Peripheral nerve injuries are one of frequent cause of disability. Results of surgical procedures are not still completely satisfactory, though fast development of microsurgical technician improve their efficiency. One of the major problems in surgical treatment of the peripheral nerve injuries is ends coaptation in the case of large loss of nerve tissue. And so finding substitute implants not rejected by the organism for the recipient and providing with the success is necessary of regeneration. Recently, several synthetic nerve guide implants have been introduced and approved for clinical use to replace autologous transplants. This alternative therapy is based on pioneering studies with experimental nerve guides. This paper present a review of published studies involving biodegradable nerve guides.

Introduction: Common etiologies of acute traumatic peripherial nerve injury include penetrating injury, crush, stretch, and ischemia. Purpose: This paper reports an epidemiological and clinical study of patients with peripheral nerve injuries who were treated for upper limb trauma, which included nerve injury, surgically treated in the Department of Orthopedics University Hospital in Bialystok, in the years 1997-2007.Materials and methods: A total of 202 patients with injury to the median, ulnar and radial nerves were involved in the study. Based on the medical history concerning day care and inpatient hospitalization, surgery books data and the analysis of medical histories, the data referring to the nerve trauma were analyzed. Results: The data allowed a reliable assessment of the population of patients with upper limb nerve injury and methods of treatment, as well as quantitative presentation of the methods of repair and reconstruction of the damaged nerve stem. Conclusions: In the Department of Orthopedics, Medical University of Bialystok, in the years 1997-2007 delayed nerve injuries were more frequently treated than the immediate ones. Surgical treatment of nerve injuries most frequently used primary suture and cable grafting reconstructions. Secondary epineural suture of the nerve was performed more rarely. Autogenic nerve graft segments were most frequently collected from the ulnar nerve of the lower extremity. The medial antebrachial cutaneous nerve grafting was much seldom performed.

Occupational ulnar neuropathy at the elbow joint develops in the course of long term direct pressure on the nerve and a persistently flexed elbow posture, but first of all, it is strongly associated with “holding a tool in a certain position” repetitively. Therefore, computer work only in exceptional cases can be considered as a risk factor for the neuropathy. Ulnar hypermobility at the elbow might be one of the risk factors in the development of occupational ulnar neuropathy; however, this issue still remains disputable. As this condition is mostly of congenital origin, an additional factor, such as a direct acute or chronic professional or non-professional trauma, is needed for clinical manifestations. We describe a patient – a computer user with a right ulnar nerve complete dislocation and left ulnar nerve hypermobility, unaware of her anomaly until symptoms of left ulnar neuropathy occurred in the course of job exposure. The patient was exposed to repetitive long lasting pressure of the left elbow and forearm on the hard support on the cupboard and desk because of a non-ergonomically designed workplace. The additional coexistent congenital abnormal displacement of the ulnar nerve from the postcondylar groove during flexion at the elbow increased the possibility of its mechanical injury. We recognized left ulnar neuropathy at the ulnar groove as an occupational disease. An early and accurate diagnosis of any form of hypermobility of ulnar nerve, informing patients about it, prevention of an ulnar nerve injury as well as compliance with ergonomic rules are essential to avoid development of occupational and non-occupational neuropathy.

Optical coherent tomography (OCT) of the retina and optic nerve disc is the gold standard for imaging in the diagnosis and monitoring of glaucoma and optical coherence tomography angiography (OCTA) is an imaging technique with great scientific and clinical potential in patients with glaucomatous neuropathy. The main advantages of OCT and OCTA are non-invasiveness, speed, repeatability and availability of the procedure. In the article, on the basis of recent meta-analyses, the discriminating abilities of the peripapillary retinal nerve fiber layer (pRNFL) and macular parameters as measured with spectral \domain (SD) and swept source (SS) OCT, in all glaucoma patients and in different glaucoma subpopulations were discussed. In addition, the results of the most important research on OCTA in the diagnosis and monitoring of glaucoma and the limitations of this method are listed.
Optyczna koherentna tomografia (OCT) siatkówki jest złotym standardem obrazowania w diagnostyce i monitorowaniu jaskry, a angiografia optycznej koherentnej tomografii (OCTA) jest techniką obrazową o dużym potencjale naukowym i klinicznym u chorych z neuropatią jaskrową. Głównymi zaletami OCT i OCTA są nieinwazyjność, szybkość, powtarzalność badania oraz dostępność procedury badawczej. W artykule, na podstawie ostatnich metaanaliz przedstawiono wartościowość diagnostyczną grubości okołotarczowej warstwy włókien nerwowych siatkówki (pRNFL) i parametrów plamki mierzonych za pomocą domeny spektralnej (ang. spectral-domain OCT, SD-OCT) lub żródła strojnego OCT(ang. swept source, SS-OCT) w ogólnej populacji chorych z jaskrą i w różnych subpopulacjach chorych z jaskry. Ponadto wymieniono wyniki najważniejszych badań z użyciem OCTA w diagnostyce i monitorowaniu jaskry oraz ograniczenia tej metody.

