Opis:
Silicon (Si) is the second most abundant element present in the lithosphere, and it constitutes one of the major inorganic nutrient elements of many plants. Although Si is a nonessential nutrient element, its beneficial role in stimulating the growth and development of many plant species has been generally recognized. Silicon is known to effectively reduce disease severity in many plant pathosystems. The key mechanisms of Si-mediated increased plant disease resistance involve improving mechanical properties of cell walls, activating multiple signaling pathways leading to the expression of defense responsive genes and producing antimicrobial compounds. This article highlights the importance and applicability of Si fertilizers in integrated disease management for crops.

This article describes Kimura’s disease as a rare case of lymphadenitis and reviews its epidemiology, clinical aspects, diagnosis and treatment methods. The role of surgical procedures was also underlined. Kimura disease is a rare and benign chronic inflammatory soft tissue disorder of unknown origin. The definitive histological description was published by Kimura et al. in 1948 and henceforth, the disease has born his name. Since that time, there has been a gradual increase in the number of reports. While most cases of Kimura disease have originated in China, Japan or Southeast Asia, there have been sporadic case reports from Europe and America. The data suggest that Kimura’s disease should be included in the list of differential diagnoses for all neck lymph node involvements. If a otolaryngologist maintains a high index of suspicion, an early diagnosis can be made to contribute to a more successful outcome of the disease.
Artykuł opisuje chorobę Kimury – rzadką przyczynę limfadenopatii, jej epidemiologię, objawy kliniczne, diagnostykę i leczenie, w tym leczenie chirurgiczne. Choroba Kimury jest rzadkim, łagodnym, przewlekłym zapaleniem tkanki łącznej nieznanego pochodzenia. Cechy histologiczne tej choroby zostały po raz pierwszy opisane przez Kimurę i wsp. w 1948 r. i nazwane jego nazwiskiem. Większość przypadków choroby Kimury pochodzi z Chin, Japonii bądź południowo-wschodniej Azji, natomiast nieliczne z Europy i Ameryki. Obraz kliniczny choroby nakazuje uwzględnienie jej w diagnostyce różnicowej powiększenia węzłów chłonnych szyi. Daje to możliwość pomyślnego wyleczenia.

Several aspects of the ecology of oospores of Phytophthora infestans were studied in the Netherlands using both observational and experimental methods. Following the introduction of a genetically variable late blight population in Europe during the 1970s, P. infestans epidemics have become more severe, leading to an increase in fungicide use in many potato production areas in Northwestern Europe. In the Netherlands, oospores are readily produced in unsprayed crops and volunteer potatoes and their incidence varied from 78% to 15% of sampled leaflets with two or more lesions, for the northeastern and southwestern region in 2000, respectively. A fungicide application following infection of plants with an A1 and A2 mating type strain significantly reduced the number of oospores produced as well as oospore viability. Several alternative hosts facilitating oospore formation have been identified in the Netherlands: S. nigrum, S. dulcamara and S. sisymbriifolium. The impact of oospores on late blight epidemics is discussed.

Background: The National Health Fund (NHF) is the public payer solely accountable for securing and organizing access to health care services in Poland. The NHF is responsible for implementing a project entitled “Primary Health Care PLUS” which aims to introduce a primary care centered model, based on coordinated, proactive and preventive methods relevant to patients’ needs and furthermore, works to keep patients well-informed and active participants in health care decision-making. The implementation period of the project is July 1, 2018 through Dec. 31, 2021. Aim of the study: The purpose of this study is to outline patient demographics and staff structures of providers that took part in the PHC PLUS pilot program, as well as the status of the program throughout the first year of its implementation. Material and methods: Following an open and transparent recruitment process, 42 urban and rural primary health care providers were selected. The purpose of the providers’ geographic spread was to ensure the model was tested in all representative regions of the country. Results: 42 PHC PLUS providers attending to 288,392 patients are participating in the project. Approximately 1,100 medical staff members are involved in the project. PHC PLUS medical teams consist of specialists including physicians, coordinators, nurses, dietitians, psychologists, physiotherapists and health educators. Out of 41,022 health risk assessments declared to be conducted during the project, 18,058 (43.1%) were performed from July 1, 2018 to April 30, 2019, including 4,537 basic and 13,521 extended assessments. Furthermore, 15,020 patients in total, participated in the disease management programs, which are also paid from the project. Conclusions: A Polish health care pilot project that centers primary care can help shift the focus toward preventive interventions, rather than the current system, which often focuses on providing medical care to patients who have already been diagnosed with diseases, often in their more advanced stages.

