Objectives Exposure to environmental tobacco smoke (ETS) constitutes a threat to the health of many people. In order to diminish ETS exposure, countries (including Poland) implemented legal restrictions of smoking in public places and worksites. Currently more attention is also paid to reduce overall and residential ETS exposure by voluntary smoke-free home policy adoption. The aim of current analysis was to evaluate the prevalence and determinants of implementing smoking bans at place of residence among economically active males and females in Poland. Material and Methods Data from cross-sectional, household study – Global Adult Tobacco Survey (GATS 2009–2010) were analyzed. The logistic regression model was applied for appropriate calculations. Results Out of 3696 studied subjects only 37.1% adopted total smoking ban within the home. Decreased likelihood of adopting total smoking bans was associated with current smoker status, low education attainment, lack of awareness on adverse health consequences of ETS, low level of support for tobacco control policies, and cohabitation with a smoker in both genders. Having smoke-free homes was also linked with age in women, place of residence and work smoking policy in indoor areas in men. Conclusions Targeted activities to encourage adopting voluntary smoke-free rules among groups least likely to implement 100% smoking bans in the home and activities to decrease social acceptance of smoking in the presence of nonsmokers, children, pregnant woman are urgently needed.

Introduction and Aim. Breast milk is the first nourishment in the infancy period containing environmentally present chemical compounds. Literature data on the presence of heavy metals in mothers’ milk is insufficient. The study is to determine the influence of environmental factors on the content of heavy metals in breast milk. Material and Methods. This paper presents results of an investigation on the occurrence of heavy metals in breast milk. Absorption spectrometry technique using VARIAN Techtron Instrument type 5DA determined the content of Cu, Cd, Pb, Zn, Ni, Mn, Fe. The mothers were 16 to 40 years old and lived at least 5 years in the industrial region (Upper Silesia and Częstochowa, Cieszyn – Poland). Results. The co-occurrence of metals in the milk and the change of heavy metals concentration depends on the lactation time. Smoking and air pollution are important factors which influence the content of heavy metals in breast milk. Conclusions. Breast milk for infants is an important source of nourishment containing heavy metals.
Wstęp i cel. Mleko kobiety jest pierwszym pokarmem zawierającym związki chemiczne obecne również w jej środowisku. Celem prowadzonych badań było określenie wpływu czynników środowiskowych na zawartość metali ciężkich w mleku kobiet. Materiał i metody. W pracy przedstawiono wyniki badań nad występowaniem metali w mleku kobiecym. Zawartości Cu, Cd, Pb, Zn, Ni, Mn, Fe oznaczano metodą AAS. Użyto aparatu VARIAN-5A. Badaniom poddano kobiety w wieku 16–40 lat, które mieszkały co najmniej 5 lat na Górnym Śląsku, w Cieszynie i Częstochowie. Wyniki. Współwystępowanie metali w mleku zależało od okresu laktacji. Czynnikiem wpływającym na zawartość metali w mleku były: stopień zanieczyszczenia przyziemnej warstwy powietrza oraz dym tytoniowy jako produkt oddziałujący na organizm karmiącej kobiety inhalowany zarówno podczas czynnego, jak i biernego palenia tytoniu. Wnioski. Mleko kobiece może być źródłem znaczącej ilości metali ciężkich.

Objectives Expanding the information on exposure to environmental tobacco smoke (ETS) at home and its associates is of great public health importance. The aim of the current analysis was to evaluate associates of exposure to environmental tobacco smoke among economically active male and female adults in Poland in their place of residence. Material and Methods Data on the representative sample of 7840 adults from the Global Adult Tobacco Survey (GATS) carried out in Poland in the years 2009 and 2010 were applied. The Global Adult Tobacco Survey is a nationally representative household study. The logistic regression model was used for relevant calculations. Results The exposure to environmental tobacco smoke in the place of living affected 59% of studied subjects. Out of non-smokers 42% of males and 46% females were exposed to the ETS in the at home. Increased risk of residential ETS exposure was associated with low education attainment, lack of awareness on adverse health consequences of second hand smoke (SHS), low level of support for tobacco control policies, living with a smoker. One of the factors associated with the ETS exposure was also the approval for smoking at home of both genders. The residential ETS exposure risk was the highest among males (odds ratio (OR) = 7.1, 95% confidence interval (CI): 6.1–13.8, p < 0.001) and females (OR = 8.1, 95% CI 6.5–11.8, p < 0.001) who declared that smoking was allowed in their place of residence compared to respondents who implemented smoking bans at their place of residence. Conclusions Campaigns to decrease social acceptance of smoking and encourage adopting voluntary smoke-free rules at home might decrease the ETS exposure and reduce related risks to the health of the Polish population. Educational interventions to warn about adverse health effects of the ETS should be broadly implemented particularly in high risk subpopulations.

