Working as a doctor, nurse, and midwife is associated with great responsibility for the health and life of patients. It is a source of many burdens, which result in marked, chronic stress leading to professional burnout and related consequences affecting all spheres of life. The research aimed to analyze healthcare workers’ preferred stress-coping strategies. The study encompassed 134 healthcare workers practicing the profession of doctor, nurse, and midwife, working in the cities of Lublin in the period from May 2022 to February 2023. We employed a self-made questionnaire and the Stress Coping Inventory (Mini-COPE). The PS IMAGO 9.0 program was used for the calculations and analysis of the results. The strategies of coping with stress most often used by doctors, nurses, and midwives were based on active coping and planning. The three surveyed groups of healthcare workers differ statistically significantly in using the denial strategy. Nurses are more likely than physicians to use denial as a coping strategy. There were no statistically significant differences in the remaining strategies of coping with stress. Healthcare workers in stressful situations often choose active ways of dealing with them, especially active coping and planning. An important factor that strengthens the choice of active strategies for coping with stress seems to be training aimed at improving the professional qualifications of healthcare workers.

The study analyses the number of midwives in the counties of Eastern and Western Galicia in census years 1869, 1880, 1890, 1900 and 1910, and the ratios per cadastral commune, 10,000 square kilometers, 10,000 civilians and 10,000 women in a county. The analysis was based on Austrian and Galician statistical reports. The results of the study confirmed that Eastern Galicia outnumbered Western Galicia in terms of midwives. However, it was in Western Galicia where the rate of growth was higher, and the effects of the 1910 collapse more moderate. This could have been due to an amendment to the 1897 midwifery regulations. The position of individual counties changed, depending on the year and the specific measure. In Eastern Galicia, Lisko county ranked the worst and Horodenka, Śniatyń, Tarnopol, Trembowla, and Brzozów counties ranked the best. In Western Galicia, Limanowa county ranked the worst, while Brzesko, Kraków, Łańcut, Wadowice, Przeworsk, and Podgórze counties ranked the best. There is a strong positive correlation between the rank and natural conditions (water, soil, climate), type of crops, agricultural development and processing, transport system (road and rail), population, and stimulating function of large urban centers. Favorable conditions were correlated with higher values in measures of the number of midwives.

Objectives The aim of the study was to assess the nutritional status and diet of midwives working on a shift schedule in public hospitals in Wrocław, Poland, and to analyze the variation in their diet according to their working hours (day shift, night shift) and on a non-working day. Material and Methods In the group of 50 midwives, employed in 4 public hospitals in Wrocław weight and body composition, waist and hip circumference, waist-hip-ratio and BMI were assessed. The nutritional habits and quality of the study participants’ diets were assessed by 3-days food dietary recall, including 1 day shift day, 1 night shift day, and 1 non-working day. Results More than half of the subjects were assessed as having excess body weight (BMI ≥25 kg/m2). Thirty percent of participants had BMI ≥25–<30 kg/m2 and 24% BMI ≥30 kg/m2, 70% had a body fat percentage >30%. Fifty-six percent of the diets had an energy value <90% of the subject’s total daily energy expenditure. Significantly higher energy value of diets on the night shift day compared to the morning shift day (1959.05±596.09 kcal vs. 1715.36±654.55 kcal, p = 0.01) were observed. The same relationship applied to cholesterol content (349.50±155.33 mg vs. 261.39±190.59 mg, p = 0.002). A high intake of phosphorus and sodium, exceeding the recommended dietary intake, was noticed. Conclusions The occurrence of a night shift in the shift work schedule is an element of that model that may have a significant impact on the nutritional and health value of shift workers’ diets. Therefore, it seems reasonable to implement nutritional education programs promoting healthy eating choices and habits during night work.

Objectives: The most frequent manner of attending childbirth imposes on midwives assuming poor body position affecting the musculoskeletal system. Long professional experience does not mitigate the negative effects. The adopted movement habit, as well as the type, number and frequency of actions influence the body posture. The aim of the study was to identify ergonomic threats of basic occupational midwives activities and how particular spinal segments arrangements while attending childbirth using the same technique in senior midwives differ from those of junior ones. It was also checked whether pain influences the working position assumed by midwives. Materials and Methods: Examinations were conducted in 95 midwives aged 21–50 (X = 29.25±9.34): 51 graduates of BSc midwifery who worked 680 h in delivery rooms during obligatory practical classes and apprenticeship and 44 senior midwives with professional experience of 7–27 years (X = 14.84±5.98). The study was threefold. The spinal alignment while performing work activities associated with attending childbirth was assessed using the OWAS system and the SonoSens Monitor, the center of gravity projection on basal plane – using the AccuGait AMTI stabilometric platform. The measurements were taken during a simulation of attending childbirth (on examination model). A survey was conducted aimed at identifying spinal pain. Results: Midwives’ working postures require unnatural body alignments. Postural instability in the working position and no maximal usage of basal plane were observed. The work overload may afflict the musculoskeletal system, which was confirmed by different pain discomforts in 67.3% of the examinees. Conclusions: Spinal alignment while attending childbirth is individually differentiated and in every case non-ergonomic. Identifying explicitly spinal overloads is difficult, but the most prevalent ones affect lumbar and cervical regions altogether. Spinal pain is frequently noted, both in junior and senior midwives, and is characteristic for midwives working in maximal movement ranges.

