Opis:
Appropriate treatment of symptomatic carotid artery stenosis can reduce ischemic cerebral strokes' risk and in some cases eliminate neurological symptoms. Endarterectomy is the most common surgical treatment. The aim of the study was to examine the influence of carotid endarterectomy on neurological symptoms and patients' life quality.Material and methods. The material comprised of 102 patients who underwent endarterectomy. All of the patients were given a questionnaire with a list of neurological symptoms (vertigos, headaches, left hemiparesis, right hemiparesis, numbness, acroparaesthesia, single syncope, recurrent syncopies, diplopia, tinnitus, concentration disturbances and aphasia) and with a numerical life quality scale to fill in before and a year after the surgery.Results. Vertigo, headache, single and recurrent syncopies and aphasia as well as cerebral stroke and amaurosis fugax were significantly more rarely observed after endarterectomy. The mean value of patients' life quality evaluated on a 10-point Likert scale after the surgery increased (3.9 vs 6.3).Conclusions. A year after carotid endarterectomy patients' life quality improves which is connected with neurological symptoms' regression and no further symptoms' occurrence due to a preventive role of the surgery.

Opis:
Paraneoplastic syndrome (PS) is a dysfunction of organs or systems, associated with neoplastic disease, but not related to the local growth of tumor, metastasis or adverse anti-cancer drugs reactions. Neurological paraneoplastic syndromes (NPSs) may affect every region of a human nervous system - both central, peripheral, and/or autonomic nervous system. The symptoms are caused by a neoplastic process in other organ or system. PNSs usually precede the development of cancer for months or even years, and can be therefore useful diagnostic markers of cancer. They present an autoimmune background associated with the response of the immune system against cancer cells. In the blood serum and cerebrospinal fluid of patients with PNS appear onconeural antibodies, reacting with tumor antigens and the brain, the spinal cord and peripheral ganglia antigens. Many authors emphasize the significance of paraneoplastic syndromes in the modern oncology. Due to this reason, the knowledge of paraneoplastic syndromes and their mechanisms is very important in the contemporary medicine. The purpose of this research review is to summarize the information about the clinical features of the most common PNSs and the pathological mechanisms of their development.

Introduction: There are different complications related to the submandibular gland resection, like damage to the mandibular branch of the facial nerve, lingual or hypoglossal nerve. Moreover, subjective complaints like dry mouth, altered sensation around the scar and taste alterations were described. Patients and methods: A prospective, non-randomized study was performed including 20 patients treated with submandibular gland resection. Results: The most common etiology was sialolithiasis affecting 9 (45%) patients. One (5%) patient presented a lingual nerve paresis, 2 (10%) patients presented a transient marginal mandibular branch paresis and no hypoglossal nerve alteration was observed. Factors like the size of the lithiasis (P = 0.293), size of the tumor (P = 0.445) or type of pathology (P = 0.694) were not related with neurological complications. Altered sensation in the skin around a scar was the most severely weighted problem (55% and 33.3%), and xerostomia was the second one (33% and 27.3%) at one month and at six months after surgery, respectively. Conclusion: The submandibular gland resection remains to be a safe technique with a low rate of surgical and neurological complications. However, patients should be informed about sequelae like xerostomia, altered sensation in the skin or pain around the scar, which may be discomforting. The cosmetic result can be satisfactory, but a surgical scar can be avoided with another approach. Although rare, the risk of MBFN, lingual and hypoglossal nerve damage should always be included in the preoperative information on surgical risks.

Opis:
Paraneoplastic neurological syndromes (PNS) are a group of rarely observed disorders, accompanying about 1% of cancer diseases. They have a typically aggressive course, leading to profound and essentially irreversible disability. The pathogenic cause behind PNS is damage of the nervous system structures as a consequence of the body’s immunological reaction induced by cancer. Onconeural antibodies generated as part of the reaction target the tumour tissues, but also the regular nervous tissue recognized as antigens. Presence of onconeural antibodies reveals a 50–60% sensitivity and 100% specificity in diagnosing PNS. Imaging and laboratory tests have a limited impact on diagnosis. What is crucial is the analysis of clinical systems following Graus’s criteria (elaborated in 2004). In the case presented here, the patient underwent treatment in many neurological departments due to the symptoms of progressive damage of the nervous system. She was eventually diagnosed with the stiff person syndrome related to breast cancer. In spite of the treatment, including an efficacious oncological therapy, the severe neurological deficit resulted in serious motor disability.

Isoprostanes are a recently discovered group of prostaglandin isomers. Results of previous studies suggest that they can be used as oxidative stress markers, because in a number of cardiovascular, pulmonary and neurological diseases their levels in biological samples considerably increase. It has been found that people suffering from diabetes, obesity, homozygous familial hypercholesterolemia, moderate hypercholesterolemia, and smokers have higher levels of isoprostanes in urine. The same refers to patients with asthma, Alzheimer disease and Down syndrome. This paper reviews the results of relevant studies.

This study aimed to evaluate possible health effects associated with long-term occupational exposure to low levels of mercury vapors. Forty-six subjects exposed to mercury and 65 healthy unexposed employees were studied. The subjects were administered a questionnaire on experienced symptoms and underwent clinical examinations as well as routine biochemical tests. Atmospheric and urinary concentrations of mercury were measured, too. Environmental concentrations of mercury were estimated to be 3.97 ± 6.28 μg/m3 and urinary concentrations of mercury in exposed and referent groups were 34.30 ± 26.77 and 10.15 ± 3.82 μg/dm3, respectively. Additionally, symptoms such as somatic fatigue, anorexia, loss of memory, erethism, blurred vision and teeth problems were significantly more common among exposed individuals. These observations indicate that occupational exposure to mercury vapors, even at low levels, is likely to be associated with neurological and psychological symptoms.

