    Opis:
    Consumer and system-wide gains remain limited by an outmoded policy regime. With scientific innovation running far ahead of public policy, physicians, researchers and patients are not receiving full advantage of the latest developments. European health systems require a seamless and rapid flow of digital information, including genomic, clinical outcome, and claims data. Research derived from clinical care must feed back into assessment, in order to advance care quality for consumers. National health systems are heterogeneous; the solutions and required fundamental approaches differ between the European member states and are not entirely portable and scalable. To date, this applies not only to general systemic aspects but particularly to cross-border reimbursement issues and the exchange of treatment and patient data. To answer those needs, an international consortium was established to implement the project “Interregional coordination for a fast and deep uptake of personalised health”: Regions4PerMed. A cycle of international events, such as conferences, in situ visits and workshops, has been planned. Interdisciplinary groups of experts will exchange thoughts and experiences to design solutions that could be implemented in the various healthcare systems. Regions4PerMed aims to coordinate regional policies and innovation programmes in personalised medicine and personalised health to accelerate the deployment of personalised health for patients. Key Action 2 is dedicated to health technologies and smart and integrated care.

    Opis:
    Croatian national health ICT implementation strategy is determined by Croatian national health strategy and plan, Croatian ICT development strategy for 21st century, and requirements specifications for the health information system. National health ICT implementation strategy components are accented: purpose of the ICT implementation strategy, information principles, needs and ICT enablement in domains of patients, healthcare professionals, policymakers and managers and public. Telemedicine and telecare positions and implementation steps are described. Based on the determinants, three organizational levels have been established - government, ministerial and project levels. General architecture of Croatian healthcare information system and respective pilot projects and results of pilot implementations as well as national ICT environmental accelerators for health ICT implementations are presented.

    Contemporary world events are demonstrating the need to embrace and further develop tele-health options for assessment and delivery of biopsychosocial healthcare services. This is now possible, given advances in communication technologies allowing virtual connections of medical personnel with constituents, as well as necessary, in light of recent challenges posed by infectious conditions and growing needs for travel restrictions, social distancing, and isolation of large portions of populations. Moreover, the opportunity to virtually connect with persons through ubiquitous computer-based and handheld communication devices allows comprehensive care provision to include underserved areas, where restricted, walk-in access to brick-and-mortar establishments has long been recognized as a limiting factor in healthcare. This review examines evolving approaches of tele-healthcare, with a specific focus on telemedicine as a bridge between traditional, in-person approaches to diagnose and treat medical conditions and new healthcare opportunities developing to meet changing societal needs. The three purposes of the review are: discuss background information, with a brief look at policy and procedure guiding applications of tele-techniques in healthcare practices; identify relevant scientific studies to show the breadth of new evidence-based research for telemedicine practices; and, discuss challenges for the further development of telemedicine as healthcare systems in the United States evolve to meet current and projected healthcare needs.

Artykuł ten zarysowuje mechanizmy integracji oraz wytyczne WHO w zakresie opieki zintegrowanej.
    Integrated care is one of the most important concepts related to management and organization of healthcare systems. According to this concept, improvement in the quality and rationalization of costs of medical services can be achieved through coordination of the provision of care and a cooperation between health care units. The role of relationship between health and social care is being increasingly highlighted. This article outlines the mechanisms of integration and the WHO guidelines in the field of integrated care.

    Podstawowa opieka zdrowotna w Polsce oparta jest na medycynie rodzinnej. Stanowi pierwszy, a w konsekwencji niezwykle istotny etap kontaktu pacjenta z systemem ochrony zdrowia. Kluczowe jest, więc prawidłowe jej umiejscowienie w systemie wraz z określeniem zadań oraz zasad organizacji. Z problematyką tą ściśle związane są pojęcia opieki koordynowanej i zintegrowanej, które wprost przekładają się na proces leczenia pacjenta oraz współdziałanie w tym zakresie między jednostkami ochrony zdrowia. Należy wręcz stwierdzić, że determinują organizację całego systemu opieki zdrowotnej, powodując podniesienie jakości i racjonalizację kosztów udzielanych świadczeń. Artykuł ten zarysowuje organizację i zakres działania podstawowej opieki zdrowotnej na gruncie ustawy z dnia 27 października 2017 r. o podstawowej opiece zdrowotnej widzianej przez pryzmat tych dwóch kluczowych pojęć tj. opieki koordynowanej i zintegrowanej.
    The primary healthcare in Poland is based on the family medicine. It is the first, and, as a consequence, materially significant stage of contact between a patient and the healthcare system. So, setting the primary healthcare correctly in the system as well as specifying its tasks and organization rules is a key element. The terms of the managed care and the integrated care are linked strictly to this issue. They translate directly into the patient treatment process and into coordination among healthcare units in this respect. It should be stated that these terms simply determine the organization of the whole healthcare system, translating into improvement of quality and rationalization of costs of provided medical services. This paper outlines the organization and the extent of activities of the primary healthcare under the Primary Healthcare Act of 27 October 2017, at the primary healthcare seen from the angle of these two key terms, i.e. the managed care and the integrated care.
