Objective: To determine whether self-reported sickness presence (SP) and self-reported sickness absence (SA) are specific risk factors for future health problems or reduced work ability in the active workforce. Materials and Methods: The study population consisted of a cohort based on a random sample (n = 2181) with data for 2004, 2005, and 2006. The subjects were employees aged from 25 to 50 years in 2004. Cross-tabulations were calculated to identify significant background factors (sex, age, education, socioeconomic position), work factors (work demands, control, adjustment latitude), and outcome factors. Block-wise multiple logistic regression analyses were performed for outcome factors (SP, SA, self-rated health, physical complaints, work ability, mental well-being). Results: SA and SP were found to have negative health consequences; this was particularly pronounced for those with frequent SP or SA. There was a dose-response relationship between the degree of SA, SP and the different health outcomes. The health risks remained, after control for background factors, prior working conditions and initial health. SP also appeared to lead to SA, whereas SA did not have a significant impact on future SP. Conclusions: The results suggest that both SP and SA are strong predictors of future poor health, physical complaints, low mental well-being and low work ability. The detrimental influence of frequent SP was most pronounced in relation to work ability and physical complaints, although all of the measured health factors were affected. The negative effects of SA on the different health outcomes were similar.

Objectives: Studies involving simulators are increasingly popular. We examined to what extent exposure to a variety of test conditions on the simulator affects the level of mood and severity of simulator sickness. In addition, we were interested in finding out to what degree the changes in mood are associated with the severity of the symptoms of simulator sickness. Material and Methods: Twelve men (aged M: 29.8, SD: 4.26) participated in the study, performing two 30-minute tasks in a driving simulator truck (fixed-base and mobile platform). For measuring mood, the UMACL questionnaire was used, and to assess the severity of the symptoms of simulator sickness, the SSQ questionnaire was used. Mood and the severity of simulator sickness symptoms were measured 3 times during the study (pretest, 2 min and 0.5 h after the test). Results: Symptoms of nausea and disorientation occurred after the tests on both simulators. In the case of the mobile platform, exacerbation of the symptoms associated with oculomotor disturbances was observed. These symptoms were particularly severe 2 min after completion of the test on the simulator, and they persisted for at least 0.5 h after the end of the test. The correlations between simulator sickness and mood explained from 35% to 65% of the variance of these variables. In particular, a strong association was observed between the oculomotor disturbances and a decrease in energetic arousal. This refers both to the effect level and the duration of these symptoms. Conclusions: Simulator sickness is a major problem in the use of simulators in both the research and the training of operators. In the conditions involving the mobile platform, not only was a higher severity of the symptoms of simulator sickness observed, but also a decrease in energetic arousal. Therefore, the implementation of the mobile platform can provide an additional source of conflict at the level of incoming stimuli and changes in mood may increase this effect. Thus, it seems important to consider the tasks performed on the simulator in the context of utility and the purpose for which we use them.

Objectives: Previous research on the association between adjustment latitude (defined as the opportunity to adjust work efforts in case of illness) and sickness absence and sickness presence has produced inconsistent results. In particular, low adjustment latitude has been identified as both a risk factor and a deterrent of sick leave. The present study uses an alternative analytical strategy with the aim of joining these results together. Material and Methods: Using a cross-sectional design, a random sample of employees covered by the Upper Austrian Sickness Fund (N = 930) was analyzed. Logistic and ordinary least square (OLS) regression models were used to examine the association between adjustment latitude and days of sickness absence, sickness presence, and an estimator for the individual sickness absence and sickness presence propensity. Results: A high level of adjustment latitude was found to be associated with a reduced number of days of sickness absence and sickness presence, but an elevated propensity for sickness absence. Conclusions: Employees with high adjustment latitude experience fewer days of health complaints associated with lower rates of sick leave and sickness presence compared to those with low adjustment latitude. In case of illness, however, high adjustment latitude is associated with a higher probability of taking sick leave rather than sickness presence.

