Opis:
Quality measurement is the first step in improving overall quality. Servqual has been identified as one of the most important methods frequently used in assessing the quality of healthcare services. This study aims to systematically review and conduct meta- analysis of conducted studies in this regard in which Servqual tools were used to assess and measure the quality of services. In this systematic review and meta-analysis, the required data were collected using several keywords (and their Persian equivalents): services quality, Servqual, quality, gap, primary health cares, health services, Iran. The databases searched were as follows: PubMed, Scopus, Google Scholar, SID, Magiran, and Iranmedex. The comprehensive meta-analysis (CMA) software, Version 2 was used for data analysis. Based on the random effect model, the total mean score of Service Consumers’ perception, Service Consumers’ expectation and the gap between them were estimated as being 3.81 (95% CI: 3.56–4.06), 4.49 (95% CI: 4.31–4.67) and -0.83 (95% CI: -065 – -1.01), respectively. The lowest score of consumers’ perception of services quality, was related to the empathy dimension (3.57) and the highest score of consumers’ expectation of services quality was related to the reliability dimension (4.60). Furthermore, there was a quality gap in all dimensions. In order to achieve desired quality and meet service receivers’ needs, attention should be always given to their views, so that proper planning can be done to address existing problems through assessing views. Should such be undertaken, we would be able to witness services quality improvement.

Opis:
The article presents the risk of occupational burnout observed in health care, as well as possibilities of prevention in this regard. Its authors used their knowledge and experience obtained during the implementation of the international Time2Grow project financed from the European Social Fund. The project was dedicated to developing a burnout prevention model mainly in the area of broadly understood health care. The project was implemented in the Polish-Belgian-Finnish partnership, thanks to which it provided great opportunities to benefit from good European practices. The article presents the results of quantitative primary research carried out on Poland in the group of nurses and midwives, as well as selected results of quantitative and qualitative research conducted in Finland on a group of practicing nurses caring for elderly people.

Background. Health education is one of the main cores of primary health care (PHC). However, there is limited evidence on the difficulties of implementing health education programs. This study explored the barriers of implementing health education programs in Iranian rural communities. Objectives. A qualitative study with conventional content analysis approach was conducted. Applying purposeful sampling, 34 rural folks and health care providers were employed to participate in the study. Data were collected through individually, semi-structured interviews. Four themes, including “Ineffective teaching and learning processes”, “Lack of health educators’ motivation”, “Communication gaps”, and “Lack of resources and facilities for teaching and learning” emerged as the barriers of implementing health education programs in rural communities. Conclusions. Several executive and communicational problems were identified as the local-level obstacles of implementing health education programs in rural areas. Better understanding on the extensive range of health education barriers in rural areas may be helpful for rural health workers and stakeholders in designing and/or revisiting health education programs in rural communities.

Opis:
To ascertain the level of agreement with the “Ten Commandments for patient-centred treatment” by physicians in Central Portugal, exploring differences by age group, sex and medical speciality. Material and methods. An online questionnaire in September of 2016, sent to doctors registered in the Central Regional Section of the Portuguese Medical Association, with weekly reminders to non-respondents. The English wording was translated to Portuguese, a questionnaire with a four-grade answer was created and its reliability determined. Variables such as sex, age group and medical specialty were considered. Descriptive and inferential statistics were performed. Results. A representative yet convenient sample of 811 doctors participated. Their description is as follows: ≤ 35 years n = 203, ≥ 36 and ≤ 55 years n = 217 and ≥ 56 years n = 373, women n = 391 (49.2%), General Practice/Family Medicine (GP/FM) n = 301, medical specialty n = 303 and medico-surgical specialty n = 173. By medical specialty, for commandment 1, there is higher prevalence of “Disagree/Completely Disagree” in the GP/FM specialty (p < 0.001). By gender, for commandment 1, women vs men doctors “Disagree/Completely Disagree” proportion of 11.7% vs 6,4% (p = 0.003). For the younger age groups, there is a greater proportion of “Disagree/Completely Disagree” for Commandment 1 (p = 0.016), for Commandment 4 (p = 0.007), for Commandment 6 (p = 0.001), for Commandment 7 (p = 0.001) and for Commandment 8 (p = 0.020). Conclusions. There is vast agreement with the “Ten Commandments for patient-centred treatment” in central Portugal. For Commandments 1, 3 and 6, the proportion of “Disagree/Completely Disagree” is higher than for the remaining. There is a need for future investigations to explain the present results.

