Introduction: Fungal paranasal sinusitis can be either invasive or noninvasive. Saprophytic infections, fungus balls (FB) and allergic fungal rhinosinusitis are noninvasive (AFR). Materials and methods: The present study examined 521 patients who underwent endoscopic sinus surgery between January 2016 and April 2017 due to chronic paranasal sinusitis at the Department of Laryngology and Laryngological Oncology of the Upper Silesian Medical Centre in Katowice. The aim of the study was to analyze histopathological and microbiological material collected intraoperatively and to determine incidence and type of fungal infections among patients treated for chronic sinusitis. Results: Chronic fungal sinusitis was confirmed in 10 of 521 operated patients. The study group consisted of 9 females and 1 male. Histopathological examination revealed dead mycelium in 5 patients and colonies of Aspergillus spp. in 4, while microbiological examination revealed Candida albicans infection in 1 case. Allergy to inhalant allergens of fungal spores of Alternaria and Penicillium was confirmed in a 73-year-old patient, which, based on the whole clinical presentation, enabled to diagnose chronic allergic fungal sinusitis. The most common location of mycelium was maxillary sinus, followed by sphenoid sinus. Discussion: The most common form of noninvansive fungal sinusitis is the so-called fungus ball, which was also confirmed in our report (95% of the test subjects). AFRS is more likely to occur in warm, moist climates that favor the growth of fungi.

Invasive fungal sphenoid sinusitis is a rare and potential life threating disease. An incidence rate of fungal sphenoiditis among patients with isolated sphenoid disease varies from 4.5% to 26.8%. Generally the symptoms of sphenoid sinusitis are non-specific and include headaches, visual disorder, post nasal drip, nasal obstruction and paralysis or paresis of single or multiplicitous cranial nerves. In this study, a case of isolated invasive fungal left sphenoiditis in immunocompetent 56-year-old woman is described. The first sinus computed tomography (CT) examination revealed complete opacification of the left sphenoid sinus, she second CT scans showed also erosions of the sphenoid sinus bony walls. Endoscopic sinus surgery was performed and intraoperatively fixed and dilated left pupil was observed thus surgery team decided to finish an operation immediately. Ophthalmologist diagnosed the left oculomotor nerve paresis. Pathologic analysis demonstrated fungal hyphae morphologically identical to Aspergillus spp. Due to suspicion of cerebrospinal fluid (CSF) leak, increasing pneumocephalus and manifestation of mental changes in our subject, endoscopic reoperation with CFS fistula closure was done nine days after the first operation. The patient was treated with intravenous voriconazole and ceftriaxone for one month, and after discharge with itraconazole for 3 months. The full recovery of the left oculomotor nerve function ensued 4 months after surgery and no signs of recurrence of the fungal disease were present up to date.
Inwazyjne grzybicze zapalenie zatoki klinowej jest rzadką ale potencjalnie zagrażającą życiu chorobą. Częstość występowania grzybiczego zapalenia zatok wśród pacjentów z izolowanym zapaleniem zatoki klinowej wynosi 4.5% do 26.8%. Najczęściej objawy zapalenia zatok klinowych są niespecyficzne i obejmują bóle głowy, zaburzenia widzenia, spływanie po tylnej ścianie gardła, niedrożność nosa oraz pojedyncze lub mnogie porażenie bądź niedowład nerwów czaszkowych. W tej pracy opisany został przypadek izolowanego, inwazyjnego zapalenia lewej zatoki klinowej u 56-letniej pacjentki bez zaburzeń odporności. Pierwsze badanie tomografii komputerowej zatok (TK) ujawniło całkowite zacienienie lewej zatoki klinowej, a drugie badanie TK wykazało również erozję ścian zatoki klinowej. Przeprowadzono endoskopowy zabieg zatok i śródoperacyjnie zaobserwowano nieruchomą i poszerzoną lewą źrenicę, z tego powodu zespół chirurgiczny zdecydował o natychmiastowym przerwaniu zabiegu. Okulista rozpoznał niedowład lewego nerwu okoruchowego. Badanie histopatologiczne wykazało obecność strzępków grzybni, podobnych do Aspergillus spp. Ze względu na podejrzenie wycieku płynu mózgowo-rdzeniowego (PMR), nasilenie odmy śródczaszkowej i pojawienie się zmian psychicznych u naszej pacjentki, endoskopowa reoperacja z zamknięciem przetoki PMR została wykonana dziewięć dni po pierwszej operacji. Pacjentka była leczona dożylnie worykonazolem i ceftriaksonem przez miesiąc, a po wypisie do domu - itrakonazolem przez 3 miesiące. Pełny powrót funkcji lewego nerwu okoruchowego nastąpił po 4 miesiącach od operacji i do tej pory pacjentka nie ma objawów nawrotu choroby grzybiczej.

