Purpose: To present the results of surgical treatment concerning pressure ulcers. Materials and methods: All patients underwent surgical treatment at the Orthopedics and Traumatology Clinic from 1997 to 2016. One operator performed the procedures. A total of 28 pressure ulcers were operated (25 patients). The clinical material consisted of 15 pressure ulcers located in the lower back area, six pressure ulcers located in the ischial area, three pressure ulcers located in the trochanteric area and four pressure ulcers located in the heel area. Results: One conducted the result evaluation based on the criteria of complications proposed by Seiler. In case of all surgically treated patients, pressure ulcers appeared after 2-5 weeks after the surgery. One patient experienced hematoma below the slide piece. One could observe seroma in case of 5 patients. In this situation, it was required to apply a local postoperative puncture. Four patients experienced marginal skin necrosis (2 of them required resection of dead skin edges in operating block conditions which were later re-stitched using “side to side” technique). During the 3-year follow-up, there was no recurrence of operated pressure ulcers. Conclusions: Deep septic pressure ulcers, according to the division introduced by Seiler, connected with bone infection require the application of muscular-skin pieces. This procedure is aimed at improving local tissue blood supply. Proper preoperative preparation of the patient, careful planning of the surgery and suitable postoperative treatment of the patient are as significant as the surgery itself.

Introduction: Pressure ulcers and their consequences can occur in any patient regardless of the underlying disease, so the knowledge of their prevention and treatment is extremely important. The aim of the study: The aim of the study was to analyze the state of students’ knowledge on pressure ulcers. Material and methods: The research was carried out in the Pomeranian Medical University in Szczecin among students of nursing. The study involved 203 full-time students. Data was collected using a questionnaire consisting of 27 questions. The questions addressed the issues of the formation, prevention and treatment of pressure ulcers. The Kolmogorow-Smirnow, Fisher-Snedecor test, (ANOVA) with Spearman’s correlation and Student’s t-test were applied for statistical analysis. The statistical significance was assumed to be p ≤ 0.05. Results: Over half of the respondents (57.64%) had a sufficient level of knowledge on pressure ulcers prevention and treatment. The level increased with the duration of the study, the highest was in the last few years, both I and II cycle of studies. The better (the higher) subjects assessed their knowledge about prevention and treatment of pressure ulcers, they were characterized by a higher level of knowledge. Conclusions: The knowledge of nursing students on pressure ulcers increases with subsequent years of study. The selfassessment of students’ knowledge about pressure ulcers, their prevention and treatment is accurate. Students of the consecutive years are better prepared to carry out anti-bedsore prevention. Students are aware of the need to have knowledge, as well as practice in the field of prevention and treatment of pressure ulcers.

Patients with cerebrovascular accidents (CVA) are at risk of developing PUs due to sensory, motor and cognitive deficits following the disease. Therefore, the aim of the present study was to investigate the severity and factors affecting the occurrence of PUs in patients with CVA. The present study was a retrospective cohort study. The data of this research was extracted from the registry system of Imam Reza Hospital located in the west of Iran. 217 patients with CVA with Braden score below 14 or with PU were included in the study. In order to investigate the relationship between independent variables and wound stage in univariate regression model, logistic regression was used, and the results were reported as odds ratio (OR). Data were analyzed with Stata statistical software version 15 and Excel 2016 software. Out of 217 CVA patients hospitalized, 180 had pressure ulcers. In total, 465 PUs were diagnosed in 180 patients in different areas. The average age of patients was (SD=61±8.78) years. 65 patients suffered only one wound, 38 patients experienced two wounds, and 37 patients experienced three wounds. The highest number of PUs was with 201 cases of stage I, followed by 180 cases of stage II. Stage IV with 2 cases was the least PU cases. Patients with a Braden score of 10-12 were 3.89 times more likely than patients with a Braden score greater than 14, and patients with a Braden score of 6-9 were diagnosed with PUS 6.67 times more than those with a Braden score greater than 14. Regarding the site of PUs, the sacrum and then the buttock were the most common sites of PU, which were observed in 65.56% and 29.49% of patients, respectively. The highest number of stage I was related to the sacrum area. The sacrum was the most common site of PU occurrence and the highest number of PUs was related to stage I. Age, fecal incontinence, hemoglobin levels and Braden’s score have been among the factors affecting PUs in patients with CVA.