Background: The use of simulation-based training is strategic in training medical staff. Aim of the study: The aim of the study was to assess the knowledge and skills of performing neonatal CPR (cardiopulmonary resuscitation) complicated by the presence of meconium in the respiratory tract in two groups of midwives (trained in the traditional tutorial-based model vs simulation-based training) with an interactive neonatal manikin. Material and methods: The knowledge and practical skills assessment was conducted in two groups of midwives. The knowledge of the participants was assessed using both a written test consisting of 25 control questions (5 questions on meconium aspiration syndrome, 20 on neonatal CPR) and practical skills including 12 activities (1-initial, 9-proper, 2-final). The complex “success” indicator was determined based on the effects of patient resuscitation (15 minutes of the scenario), and the reference point for knowledge and skills was to obtain a minimum of 70% correct answers/activities performed, evaluated by three independent experts. Results: In the knowledge test, the respondents scored 17.93 ± 3.11 points out of 25 (71.7%). Practical skills for all respondents were 17.57 ± 2.49 points out of 21 (83.66%), with a tendency to obtain higher points in cleaning of the airways and stimulation of the newborn, and lower for ventilation and the use of an alternative method of intubation. Despite the similar level of practical skills in both groups, only a higher statistical tendency was observed in three out of nine instrumented “proper activities” in the simulation trained midwives. Conclusions: The scenario used by us assessing the knowledge and skills of midwives taught in traditional tutorial-based and simulation-based training, indicates the advantage of acquiring individual competences in a short time using simulation-based training. This increasingly popular scientific model allows the acquisition of “invasive” competencies, but it requires further research.

SummaryProfessional and disciplinary liability are an inherent part of obligatory membership of a professional association. Obligatory membership of a professional self-governing body means that there is a defined scope of members’ rights and duties, and hence a defined sphere of relations between the defendant and the given professional self-governing body under Polish administrative law. Only persons practising in a specific profession, that is belonging to a specific professional association, are subject to its professional liability. Nurses, midwives and pharmacists are still professionally liable even if what they have done or failed to do is not subject to penalisation under criminal law. However, they may be penalised under administrative law, that is provisions of the legislation on practice in the particular profession in question. Te criterion regarding rights clearly distinguishes between proceedings for professional liability and the criminal procedure, and at the same time indicates that the character of the former is based on administrative law.
Odpowiedzialność zawodowa i dyscyplinarna nieodzownie wiążą się z obowiązkową przynależnością do samorządu zawodowego. Z obowiązku przynależności do samorządu zawodowego należy wywieść także ograniczony zakres podmiotowy i przedmiotowy oraz tym samym zamkniętą sferę stosunków administracyjnoprawnych pomiędzy obwinionym, a organami samorządu zawodowego. Należy podkreślić w odniesieniu do ograniczenia podmiotowego, że odpowiedzialności zawodowej podlegają wyłącznie osoby wykonujące określony zawód, to znaczy przynależące do korporacji zawodowej. Zaś odwołując się do kryterium przedmiotowego, trzeba zauważyć, że pielęgniarki i położne oraz farmaceuci zostaną pociągnięci do odpowiedzialności zawodowej, nawet wówczas gdy prawo karne nie będzie penalizowało takich działań czy zaniechań. Będą one natomiast sankcjonowane przez prawo administracyjne, czyli przepisy ustaw związanych z wykonywanym zawodem. Kryterium przedmiotowe ewidentnie odróżnia postępowanie w przedmiocie odpowiedzialności zawodowej od postępowania karnego, wskazując równocześnie na jego administracyjnoprawny charakter.