Objective: Congenital cholesteatomas of the petrous apex are rare lesions that develop insidiously, which delays diagnosis and requires a high index of suspicion. A case report of supralabyrinthine congenital petrous bone cholesteatoma and review of recent literature are presented. Methods: A 27-year-old woman presented with progressive facial palsy. Otomicroscopy did not reveal any abnormalities, and hearing was normal. Although there were no complaints of vertigo or instability, vestibular examinations indicated a non-compensated peripheral vestibular lesion. Radiological examinations revealed a lesion in the petrous apex and epitympanum that had features of a cholesteatoma. Results: To preserve normal hearing, the middle fossa approach was chosen for surgery. A damaged part of the facial nerve was reconstructed with cable graft. Conclusion: Advances in radiological imaging facilitate surgical planning, and improvements in the techniques of lateral skull base surgery enable safe and radical removal of petrous bone cholesteatomas with minimal morbidity.

The aim of this work was to describe, interpret and highlight the impact of neuroanatomy in the region of the larynx on intraoperative neuromonitoring (IONM) during thyroidectomy. A rich network of anastomoses of the recurrent laryngeal nerve (RLN) and superior laryngeal nerve (SLN) may have impact on the results of thyroidectomy and partial laryngectomy. Intraoperative neuromonitoring is a useful tool in the armamentarium of a head and neck surgeon but it will never replace profound knowledge of surgical anatomy and good surgical technique.

Introduction: The most common mechanism of post-traumatic facial nerve palsy are road accidents and falls. Treatment schemes as well as proper timing of surgery are still controversial. Aim: The aim of the study was the evaluation of the effects of surgical treatment in patients with post-traumatic facial nerve palsy. Treatment results were correlated with epidemiological factors, mechanism of injury, level of nerve damage, time of surgery and its extent. Material and methods: 9 patients with facial nerve palsy after head trauma were analyzed. In all patients complete paresis of the VII nerve occurred immediately after the injury. In 5 patients the nerve was damaged in the course of the longitudinal fracture of the temporal bone, in 3 as a result of its transverse fracture while in one woman there was no evident fracture line. In all cases, surgical treatment was performed between 4 days and 13 weeks after the trauma. In all cases transmastoid approach was used. Edema lesions of the nerve dominated in 6 patients, in two cases a bone fragment was noted along its course, in one person nerve was disrupted but primary reconstruction was not possible – the man was excluded from further analysis. The results of treatment were assessed by House-Brackmann (HB) scale 12 months after the procedure. Results: Very good (HBI) or good (HBII) recovery of facial nerve function was achieved in 2 and 4 out of 8 patients respectively. Surgical timing, the extent of surgery, patient’s age, mechanism of injury and level of nerve damage had no effect on the final outcome. Conclusions: The management of post-traumatic facial nerve palsy should be individual. The commonly accepted recommendation on surgical treatment is to undertake it in patients with immediate-onset and complete paralysis. Patients who, due to their severe general condition, cannot undergo early facial nerve decompression may benefit from delayed treatment for up to 3 months after the injury.

Schwann cell tumour is a rare benign neoplasms, which in approximately 25–45% of cases localises in the head and neck region. The half of them is found intracranially, within the cochleovestibular nerve (n. VIII) leading to deef. The slow growth and the lack of characteristic clinical signs influence clinically silent development of the tumour. Presence of the clinical signs may by connected with the pressure on the surrounding anatomical structures or the damage of the affected nerve. The treatment of the choice of the extracranial tumours is surgical resection. In presented case, the tumour development was signless. The attempt of the tumour enucleation was taken. Because of the difficulties with the tumour extracapsular enucleation, the decision of radical resection of the tumour and a part of the vagal nerve had been made. The vagal Schwann cell tumour is benign and rare neoplasm, that should be resected, despite it doesn’t influence the suspected patients survival. The main problem stands for the complications due to the nerve damaging during the surgery.