Objectives This article investigates sickness presence among secondary school students in 5 European countries. The research questions asked are: What characterizes students with high sickness presence in secondary schools? Does high sickness presence influence future sickness absence? Material and Methods A group of 7008 students aged 16–19 years participated in the first study (2016), and 5002 of them also participated in the follow-up study (2017). The participants came from 25 schools in Belgium, Estonia, Finland, Italy and Latvia. The response rate was high. A multivariate binomial logistic regression analysis was used. Results In 2016 high sickness presence (≥ 5 incidents) was reported by 16% of the students, and in 2017 by 15% of them. In 2016 there were significant differences between countries, and students from Latvia were most likely to report high sickness presence (adjusted odds ratio [aOR] = 3.45). Students with high absence (aOR = 1.86) and high school motivation (aOR = 1.16, for 1 pt increase on a 5-point scale) were overrepresented among those reporting high sickness presence. Country, absence and motivation were also significant factors for sickness presence in 2017. Furthermore, there was a significant positive association between high sickness presence and high sickness absence the following year. Conclusions Country, absence, and motivation were important factors for high sickness presence in secondary schools. Engaging in high sickness presence seemed to influence future sickness absence. Managers from 192 companies filled out the Employee Direct Participation in Organisational Change questionnaire measuring employees’ direct participation (DP) in organisational decisions. Four main forms of DP were identified: individual and group consultations, and individual and group delegation. Workplace safety was measured with the number of accidents, the number of employees working in hazardous conditions, accident absenteeism and sickness absence. Results showed that the 2 latter indicators were significantly related to some parameters of DP. Thus, companies that used face-to-face individual consultation had lower accident absenteeism than ones that did not. The same effect was true for group consultation with temporary groups, and individual and group delegation. Workplaces with high scores for scope for group consultation had lower accident absenteeism, and those with high scores for scope for group delegation had lower sickness absence. It was concluded that employee DP had a positive influence on workplace safety, even if involvement was not directly related to safety.

Objective: The number of lowland dwellers traveling at high altitudes has greatly increased in recent decades. Business travelers flying to La Paz or Lhasa are at risk of altitude illnesses, similarly to miners and soldiers transported to high altitudes. Traveling to high altitude requires adaptation and if this process fails due to too rapid an ascent rate or susceptibility of the climber, acute mountain sickness (AMS) may result. Doctors and nurses in travel clinics, health centers and occupational health care clinics may face patients asking advice on how to plan their trips or manage AMS, or the health care personnel may take part in a travel to high altitude environment. Methods: Two patients, aged 29 and 47 years, who fulfilled the criteria for AMS were studied. The clinical findings and treatment in the field are described including the review of the current recommendations for prevention and treatment of AMS. Results: Both patients developed a severe AMS due to too rapid ascent and their denial of the symptoms. Conclusions: Prevention is the safest and the most efficient method in the care concerning AMS. Realizing the risk of mountain sickness, active inquiry about the symptoms and correctly timed reaction to them, in other words interrupting the ascent or descending, help to reduce and even to prevent the development of serious problems.

Objectives This study is analyzing the unexpected reversed or lacking association between high adjustment latitude and sickness presence by examining whether it is due to confounding. Material and Methods Questionnaires were sent in 2004 and 2005 to a cohort of individuals aged 25–50 years, selected from the Statistics Sweden’s register of the Swedish population. Information from 2397 individuals who answered both questionnaires was analyzed by the use of logistic regression analysis. Results The odds ratio for sickness presence among the individuals with a low adjustment latitude compared to those with high adjustment latitude was 1.7 (range: 1.4–2.2). This increased likelihood was almost entirely unaffected in the analysis of potential confounders. Conclusions If the reversed association between adjustment latitude and sickness presence does not reflect confounding, it may be due to reporting bias, which may cause problems in research on sickness presence. We argue that more detailed studies are needed to explore the different sources of possible reporting problems.

Social insurance is part of social security which consists as well as of social assistance and services, health services and health care insurance. Everyone has the right for social insurance when retired or in case of incapacity of work under a certain system established by a law. The right of social insurance is part of labor rights. In Albania the mandatory social insurance scheme is based on the pay-as-you-earn principle, on the awareness of the individual about the risks in social field in its future and in the principle of agreement between generations. This is a scheme financed out of contributions from the employers, the employed persons and self-employed. The benefits are provided in case of sickness, maternity, old-age, disability, loss of breadwinner, employment accidents/occupational diseases and unemployment.