In the recent years one of the common causes of cancer related deaths in humans is associated with the development of gastric carcinoma leading to gastric cancer. It had long before reported that the potent agent that is intimately related with the gastric cancer is the infection of the bacterium Helicobacter pylori (H. pylori). H. pylori infection in humans is characterised by the development of acute peptic ulcers which creates a lots of gastroenterological symptoms. Co-existence of H. pylori in the human gastric environment is well adapted by the pathogen by evading the immune responses of the host. Gastric biopsy in patients suffering from H. pylori infection reported the development of gastric lesions having the chance to proceed towards development gastric cancer. Recently it was reported that eradication of the H. pylori colonization is not possible even after continued current antibiotic treatment over long periods and even combined regimes are unsuccessful.Moreover alterations of the genetic background of the host gastric cells were also reported along with infection of H. pylori, the primary concern for the evolution of the genetically transformed cells which is the hallmark for the development of malignancy. Clinical studies have reported the limited efficacy in decreasing the incidence of gastric carcinoma even after the eradication therapy has been completed for the bacterial colonization. Research based on the biopsy specimens obtained from conventional gastric endoscopy of affected patients has revealed some genetic or epigenetic alterations in the gastric mucosa. The cause of concern to human life lies in the fact that the number of sporadic cases of primary or metachronous gastric carcinoma has increased after successful eradication.The purpose of the present review is to assess the current status of the research based on the concept of development of gastric carcinoma in association with H. pylori infection in humans and to throw some light in the remedial measures against the pathogen which is a chronic burden in the well-being of the human society.

This paper proposes an efficient in vitro propagation method for a red dye yielding and medicinal herb - Oldenlandia umbellata L. The plant is well-known in Siddha medicine for its styptic property. It is also used as drug for bronchial asthma, as a febrifuge and to treat poisonous bites and ulcers. Moreover, the red dye from its roots has been used in diverse applications since ancient times. These varied applications have increased the utilization and exploitation of this plant for medicinal and dye extraction purposes. Hence, a protocol for rapid propagation is standardized in the present study. Herein, the nodal and internodal of Oldenlandia umbellata L. were cultured on MS medium supplemented with cytokinins, 6-Benzylaminopurine (BAP), GA3, Kinetin (KIN), and auxins 1-Naphthaleneacetic acid NAA, 2, 4-Dichlorophenoxyacetic Acid (2, 4-D). Direct organogenesis was the intent. In doing so, the nodal and internodal explants showed maximum number of shoots in the combination of 2.5 mg/l BAP, 2.0 mg/l GA3, 0.5 mg/l. In addition, the explants growing on medium supplemented with 10 mg/l BAP was found to have generated more shoots. The developed shoots were then transferred for rooting to half strength MS medium fortified with IAA, IBA and NAA ranging from 2-5 mg/l. The best results were achieved on 2.0 mg/l 89.6 % (IBA) Indole-3- butyric acid. For hardening, the rooted plantlets were subsequently transferred to paper cups containing FYM, red soil and sand in the ratio of 1:2:1. They were then transferred to the growth chamber for acclimatization. After acclimatization, the plantlets were transfer for further development under field conditions. About 95% survival was achieved in the field.

Introduction: Herpes Simplex Virus (HSV) is a human herpesvirus responsible for systemic infections. Ocular involvement spans all ocular tissues. Corneal lesions, such as dendritic or geographic ulcers, are frequently found in HSV keratitis while conjunctival lesions have been described as infrequent. Clinical case: A 62-year-old man diagnosed with Birdshot chorioretinopathy presented a stye in his eyelid that was treated with oxytetracycline-hydrocortisone ointment. One week later he returned presenting several small dendritic corneal ulcers in both eyes and a conjunctival ulcer in one of them. All lesions healed completely after treatment with topical acyclovir ophthalmic ointment at 3%. Conclusions: We should pay attention to the conjunctiva and not merely focus on the cornea when examining patients with herpetic eye disease, especially if being treated with inmunosupressors.

The aim of this study was to assess the course of the healing process following the use of dibutyrylchitin (DBC) dressing, a fully degradable material used in the treatment of ulcers which occur during chronic venous insufficiency common in patients suffering from type 2 diabetes. These diseases have a significant impact on the patients’ standard of living, including the potential employment, and on the declining attendance at the current workplace. The implementation of this innovative therapeutic solution may positively affect the above-mentioned difficulties. An analysis of the healing process, following the application of the DBC dressing, was performed. Once the dressing was positioned on the wound, the analysis indicated that it underwent a process of degradation facilitated by the enzymes occurring naturally in the wound. When fully degraded, a further layer was applied. This process was repeated until the wound was fully healed. The study group consisted of 4 patients previously diagnosed with type 2 diabetes. During the observation period, the ulcers in all 4 cases had healed. The examined wound dressings adhered well to the wound surface and degraded within it. No side effects or adverse effects of the applied innovative therapy were observed. An addition of the biodegradable DBC dressing to the standard therapy procedure of ulcers occurring during chronic venous insufficiency among patients with type 2 diabetes indicate safe and effective treatment, which may have a direct reflection in the patient’s professional capacity enhancement. It resulted in the complete healing of all ulcers in each of the observed cases.