Opis:
Issues related to the functioning of the healthcare sector always result in turbulent and endless discussions. Most of those voices concern: the reduction of costs, spending on healthcare, walk outs, multi-occupation of medical staff, operating and health sector entities’ debts. The criteria of economic analysis in the health sector require to look at the activities of medical units, from the perspective of the society and its char- acteristics that determine them, and taking into account the effects connected with the geographic diversification of the management, financing and organization of the health- care system. The aim of this paper is to apply the spatial shift-share methods that are based on information about of the structural changes of economic and social phenomena, developed in geographic space, within a specified period of time, to analyze the state of the health sector. The attempt to identify regional trends in the health care sector, depending on the sector's major characteristics, will be made. The analysis also aims to identify areas and determinants of health care sector that may play a key role in specialization and the development of the regions.

Opis:
The article is devoted to the legal situation of people in a crisis of homelessness from the perspective of the implementation of human and citizen rights. To this end, the law was reviewed in the scope of satisfying housing needs, the possibility of obtaining an ID card, employment, active voting rights and health care. These are, of course, only selected areas in which the particular situation of homeless people is reflected. Attempts have been made to indicate that the legislator, as a rule, takes into account the specific conditions of existence of homeless people by introducing regulations that allow them to use the full catalogue of human rights and freedoms while respecting the principle of non-discrimination due to the lack of housing or residence. Despite this, there are still areas where the welfare of homeless people is not properly protected. The reason for this situation are difficulties in the practical implementation of solutions contained in normative acts, imprecise legal provisions, as well as the very diverse situation of people experiencing homelessness.

W pracy przedstawiono uwarunkowania oraz oddziaływanie społeczeństwa informacyjnego a także rozwój technologii informacyjno-komunikacyjnych (ang. Information and Communication Technologies ICT), na stwarzanie możliwości uzupełnienia modelu świadczeń zdrowotnych poprzez wprowadzenie telopieki. Społeczeństwo informacyjne w pełni wykorzystuje elektroniczne środki komunikacji, takie jak internet, zasoby informacyjne i inne formy usług dostępnych poprzez urządzenia mobilne, m.in. GPS, GSM, oraz inne formy tzw. e-usług. Wydłuża się czas aktywności zawodowej, a wielu ludzi coraz więcej czasu poświęca pracy. W wielu krajach obserwuje się zjawisko starzenia się społeczeństwa, wydłużenia aktywności zawodowej i narastania problemów opieki zdrowotnej. Według demografów proces starzenia się społeczeństwa rozpoczął się już w 2005 roku, a jego szczyt przypadnie na lata 2010-2020. W następnej dekadzie w wiek emerytalny (kobieta 60 lat, mężczyzna 65 lat) osiągnie prawie 2 mln ludzi są to osoby urodzone w powojennym wyżu lat 50. W 2020 roku co piąty Polak będzie emerytem (21%) i to pod warunkiem, że współczynnik narodzin w Polsce będzie utrzymywał się na wyższym poziomie niż obecnie. Sposobem złagodzenia problemów służby zdrowia może być teleopieka, co może przyczynić się do poprawy komfortu życiowego ludzi starszych.
Determinants, as well as the influence of information society on the development of Information and Communications Technologies (ICT) is discussed in this paper. It is pointed out that telecare technologies may have several medical benefits in the existing health care systems. Information society exploits electronic mass-media like internet communication and information, as well as various mobile technologies like GPS, GSM and other forms of so called e-services. Many people work more extensively than some years ago. From the other hand, the aging of societies in the developed countries is observed. Demographic data point out that the process of society aging started in 2005 and will be specially extensive in years 2010-2020. In the following decade, 2 million people will reach the retirement age (women: 60 years old, men: 65 years old). In years 2020 every fifth Polish society will be retired (21%), providing that current birth rate will increase. Telecare may help to solve some medical care problems and to improve life standard.