The author of this paper analyses selected solutions of sickness insurance in terms of the implementati on of the principle of solidarity, with the considerations referring to sickness and maternity allowances. A thorough analysis of the regulations concerning the personal scope of sickness insurance, the rules for financing contributions, conditions for the acquisition or continuation of sickness and maternity allowances, the possibility of receiving these benefits after the lapse of the insurance coverage and the basis for contribution assessment leads to the conclusion that sickness insurance is essentially based on the principle of solidarity. However, some detailed solutions violate this principle, by allowing for some abuses at the expense of the risk community or by allowing benefits to be received in a situation where there is no social risk.
W niniejszym opracowaniu autor analizuje wybrane rozwiązania ubezpieczenia chorobowego pod kątem realizacji zasady solidarności, przy czym rozważania dotyczą zasiłków chorobowego i macierzyńskiego. Dogmatyczna analiza przepisów dotyczących zakresu podmiotowego ubezpieczenia chorobowego, zasad finansowania składek, warunków nabycia lub kontynuowania pobierania zasiłków chorobowego i macierzyńskiego, możliwości pobierania tych świadczeń po ustaniu tytułu ubezpieczenia oraz wysokości podstawy wymiaru składki prowadzi do wniosku, że zasadniczo ubezpieczenie chorobowe opiera się na zasadzie solidarności. Niektóre szczegółowe rozwiązania zasadę tę jednak naruszają, pozwalając na pewne nadużycia kosztem wspólnoty ryzyka czy na pobieranie świadczeń w sytuacji, w której nie występuje ryzyko socjalne.

W konsekwencji przemian kulturowych i postępu technologicznego w medycynie we współczesnych społeczeństwach nastąpiła zmiana postrzegania śmierci. Staje się ona coraz częściej tematem tabu. Jednocześnie cały czas pojawiają się trudności komunikacyjne w relacjach na płaszczyźnie umierający pacjent – otoczenie – personel medyczny. Rozwiązanie wspomnianych trudności jest możliwe dzięki umiejętnościom przekazania przez lekarzy diagnozy bolesnej dla pacjenta i jego rodziny oraz jej przyjęcia przez zainteresowanych. Niezbędne we wzajemnej komunikacji wszystkich uczestników procesu leczenia (także pracowników medycznych, chorego i jego rodziny) jest świadomość naturalnych reakcji psychosocjologicznych związanych z przyswajaniem traumatycznych wydarzeń, takich jak śmierć, oraz nieunikanie prawdy o śmiertelnej chorobie.
As a consequence of cultural changes and technological progress in medicine, in nowadays societies there has been a shift in perception of death. Death becomes more often a taboo. At the same time, there are still difficulties in communication in relationship: dying patient- his environment- medical personnel. The solution of the difficulties mentioned above is possible thanks to certain skills: communication of a painful diagnosis by the doctors to the patient and his family as well as its acceptance by all the involved. It is crucial for all the participants of healing process (medical personnel, as well as the sick person and his family) to have awareness of natural psycho- and sociological reactions to traumatic events such as death. It is also necessary not to avoid the truth about the deadly sickness

It is acknowledged that ancestry may play a role in the likelihood of reporting motion sickness, based upon questionnaires in which symptoms are reported more frequently in individuals with Asian ancestry. This study compares motion sickness and related vertigo syndromes in Thai and Thai-Chinese populations. The Motion Sickness Questionnaire; Albany Panic and Phobia Questionnaire; Acrophobia Questionnaire; Body Symptoms Questionnaire and the Situational Characteristics Questionnaire were administered to 128 participants. Eighty-eight participants had a father, mother and all grandparents of Thai origin, while 44 participants had with at least one Chinese ancestor among parents or grandparents. All responses were similar between groups except regarding fear of heights, which is significantly higher in Thai participants without recent Chinese ancestors. Reported motion sickness sensitivity is similar between Thai and Chinese populations. The group differences for some fear of heights items may be linked to each group’ previous experience with heights. Results also suggest that although conquering a fear of heights might require specific visuo-vestibular adaptations, these adaptations alone may not be sufficient to lessen an individual’sfear of heights.

