The port areas are specific in terms of navigational accidents. Usually, accidents with low consequences dominate, but due to many users with contradictory needs, there is a requirement to keep the balance between business and safety. This balance is usually achieved by the Port Authority or like in Poland by Maritime Office together with Port Authority. Maritime Offices are the governmental body responsible to maintain the acceptable navigational safety level. Such an approach leads to several frictions between the users that needs are often contradictory. To ensure minimal safety level and introduce new or amended port regulations the scientific methods are demanded to support this process. The paper present methodology to adjust the port regulations of Kołobrzeg Port where the passenger ship owners moored in close vicinity of the waterway and therefore restricted it to the other users. The compromise was achieved with the application of real-time simulation method. Moreover, the paper presents the important role of harbor regulations in whole navigation risk management process within the port area. Today’s port regulations are created mostly based on good practice of pilots and experts, whereas the quantitative methods are used less frequently. The intention of the presented case study was to demonstrate how the quantitative risk assessment could be used in port policy development.

The paper presents research on the actual ferry crossing traffic in Świnoujście by using laser measurement system. W artykule przedstawiono badania rzeczywiste ruchu promów na przeprawie miejskiej w Świnoujściu z wykorzystaniem metod laserowych. Celem pracy było określanie bezpieczeństwa współistnienia przeprawy w aspekcie modernizacji stanowiska nr 6 Bazy Promów w Świnoujściu oraz weryfikacja badań symulacyjnych wykonanych w 1996 roku w Instytucie Inżynierii Ruchu Morskiego w Szczecinie.

In the safety evaluation of manoeuvres carried out on the basis of INS indications, the real ship's dimensions are used to determine navigation safety level. The paper presents a new approach to this problem. The ship's maneuvering area and collision probability were calculated incorporating uncertainty areas of ship's plan geometry at given probability level for typical configuration of navigational equipment applied in existing pilot systems. The widening coefficients of safety factors for each configuration set were determined and discussed. The most economically advantageous configuration of navigation system was chosen.

Ship traffic on the Baltic Sea grows each year affecting the shipping safety and boosts chances of collision with other vessel. In this article the modelling of hazard of collision is presented for ship routes crossing, taking advantage of function Copula and methods of queuing theory.
Gęstość ruchu statków na Bałtyku wzrasta każdego roku, powodując zwiększone ryzyko wystąpienia kolizji statków. W pracy przedstawiono modelowanie zagrożeń dla kolizyjnych strumieni transportowych, wykorzystujące funkcje Copula i metody obsługi masowej.

The paper presents the method of estimation of safety navigation. The proper criteria, measures and rate will need the estimation of safety navigation. These are very often expressed by risk. Four different definitions are used. The mostly used definition is a combination of accident probability with connection to its results. Probability of accident can be expressed in different manner. Three kinds of them are presented. Also the consequences of accident may be calculated in many aspects. There are necessary activities leading to efforts of unifying of these quantities.

Opis:
Ensuring the safety of navigation requires searching and implementation new methods of operation by International Maritime Organization. Greater vessel dimensions, large quantities of dangerous goods transported by sea, require enforcing new regulations for waters of the increased risk of occurrence of a dangerous situation or collision. Identification of hazards allows predicting different scenarios of events and finally the diagnosis for risk assessment, its control and limitation. Activity connected with the improvement safety of navigation should be unified and the same tools for such action are recommended to be applied worldwide. The paper discusses the role of formal safety assessment as a tool used by the International Maritime Organization in the decision-making process. The applicability of this methodology in national and shipowners’ regulations concerning the safety of navigation is presented in this article.

It is well-known that operator error – “human factor” – is the cause of most accidents. This is a great concern to navigators. Recent crashes of ferries with large numbers of human victims have occurred because of navigator errors or because of their inability to make adequate decisions in an unexpectedly arisen emergency. This paper presents a possible method for quantitative estimation of the “human factor” based on analysis of the physical and mental conditions of the operator and the working environment.

