methods: 79 patients were included the tests, divided into 4 groups: Group 1–23 people with T2DM, Group 2–23 people with large intestine CC, Group 3–10 people with large CC and T2DM, and Group 4–23 people without T2DM or CC. Each patient had a colonoscopy and those with cancer were confirmed in a histopathological examination. Laboratory measurements included fasting glucose, insulin, C-peptide. The concentration of interleukin-2 in serum was determined with the immunoenzymatic (ELISA) method. Results: The results obtained showed that in patients with T2DM and CC the concentration of interleukin-2 was statistically higher than in the other groups.(4.21±1.61 pg/ml vs. Group 1 -1.64±0.44 pg/ml, Group 2–1.54±0.21 pg/ml, and Group 4–1.70±0.36 pg/ml; p<0.05). Insulin levels, C-peptide and HOMA-IR did not differ significantly between groups, but a tendency was observed to higher values of HOMA-IR and insulin levels in the groups with T2DM alone and T2DM with concomitant CC. Conclusions: The data show differences in the immunity state of patients with T2DM and CC, compared with people without those two diseases. Elevated level of interleukin 2 found in this group, after confirmation in other studies with more patients, could be used as a marker of an increased risk of CC in people with T2DM

Patients with colorectal carcinoma showed statistically significant lower values of transferrin saturation, total iron binding capacity and serum iron level as compared with control group, while the level of ferritin and the size of labile iron pool in carcinoma patients were higher, although this difference was not statistically significant. Our observations are in favour of the hypothesis which suggests that changes in iron metabolism restrict iron availability for tumour cells and as consequence, slow their growth.

In Poland there there are about 15‑16 thousand cases of colon cancer per year. The health care system allows the treatment of patients with colorectal cancer in highly specialized hospitals, oncology centers and district hospitals. The results of treatment within different reference level differ. The aim of the study was to evaluate the results of surgical treatment of patients with colorectal cancer at a district hospitals compared with the results of highly specialized center. Material and methods. A retrospective study. The material consisted of 171 consecutively operated patients diagnosed with colorectal cancer treated in the Department of Surgery, District Hospital in Wołomin. The control group consisted of 200 patients treated surgically at the Department of General and Colorectal Surgery, University Hospital in Łódź. In both centers, the patients were operated on by surgeons with experience in operations on the large bowel. The demographic data, information on the type of indication (elective vs emergent), and the severity of the disease by AJCC / TNM scale were collected. In the district hospital there were patients with more advanced disease (p <0.001), older (p = 0.0001), and often operated under emergent indication (p = 0.0001). The telephone survey collected data on survival or the date of death of the patient and set the percentage of five-year survival. Results. The proportion of five-year survival in the study group and control group was respectively 46% and 71% (p <0.0001). The percentage of five-year survival among patients undergoing elective procedure in both centers were respectively for Wołomin and Łódź 58% and 73% (p = 0.008). The proportion of 5-year survival among “younger” patients (<70) was respectively in Wołomin and Łódź 64% and 81% (p = 0.004) for “older” patients with (> 70) 50% and 60% (p = 0.6747) Conclusions. Overall results of surgical treatment of patients with colorectal cancer in the district hospital are inferior to treatment results in a highly specialized center. The population treated in the district hospital is statistically significantly different in comparison to patients treated in highly specialized center. The following differences were captured: severity of the disease, age and type of indication (elective vs emergent). The diffrences has an influence on the outcomes. The five years survival for patients > 70 years undergoing elective procedure is not statistically different between the district hospital and highly specialized center.

Prebiotics are defined as selectively fermented food ingredients that induce specific changes in the composition and/or activity in the gastrointestinal microbiota beneficial to the host well-being and health. The aim of the presented experiment was to investigate the effect of a prebiotic applied alone or in combination with Hyppocastani extractum siccum, and Lini oleum virginale in rats with dimethylhydrazine induced colon cancer. Wistar albino rats were fed high fat diet supplemented with the prebiotic alone or in combination with Horse chestnut and flaxseed oil. The activity of faecal glycolytic enzymes, lipid parameters, bile acids, short chain fatty acids and counts of coliforms and lactobacilli were determined. Treatment with the prebiotic alone and in combination with selected substances significantly decreased the activity of glycolytic bacterial enzyme β-glucuronidase (P<0.001) and increased activities of β-galactosidase and β-glucosidase. Bile acids concentration was significantly decreased (P<0.01) except for the combination of the prebiotic with Horse chestnut. The prebiotic alone decreased the lipid parameters (P<0.001) and enhanced production of short chain fatty acids. Application of prebiotic and bioactive natural substances significantly reduced number of coliforms (P<0.05). Prebiotic alone significantly increased the count of lactobacilli (P<0.05). These results show that prebiotics have a protective effect and may be the useful for colon cancer prevention and treatment.

Opis:
Introduction. Abdominal ultrasound is considered an initial examination of the the gastrointestinal tract. The case is presented in which ultrasound (US) detected a ‘pseudokidney’ sign that enabled the tentative diagnosis of a colonic neoplastic lesion. Case report. A 78-year-old patient was admitted to the Department of Internal Diseases due to weight loss and anaemia. US demonstrated abnormalities in the form of intestinal wall thickening with a hyperechoic centre. This finding led to the tentative diagnosis of a neoplastic lesion. After further diagnostic procedures, i.e. colonoscopy and histopathological examination of biopsy specimens, the patient was diagnosed with colon adenocarcinoma. Conclusions. In many cases, experienced US radiologists can initially assess the intestinal wall and other organs, which effectively directs towards further diagnostic management, hence accelerates the diagnosis and decision to apply a given therapeutic option or otherwise.
