This study assessed the quality of referral letters to the Glaucoma Outpatient Clinic and patients’ first consultation results. A retrospective study included 196 patients newly referred to the clinic. Referrals were mainly issued by ophthalmologists (156; 79.6%). Regarding their urgency level, the planned referrals (184; 93.9%) were dominant. The majority of referral diagnoses (147; 75%) were confirmed as glaucoma in the clinic, whereas 17 (8.2%) glaucoma diagnoses were rejected. 64 (32.7%) patients did not require further ophthalmological review. The assessment of the quality of referral letters revealed that important clinical data was missing. There is a need to improve the referral system to specialist glaucoma clinics in Poland.

Opis:
Background. Adequate utilization of primary care directly reflects the health status of the population. In Georgia (Republic), many patients seek care without a referral by a primary-care provider, and as a result, patient referral rates to family physicians are low. A tendency of patient self-referral behavior may reduce the effectiveness of the healthcare system. Objectives. The purpose of the research is to study the problem of the low rate of patient referrals to family physicians in Georgia. Material and methods. Within the quantitative survey, 20 family physicians and 300 patients were interviewed through a semi-structured questionnaire in different regions of Georgia. Results. Patient referral rates to family physicians are low. 15% (n = 3) of family physicians recognize that patients often address them only for a referral to specialists. Only 5% (n = 3) of family physicians provide preventive consultations on occasion and 50% (n = 10) – in the case of need only. 70% (n = 14) of family physicians think that their remuneration is not adequate for their work and that they work more than they are paid. 35% (n = 105) of respondents in the case of a health problem address both a family physician and a specialist- -physician. 42% (n = 126) of patients visit a family physician once a year or do not visit at all, and 47% (n = 141) of patients believe that the family physician institute needs some changes. Conclusion. The result suggests that the low rate of patient referrals to family physicians is due to distrust towards family physicians, which is related to a lack of qualification of physicians and low public awareness of the competence of the family physicians. Due to inadequate reimbursement, family physicians do not have enough motivation to provide adequate service, and the lack of continuous professional education negatively affects their professional development. It is recommended to raise public awareness about primary care, to introduce effective methods for payment of family physicians and to increase the role and affordability of continuous professional education.

More and more business customers use social media to express their opinions about products, services and brands. This study aims to make a step further in understanding consumer engagement in electronic word of mouth (eWOM) on Facebook and its impact on recommending and purchasing products. To test our conceptual framework, structural equation modeling techniques have been applied to data collected through a self-administrated survey addressed to 640 Facebook users from Romania. Our model shows that Facebook’s perceived usefulness infl uences customer brand engagement, which in turn has an important effect on the intention to purchase the brand and recommend it to others (word of mouth). Therefore, people who use Facebook as a source of information about products and services are top eWOM providers. A brand that seeks to build authority on Facebook should look for eWOM providers, convert them into fans and “feed” them with attention-grabbing information that is worth spreading. This way, most of the fans will provide free word-of-mouth advertising, becoming true brand ambassadors. Our study has established that Facebook’s perceived usefulness not only engages online users with brands, but also makes this medium a perfect eWOM tool: it builds trust in friends’ recommendations, and eventually, makes the users sincere supporters of the brands they like.

Background. Oral health care services are often sparse and inconsistent in India therefore it is often difficult for poor people to get access to the oral health care services. The approach by dental institutions with the help of community outreach programs is a step ahead in overcoming this situation. Objectives. The study was conducted to evaluate the number of patients, disease pattern and the services provided in the outreach programmes and also effectiveness of patient referral. Methods. A retrospective study was conducted and the data were obtained from records of outreach programs conducted, in last 2 years by Pacific Dental College and Hospital. The data were analysed using descriptive statistics for the computation of percentages Chi-square test was applied to know the association of effectiveness of referral with age and gender. Confidence level and level of significance was fixed at 95% and 5% respectively. Results. A total of 22982 individuals in the age group of 4-80 years attended the outreach program. Dental caries (42.3%), periodontal diseases (63.2-69.0%) and dental fluorosis (33.7-35.0%) were commonly observed diseases. Effectiveness of referral was significantly high among the middle age adults and females (P<0.05). The effectiveness of referral was highly improved in 2013 after establishment of certain guidelines and strategies. Conclusion. The approaches by dental institutions with the help of community outreach programs can spread awareness and disseminate treatment and thereby enhancing access to care and eliminating access to care within the rural communities.