BackgroundThe aim of this study was to describe, explain, and compare the correlations between workload, job satisfaction, and occupational stress levels in Polish midwives working before and during the COVID-19 pandemic.Material and MethodsThis study was cross-lagged in nature and conducted in 2 phases at public health facilities. The first phase took place between March and August 2018 before the current pandemic, and the second in February 2021. In total, 133 midwives working before the COVID-19 pandemic and 92 midwives working during the pandemic were surveyed.ResultsAnalysis revealed that working during the pandemic, personal SARS-CoV-2 infection and workload were positive predictors of occupational stress. Midwives working during the pandemic experienced significantly higher levels of occupational stress compared to the period before the pandemic. Job satisfaction was found to be a negative predictor of stress in both groups, and to mediate the correlation between workload and occupational stress. The positive influence of job satisfaction on coping with stress was stronger in the group working during the pandemic.ConclusionsWorkload increased, and job satisfaction decreased, occupational stress levels in both groups of midwives. The COVID-19 pandemic, directly and indirectly, increased midwives’ occupational stress levels, and reinforced the negative correlation between workload and job satisfaction. The results confirm the important role of working conditions in shaping occupational stress levels.

Ovarian cancer is the second most common cause of mortality due to female genital malignancies in Poland and thus a considerable problem in gynecologic oncology. In spite of introduction of novel therapies and new diagnostic modalities, treatment outcome is far from satisfactory. Therapeutic failures are associated first and foremost with detection of the disease at a late clinical stage in most patients. Treatment of ovarian cancer, characterized by a high recurrence rate, is prolonged and cumbersome for the female organism, due mainly to multiple side effects associated with administration of cytostatics. The purpose of this study was to evaluate the work of nurses and midwives, caring for patients undergoing chemotherapy at the Department of Gynecologic Oncology. In 2009 and 2010, 100 hospitalized patients filled-in a custom-designed questionnaire. The questionnaire included a general part, devoted to sociodemographic and clinical data, and a detailed part, consisting of 6 sections, assessing particular aspects of nursing care stemming from nurses’ and midwives’ professional functions. Results obtained have led us to conclude that patients’ level of satisfaction concerning the quality of nursing care is in fact very high. Not a single negative mark appeared in the study, evidencing good quality of services provided. Only a few aspects of care assessed by the patients require institution of reparative action. These include patients’ education and sense of intimacy during examination and nursing procedures. Comparative analysis included results of studies performed at Center of Oncology in Warsaw, at two oncologic departments in Lublin and at the Regional Center of Oncology in Kielce. Results obtained demonstrate that the highest level of satisfaction with quality of nurses’ work was obtained at the Department of Gynecologic Oncology in Poznań.
Rak jajnika stanowi drugą co do częstości przyczynę zgonów z powodu nowotworów narządów rodnych w Polsce i jest dużym problemem dla onkologii ginekologicznej. Mimo wprowadzania do terapii nowoczesnych leków oraz nowych badań diagnostycznych efekty są bardzo niezadowalające. Niepowodzenia w postępowaniu terapeutycznym wynikają przede wszystkim z rozpoznawania choroby w znacznym stopniu zaawansowania u większości pacjentek. Leczenie raka jajnika, cechującego się dużą skłonnością do nawrotów, jest długotrwałe i obciąża organizm kobiety z powodu wielu skutków ubocznych występujących podczas leczenia cytostatykami. Celem przeprowadzonych badań była ocena jakości pracy pielęgniarek i położnych świadczących opiekę chorym leczonym chemioterapią w Klinice Onkologii Ginekologicznej. Opracowane na potrzeby pracy ankiety wypełniło 100 pacjentek hospitalizowanych na oddziale w latach 2009 i 2010. Kwestionariusz podzielono na część ogólną, zawierającą dane socjodemograficzne i kliniczne, oraz szczegółową, składającą się z 6 części, w których wystawiane oceny dotyczyły różnych aspektów opieki wynikających z funkcji zawodowych pielęgniarek i położnych. Uzyskane wyniki pozwalają wnioskować, iż poziom satysfakcji pacjentek oceniających jakość pracy zespołu jest bardzo wysoki. W badaniu nie pojawiła się ani jedna ocena negatywna, co świadczy o dobrym poziomie świadczonych usług. Tylko nieliczne aspekty opieki opiniowane przez chore wymagają wprowadzenia działań naprawczych. Należą do nich edukacja pacjentów i poczucie intymności w trakcie badań i zabiegów pielęgniarskich. Do analizy porównawczej użyto wyników badań przeprowadzonych w warszawskim Centrum Onkologii – Instytucie, w dwóch oddziałach onkologicznych w Lublinie i Świętokrzyskim Centrum Onkologii. Uzyskane dane jednoznacznie wykazały, iż najwyższy poziom satysfakcji pacjentów z oceny jakości pracy pielęgniarek i położnych uzyskano w Klinice Onkologii Ginekologicznej w Poznaniu.