Opis:
New generation of membrane materials can play role in regeneration process in living organism e.g. creation of optimal conditions for regeneration of bone tissue (GBR/GTR technique) or defected peripheral nerve (GNR technique). However, biodegradable polymeric materials which are now widely used in GNR technique (PLA, PCL, collagen) does not have satisfactory mechanical properties such as strength (RM) or Young's modulus (E) because it is difficult to control their porosity [1,2]. Materials suitable for nerve regeneration should exhibit electrical properties which stimulate the regeneration [3]. The main idea of the guided nerve regeneration is utilisation of a membrane separating two tissues i.e. defected nerve tissue and connective tissue. Inside the defected nerve tissue surrounded by the membrane should be present factors influencing the regeneration process such as: ECM protein, nervotrofic factors. On the other hand, the membrane should act as a barrier for fibroblast cells inflowing into the defected area. The work presents results of investigations on porous nanocomposite materials basing on bioresorbable aliphatic polyesters i.e. poly-(L/DL)-lactide and carbon nanotubes (CNT). All materials i.e. nanocomposite foils and porous materials were prepared using synthetic co-polymer of L/DL-lactide with L/DL ratio of 80/20 from Purac®. The polymer had the FDA attestation confirming its biocompability. As the nanofillers, two types of CNTs produced by Nanostructured and Amorphous Materials (Inc. Huston, USA) were used: MWCNTs (multi-wall carbon nanotubes; diameter 10-30 nm and length 1-2 μm) and SWCNTs (single-wall carbon nanotubes; diameter 0.7-2 nm and length 15-30 μm). Nanocomposite membrane materials (PLDLA/0.5% wt. MWCNTs and PLDLA/0.5% wt. SWCNTs) were prepared using combined methods: phase inversion and freeze-drying. Porous microstructure of the nanocomposites was investigated using SEM/EDS. It was found, that the presence of the CNT influenced shape, size (5-50 μm) and distribution of pores in the material (total porosity of PLDLA/ MWCNTs was about ~65% and PLDLA/S WCNTs was about ~35%). The nanoadditives increased mechanical properties of the membrane materials. For example addtition of the SWCNTs increased the membrane strength (RM) form 16 to 24 MPa. Physicochemical properties of the materials surface were investigated by means of wettability and surface energy measurements. It was shown that dispersion part of surface free energy decreased when SWCNTs were used as additives (from 4.5 mm/mJ PLDLA membrane to 0.7 mm/mJ PLDLA/SWCNTs), while in the case of the MWCNTs addition dispersion part of surface energy increased from 4.5 mm/mJ to 6.9 mm/mJ. Such PLDLA-based materials modified with CNTs (MWCNTs, SWCNTs) may be an attractive support for adhering cells. SWCNTs were more suitable nanoad- ditives for PLDLA-matrix membranes than MWCNTs, because such membranes were stronger, hydrophilic and much more bioactive.

Opis:
Botulinum toxin (BTX) belongs to a family of neurotoxins which strongly influence the function of autonomic neurons supplying the urinary bladder. Accordingly, BTX has been used as an effective drug in experimental therapies of a range of neurogenic bladder disorders. However, there is no detailed information dealing with the influence of BTX on the morphological and chemical properties of nerve fibres supplying the urinary bladder wall. Therefore, the present study investigated, using double-labeling immunohistochemistry, the distribution, relative frequency and chemical coding of cholinergic and noradrenergic nerve fibers supplying the wall of the urinary bladder in normal female pigs (n=6) and in the pigs (n=6) after intravesical BTX injections. In the pigs injected with BTX, the number of adrenergic (DβH-positive) nerve fibers distributed in the bladder wall (urothelium, submucosa and muscle coat) was distinctly higher while the number of cholinergic (VAChT-positive) nerve terminals was lower than that found in the control animals. Moreover, the injections of BTX resulted in some changes dealing with the chemical coding of the adrenergic nerve fibers. In contrast to the normal pigs, in BTX injected animals the number of DβH/NPY- or DβH/CGRP-positive axons was higher in the muscle coat, and some fibres distributed in the urothelium and submucosa expressed immunoreactivity to CGRP. The results obtained suggest that the therapeutic effects of BTX on the urinary bladder might be dependent on changes in the distribution and chemical coding of nerve fibers supplying this organ.