Objectives: The aim of the study was to use meconium samples to assess fetal exposure to compounds present in environmental tobacco smoke (ETS). Material and Methods: In order to assess fetal exposure to toxic tobacco smoke compounds, samples of meconium from the offspring of women with different levels of tobacco smoke exposure, and the samples of saliva from the mothers were analyzed. Thiocyanate ion as a biomarker of tobacco smoke exposure, and other ions that are indices of such exposure were determined by means of ion chromatography. Results: The results of ion chromatography analysis of the meconium and maternal saliva samples for the presence of cations and anions (including thiocyanate ion) indicate that the concentration level of specific ions depends on the intensity of environmental tobacco smoke exposure of pregnant women. Conclusions: Based on the results, it can be concluded that meconium samples can be used to determine the substances from tobacco smoke. The results confirm the effect of smoking during pregnancy on the presence and content of substances from tobacco smoke.

Smoking prevalence in Poland at the background of European Union countriesSmoking prevalence in Europe is worth analyzing in order to observe exposure of population to tobacco smoke, what is informative and  mportant from the public health point of view. Aim of the paper was to compare smoking prevalence between European Union countries, with the special attention to Poland. The analysis was conducted separately by age, sex and smoking categories. Results part contains systematic review of the 2002 data for Poland at the background of the rest of the EU countries. Presented analysis shows past and present state of tobacco epidemics in Poland and is a good indicator of tobacco-related health threats in Polish population. Discussion part contains between others methodological considerations on how to read the smoking prevalence analysis results to avoid producing misleading message.

ObjectivesAt the end of the 20th century, after years of negligence in the prevention of cardiovascular diseases, Poland was struggling with very high premature mortality. The period of 1991–2005 brought significant improvements since the general public introduced beneficial dietary modifications. This paper aims to analyze the changes in the rate of premature mortality due to tobacco-dependent cardiovascular diseases in Poland in 2008–2017.Material and MethodsThe time trends of deaths occurring under the age of 65 years caused by ischemic heart disease, cerebrovascular disease, atherosclerosis and aortic aneurysm were analyzed. Both standardized and crude premature mortality rates were used, as well as mortality rates for patients grouped into 5-year age ranges with a breakdown by gender. The joinpoint model was used to determine these time trends.ResultsPremature mortality due to the analyzed cardiovascular diseases decreased linearly in 2008–2017. In the case of ischemic heart disease and cerebrovascular diseases, the decrease amounted to approx. 5% per year, both in the female and male population. However, in the case of atherosclerosis and aortic aneurysms, the rate of mortality reduction ranged 4–7% per year. The reduction concerned all the examined age groups, but with different dynamics. The most considerable annual decrease was observed in the group of patients aged 40–44 years (7.9% for females and 8.9% for males). Along with the increase in age, the dynamics of reduction decreased.ConclusionsIn 2008–2017, Poland experienced a decline in premature mortality due to tobacco-related cardiovascular diseases, particularly in the age group of 40–44 years. The decline may have been associated, among other things, with a reduction in exposure to tobacco smoke, one of the cardiovascular risk factors.

