Opis:
During organogenesis in the developing goat embryo, a four-chamber stomach is formed from an extension of the primary intestine. Due to histological and morphological changes, each chamber of the stomach takes on characteristic features associated with its function. The histological structure of the stomach wall consists of four layers, the mucosa, submucosa, tunica muscularis and serosa. This work presents the current state of knowledge of the changes observed in each layer of the developing stomach and its morphogenesis in the prenatal period. Practical applications of this information in prenatal ultrasound diagnostics in goats are suggested as well.

The article presents a perspective of somatic present in the prose of Nikolai Gogol. The body is treated here as a project — a collection of body fragments. Particular attention is paid to the figure of nose, which occurs in two selected short stories entitledNose and Diary of a Madman. Therefore, any attempt to emancipate the body part is a threat to the need to preserve the face. Nose is a sense of belonging. This part of the body is thus a limit to what we used to be referred to as belonging to us. Its location creates the last bastion of perception, the selfness.

The gastrointestinal tract is an extraordinary human organ in terms of its morphology and function. Its complex structure and enormous length as well as frequent presence of gas discourage many doctors performing ultrasound examination from its exploration. Moreover, there are anatomical structures in multiple locations which can mimic certain abnormalities. It is difficult to present an exhaustive account of the problem of gastrointestinal tract ultrasound imaging errors in a single work; therefore, this study focuses mainly on false positive errors which usually result from a lack of knowledge of anatomical variants of the gastrointestinal tract structure. In the case of the stomach, rugae and muscle layer thickening towards the pylorus have been mentioned, which constitute variants of the structure of this organ examined when empty. Diagnostic pitfalls in the small intestine may include the dudenojejunal flexure (ligament of Treitz), the horizontal part of the duodenum and the ileocaecal valve. The status of the apparent lesions in all of the cases mentioned will be resolved following fluid intake by the patient. In the colon, the varied structure of semilunar folds should be taken note of. Their large thickness can warrant suspicion of wall invasion or a polyp. In addition, the study emphasises the importance of appropriate preparation of a patient for gastrointestinal tract examination since it determines the accuracy of the diagnosis. The authors also take note of common ‘sins’ of physicians such as hasty examination and failure to comply with the stomach and appendix examination protocol.
Cewa pokarmowa to wyjątkowy pod względem morfologicznym i czynnościowym narząd ciała ludzkiego. Jej złożona budowa, ogromna długość oraz częsta obecność w niej gazu zniechęcają do jej eksploracji wielu lekarzy wykonujących badanie ultrasonograficzne. Ponadto w wielu miejscach można spotkać struktury anatomiczne mogące pozorować istnienie zmian patologicznych. W jednym opracowaniu trudno przedstawić całość problematyki związanej z błędami popełnianymi w diagnostyce ultrasonograficznej cewy pokarmowej, dlatego w prezentowanej pracy skupiono się głównie na błędach fałszywie pozytywnych, które w przeważającej mierze wynikają z nieznajomości wariantów anatomicznych budowy przewodu pokarmowego. W odniesieniu do żołądka zwrócono uwagę na tzw. ufałdowanie i grubiejącą w kierunku odźwiernika błonę mięśniową – zmienności budowy ściany tego narządu badanego na czczo. W jelicie cienkim pułapkę diagnostyczną mogą stanowić: zgięcie dwunastniczo-czcze Treitza, poziomy odcinek dwunastnicy i zastawka krętniczo-kątnicza. We wszystkich wymienionych przypadkach rozstrzygające będzie zachowanie się tych tzw. rzekomych zmian po napojeniu osoby badanej. W jelicie grubym należy zwracać uwagę na zróżnicowaną budowę fałdów półksiężycowatych, których duża grubość może nasuwać podejrzenie nacieku ściany bądź polipa. Ponadto w pracy podkreślono znaczenie prawidłowego przygotowania pacjenta do badania cewy pokarmowej, od tego bowiem zależy trafność stawianych rozpoznań. Autorzy zwracają także uwagę na częste „grzechy” w pracy lekarzy, takie jak pospieszne badanie oraz nietrzymanie się metodycznego standardu badania USG żołądka i wyrostka robaczkowego.