Introduction. There is need for any healthcare setup to rapidly identify Sinus headache patients from other cases of Neurological and Primary headaches like Migraine and Chronic Tension type headache. Materials and Methods. 117 cases of confirmed sinusitis were evaluated and analysed for common traits which could help in rapid diagnosis of Sinusitis. Results. Unilateral, dull aching headache and facial pain, of changing intensity, lasting all day on an average of 4 to 6 hours with a previous short history of common cold in acute sinusitis is the most common pattern observed in this study. Discussion. Unilateral, dull aching headache and facial pain, of variable intensity, lasting all day on an average of 4 to 6 hours along with is highly sensitive criterion (91%) for diagnosis of acute sinusitis. This, along with a history of previous Upper Respiratory tract infection, is useful to rapidly screen patients for acute sinusitis (95% sensitive). Though a subset of other headache cases will also be included by the criteria, imaging or Endoscopy must be done in these positive cases to increase the specificity of the diagnostic criteria.

Simulated maxillary sinusitis was observed in guinea pigs following the surgical incision in superior cervical sympathetic ganglion. Additionally, toxic hepatitis development was stimulated after exposure to tetrachlormethane in experimental animals. The treated and control animals were observed for three months. A significant increase in the content of reactive oxygen species (ROS) in neutrophils and lymphocytes was noted in artificially developed sinusitis. The ROS values were slightly higher in the case of combined experimental sinusitis and toxic hepatitis. Based on the obtained data, the role of ROS in the development of sinusitis was discussed.

Fungal sinusitis can be divided into invasive and non-invasive. Fungal ball of the sinus is a type of chronic non-invasive infection and it is most often related to the previous endodontic treatment. This case report describes non-invasive fungal rhinosinusitis in a patient with cough and obstructive sleep apnea syndrome. The patient did not present any other symptoms and he was treated by a therapeutic specialist. The patient underwent antibiotic therapy but no improvement was observed. Computed tomography of the face showed the lesion with calcification of the right maxillary sinus. It was decided to perform sinusotomy through the anterior wall of the maxillary sinus under endoscopic control. The lesion was removed during surgery and the sinus was rinsed. General condition of the patient improved after surgery. No cough or obstructive sleep apnea syndrome was found.

Understanding the appropriate use of diagnostics and treatment in acute rhinosinusitis is of immense importance given the high prevalence of this disease in the general population. The ability to differentiate between the principal phenotypes of acute sinusitis, namely acute viral infection (cold), acute post-viral sinusitis and acute bacterial sinusitis, determines the future management and is fundamental to providing rational therapeutic recommendations – especially as regards antibiotic treatment, which is very often overused in acute sinusitis even though bacterial phenotypes only account for 0.5–2% of all cases of the disease. The latest therapeutic recommendations contained in the EPOS2020 position paper introduce a system based on integrated care pathways (ICPs), which comprise pharmacy-supported self-care and e-health as the first level, followed by primary care as the second, with specialist care being reserved for patients who develop a more severe course of the disease, have suspected complications or suffer from recurrent acute sinusitis. Management of acute sinusitis is primarily based on symptomatic treatment modalities, with phytotherapeutic support, as well as on antiinflammatory treatment, while antibiotic therapy is used in very specific and limited indications. Complications are relatively rare in acute sinusitis and they are not considered to be associated with antibiotic intake. Considering the high prevalence of acute forms of sinusitis, their significant impact on quality of life and high direct and indirect costs of treatment, the right diagnosis and management, without unnecessary escalation of therapy, can substantially translate into a number of public health benefits.