Four-month-old female Wistar rats were exposed for 20 days to tobacco smoke obtained from non-filter cigarettes. During the exposure, concentration of tobacco smoke was monitored indirectly by measuring the CO level (1500 mg/m3 air). The efficacy of exposure was assessed by measuring urine nicotine and cotinine levels. Cigarette smoke did not change total cytochrome P450 and b5 protein levels in any of the organs studied, and most of these organs did not show any changes in the activity of reductases associated with these cytochromes. Following exposure to tobacco smoke, fetal rat liver expressed CYP2B1/2 protein; in newborns (day 1) both liver and lung showed CYP2B1/2 protein expression and very low pentoxyresorufin O-dealkylase activity. Western blot analysis of adult liver, lung, heart, but not of brain microsomes, showed that tobacco smoke induced CYP2B1/2 in both nonpregnant and pregnant rats, though its expression was lower in the livers and hearts of pregnant females. In the rat and human placenta, neither rat CYP2B1/2 nor human CYP2B6 showed basal or tobacco smoke-induced expression at the protein level. This study shows clearly that the expression of CYP2B1/2, which metabolizes nicotine and some drugs and activates carcinogens, is controlled in rats by age-, pregnancy-, and tissue-specific regulatory mechanisms.

Dym tytoniowy zawiera tysiące czynników chemicznych szkodliwych dla zdrowia człowieka. Mają one działanie drażniące, toksyczne i rakotwórcze. Bierne palenie tytoniu w miejscach publicznych i narażenie na środowiskowy dym tytoniowy także w miejscu pracy stanowią ogromny problem medyczny. Wynika z niego potrzeba uświadamiania pracowników w zakresie szkodliwości nałogu palenia tytoniu oraz stworzenia aktów prawnych mających na celu wyeliminowanie narażenia na dym tytoniowy. W niniejszej pracy przedstawiono wyniki przeglądu piśmiennictwa na temat biernej ekspozycji na dym tytoniowy i jej wpływu na zdrowie człowieka oraz dokonano przeglądu obowiązujących w Polsce aktów prawnych dotyczących palenia papierosów w miejscu pracy. Dotychczas opublikowane dane wskazują, że ekspozycja na dym tytoniowy w okresie życia płodowego zwiększa ryzyko m.in. porodu przedwczesnego i małej masy urodzeniowej noworodka, nagłej śmierci niemowląt oraz w przyszłości – upośledzenia funkcji płuc, astmy i ostrych schorzeń dróg oddechowych. Narażenie na dym tytoniowy, także wyłącznie w wieku dorosłym, jest natomiast niezależnym czynnikiem ryzyka rozwoju schorzeń układu krążenia i układu oddechowego oraz nowotworów złośliwych. Podniesienie świadomości społecznej na temat szkodliwości ekspozycji na dym tytoniowy powinno być priorytetem w ochronie zdrowia pracujących. Lekarz sprawujący opiekę profilaktyczną nad pracownikami jest przedstawicielem służby zdrowia, który ma najczęstszy kontakt z aktywnymi zawodowo dorosłymi palaczami tytoniu. Wynika z tego niezwykle istotna rola służby medycyny pracy w zwiększaniu świadomości pracowników i pracodawców w zakresie profilaktyki negatywnych skutków zdrowotnych nałogu palenia tytoniu. Med. Tobacco smoke contains thousands of xenobiotics harmful to human health. Their irritant, toxic and carcinogenic potential has been well documented. Passive smoking or exposure to second-hand smoke (SHS) in public places, including workplace, poses major medical problems. Owing to this fact there is a strong need to raise workers’ awareness of smoking-related hazards through educational programs and to develop and implement legislation aimed at eliminating SHS exposure. This paper presents a review of reports on passive exposure to tobacco smoke and its impact on human health and also a review of binding legal regulations regarding smoking at workplace in Poland. It has been proved that exposure to tobacco smoke during pregnancy may lead to, e.g., preterm delivery and low birth weight, sudden infant death syndrome, lung function impairment, asthma and acute respiratory illnesses in the future. Exposure to tobacco smoke, only in the adult age, is also considered as an independent risk factor of cardiovascular diseases, acute and chronic respiratory diseases and cancer. Raising public awareness of tobacco smoke harmfulness should be a top priority in the field of workers’ health prevention. Occupational medicine physicians have regular contacts with occupationally active people who smoke. Thus, occupational health services have a unique opportunity to increase employees and employers’ awareness of adverse health effects of smoking and their prevention. Med Pr 2015;66(6):827–836

