Introduction: Sarcoidosis is a multi-organ disease characterized by noncaseating granulomas. The lesions are usually localized within the lungs and hilar lymph nodes. The sarcoidosis of the upper respiratory tract (SURT) rate is estimated at 0.5–6%. The involvement of the larynx is extremely rare – it occurs in 1.2% of cases of SURT. Materials and methods. We present a case of a patient with laryngeal sarcoidosis. Results: A 48-year-old man had had a diagnosed of dysphonia for 5 years. A diagnosis of chronic hypertrophic laryngitis was made. Empirical course of methylprednisolone was attempted with a short-term improvement. Due to coexisting skin lesions, he was diagnosed with allergy. Chest CT showed moderately enlarged mediastinal and hilar lymph nodes as well as bilateral nodules around the hila. The bronchial biopsy showed no granulomas. Samples taken during microlaryngoscopy revealed sarcoid infiltrates. Conclusions and discussion: There is no unambiguous test confirming the diagnosis of sarcoidosis. It is a typical example of a diagnosis by exclusion. Laryngeal involvement is extremely rare and occurs in 1.2% of cases of SURT. The clinical course of the disease is often difficult to predict. In many patients, the lesion may undergo spontaneous regression. In patients poorly responding to steroid therapy or developing side effects, cytotoxic drugs may be considered.

Opis:
The paper presents the possibilities of quantitative analysis of results obtained from CT examination of organs and anatomical structures of the upper respiratory tract. The presented results of the analysis were obtained using proprietary software developed in the MATLAB 2018b environment (Image Processing toolbox). The software enables to visualize the original results of CT scan and, after evaluating the visible structures, enables to select the area to be subjected to quantitative analysis. After the initial identification of an area of interest requiring detailed diagnostics, its volume and the surface areas of individual cross-sections are calculated in the area separated for examinations. A graphical presentation of the analysis results - the surface areas of selected cross-sections possible to visualize in two- and three-dimensional space - enables quick analysis of changes in the examined region.

Cancer patients are predisposed to fungal infections caused by Candida albicans, especially to oral or respiratory tract candidiasis. The aim of this study was to estimate genetic diversity by RAPD-PCR (random amplified polymorphic DNA-polymerase chain reaction) of C. albicans isolated from upper respiratory tract of 100 patients with non-small cell lung cancer. Among 52 strains, 34 genotypes were defined. 10 clusters comprising 28 (53.85%) isolates with similarity coefficient ≥ 80% were formed. The remaining 24 (46.15%) isolates represented individual genotypes. The RAPD-PCR technique revealed genomic variability within C. albicans isolated from upper respiratory tract of the cancer patients.

Opis:
Acute infections of the upper respiratory tract and upper gastrointestinal tract (mouth and throat) are still the most common reason for reporting to the general practitioner or ENT specialist. Despite the fact that the most common causative agent of these diseases is a virus, antibiotics are still flavored in about 60-80% of patients who report to the doctor. In consensuses such as EPOS, WHO reports and other local/ / national reports, there is a need to limit antibacterial treatment for and to replace it with symptomatic and anti-inflammatory drugs. In addition to intranasal steroids and mucocutaneous decongestants, supplementary treatment should be considered which resolves persistent symptoms. Plant preparations are used here. The growing role of rational phytotherapy is emphasized, especially due to the most frequent the etiological factor of these infections, i.e. viruses. Modern phytotherapy is a part of pharmacology, and all preparations based on plants meet the standards for of medicines and are subject to standard registration procedures. The article discusses the use and mechanism of action of selected herbs, among others: sorrel, elderberry, yarrow, and horsetail in the treatment of upper respiratory tract infections and upper gastrointestinal tract. The phytotherapy shows good efficacy and tolerance and can be used both in children and adults in recurrent infections of the upper respiratory tract. It is worth paying attention to the combined preparations, the components of which have the effects confirmed by randomized trials and long-term registration for use in treatment, both during the first signs of cold and its further duration, and supportive as support in chronic inflammation especially of the paranasal sinuses.