Efficacy of Rectal Artesunate on parasitaemia and febrile condition of children was assessed in 905 children below the age of 5 years. Grid systematic method was employed in selecting sixteen study centres from 8 Local Government Areas of Ogun State. Ethical approvals were obtained in addition to interactive sessions with parents of the children and PHCs medical practitioners. Body weight and temperature were taken and drug was administered at a dose of 5 to 10mg/kg of the body weight per rectum at 0 hour, 24 hours and 48 hours. Efficacy of treatment was monitored by analysing blood samples taken at 0 hour, 24 hours and 48 hours for Malaria Parasites Count (MPC/µl), parasitized red blood cells (PRBC). Data obtained were analyzed using SPSS version 20 to assess association with p-value of < 0.05. A significant (p < 0.05) difference was observed between malaria parasites prevalence at 0 hour, 24 hours and 48 hours. Also a baseline mean malaria parasite density (MPC/µl) of 3,320/μl was observed at 0 hour and a reduction in mean MPC/µl of 1,230/μl and 420/μl at 24 hours and 48 hours respectively with a significant (p < 0.05) difference between MPC/µl at 0 hour, 24 hours and 48 hours. Baseline mean parasitized red blood cells (PRBC) of 4.2 % was observed at 0 hour with reduction in mean PRBC of 2.4 % and 1.1 % at 24 and 48 hours respectively with a significant (p < 0.05) difference between PRBC at 0 hour, 24 hours and 48 hours. Fever Subsidence Ratio between 0 hour, 24 hours and 48 hours were 1.4:1.1. Rectal artesunate is highly effective and well tolerated antimalarial suppository for pre-referral and parenteral therapy.

Objectives. To assess occupational injuries in the 1st versus 2nd half of a working shift in terms of (a) the likelihood of hospital referral following an injury event; (b) the external causes of injuries. Methods. Logistic regression analyses of data from a 16-year (1991–2007) experience of ongoing surveillance of occupational injuries in a synthetic fiber factory in Iran. Results. The likelihood of a hospital referral following an injury in the 1st half of a shift was higher than in the 2nd half (adjusted odds ratio [OR] 1.50, 95% confidence interval [CI] 1.10–2.00). Comparing the 2 halves of the shift, an injury occurring in the 2nd half was more likely to be due to exposure to smoke, fire and flames (OR 2.34, 95% CI 1.06–5.19) or transport accidents (OR 1.84, 95% CI 1.06–3.21). Conclusions. Time-specific safety interventions could be used in the risk management of occupational injuries. Further studies to investigate the effect of time-dependent interventions are recommended.

Introduction. Poland is among the countries which reported the highest rates of mortality from cancer. The health behaviours of people are influenced, among other things, by their beliefs about cancer, but their evaluation of oncological institutions and specialists seems also to be of great importance. Objectives. 1. How the respondents evaluate Polish oncology: a) the conditions of treatment in oncology hospitals, b) access to oncological medical services, c) the competence of oncologists. 2. What are the socio-cultural factors of the assessment of Polish oncology? 3. What is the influence of the grade level of the assessment of Polish oncology on the degree of fear and the knowledge about cancer? Materials and method. The study was carried out with a sample of 910 adult residents of Wroclaw in south-west Poland. Quota sampling was used. An interview questionnaire was used as the method. Results. Analysis of the data showed a negative image of Polish oncology, according to the study participants: dissatisfaction with both treatment conditions and with access to medical services. Assessment of Polish oncology depends primarily on education, age and economic situation, as well as ‘family history of cancer’, and attitude towards doctors. The lower the rating of Polish oncology, the lower the medical knowledge, and the higher the level of fear of cancer. Conclusions. Negative assessment of Polish oncology perpetuates the fear of cancer in society. There is a need for constant improvement of the quality of medical oncology services, for building public trust in physicians, to fight inequalities in health, and to take into account the lay perspectives in developing strategies to combat cancer.