Opis:
Badano wpływ dymu tytoniowego, aniliny, relanium oraz diety niskobiałkowej na zdolność rozrodczą i potomstwo szczurów. Badania wykonano na sześciu miotach białych szczurów. W pierwszym etapie badano efekty działania dymu tytoniowego, którego stężenie kontrolowano drogą pomiaru tlenku węgla w powietrzu komory doświadczalnej.
The effect of tobacco smoke and low-protein diet on rat reproducibility and offspring was investigated in six litters of white rats. Smoke concentration was controlled by determining the air carbon oxide content which aberaged 1000 mg/m3. During the experiments blood carboxyhemoglobin level was measured; by the end of exposure, each generation displayed closely similar concentrations amounting to 23.9-26.6%. There were no diet-related differences in blood carboxyhemoglobin concentrations.

Opis:
Wprowadzenie. Biorąc pod uwagę negatywny wpływ używek, w tym alkoholu, nikotyny oraz nadmiernego spożycia kofeiny na organizm dziecka i matki, bardzo ważnym elementem profilaktyki jest zaprzestanie lub ograniczenie ich stosowania, zwłaszcza w okresie ciąży, i laktacji. Cel badań. Celem pracy była ocena spożycia alkoholu, palenia papierosów, narażenia na dym tytoniowy oraz spożycia kofeiny w wybranej grupie kobiet karmiących piersią z województwa mazowieckiego. Materiał i metoda. Badanie przeprowadzono w okresie od września 2010 do marca 2011 roku, wśród 102 kobiety w wieku od 19 do 38 lat. Informacje o spożyciu alkoholu, paleniu tytoniu oraz narażeniu na dym tytoniowy uzyskano metodą wywiadu kwestionariuszowego. Do oceny spożycia kofeiny zastosowano metodę 3-dniowego bieżącego notowania i metodę częstotliwości spożycia. Źródłem informacji o zawartości kofeiny w spożywanych produktach były dane literaturowe, a w przypadku napojów energetyzujących dane producenta zamieszczone na etykietach. Wyniki. Spośród wszystkich badanych kobiet (n=102), aż 17% respondentek deklarowało spożycie alkoholu, 6% palenie papierosów, a 15% bierne narażenie na dym tytoniowy. Średnie spożycie kofeiny, w grupie tzw. „konsumentów kofeiny” (n=94) wynosiło 127,4 ±76,0 mg/os/dzień dla metody 3-dniowego bieżącego notowania i 163,4 ±100,6 mg/os/dzień dla metody częstotliwości spożycia. Współczynnik korelacji pomiędzy zastosowanymi metodami wynosił r=0,71 (p<0,001). Głównymi źródłami kofeiny niezależnie od zastosowanej metody zbierania danych były: herbata czarna, która dostarczała około 60% kofeiny oraz kawa mielona (około 20%) i kawa instant (około 13%). Wnioski. Mimo powszechnej wiedzy nt. szkodliwości palenia papierosów/narażenia na dym tytoniowy oraz spożycia alkoholu i produktów bogatych w kofeinę, część respondentek nie zaprzestała stosowania tych używek w okresie laktacji, co wskazuje to na konieczność prowadzenia edukacji w tym zakresie.
Background. Taking into account the negative impact of stimulants, including alcohol, nicotine and excessive consumption of caffeine on the baby and his mother, a very important is to stop or to restrict their use, especially during pregnancy and lactation. Objective. Purpose of the study was to evaluate alcohol consumption, cigarette smoking, tobacco smoke exposure and caffeine consumption in breastfeeding women from Masovian Province. Material and method. The survey was conducted from September 2010 till March 2011. The study group consisted of 102 breastfeeding women aged 19-38 years. Information on alcohol consumption, smoking and exposure to tobacco smoke was obtained by questionnaire interview. The results about caffeine intake were obtained using 3-day dietary records method and food frequency questionnaire method. Source of information about the caffeine content in products were the published literature, in the case of energy drinks the manufacturer’s label. Results. Among all women surveyed (n = 102), up 17% of respondents declared alcohol consumption, cigarette smoking of 6% and 15% of passive exposure to tobacco smoke. The average caffeine consumption in a group called “caffeine consumers” (n = 94) was 127.4 ± 76.0 mg/person/day for 3-day dietary records method and 163.4 ± 100.6 mg/person/day for the food frequency questionnaire method. The correlation coefficient between the used methods was r=0.71 (p <0.001). The main sources of caffeine, regardless of the method of data collection were: black tea, which provided about 60% of caffeine and ground coffee (about 20%) and instant coffee (about 13%). Conclusions. Despite general knowledge about the harmful effects of smoking cigarette/ tobacco smoke exposure and the consumption of alcohol and foods high in caffeine, some respondents did not halt the use of these stimulants during lactation, indicating a need for an education in this field.

