Introduction and objective. Musculoskeletal disorders are frequently met in dentistry. Objectives. To show the efficiency of rehabilitation and to make correlations among patients’ pain levels, their overall health status, and the number of days of work absenteeism. Materials and method. A total of 390 dentists diagnosed with low back pain, scapulohumeral periarthritis, cervicobrachial neuralgia, hand osteoarthritis, tendinitis or tenosynovitis of the upper limb, carpal tunnel syndrome, spinal deformities and fibromyalgia, were followed in a 2-year prospective study. For each ailment the patients were divided into two groups. Group 1 followed both medical and rehabilitation treatment, while group 2 followed medical treatment. The patients were assessed by the visual analogue scale (VAS), the Health Assessment Questionnaire adapted for Dentists (HAQD) and the number of days of absenteeism. Results. VAS scores did not significantly differ between the two groups at the beginning of the study but were significantly lower at final assessment. HAQD scores were significantly lower at one-year and two-year assessments in Group 1. The number of days of absenteeism did not differ significantly between the two groups at the initial assessment. Nevertheless, the number of days of absenteeism was significantly higher for Group 2 patients at the end of the study. For increased values of the visual analogue scale at the beginning and at the end of the study, the significantly increased numbers of days of absenteeism and of health assessment questionnaire scores were associated. Conclusions: Improvements of functional parameters and increase in work productivity were recorded in dentists who followed physical therapy.

Introduction and objective. This study aimed to determine the environmental conditions for the occurrence of carpal tunnel syndrome (CTS) in a non-industrial area, and patient-reported outcomes after surgical release. Materials and method. This observational study utilized convenience sampling to screen 100 consecutive patients for carpal tunnel syndrome at the Orthopedic Clinic, using two questionnaires. Data were collected from the Disability of Arm Shoulder and Hand (DASH) questionnaire, and the PROMIS® (Patient-Reported Outcomes Measurement Information System) Upper Extremity and PROMIS® SF 3a questionnaire (Pain Intensity). The relationship of various repetitive musculoskeletal disorders to CTS was validated by questionnaire scores, PROMIS® T-score, and correlation coefficients. Results. Finally, CTS was confirmed by electromyography in 69 patients (55 females and 14 males; average age: 47.5 years). Aging significantly influenced the occurrence of symptoms associated with pain (neck, thoracic, lower back, shoulder, and CTS (p<0.001)). Those employed for longer more frequently declared performing exercises to prevent overload pain (p<0.001). DASH results significantly correlated with the PROMIS Upper Extremity score (r = -0.64; p<0.05). Conclusions. Geographical and environmental conditions indicate that even though working with a computer is described as an essential risk factor for CTS, the study group showed a predominance of elements that were unrelated to working at a computer. CTS also occurred among people working physically, and even among unemployed individuals. The existence of a statistically significant, strong, negative correlation (r=-0.64; p < 0.05) between the results obtained in the questionnaires DASH and PROMIS Upper Extremity has been demonstrated.

Knuckle pads are rare harmless subcutaneous nodules that must be differentiated from joint disease of the proximal interphalangeal or rarely of the metacarpophalangeal joints as well as from other masses of the paraarticular tissues. We present a case of an otherwise healthy 36-year-old woman presenting with bilateral knuckle pads located at the dorsal aspect of the proximal interphalangeal joints. No predisposition to a specific musculoskeletal disorder was noted. Ultrasound revealed well-delimited subcutaneous hypoechoic masses without internal flow signals at color Doppler. Histology showed proliferation of myofibroblasts with a decrease of elastic filaments in the deep dermis. The clinical picture, the family history in addition to the histology allowed us to make the diagnosis of knuckle pads. We present the ultrasound findings of knuckle pads and discuss the differential diagnosis of a “swelling” in the dorsal region of proximal interphalangeal joints and metacarpophalangeal joints.
Palce poduszeczkowate (knuckle pads), potocznie zwane guzkami kłykciowymi, są rzadkimi i łagodnymi zmianami podskórnymi, które należy różnicować z chorobą stawu międzypaliczkowego bliższego i, rzadziej, stawu śródręczno‑paliczkowego,a także z innymi masami/zmianami okołostawowymi. Prezentujemy przypadek 36‑letniej zdrowej kobiety, u której stwierdzono obustronne guzki zlokalizowane po stronie grzbietowej stawów międzypaliczkowych bliższych. Badanie ultrasonograficzne wykazało wyraźnie odgraniczone, hipoechogeniczne guzki zlokalizowane podskórnie, bez cech obecności nieprawidłowego unaczynienia w badaniu kolorowym dopplerem. W badaniu histopatologicznym ujawniono proliferację miofibroblastów oraz zmniejszenie liczby włókien elastycznych w głębokich warstwach skóry właściwej. Obraz kliniczny, wywiad rodzinny oraz badania histologiczne pomogły w ustaleniu rozpoznania. W artykule przedstawiamy ultrasonograficzny obraz palców poduszeczkowatych oraz diagnostykę różnicową pogrubienia tkanek miękkich okolicy grzbietowej stawów międzypaliczkowego bliższego i śródręczno‑paliczkowego.

Objectives Carpal tunnel syndrome (CTS) is the most common nerve entrapment syndrome, which since the beginning of the seventies has been linked to the keyboard and visual display unit (VDU). The objective of this study was to investigate the prevalence and personal factors associated with CTS in female VDU workers in Italy. Material and Methods Participants in this study were female adult subjects, working ≥ 20 h/week (N = 631, mean age 38.14±7.81 years, mean working age 12.9±7.24 years). Signs and symptoms were collected during compulsory occupational medical surveillance. The binary logistic regression was used to estimate adjusted odds ratios for the factors of interest. Results Diagnosis of CTS was reported in 48 cases (7.6%, 11 of them or 1.7% after a surgical correction) for the incidence of 5.94/1000 person-years. In general, signs and symptoms of CTS were associated with the following demographic factors: previous trauma of upper limb (adjusted odds ratio (ORa) = 8.093, 95% confidence interval (CI): 2.347–27.904), history (> 5 years) of oral contraceptives therapy/hormone replacement therapy (ORa = 3.77, 95% CI: 1.701–8.354) and cervical spine signs/symptoms (ORa = 4.565, 95% CI: 2.281–9.136). Conclusions The prevalence of CTS was similar to the estimates for the general population of Italy. Among personal risk factors, hormone therapy, previous trauma of the upper limb and signs/symptoms of the cervical spine appeared to be associated with a higher risk of CTS syndrome. Eventually, the results reinforce interpretation of CTS in VDU workers as a work-related musculoskeletal disorder rather than a classical occupational disease.