Introduction: Endoscopic surgery of the paranasal sinuses is a surgical technique that has revolutionized the surgical treatment of patients with chronic sinusitis. Application of this method now goes far beyond the conditions of the nasal cavity. Aim: It allows among other operations within the eye socket, acting alternative technique for the operation of the external appproauch. Material and Methods: This work is devoted to the analysis of the effectiveness of endoscopic surgery in the treatment of patients with orbital complications in the course of sinusitis in the Department of Otolaryngology SU in Cracow in the years 2004-–2016, aim of this study is to search for optimal treatment of patients with the above disorders. Results: In the group of patients operated on because of orbital abscess from reaching the outside in all patients had symptoms of revocation on the part of the orbit and ocular, and postoperative complications were not observed. Among patients undergoing endoscopic trocar in one patient there was no improvement in the function of the eye. It could lead to a too long period of time from the first appearance of symptoms (over 48 hours), and sometimes surgical intervention, as well as the occurrence of orbital hematoma in the second postoperative day and the necessity of re-operation of the external handle.

We present a case report a patient with orbital subperiosteal abscess in chronic sinusitis, in which in the age of admission to an experienced significant disorders of the ocular. The patient was performed orbital abscess drainage by intranasal, complicated by the occurrence of orbital hematoma and the need to re-perform the surgery.

Introduction: CRS is a complex systemic disease affecting more than 10% of the population. There are two main types of CRS phenotypes: CRSwNP and CRSsNP. In the Caucasian population, the prevalence of inflammation markers typical of the Th1 profile is observed in CRSsNP, whereas Th2 and Th17 in CRSwNP. Th2 inflammation is observed in the CRSwNP phenotype with concomitant allergies, asthma or hypersensitivity to NSAIDs. Objectives: The aim of the study was to evaluate, based on the authors’ own material, whether allergies, asthma or hypersensitivity to NSAIDs were a risk factor for the development of a specific CRS phenotype. An attempt was also made to investigate the influence of comorbidities on the extent of sinus endoscopic procedures, which depended on the severity of inflammation. Methods: In the years 2006–2015, ESS was performed on 2217 patients with different CRS phenotypes. Patients with an allergy, bronchial asthma and hypersensitivity to NSAIDs were subjected to analysis. Results: Based on logistic regression, it was found that among the mentioned comorbidities, only asthma (P < 0.0001) and hypersensitivity to NSAIDs (P = 0.0007) significantly affect the occurrence of the phenotype with polyps, whereas the impact of allergies is statistically insignificant (P = 0.1909). The relationship between the type of ESS and CRS phenotypes is statistically significant (P < 0.0001). Conclusions: Bronchial asthma and hypersensitivity to NSAIDs have a statistically significant effect on the occurrence of the CRSwNP phenotype. This effect was not observed in allergies. The impact of allergies, asthma and hypersensitivity on the phenotype was observed in the group of patients subjected to the most extensive surgery (ESS 4).

The main objective of the study was to determine the validity of intraoperative antiseptic treatment during endoscopic sinus surgery and the impact of such a treatment on the postoperative outcomes. Fifty-five patients with chronic sinusitis, qualified for surgical treatment were enrolled into the trial. It was designed as a prospective, randomized, blinded study. The surgical procedures were performed on both sides, in the same scope. In the next stage, after opening, one side was flushed with saline solution, and the other side with octenidine solution. The analysis showed a statistically significant reduction in postoperative crusting measured using the Lund-Kennedy scale between the test and the control group. Intraoperative lavage of the paranasal sinuses in both control and study group showed an effect on decreasing the total number of positive postoperative cultures relative to preoperative ones. Study showed a beneficial effect of the intervention consisting in rinsing with Octenisept on the reduction of crusting in the postoperative assessment.

Background Radon-222-enriched hot spring therapy, which is characterized by a safe level of radioactivity, is used for the treatment of rheumatic disorders, and its efficacy has already been studied in several clinical trials. Radon-water inhalation therapy for the treatment of upper and lower airway inflammatory diseases is used in many hot springs centers. However, its application has not been reviewed to date. Methods We systematically searched the PubMed and Scopus databases for clinical trials published in the last 20 years in which objective parameters of upper and lower airway function had been tested before and after radon-enriched inhalation treatment. Results Four prospective studies were found: 1 asthma trial, 1 placebo-controlled chronic rhinosinusitis trial, 1 upper respiratory tract inflammation with nasal obstruction trial, and 1 case-control allergic rhinitis trial. Patients were treated with nasal inhalations of radon-enriched water for 12 to 28 days and were assessed at baseline and after therapy. After 2 weeks of treatment, nasal resistance decreased, flow increased, mucociliary clearance was enhanced, ciliated-to-muciparous cell ratio increased, and %FEV1 increased in asthmatic patients. Conclusion Radon-enriched inhalation therapy improves objective indicators of nasal function in allergic rhinitis and chronic rhinosinusitis, and causes relief of pulmonary obstruction in asthma.