ObjectivesIt is especially difficult for hospitality workers to avoid secondhand smoke (SHS), meaning that they are likely particularly vulnerable to the effects of SHS. The authors aimed to determine the degree to which smoke-free laws protect hospitality workers from SHS exposure, by examining biochemical markers of such exposure.Material and MethodsThis was a cross-sectional study examining SHS exposure in non-smoking employees working in hospitality settings where smoking is prohibited or permitted. The following biomarkers were selected: cotinine and tobaccospecific nitrosamines, which are known to measure SHS exposure, and 2 representative carcinogens: 4-(methylnitrosamino)-1-(3-pyridyl)-1-butanol and 4-(methylnitrosamino)-1-(3-pyridyl)-1-butanone (NNK). The authors compared these biomarkers between 3 hospitality settings. A descriptive analysis was performed. In addition, they conducted 1-way and 2-way analysis of covariance (ANCOVA) to compare the biochemical markers.ResultsSmoking substances were identified by smoking ban levels. In the case of hair nicotine and urine cotinine, their concentrations were lower in areas with a complete smoking ban than in both areas with a separate smoking room and no smoking ban; however, there was no statistically significant difference between these. In the case of dust NNK, its level was the lowest in areas with a complete smoking ban. To confirm the smoking ban effect by hospitality settings, the authors checked the results of the 2-way ANCOVA. In karaoke and billiard halls, the dust NNK concentrations were significantly higher in areas with no smoking ban than in areas with a separate smoking room.ConclusionsExposure to SHS is more prevalent in places that are more lenient when it comes to smoking (e.g., Internet cafés) than in places that are not (e.g., restaurants and cafés), even when smoking is similarly prohibited in both types of places.

Objectives Secondhand smoke exposure causes serious health problems. Banning smoking in public venues decreases exposure to secondhand smoke. However, the implementation of smoke-free rules in a private setting (including homes) is largely voluntary. This study aimed to assess the prevalence and characteristics of voluntary smoke-free home rules in Poland during the COVID-19 pandemic as well as to identify factors associated with the voluntary implementation of smoking bans at home. Material and Methods This cross-sectional survey was conducted in March 2022 with a nationally representative sample of 1090 individuals aged ≥18 years in Poland. Data were collected using a computerassisted web interviewing (CAWI) technique. The research tool was an original questionnaire developed for the purpose of this study. Results Nationally, 60.6% of individuals had total ban on smoking in home (100% smoke-free home rules), 34.0% had implemented a partial smokefree home rule and 5.4% had not implemented any smoke-free home rules. Over three-quarters of non-smokers (76.8%) and only one-fifth of smokers (20.7%) had adopted a full smoke-free home rule. In multivariate logistic regression analysis, males (OR = 1.65, 95% CI: 1.22–2.22, p < 0.01), non-smokers (OR = 13.78, 95% CI: 9.80–19.38, p < 0.001), respondents who had higher education (OR = 1.57, 95% CI: 1.15–2.14, p < 0.01) as well as those who lived alone (OR = 2.44, 95% CI: 1.52–3.90, p < 0.001) had higher odds of having a 100% smoke-free home rule. Conclusions This study demonstrated the negative impact of the COVID-19 pandemic on the implementation of smoke-free home rules in Poland. Less than two-thirds of the Polish population has adopted a total smoke-free home rule, with significant gaps between smokers and nonsmokers. Information on current voluntary smoke-free rules will be useful for further implementation of the smoke-free law in Poland. Int J Occup Med Environ Health. 2023;36(1):84–95
