Opis:
Use of bioresorbable materials in bone surgery opens up new possibilities in treatment of injuries and orthopaedic illnesses. Lack of necessity for implant removal surgery enables faster treatment and substantial reduction in costs of treatment. In recent years, these advantages have caused growing interest in materials of this type. It must be noted that bioresorbable materials in most cases are based on macromolecular materials – polymers which have completely different mechanical characteristics than traditional materials used in bone surgery. Bioresorbable properties give enormous possibilities but also present difficult challenges related to construction of new types of implants based on materials of this type. Full characteristics of new materials is indispensable in construction process, especially of mechanical properties and surface properties. The latter is responsible for contact with biological environment and bioactivity is determined on the basis of hydroxyapatite precipitations amount on material surface. In case of polymer based composites, influence of phases in powder and fibre form on properties is fundamental to be proved. Addition of particle modifier into polymer matrix causes reduction in strength of polymer. This phenomenon is mostly associated with homogenic particle distribution difficult to be achieved in case of higher filler content. However biological interplay is insufficient by low filler contents. Therefore in the below presented investigations different filler contents were applied and their influence on mechanical properties was investigated to achieve compromise between mechanical properties and bioactive, antibacterial or anti-inflammatory effect of the applied particles. Acquaintance with these results allowed for elaboration of full range of material and constructive solutions for implants in bone surgery. Biodegradable biopolymers have low mechanical properties (Young’s modulus, strength, viscoelasticity). Construction process of bioresorbable implants requires to be completely changed in way of conduction. Computer simulation based investigations of stress-strain characteristics is necessary to be carried out in each stage of research. Bigger cross-sections and different shapes of implants are required to achieve suitable stiffness. Application of bone screws made of resorbable materials requires design of these elements from the bottom up. Design of optimal shape of screw thread and point of contact between screw head and bone plate is particularly important. 9 different implants were designed within the confines of conducted research – including for tibia, humerus, radius, ulna, phalanges, clavicle, calcaneus, pelvis and metatarsus. Stress values were determined in analyzed implants. Acceptable loads were determined for individual types of plate. Plates made of PLA can bear small loads and play role mainly as grasping and holding bone element. In most cases additional immobilization of operated bone is required in order to prevent plate or screw destruction. In case of stabilization function for resorbable materials – as many screws as possible are recommended – thanks to it, screws are less loaded and the plate-bone system is more immobilized.

The external osteosynthesis is one of the methods of healing bone fractures. The idea of external fixators design consists in inserting into the bone fragments elements, which are coupled outside the limb by an element, called the frame of the fixator, having the fracture set. The external fixation is based on the principle of "load transfer". The design of a new generation of external fixators ought to employ some methods of mechanical analysis. Selected problems related to the modelling and simulation of physical performance of the unilateral external mechatronic orthopaedic fixator-bone system are presented. The majority of works makes use of the rigid finite element method for analysing the rthopaedic devicebone system. The paper presents some problems regarding mechanics applied in the external osteosynthesis design.
Stabilizacja zewnętrzna jest jedną z metod leczenia złamanych kończyn. Konstrukcja stabilizatorów zewnętrznych składa się z ramy nośnej umieszczonej po zewnętrznej stronie złamanej kończyny oraz wkrętów kostnych zamocowanych w odłamach złamanej kończyny i zablokowanych jednocześnie w ramie nośnej. Ideą konstrukcji stabilizatorów zewnętrznych jest "przenoszenie obciążenia" podczas procesu zrostu złamanych odłamów kostnych przez ramę nośną, a tym samym odciążenie szczeliny zrostu kostnego. W projektowaniu stabilizatorów nowej generacji wykorzystywane są wybrane zagadnienia mechaniki, które umożliwiają analizę układów biomechanicznych, takich jakim w prezentowanym artykule jest system stabilizator zewnętrzny-odłam kostny. Artykuł prezentuje zastosowanie wybranych zagadnień mechaniki w projektowaniu ortopedycznych stabilizatorów zewnętrznych nowej generacji.

Ankle fractures are complex injuries with variable prognoses that depend upon many factors. The aim of the treatment is to restore the ankle joint biomechanical stability with maximum range of motion. Most ankle fractures are fibular fractures, which have a typical oblique fracture line in the distal fibula located in the area of the tibiofibular syndesmosis. The aim of this study was to simulate numerically several fixation techniques of the distal fibular fractures, evaluate their stability, determine their impact on surrounding tissue load, and correlate the results to clinical treatment experience. The following three models of fibular fracture fixation were used: (a) plate fixation with three screws attached above/below and lag screws, (b) plate fixation with two screws attached above/below and lag screws, and (c) three lag screws only. All three fracture fixation models were analyzed according to their use in both healthy physiological bone and osteoporotic bone tissue. Based on the results of Finite Element Analysis for these simulations, we found that the most appropriate fixation method for Weber-B1 fibular fractures was an unlocked plate fixation using six screws and lag screws, both in patients with physiological and osteoporotic bone tissue. Conversely, the least appropriate fixation method was an unlocked plate fixation with four screws and lag screws. Although this fixation method reduces the stress on patients during surgery, it greatly increased loading on the bone and, thus, the risk of fixation failure. The final fixation model with three lag screws only was found to be appropriate only for very limited indications.