The article focuses on the understanding of sin in John 5:14, as well as the relationship between sin and sickness presented in this verse. It provides a thorough status quaestionis on both of these issues. After examining various hypotheses regarding the meaning of sin in John 5:14, the Johannine notion of sin as unbelief is expounded as the most convincing. This sin encompasses the past, present, and future life of the healed man. Thus, contrary to the exegetical opinio communis, the cause-and-effect relationship between sin and sickness does not apply to John 5:14. In light of this explanation, the messages of John 5:14 and 9:2–3 do not contradict each other. Jesus’ words in 5:14, intentionally pronounced in the temple, should be understood as an invitation to follow him in faith.

The safe transition from a higher pressure atmosphere to a lower pressure atmosphere is accomplished by planning the decompression process, typically through changes in pressure and/or composition of the breathing mix in a function of time. However, the decompression process is affected by a much greater number of inherent factors than changes in pressure and composition of the breathing mix. Their values should be kept within certain ranges, however, there are circumstances when it is not possible to maintain control over them. In this situation, they become elements of the residual risk of the decompression process. The safety of decompression should be ensured, inter alia, by analysing the residual risk for each execution of the decompression process.
najczęściej na drodze zmian ciśnienia i/lub składu czynnika oddechowego w funkcji czasu. Jednak na proces dekompresji ma wpływ większa liczba inherentnych czynników niż tylko zmiany ciśnienia i składu czynnika oddechowego. Ich wartości powinny być utrzymywane w pewnych zakresach, jednak zdarza się, że kontrola nad nimi nie może być dotrzymana. W taki przypadku stają się one elementami rezydualnego ryzyka procesu dekompresji. Bezpieczeństwo dekompresji powinno być zapewniane, między innymi, poprzez analizę rezydualnego ryzyka dla każdej realizacji procesu dekompresji.

Background: The effect of sleeping positions during acclimatization to high altitude on Acute Mountain Sickness (AMS) and High Altitude Pulmonary Edema (HAPE) is unknown. We tested whether sleeping with the upper body raised by 5° reduces prevalence and severity of symptoms of AMS as well as of elevated pulmonary artery systolic pressure (PASP) values as a risk factor of HAPE. Methods: Randomly assigning trekking tourist volunteers n = 44 (25 m, 19 f; mean age 42.9 yr) sleeping at 4280 m or 5170 m to the experimental group (upper body elevated by 5°), or to the control group. After exclusion of other reasons for AMS-like symptoms those assumed to be related to AMS were rated by Lake Louise Score questionnaire in the evening and the following morning of the study. Transthoracic echocardiography was performed on both occasions to estimate PASP. Results: In the study group, symptoms of AMS were significantly reduced in younger subjects (p = 0.021), prevalence of AMS was reduced in women (p = 0.156), and PASP values were significantly reduced in older subjects and men (p = 0.032; p = 0.031 respectively). Conclusion: Results suggest that sleeping with the upper body in elevated position during a high altitude ascent may benefit those suffering from AMS or at risk of HAPE due to elevated PASP values.

Opis:
Japanese diving fishermen are known to have a high prevalence of dysbaric osteonecrosis. Their typical dive practices are characterized by long exposure to high pressure and by repetitive diving. The etiology of dysbaric osteonecrosis still remains controversial. Because the development of dysbaric osteonecrosis may occur silently, the diver may be unaware of the actual ischemic event. Moreover, key etiologic evidence is often lost before the diver is examined. The early stage of dysbaric osteonecrosis was evaluated in four autopsy cases of divers who died of acute decompression sikness. Hypercoagulability of blood was present in these cases. Hypercoagulability of blood has also been observed in experimental rats after decompression. Dysbaric osteonecrosis has been experimentally induced in six sheep at the University of Wisconsin - Madison. These findings are reviewed by comparing dysbaric osteonecrosis in both divers and experimental animals. In conclusion, a bone compartment syndrome and hypercoagulability appear to be pathogenetic factors in dysbaric osteonecrosis.