This review article presents immunological issues in the course of the turkey rhinotracheitis (TRT) emphasizing local immunity mechanisms, both humoral and cell-mediated, in the upper respiratory system. Studies on the influence of the humoral immunity in the course of infection and vaccinations against TRT have revealed many times the absence of correlation between the titre of specific IgY anti-aMPV (avian Metapneumovirus) antibodies in the serum and in the upper respiratory washings and the immunity against the occurrence of the clinical form of the TRT. Considering the above, T cells are increasingly often regarded as the main factor involved in the upper respiratory immunity against the TRT. However, there have been just a few reports on the role of the T cells in the local immunity processes in the infection with aMPV in turkeys. Additionally, studies of the T-cell-associated immunity against the TRT have given ambiguous results. Immunoprophylaxis issues against the aMPV infections are a significant part of the work where the authors confront current vaccination programmes against the perspectives of use of the future vaccines against the TRT. Future vaccines should face the following criteria: absence of the risk of immunosuppressive effect and reversion of vaccine strains virulence, ease-of-use combined with the possibility of administration of the vaccine to the large numbers of turkeys. The leading role in future vaccination programs for birds against the TRT is likely to be played by the in ovo technique and the recombinant vaccines. Great hopes are also linked with the development of subunit vaccines against the aMPV.

Coronavirus disease (COVID-19) is caused by the SARS-CoV-2 virus and often presents with flu-like symptoms that can have varying degrees, which may subsequently lead to acute respiratory distress (ARDS). The genetic material of the virus in samples of respiratory secretions is identified by way of basic diagnostic tests. Due to the altered course of the respiratory tract, patients after total laryngectomy require special attention in the diagnosis of SARS-CoV-2 infection. We present a case of a patient after laryngectomy who obtained different results of COVID-19 tests depending on the site of sampling.
Choroba COVID-19 wywoływana jest przez wirusa SARS-CoV-2 i często manifestuje się objawami grypopodobnymi, które mogą przebiegać z różnym nasileniem. W dalszej kolejności może prowadzić do ostrej niewydolności oddechowej (ARDS). Podstawowe testy diagnostyczne identyfikują materiał genetyczny wirusa w próbkach wydzielin z dróg oddechowych. Chorzy po laryngektomii całkowitej z uwagi na zmieniony przebieg dróg oddechowych wymagają szczególnej uwagi pod względem diagnostyki w kierunku zakażenia SARS-CoV-2. W naszej pracy przedstawiamy przypadek pacjenta po laryngektomii, u którego uzyskano różne wyniki testów w kierunku COVID-19 w zależności od miejsca pobrania próbek. Opis:
The aim of this study was to perform molecular analysis of canine adenovirus 2 (CAV-2) E1B 19K gene fragment isolated from 20 dogs of various breeds (12 males and 8 females aged 1-9 years), with clinical symptoms of upper respiratory tract infections, from the Lubelszczyzna region. Nasal swabs were taken from dogs. DNA of CAV-2 was detected using the PCR method in 16 swabs. All PCR products were sequenced, and the obtained sequences were compared with each other and with the sequence of the E1B 19K gene of the CAV-2 strain from an online database of NCBI GenBank: AC 000003. Based on analysis of the obtained sequences, three polymorphic variants of CAV-2 (No.1-3) with homology of 78 - 100% were distinguished. The nucleotide and amino acid sequences of the most frequently represented polymorphic variant, No. 1, differed from the sequences of polymorphic variant No. 2 with one substitution. The nucleotide and amino acid sequence of the E1B 19K gene of CAV-2 AC 000003 differed from the analogous sequences of representatives of variant No. 1 with 44 nucleotide and 19 amino acid substitutions. The small number of nucleotide differences in the E1B 19K CAV-2 gene among the examined own isolates, compared with AC 000003, suggest that the infections in dogs were caused by a relatively genetically stable virus which occurs in eastern Poland.

This article presents selected physical diagnostic methods used in otorhinolaryngology and results of their application. In addition to the applications of methods using the capabilities of selective sensors, selected methods of hybrid diagnostics were also presented - for assessment of parameters of respiratory processes, with polysomnography as an example of using both typical diagnostic methods dedicated to otolaryngology, as well as standard EEG and ECG methods. It has been shown that in some special cases of respiratory disorders, measurements of the air flow in the respiratory tract can be supplemented with pressure measurements in selected positions within the airways. The presented optical methods and diagnostic systems are very often used in the diagnosis of diseases not specific for otolaryngology occurring in the area of the head and neck. The presented material is the second part of the study discussing both standard and widely used diagnostic methods. All presented methods are dedicated to otolaryngology. This text is a continuation of the material published in No 4 of 2021 [1].