Introduction. Every farming method, whether conventional or organic, has been associated with some sort of risky behaviors leading to health issues among farmers. Substantial evidence is not available in the literature to determine whether the magnitudes of health outcomes vary between conventional and organic farmers. The study investigated whether selfreported neurological and mental health symptoms differ between conventional and organic farmers living in Indiana, USA. Materials and method. A self-reported questionnaire survey collected information from 200 conventional and 157 organic farmers of Indiana on demographic characteristics, depression and neurological symptoms. Statistical analyses were conducted to observe the differences in self-reported symptoms by groups of farmers. Results. It was observed that the conventional farmers had significantly higher age-adjusted mean neurological symptom score (p<0.01) than the organic farmers. Regression models revealed positive and significant associations of conventional farming with total (β =1.34; p=0.02), sensory (β =0.83; p=0.001) and behavioural (β =0.09; p=0.03) symptoms after accounting for age, income, education and years in farming. Positive but non-significant associations were also observed in conventional farmers with cognitive and motor symptoms, and with all subscales of depression symptoms in the adjusted models. Conclusion. The findings obtained suggest the importance of a larger study to further explain the difference in mental and neurological health effects in these two categories of farmers.

Opis:
Abstract: Intracerebral haemorrhage is a subtype of stroke which has highest mortality and morbidity rates and currently has no cure. Cannabinoid 2 (CB2) receptor expression is up-regulated in neuronal injuries. CB2 receptor agonists were found to be neuroprotective in brain injuries including ischaemic and haemorrhagic stroke. This study was carried out to investigate the effects of cannabinoid 2 receptor (CB2R) activation by JWH133 on intracerebral haemorrhage (ICH) induced blood brain barrier disruption and oedema formation. ICH was induced in experimental animals by collagenase injection which causes significant vascular disruption and concomitant increase in oedema; animals were then treated with JWH133. Sixty CD-1 mice were randomly divided into sham, vehicle, and JWH133-treated groups (1 mg/kg and 10 mg/kg). Neurobehaviour, brain water content, Evans Blue dye extravasation, haemoglobin content, lung water content, and body weights post ICH were assessed. JWH133 treatment attenuated neurological deficits at 24 and 72 hours post-ICH. The treatment also reduced brain water content and Evans blue dye extravasation but had no effect on haemoglobin content and lung water content. Administration of JWH133 treatment mitigated weight loss at 48 and 72 hours after ICH. The reduction in brain water content and Evans blue dye extravasation indicate that CB2 receptor activation decreases blood brain barrier disruption and brain oedema, resulting in improved neurological functioning. This suggests that activation of the CB2 receptor by JWH133 is neuroprotective after ICH and may be a therapeutic target. Further study is needed to explore the mechanisms by which these effects occur.

Opis:
We evaluated the effectiveness of three different rehabilitation programs for chronic behavioral disorders in traumatic brain injury (TBI) patients with orbito-frontal damage (OFC), incurred from 13 to 15 years prior to enrollment. From among 547 adult patients with severe TBIs treated neurosurgically from 1999 to 2003, 60 were selected for the study, divided into three groups of 20, matched for age and sex. Each group received one of three different rehabilitation protocols. Group A received a comprehensive program with two components: (1) a 6-month early neuropsychological rehabilitation program targeting cognitive deficits, beginning immediately post-injury, and (2) the “Academy of Life” program, with weekly meetings aimed at reducing behavioral disorders in social transactions. Group B received only cognitive rehabilitation, while Group C received only the “Academy of Life” progam. The Frontal Behavioral Inventory (FBInv) was used to assess behavioral disorders at baseline and again after completion of the study. At baseline, all 60 patients still displayed all or most of the common behavioral symptoms characteristic of frontal syndrome. The improvement achieved by group A (the comprehensive program) was significantly better upon completion of the study than that shown by the patients in groups B (cognitive only) and C (social skills only). A comprehensive program of rehabilitation, which combines early neuropsychological rehabilitation of cognitive deficits with the “Academy of Life” program, aimed at improving social skills, proved to be more effective than either of these two components administered without the other

Opis:
Do najczęstszych jednostek chorobowych powodujących trudności językowe o typie afazji należą choroby naczyniowe mózgu, a wśród nich udary niedokrwienne.
The most common diseases causing language difficulties of aphasia type include cerebral vascular diseases, including ischaemic strokes. Speech disorders resulting from brain damage are usually dynamic. The clinical picture, and further on, the effective diagnosis and speech therapy of a patient after a neurological incident, is thus greatly influenced both by the circumstances in which the patient found himself and the time factor. The paper presents the dynamics of aphasic disorders in a patient after ischaemic stroke within the left hemisphere. The aim is to draw attention to the changes in the image of aphasic disorders in a 63-year-old man after a neurological incident in which semantic aphasia was diagnosed, which is the evidence of differences in the occurrence of symptoms (including aphasic disorders) in the patient immediately after the stroke and over a dozen months after the incident, after an of intensive speech therapy.

Neurological disorders are diseases of the brain, spine and the nerves that connect them. There are more than 600 diseases of the nervous system, such as epilepsy, Parkinson's disease, brain tumors, and stroke as well as less familiar ones such as multiple sclerosis or frontotemporal dementia. The increasing capabilities of neurotechnologies are generating massive volumes of complex data at a rapid pace. Evaluating and diagnosing disorders of the nervous system is a complicated and complex task. Many of the same or similar symptoms happen in different combinations among the different disorders. This paper provides a survey of developed selected data mining methods in the area of neurological diseases diagnosis. This review will help experts to gain an understanding of how data mining techniques can assist them in neurological diseases diagnosis and patients treatment.