Opis:
Frontal sinusitis is a common encounter in otorhinolaryngology practice. Most patients will present with frontal headache which worsens on bending forward, reduced sense of smell and sometimes cacosmia. Pott’s puffy tumour is an osteomyelitis of the frontal bone, which was originally described as a result of trauma, but it can also develop as a result of frontal sinusitis. We present a rare case of a rapidly progressing Pott’s puffy tumour involving the frontal sinus. The oedema subsided after antibiotic treatment and the patient underwent functional endoscopic sinus surgery.
Zapalenie zatok czołowych jest często spotykanym schorzeniem w praktyce otorynolaryngologicznej. U większości pacjentów obserwuje się ból w okolicy czołowej nasilający się podczas pochylania do przodu, upośledzenie zmysłu węchu, a w niektórych przypadkach kakosmię. Guz Potta to zapalenie kości i szpiku w obrębie kości czołowej – pierwotnie opisywano go jako następstwo urazu, jednak może być również spowodowany zapaleniem zatok czołowych. W pracy przedstawiono przypadek guza Potta obejmującego zatokę czołową i charakteryzującego się szybką progresją. Po ustąpieniu obrzęku w wyniku zastosowanej antybiotykoterapii pacjenta poddano zabiegowi czynnościowo-endoskopowej chirurgii zatok przynosowych.

Objectives: The study aimed to identify the patients with pathological lesions in the maxillary sinuses in which the reported symptoms might be indicative of odontogenic origin, as well as to establish specific causative risk factors promoting their development. Methods and Materials: The study covered 44 patients with suspected odontogenic maxillary sinusitis. Dental examination and Cone Beam Computed Tomography were completed. The age of patients ranged between 19 and 69 years, and the mean age was 43 (SD = 13.9) years. Results: Out of 44 patients, 22 (50%) had non-odontogenic lesions in maxillary sinuses, while in 15 (34.1%) dental origin was established. In the remaining 7 (15.9%) patients, no pathological changes were found in the sinuses. The median of reported symptoms was 10 months (Q1 = 4, Q3 = 24). The reported complaints were not associated with the actual cause of pathological lesions. Conclusions: Odontogenic cause of the lesions in the sinuses should primarily be hypothesized, especially in patients with long-term disease symptoms, also regarding any unilateral inflammations.

Chronic rhinosinusitis with nasal polyps (CRSwNP) is a disease with still not enough known pathogenesis despite the development of genetics, immunological and microbiological research. The number of patients with CRS has been constantly growing. The coexistence of CRS, bronchial asthma and aspirin intolerance (aspirin triad) is an adverse prognostic factor with higher risk of recurrences. The aim of study was to compare the severity of CRSwNP depending of coexistence of bronchial asthma and/or aspirin intolerance. The research was performed in the group of 204 patients operated 2009-2013 with 5 years follow-up. Higher nasal polyps growth in groups of patients with aspirin triad and CRSwNP and bronchial asthma in endoscopic examination (p=0,0005 and p=0,0030 respectively) and CT-scan according to Lund-Mackay point scale (p<0,0001 and p=0,0009) was showed. Also, these patients presented increased severity of nasal symptoms before surgical treatment according to VAS scale (p=0,0126 for CRSwNP with bronchial asthma; p=0,0390 for aspirin triad). Similarly, 6 months after surgery the same groups of patients presented higher severity of the disease symptoms (p<0,0001 for aspirin triad’ patients; p=0,0174 for CRSwNP and bronchial asthma’ patients) . Patients with aspirin triad had also statistically more surgeries in past (p=0,001), what proves that recurrences in this group are very likely to be observed in spite of the use of proper conservative treatment. No such differences have been shown in the group of patients with CRSwNP and isolated aspirin intolerance (without bronchial asthma). Allergy to inhaled allergens, hypersensitivity to aspirin are factors significantly worsening the course of CRSwNP. It would be advisable to consider, despite a lack of history of aspirin intolerance, a hypersensititvity to aspirin test in patients with particularly severe CRSwNP, especially those associated with bronchial asthma. It also seems reasonable to carry out such a test on every patient with newly diagnosed CRSwNP and bronchial asthma in order to be able to plan further treatment in this group of patients accordingly including biological treatment with antimonoclonal therapy against interleukin 4, 5 or13.