Over the last decade, gastric cancer treatment has changed from extensive multiorgan resections towards less invasive approaches with limited resections and a more selective lymphadenectomy. Despite all available trials, the conclusions on the extent of necessary resections still remain debatable. The aim of the study was to assess the short term outcomes (morbidity and mortality) of a total gastrectomy depending on the simultaneous splenectomy status. Material and methods. We performed a retrospective analysis of the records of all patients treated with a curative intent using a total gastrectomy for gastric cancer between 1997 and 2003. 49 patients fulfilled the inclusion criteria. Patients were divided into two groups: S(+) gastrectomy with splenectomy group (29 patients) and S(-) total gastrectomy with spleen preservation (20 patients). Results. Survival analysis at one year showed that there was no difference in survival between the two groups (p=0.84). There were six recurrences, one in the group S(+) and five in group S(-) (p>0.05). Dissemination was observed in three patients in group S(-) (p>0.05). Other complications including infectious complications, exenteration, subileus, cardiovascular insufficiency, multiorgan failure were more frequent in the S(+) group (31% v 15%) although the difference was not significant (p=0.17). Conclusions. Splenectomy during gastrectomy for cancer has no statistically significant impact on short-term morbidity and mortality. Even though it does not show benefit in terms of 5-year overall survival rates it might be performed when needed in more advanced cases in properly selected patients (e.g. upper gastric T3/4 gastric cancer)

Two molecules - vascular endothelial growth factor involved in new vessels formation and survivin - antiapoptotic protein, reported to be associated with worse prognosis in various malignancies have been chosen for the study. Both are potential target for novel therapiesThe aim of the study was to determine the immunostaining of VEGF and survivin in gastric carcinoma and to analyse their relationship to the selected clinicopathological features and survival.Material and methods. Formalin-fixed, paraffin-embedded sections from 41 gastric adenocarcinomas were used for immunohistochemical reaction with monoclonal antibodies against vascular endothelial growth factor and survivin. The results were compared with selected clinicopathological features and survival.Results. Positive immunohistochemical reaction for vascular endothelial growth factor and survivin was revealed in 24 (58,53%) and 30 (73,17%), gastric carcinomas respectively. Vascular endothelial growth factor-negative gastric carcinomas were significantly more common in cases without metastases to regional lymph nodes and distant organs and in less advanced cases. Similar, distant metastases were also statistically less common in survivin-negative carcinomas. The differences in immunohistochemical reactions for survivin between less and more advanced cases almost reach statistical significance. The only factors significantly influenced 1, 2 and 3-year survival were vascular endothelial growth factor and survivin status. Statistically significant higher percentage of survival was noted in patients with vascular endothelial growth factor- and survivin-negative tumors.Conclusions. It seems that vascular endothelial growth factor and survivin play role in local invasion and spread of gastric adenocarcinoma and negatively influences survival. However, further studies are required to assess their true usefulness in the clinical practice.

Ectopic pancreas is a rare congenital disorder defined as pancreatic tissue lacking vascular or anatomic communication with the normal body of the pancreas. Most cases of ectopic pancreas are asymptomatic, but it may become clinically evident depending on the size, location and the pathological changes similar to those observed in case of the normal pancreas. It is often an incidental finding and can be located at different sites in the gastrointestinal tract. The most common locations are: the stomach, duodenum or the proximal part of small intestine. The risk of malignancy, bleeding and occlusion are the most serious complications. Despite the development in diagnostics, it still remains a challenge for the clinician to differentiate it from neoplasm. In this report, we described a case of 28-years old woman who presented recurrent epigastric pain. The upper gastrointestinal endoscopy revealed gastrointestinal stromal tumor on the border of the body and antrum of the back wall of great curvature of the stomach. The histopathological examination after surgery showed heterotopic pancreatic tissue. Ectopic pancreas should be considered in the differential diagnosis of gastric mass lesions.