People live with one or more chronic diseases, medical and surgical treatments of which may affect their quality of life negatively. Thermobalancing therapy (TT) and Dr Allen’s therapeutic Devices (DATD) for the first time uses own body energy to treat chronic internal diseases. This invention received a US patent. Here we observe condition of people with benign prostatic hyperplasia (BPH), kidney stone disease and chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) before and after use of TT and DATD. 10 years of empirical evidence has been studied in people with kidney disease, CP/CPPS and BPH. 2 clinical controlled trials on TT with DATD were conducted. The dynamics of clinical characteristics and parameters, such as pain, urinary symptoms and ultrasound prostate volume (PV), were compared with the no-treatment control groups. 124 men with BPH and 45 men with CP/CPPS used DATD for 6-month period, as mono-therapy, in clinical trials. The outcomes confirmed that TT with DATD reduced pain and PV in men with CP/CPPS and PV and urinary symptoms in men with BPH, while there were no positive changes in the control groups. 4 people with kidney stones, random from the 10-year follow-up, confirmed that after using DATD kidney stones were dissolved without side effects and complications. DATD is a class-1 medical device, which do not need an involvement of notified body, and it is easy use this device. Therefore, people with kidney stones, CP/CPPS and BPH should use DATD, as a tool for self-management in the first place.

Potato is one of the most important food crops with a capacity to abate hunger among farmers in the region. However, its productivity has stagnated and remained low due to poor agronomic practices applied by smallholder farmers. To avert this situation, dissemination through training of already existing better technologies and practices should be prioritized. A lot of research has been done with better recommendations- use of disease-free tuber seeds from high yielding and tolerant varieties, adequate application of balanced nutrients, timely scouting and integrated management of pests and diseases and good harvesting practices are available for immediate adoption. The adoption and dissemination could be facilitated by currently existing policies that promote cross-border movement and diffusion of research technologies and new varieties.

The aim of the studywas to evaluate the cost of surgical care and short-term outcomes of Crohn’s disease(CD) in the era of laparoscopy and biologic therapy. Material and methods. Review of a prospective database identifiedCD patients that underwent surgical management. Patients were stratified into laparoscopic, open, and converted approaches. Main outcome measures were short-term patient outcomes and cost of care by approach. Results. 92 patients were analyzed- 63.1% laparoscopic, 32.6% open, 4.3% converted. The majority was elective (100% converted, 94.8% laparoscopic, 90% open) and segmental resections (75% converted, 70.7% laparoscopic, 43.3% open). Operative times were similar between laparoscopic and open (152 minutes and 138 minutes, respectively). More open patients required ICU care (20% versus 12.1% laparoscopic and 0% converted). The median LOS was 3 (1-25) days laparoscopic, 4 (1-29) open and 4 (3-8) converted. The laparoscopic complication rate was 15.5%, readmission rate 12.1%, and reoperation rate 8.6%. The mean total hospital cost was $9,702 laparoscopic, $10,782 open, and $13,293 for converted cases (US Dollars). Conclusions. Laparoscopy is appropriate for most CD cases. When necessary to combine with open surgery, this results in efficient and effective patient outcomes and healthcare utilization. These results are important when weighing the cost of ongoing medical therapy versus surgical intervention.

Introduction and objective. The healthcare system is positioned in the patient’s environment and works with other determinants of the treatment. Patient care requires a whole system compatible to the needs of organizational and technical solutions. The purpose of this study is to present a new model of patient-oriented care, in which the use of information and communication technology (ICT) can improve the effectiveness of healthcare for patients with chronic diseases. Material and methods. The study material is the process of healthcare for chronically ill patients. Knowledge of the circumstances surrounding ecosystem and of the patients’ needs, taking into account the fundamental healthcare goals allows us to build a new models of care, starting with the economic assumptions. The method used is modeling the construction of efficient healthcare system with the patient-centered model using ICT tools. Results. We present a new systemic concept of building patient’s environment in which he is the central figure of the healthcare organization – so called patient centered system. The use of ICT in the model of chronic patient’s healthcare can improve the effectiveness of this kind of care. The concept is a vision to making wide platform of information management in chronic disease in a real environment ecosystem of patient using ICT tools. Conclusions. On the basis of a systematic approach to the model of chronic disease, and the knowledge of the patient itself, a model of the ecosystem impacts and interactions through information feedback and the provision of services can be constructed. ICT assisted techniques will increase the effectiveness of patient care, in which nowadays information exchange plays a key role.

Przyjęte w Polsce rozwiązania prawne pozwalają w sytuacji wystąpienia choroby zakaźnej ludzi na podjęcie stosownych działań, zarówno w oparciu o postanowienia ustawy z dnia 5 grudnia 2008 r. o zapobieganiu i zwalczaniu zakażeń i chorób zakaźnych u ludzi, jak i ustawy z dnia z dnia 18 kwietnia 2002 r. o stanie klęski żywiołowej. Mając na uwadze powyższe należy rozważyć zasadność funkcjonowania w polskim systemie prawnym w obszarze zarządzania bezpieczeństwem w państwie dwóch odmiennych stanów, tj. stanu nadzwyczajnego, jakim jest stan klęski żywiołowej i „stanów nienadzwyczajnych” jakimi są stan zagrożenia epidemicznego i stan epidemii, których proklamowanie jest uwarunkowane możliwością wystąpienia bądź wystąpieniem tego samego zagrożenia, jakim jest choroba zakaźna ludzi.
The legal solutions adopted in Poland in the field of the security of the state and its citizens allow, in the event of a threat, including the occurrence of an infectious disease of people, to take appropriate measures, both on the basis of the provisions of the Act of December 5, 2008 on the prevention and combating of infections and diseases infectious in humans, as well as the Act of 18 April 2002 on the state of natural disaster. Bearing in mind the above, it is necessary to consider the legitimacy of the functioning of two different states in the Polish legal system relating to the management of security in a state, i.e. an emergency state, which is a state of natural disaster and "extraordinary states", such as an epidemic threat and a state of epidemics, the proclamation of which is conditional on the possibility of the occurrence or occurrence of the same threat as human infectious disease.