Opis:
Venous leg ulcer is a big therapeutic and care problem among the adult population. Looking after the patients with a chronic wound should be carried out by qualified personnel in a systematic and planned manner – it is possible, in a situation of providing the patient with long-term nursing home care. Local treatment of ulcers should be carried out in accordance with the strategy of TIME, with the use of specialized dressings accelerating the healing process of tissues. Systematic assessment of ulcers by a nurse qualified in wound healing allows for quick reactions involving the adjustment of therapeutic procedures according to the current clinical condition. Intensive nursing care in our case, allows to decide to withdraw from the amputation of the lower limb covered with ulcers, which undoubtedly positively affects the quality of life of the patient in question.
Owrzodzenie żylne podudzia stanowi duży problem terapeutyczny i pielęgnacyjny wśród populacji osób dorosłych. Opieka nad chorym z raną przewlekłą powinna być prowadzona przez wykwalifikowany personel w sposób systematyczny i zaplanowany – jest to możliwe, w sytuacji objęcia pacjenta pielęgniarską opieką długoterminową domową. Leczenie miejscowe owrzodzenia należy prowadzić zgodnie ze strategią TIME, z wykorzystaniem specjalistycznych opatrunków przyspieszających proces gojenia tkanek. Systematyczna ocena stanu owrzodzenia przez wykwalifikowaną w leczeniu ran pielęgniarkę, pozwala na szybką reakcję polegającą na dostosowaniu postępowania terapeutycznego do aktualnego stanu klinicznego. Intensywna opieka pielęgniarska w opisywanym przypadku pozwala na podjęcie decyzji o odstąpieniu od amputacji objętej owrzodzeniem kończyny dolnej, co niewątpliwie wpływa korzystnie na jakość życia opisywanego chorego.

Pressure injuries are a serious medical problem that both negatively affects the patient's quality of life and results in significant healthcare costs. In cases where a patient doesn’t receive appropriate treatment and care, death may result. Nurses play critical roles in the prevention, care, and treatment of pressure injuries as members of the healthcare team who closely monitor the health status of the patient. Today, the use of artificial intelligence is becoming more prevalent in healthcare, as in many other areas. Artificial intelligence is a method that aims to solve complex problems by using computers to mathematically simulate the way the brain works. In this article, we compile and share information about a deep learning model developed for the detection and classification of pressure injuries. Deep learning can operate on many types of data. Convolutional Neural Networks (CNN) prefer images because they can handle 2D arrays. In this case, the images, annotated according to the National Pressure Injury Advisory Panel pressure injury classification system, have been fed into a deep learning model using CNN. The developed CNN model has a 97% success in detecting and classifying pressure injuries, and as more images are collected and fed into the CNN, the prediction accuracy will increase. This deep learning model allows for the automatic detection and classification of pressure injuries, an indicator of health outcomes, at an early stage and for quick and accurate intervention. In this context, it is expected that the quality of nursing care will increase, the prevalence of pressure injury will decrease, and the economic burden of this health problem will decrease.
Odleżyny są problemem zdrowotnym, który negatywnie wpływa na jakość życia pacjenta i powoduje poważne koszty opieki. W przypadku braku odpowiedniego leczenia i opieki może to doprowadzić do śmierci pacjenta. Pielęgniarki odgrywają kluczową rolę w zapobieganiu, opiece i leczeniu odleżyn jako członkowie zespołu opieki zdrowotnej, którzy ściśle i stale monitorują stan zdrowia danej osoby. Obecnie w dziedzinie zdrowia, podobnie jak w wielu innych dziedzinach, coraz częściej wykorzystuje się sztuczną inteligencję. Sztuczna inteligencja jest metodą, która ma na celu rozwiązywanie złożonych problemów poprzez matematyczne symulowanie sposobu działania mózgu z wykorzystaniem komputerów. Niniejszy artykuł jest przeglądem zaprojektowanym w celu podzielenia się informacjami na temat modelu deep learning opracowanego do wykrywania i klasyfikacji odleżyn. Deep learning może działać na wielu typach danych. Konwolucyjne sieci neuronowe (ang. convolutional neural networks, CNN) preferują obrazy, ponieważ mogą obsługiwać macierze 2D. Obrazy, uporządkowane zgodnie z systemem klasyfikacji odleżyn według National Pressure Injury Advisory Panel (NPIAP), zostały przekształcone w "Deep Learning Model" z wykorzystaniem CNN. Opracowywany model CNN ma 97% skuteczności w wykrywaniu i klasyfikowaniu odleżyn, a im więcej obrazów zostanie zebranych i wykorzystanych w CNN, tym większe będzie prawdopodobieństwo trafnej prognozy. Ten model deep learning daje możliwość automatycznego wykrywania i klasyfikacji odleżyn, które są wskaźnikiem jakości zdrowia, na wczesnym etapie oraz dokładnej i szybkiej interwencji. W tym kontekście oczekuje się, że jakość opieki pielęgniarskiej wzrośnie, zmniejszy się częstość występowania odleżyn oraz obciążenie ekonomiczne związane z tym problemem zdrowotnym.