The need to develop and adopt legal standards for safety management was identified at the Conference of General Directors of Civil Aviation dedicated to the global strategy of aviation safety (Montreal, March 20-22, 2006) and additionally supported by the recommendations of the Safety Conference (Montreal, March 29 - April 1, 2010). The year 2006 saw the introduction of issues related to the safety management system to the Convention’s annexes. The existing fundamental rules related to SMS were included in the annexes: 1 - Personnel licensing, 6 - Operation of Aircraft, 8 - Airworthiness of Aircraft, 11 - Air Traffic Services, 13 - Aircraft Accident and Incident Investigation, and 14 - Aerodromes. In addition, due to the dynamic development of aviation and, at the same time, the growing need to create the highest standards in the field of safety management systems, the Air Navigation Commission established the Safety Management Panel (SMP) to develop the content of Annex 19. After several years of reconciliation and consultation, this document was finally adopted by the ICAO Council in 2013. It has become the ipso facto document that provides the basis for creating an effective safety management system, as it is compatible and also adapted to the most effective practices indicated in the ICAO Doc 9859 Safety Management Manual. Annex 19 sets out the responsibility of States, the safety management system and the collection of information related to safety. The National Civil Aviation Safety Programme and the structure of this system were also dealt with. The whole idea is to support states in the process of hazard identification and risk management. The purpose of the article was: to highlight selected elements of the risk management process in a practical dimension, to present the principles for assessing the degree of risk by an air navigation service provider, and to identify ways of improving this process. Searching for an answer to the main research problem contributed to achieving this goal. The research problem was expressed in the form of a question: How does the air navigation service provider manage the risk under the air traffic safety management system in order to achieve compliance with regulatory requirements and to achieve the assumed parameters (indicators) in the area of safety? With regard to the main research problem, the following research hypotheses were formulated: 1. I believe that an air navigation service provider undertakes an assessment of the degree of possible danger in civil aviation to identify hazards in air traffic as part of a safety management system. 2. I suppose that the air navigation service provider manages the risk in a manner consistent with the provisions of law and by using procedures included in normative documents.

The risk analysis of ships’ accidents leads to the conclusion that one of the most important and dangerous stages existing during a ship’s trip is harbour entering and leaving. The use of the modern methods of risk estimation should improve both ship/harbour safety and the economic profitability of the taken decisions. This concerns especially the harbour entering by maximum acceptable size ships for the given harbour. The analysis of the indispensable data sets and the decision model algorithm proposed for the system of Szczecin– Świnoujście ports were described in the article. The use of the decision model, taking into consideration the dynamic current data and hydro-meteorological conditions, including their changes tendency and the possible changes of ship movement parameters, should increase the decision efficiency and hold the same safety level.

The paper introduces a visualization method that enables the navigator to estimate an encounter situation and choose collision avoidance manoeuvre, if necessary. It is based on the Collision Threat Parameters Area method and offers new features: fuzzy sectors of forbidden speed and course values, the possibility to use any given ship domain and a new formula of collision risk assessment. All these elements result in a method, which enables the navigator to differentiate between varying levels of risk and to point out the direct threats. The method is fast enough to be applied in the real-time decision support system.

Wreck of a ship is an incident that must be avoided. Ship accidents are generally caused by a several cases, such as human error, natural disaster, technical errors, missed communication, poor condition of the ship, and many more. Ship wreckage have huge impact for ship navigation, environment, economics, and others. Those impact have many disadvantages for the shipowners, and also for environment. For examples the fuel spills that pollute the environment, make disturbance to sailing ship because the track for those navigation is blocked by the ship wreck and their cargo especially on shallow location (<50 m). These research will discuss the effect the container when it is floats on the sea and its interference other ships. The main objective of this study is to present a risk assessment on the environmental impact of the wreck and container cargo. Wrecks on the seabed is likely to pose a risk to passing ships. container and its contents as well as the possibility of refloat, and also their environmental risks emanating from the wreck and container cargo, such as fuels, lubricants, and chemical cargo. Variations scenario is a collision between ships that pass by floating containers. The frequency of refloating container, and the consequences of the passing ship depends on several factors, which will be the subject of research. However, because of the frequency of refloating containers is unlikely, then the risk is low and does not pose a danger to navigation. These risk assessment using risk matrix 5x5 which is the combined value of the frequency and consequences of the incident. The results of this study indicate the level of risk, whether the risk is accepted, not accepted or received by considering the costs and benefits (ALARP). To consequence, there are two parameters which energy is absorbed and the penetration occurs. The absorbed energy is divided into two, namely the energy absorbed by ship and the energy absorbed by containers. In this study were taken 5 groups based on the size of the vessel. In this cases any 5 size group of vessels is based on the size of the ships that pass in the shipping lanes at the site of the sinking. Assumed these vessels have speed 10 knots at the location. As well as speed drifting containers having 0 to 3 knots.