The rapidly developing measurement techniques and emerging new physical methods are frequently used in otolaryngological diagnostics. A wide range of applied diagnostic methods constituted the basis for the review study aimed at presenting selected modern diagnostic methods and achieved diagnostic results to a wider group of users. In this part, the methods based on measuring the respiratory parameters of patients were analysed. Respiration is the most important and necessary action to support life and its effective duration. It is an actual gas exchange in the respiratory system consisting of removing CO2 and supplying O2. Gas exchange occurs in the alveoli, and an efficient respiratory tract allows for effective ventilation. The disruption in the work of the respiratory system leads to measurable disturbances in blood saturation and, consequently, hypoxia. Frequent, even short-term, recurrent hypoxia in any part of the body leads to multiple complications. This process is largely related to its duration and the processes that accompany it. The causes of hypoxia resulting from impaired patency of the respiratory tract and/or the absence of neuronal respiratory drive can be divided into the following groups depending on the cause: peripheral, central and/or of mixed origin. Causes of the peripheral form of these disorders are largely due to the impaired patency of the upper and/or lower respiratory tract. Therefore, early diagnosis and location of these disorders can be considered reversible and not a cause of complications. Slow, gradually increasing obstruction of the upper respiratory tract (URT) is not noticeable and becomes a slow killer. Hypoxic individuals in a large percentage of cases have a shorter life expectancy and, above all, deal with the consequences of hypoxia much sooner.

The paper examines the features of segmentation of the upper respiratory tract to determine nasal air conduction. 2D and 3D illustrations of the segmentation process and the obtained results are given. When forming an analytical model of the aerodynamics of the nasal cavity, the main indicator that characterizes the configuration of the nasal canal is the equivalent diameter, which is determined at each intersection of the nasal cavity. It is calculated based on the area and perimeter of the corresponding section of the nasal canal. When segmenting the nasal cavity, it is first necessary to eliminate air structures that do not affect the aerodynamics of the upper respiratory tract - these are, first of all, intact spaces of the paranasal sinuses, in which diffuse air exchange prevails. In the automatic mode, this is possible by performing the elimination of unconnected isolated areas and finding the difference coefficients of the areas connected by confluences with the nasal canal in the next step. High coefficients of difference of sections between intersections will indicate the presence of separated areas and contribute to their elimination. The complex configuration and high individual variability of the structures of the nasal cavity does not allow segmentation to be fully automated, but this approach contributes to the absence of interactive correction in 80% of tomographic datasets. The proposed method, which takes into account the intensity of the image elements close to the contour ones, allows to reduce the averaging error from tomographic reconstruction up to 2 times due to artificial sub-resolution. The perspective of the work is the development of methods for fully automatic segmentation of the structures of the nasal cavity, taking into account the individual anatomical variability of the upper respiratory tract.
W pracy przeanalizowano cechy segmentacji górnych dróg oddechowych w celu określenia powietrznego przewodnictwa nosowego. Przedstawiono zdjęcia 2D i 3D procesu segmentacji oraz uzyskanych wyników. Podczas formowania analitycznego modelu aerodynamiki jamy nosowej głównym wskaźnikiem charakteryzującym konfigurację kanału nosowego jest ekwiwalentna średnica, którą wyznacza się na każdym skrzyżowaniu jam nosowych. Jest ona obliczana na podstawie pola powierzchni i obwodu odpowiedniego odcinka kanału nosowego. Podczas segmentacji jamy nosowej w pierwszej kolejności należy wyeliminować struktury powietrzne, które nie wpływają na aerodynamikę górnych dróg oddechowych – są to przede wszystkim nienaruszone przestrzenie zatok przynosowych, w których dominuje rozproszona wymiana powietrza. W trybie automatycznym jest to możliwe dzięki eliminacji niepołączonych izolowanych obszarów i znalezieniu, w kolejnym kroku, współczynników różnicy obszarów połączonych konfluencjami z przewodem nosowym. Wysokie współczynniki różnic przekrojów pomiędzy skrzyżowaniami będą wskazywały na obecność wydzielonych obszarów i przyczynią się do ich eliminacji. Złożona konfiguracja i duża zmienność osobnicza struktur jamy nosowej nie pozwala na pełną automatyzację segmentacji, jednak takie podejście przyczynia się do braku konieczności interaktywnej korekcji w 80% zestawów danych tomograficznych. Zaproponowana metoda, uwzględniająca intensywność elementów obrazu znajdujących się blisko konturu, pozwala na nawet 2-krotne zmniejszenie błędu uśredniania z rekonstrukcji tomograficznej, wynikającego ze sztucznej subrozdzielczości. Perspektywą pracy jest opracowanie metod w pełni automatycznej segmentacji struktur jamy nosowej z uwzględnieniem indywidualnej zmienności anatomicznej górnych dróg oddechowych.