Opis:
The method of objective assessment of the correctness mandible bones fracture slit was proposed in the research. At present, treatment results are evaluated by comparing pantomographic pictures made before and after integration. It is not an objective method; the estimation of the integration correctness coefficient is not possible, either. The use of computer processing enables introducing the relative mandible faction dislocation coefficient. Thanks to this, measurable determination of bone faction reposition correctness on pantomograms made after osteosynthesis is possible.

Opis:
At present, in mandibular fractures surgical treatment reposition and titanium miniplates osteosynthesis is the most common method used. The treatment results are assessed on the basis of comparative analysis of two radiograms taken before and after fixations of fractures. In this article the objective method of radiological assessment of osteosynthesis is presented. The crucial part of the method is the computer program which was created at The Institute of Computer Biomedical Systems, Silesian University in Katowice. Comparison of placement of bone fragments on a pair of pantomograms taken before and after a particular surgical operation was done. To make the process of the results evaluation an objective one a new parameter - the relative dislocation coefficient - was implemented. Achieved results enable to assess correctness of bone fracture reposition on the pantomographical pictures taken twice, once before and the next time after osteosynthesis.

Based on the analysis of surgical instruments withdrawn from the operating theatre, it was decided to briefly present the history of osteosynthesis in the orthopaedic department in Gdansk, Poland. Boxes with orthopaedic instruments withdrawn from the operating theatre were obtained. The instruments were grouped, numbered, photographed and catalogued. Not all obtained instruments were used in orthopaedics; the boxes contained several instruments typically used in other fields of surgery. Attempts have been made to divide orthopaedic instruments into several groups, depending on their use in bone fixation. Some specific instruments are used only in one method. The most interesting tools are discussed in detail, with attention to their features and role in shaping modern orthopaedics. Some of the obtained equipment could have been made in the Orthopaedic Workshop in the hospital. They are specific exhibits related to the development of orthopaedics in this hospital that required preservation.
Na podstawie analizy narzędzi chirurgicznych wycofanych z sali operacyjnej postanowiono pokrótce przedstawić historię osteosyntezy na oddziale ortopedycznym w Gdańsku. W badaniu wykorzystano pudła z narzędziami ortopedycznymi wycofanymi z sali operacyjnej. Narzędzia zostały pogrupowane, ponumerowane, sfotografowane i skatalogowane. Nie wszystkie narzędzia były wykorzystywane w ortopedii; pudła zawierały również kilka narzędzi typowo stosowanych w innych dziedzinach chirurgii. Podjęto próby podzielenia narzędzi ortopedycznych na kilka grup, w zależności od ich zastosowania w zespalaniu kości. Istnieje kilka specyficznych narzędzi, które są używane tylko w jednej metodzie. Szczegółowo omówiono najciekawsze narzędzia, zwracając uwagę na ich cechy i rolę, jaką odegrały w kształtowaniu współczesnej ortopedii. Część uzyskanego sprzętu mogła zostać wykonana w Pracowni Ortopedycznej w szpitalu. Są to specyficzne eksponaty związane z rozwojem ortopedii w tym szpitalu, które wymagały konserwacji.

Opis:
Purpose: The purpose of this study was to analyze three patterns of mandible angle fracture treatment by means of the finite element analysis. Methods: Investigation has been based on the mandible geometry reconstructed with use of hospitalized patient CT data. The KLS Martin mini-plates with corresponding screws were used to establish proper fracture stabilization. Models were run assuming isotropic and elasto-plastic material properties of connecting devices and cortical bone. The main masticatory muscles and artificial temporomandibular joint have been incorporated to assure mandible physiological movement. The gage loading has been applied in three different locations to cover wider range of possible mastication loading cases during daily routine. A different contact conditions have been applied to the fracture plane to simulate both load bearing and sharing behaviors. Prepared FEM models reflect the most frequently used surgery’s approaches to mandible angle fracture treatment. A specific nomenclature has been introduced to describe particular model. The tension plate, with one connecting mini-plate, two-point fixation and combined fixation, both using two mini-plates respectively. Results: Performed analysis allowed for a detailed estimation of the mini-plate connection response under the applied gauge loading. The equivalent stress within the mini-plates and surrounding cortical bone have been compared between all models. Regarding the fracture plane, the contact status and pressure have been considered. Conclusions: The combined fixation model, acting as a biplanar fastener system, presents the highest flexibility and connection efficiency.