Opis:
The cutaneous recording of gastric myoelectrical activity of the stomach known as electrogastrography (EGG) seems to be the promising tool for the non-invasive assessment of gastric motility. Unfortunately the EGG recording is usually severely contaminated both by motion artefacts and endogenous biological noise source. In order to use EGG signals as reliable diagnostic tool it is necessity to look for the effective artefacts removal methods. In this paper Continuous Wavelet Transform (CWT) was applied for detection motion artefacts from the EGG data. The set of own mother wavelets extracted directly from EGG signal was created and applied for detecting motion artefacts from one channel EGG recording. The results was compared with the effects obtained by using standard mother wavelets. The proposed method based on CWT with own mother wavelet presents very good performance for detecting motion artefacts from the EGG data.

Celem pracy jest przedstawienie możliwości programu komputerowego WIN-ESTIMATE, stworzonego w 1984 r. przez Keichi Maruyamę – ordynatora oddziału chirurgii w Narodowym Szpitalu Onkologicznym w Tokio, a następnie zmodyfikowanego i przystosowanego do środowiska Windows przez zespół Rüdigera Siewerta z Kliniki Chirurgii Uniwersytetu Technicznego w Monachium. Program ten w obecnej wersji 2.5 szacuje przedoperacyjną obecność przerzutów w węzłach chłonnych u poszczególnych chorych poprzez porównanie ich danych z informacjami zawartymi w bazie danych pacjentów leczonych w latach 1969-1989. W analizie retrospektywnej sprawdzono zastosowanie programu Maruyamy w polskim ośrodku chirurgicznym. Badaniami objęto 40 chorych na raka żołądka, w tym 9 kobiet i 31 mężczyzn. Wszyscy pacjenci przebyli resekcję żołądka z limfadenektomią w ówczesnej II Katedrze i Klinice Chirurgii Ogólnej Akademii Medycznej w Lublinie, a całość danych klinicznych i patologicznych wprowadzano do programu i poddano analizie statystycznej z zastosowaniem nieparametrycznego testu Kruskala- Wallisa. Analiza wykazała, że program ten umożliwia niezależnie od przyjętych standardów dokładną i indywidualną ocenę rozległości zabiegu operacyjnego.
The aim of the study is evaluation of possibilities of WINESTIMATE computer program application, created in 1984 by Keichi Maruyama, the head of the Surgery Department in National Cancer Center in Tokyo, and subsequently modified and Windows-adapted by Rüdiger Siewert’s team of the Surgery Department, Technical University of Munich. The upgraded 2.5 version of the program estimates preoperatively the presence of lymph nodes metastases in patients and compares data with records of patients who underwent surgical treatment between 1969 and 1989. Presented retrospective analysis shows how the application of Maruyama program complies with the situation of surgery centres in Poland. Forty gastric cancer patients (31 men and 9 women) were included in the study. All patients underwent stomach resection with lymphadenectomy in 2nd Department of General Surgery, Medical University of Lublin. The clinical and pathological data were introduced into the program. Kruskal and Wallis’ non-parametric test was used in the statistic analysis. Results evaluation proved that regardless of the adopted standards, computer program allows a thorough and individual estimation of the extent of the necessary surgical treatment.