Gastroesophageal reflux disease (GERD) is defined as a condition in which the reflux of stomach contents causes troublesome symptoms and/or complications. This common disease may also present with atypical, extraesophageal symptoms. Laryngopharyngeal reflux (LPR) is the reflux of gastric contents into the throat and larynx, which causes symptoms such as globus, throat clearing, hoarseness and chronic cough. GERD and LPR may be related to many dis-eases, including laryngitis, asthma, COPD, chronic rhinosinusitis, otitis media, dental erosions, and even laryngeal cancer or life-threatening events. The diagnosis of LPR is based on clinical symptoms (measured by RSI), laryngoscop-ic signs (evaluated in RFS), an empiric trial of proton pomp inhibitor (PPI) therapy, 24-hour pH monitoring, impedance monitoring, esophageal manometry and endoscopic examination. The most common management is double-dose PPI therapy for at least six months. When this treatment is ineffective, the surgery should be considered. However, di-agnosis and treatment of LPR is still controversial. Further studies are necessary to establish an optimal algorithm for the management of LPR.

With the rising awareness of the adverse effects of chemical pesticides, people are looking for organically grown vegetables. Consumers are increasingly choosing organic foods due to the perception that they are healthier than those conventionally grown. Vegetable crops are vulnerable to a range of pathogenic organisms that reduce yield by killing the plant or damaging the product, thus making it unmarketable. Soil-borne diseases are among the major factors contributing to low yields of organic produce. Apart from chemical pesticides there are several methods that can be used to protect crops from soil-borne pathogens. These include the introduction of biocontrol agents against soil-borne plant pathogens, plants with therapeutic effects and organic soil amendments that stimulate antagonistic activities of microorganisms to soil-borne diseases. The decomposition of organic matter in soil also results in the accumulation of specific compounds that may be antifungal or nematicidal. With the growing interest in organic vegetables, it is necessary to find non chemical means of plant disease control. This review describes the impact of soil-borne diseases on organic vegetables and methods used for their control.

In the years 1993-1995 experiments on integrated pest management (IPM) were conducted. A three year experiment with winter wheat was performed to evaluate the effect of chemical control of diseases on their occurrence and grain yield, at various conditions of N fertilisation. The economical profit was analysed.
W latach 1993-1995 podjęto doświadczenia dotyczące stosowania Integrowanych Metod Ochrony Roślin (IPM). Doświadczenia przeprowadzono na pszenicy ozimej, oceniano efekt zwalczania chorób po zastosowaniu fungicydów przy różnych poziomach nawożenia azotowego. Nawożenia azotowe ustalono na poziomie dawki standardowej - zgodnie z analizą chemiczną. Określono efekt ekonomiczny i stwierdzono, że przy niskim porażeniu pszenicy przez choroby największy wpływ na plon miały wzrastające dawki azotu. Zastosowanie fungicydów było ekonomicznie uzasadnione tylko przy najniższym poziomie nawożenia N w warunkach niskiego porażenia pszenicy ozimej.

Potato virus Y (PVY) is one of the most destructive viruses infecting potato in Egypt and worldwide. Recent research has shown that a necrotic PVY-NTN strain is infecting potato in Upper Egypt. Chemical control is not effective to control this viral pathogen. An alternative to control PVY infecting potato is using a mild PVY strain to elicit systemic cross protection in potato plants against infection with a severe necrotic strain of PVY. Results of this study showed that a PVY necrotic strain produced a significant lesser number of local lesions on diagnostic plants (Robinia pseudoacacia L.) when these plants were treated first with a mild PVY strain. Data obtained from greenhouse and field experiments indicated that treatment of potato plants (variety Burna) with a mild PVY strain significantly protected potato from infection with a severe necrotic PVY strain, and resulted in a significant increase in tuber yield compared with infected plants without prior treatment with a mild PVY strain. The highest increase in potato tuber yield was obtained when potato plants were inoculated with a mild PVY strain 3 days before challenging with the severe necrotic PVY strain. This study proved that using a mild strain of PVY can significantly protect potato plants from infection with a severe strain of this virus under both greenhouse and field conditions and can present a potential method to reduce losses due to infection of this virus in Assiut governorate and Upper Egypt.