ntroduction: A proper level of nutrition is significant in the period of convalescence in patients subject to major surgical procedures, particularly due to neoplastic disease. Bioelectrical Impedance Analysis, or BIA is a widely used method in assessing body mass composition. BIA measurement is easy, quick, cheap and repetitive. Material and methods: We assessed the body composition of 56 patients (25 women and 31 men) hospitalized and operated at the Department of General and Oncological Surgery of the Wroclaw Medical University in the years 2017–2018 using bioelectric impedance. Results: The average body weight loss in the 4th postoperative day was 1.32% of body mass and on the day of release from hospital – 4.23% of body mass in relation to body mass upon admission. The percentage of body fat (FM – Fat Mass) in patients admitted to the department is above the normal range. The change in body composition in hospitalized patients mainly concerns the amount of adipose tissue and the amount of extracellular and intracellular water (ECW – Extracellular Water; ICW – Intracellular Water). Conclusions: Bioelectrical impedance can be an easy and effective method of assessing body composition and its change in patients undergoing major surgery. Amongst the analyzed groups, patients operated for pancreatic cancer lose the largest percentage of body weight until discharge from the department. Loss of body mass mainly occurs as loss of fat mass (FM).

Introduction: Helicobacter pylori plays the main role in the development of chronic pathology of the gastroduodenal area. Helicobacter pylori infects 25% of the population in developed countries and up to 90% in developing countries. The most virulent are CagA positive (+) strains of Helicobacter pylori, which cause an intense cellular response, i.e. inflammation of the stomach mucous membrane, cell proliferation and cell death. Material and methods: We examined 75 children, aged 10 to 18 years, with chronic gastroduodenal pathology associated with Helicobacter pylori. A morphological and morphometric study of the stomach mucous membrane was performed depending on the presence of cytotoxic strains of Helicobacter pylori. Results: In children infected with CagA (+) strains of Helicobacter pylori, active and deep chronic gastritis and gastroduodenitis [21 (28.00%), 20 (26.67%), respectively] were more common than in children with CagA (−) strains [10 (13.33%), 13 (17.33%), respectively]. Discussion: Morphological studies provide an opportunity to determine quantitative and qualitative changes in the mucous membrane of the stomach in gastroduodenal pathology associated with Helicobacter infection. This increases the quality of diagnosis and predicts the course of the disease. Conclusion: A morphological study has shown that the presence of cytotoxic strains of Helicobacter pylori contributes to more significant changes in the gastric mucosa in children with chronic gastroduodenal pathology.
Wstęp: Helicobacter pylori odgrywa główną rolę w rozwoju przewlekłej choroby żołądka i dwunastnicy. Zakażenie tą bakterią dotyczy 25% populacji w krajach rozwiniętych i około 90% w krajach rozwijających się. Najbardziej wirulentne szczepy CagA-pozytywne (+) powodują intensywną odpowiedź komórkową w postaci stanu zapalnego błony śluzowej żołądka, proliferacji komórek i apoptozy. Materiał i metody: Badaniem objęto 75 dzieci w wielu 10–18 lat z przewlekłą chorobą żołądka i dwunastnicy związaną z zakażeniem Helicobacter pylori. Ocenę morfologiczną i morfometryczną błony śluzowej żołądka przeprowadzono w zależności od obecności cytotoksycznych szczepów Helicobacter pylori. Wyniki: W grupie dzieci zakażonych szczepami CagA (+) postać aktywna i głęboka zapalenia żołądka i dwunastnicy [odpowiednio 21 (28,00%) i 20 (26,67%)] występowała częściej niż w grupie dzieci zakażonych szczepami CagA (−) [odpowiednio 10 (13,33%) i 13 (17,33%)]. Omówienie: Badania morfologiczne umożliwiają ocenę ilościową i jakościową zmian w błonie śluzowej żołądka u pacjentów z chorobą żołądka i dwunastnicy związaną z zakażeniem Helicobacter pylori. Zwiększają tym samym jakość diagnostyki i pozwalają przewidzieć przebieg choroby. Wnioski: Badanie morfologiczne wykazało, że zakażenie cytotoksycznymi szczepami bakterii Helicobacter pylori przyczynia się do powstawania istotnych zmian w błonie śluzowej żołądka u dzieci z chorobą żołądka i dwunastnicy.
