Opis:
Conventional therapeutic approaches like chemotherapy, surgery and radiation for cancer and other malignancies have limited success rate. Several factors such as drug resistance, non-penetrability into solid tumors and unwanted toxicity due to non-specific action contribute to treatment failure. To overcome these issues, immunotoxins were developed having a target specific action. Immunotoxins are chimeric cytotoxic proteins which are formed by linking a toxin moiety to intact antibody or its fragment. Bacterial and plant based toxins are used in immunotoxins construction but they have many drawbacks in terms of immunogenicity and non-specific toxicity. Recently, a novel class of immunotoxins of human origin has been introduced. Immunotoxins fight back cancers by targeting the neoplastic cells and cause cells death by inhibiting protein synthesis. They have been categorized into two main classes, chemically conjugated immunotoxins and recombinant immunotoxins. This review summarizes recent advances in immunotoxins development and present a comprehensive illustration of sources and generations of immunotoxins. Moreover, it also highlights future directions to be explored. This review also focuses the limitations in immunotoxins development and its use.

Breast cancer is the most common malignancy among women. It is at present the most common cancer standing behind approx. 17.4% of all morbidity and approx. 22.2% of all deaths. Every year in Poland nearly 11,000 new cases of breast cancer are recorded and this number is constantly growing. The main reason for this state of affairs is seen in a small number of screening. It is therefore important to spread knowledge about breast cancer prevention, conducting training in currently available knowledge regarding risk factors as well as the promotion of healthy behaviours of women in the early detection of breast cancer. The simplest examination used to detect cancerous changes is breast self-examination. It helps to detect adverse effects in the breast and increases the chances of their recovery. In accordance with the recommendations of the Polish Gynecological Society regarding the prevention and early diagnosis of changes in the mammary gland, it is recommended that women over the age of 20 should regularly once a month carry out breast self-examination. Women menstruating should perform a test on the second or third day after menstruation, and pregnant women and post-menopausal women should conduct this always on the same day of the month. Breast self-examination should consist of a visual inspection and palpation. The aim of this study is to present the methodology of breast self-examination to detect cancer lesions at an early stage of their development and provide an overview of the results of Polish research on the knowledge of breast self-examination techniques, awareness of women surveyed regarding the age at which breast self-examination should be started, the frequency with which breast examination must be carried out, knowledge of risk factors for breast cancer or symptoms of breast cancer.

Introduction. The overweight and obesity epidemic represents a rapidly growing threat to the health of populations in an increasing number of countries. Nearly one-third of the world’s population has excess adipose tissue. Nowadays, obesity occurrence is so common that it is replacing more traditional problems, such as an undernutrition and infectious diseases, as the most significant causes of ill health. If the current trend continues, almost half of the world’s adult population will be overweight or obese by 2030. Objective. The aim of this study is to show the connection between recent trends in body mass index, and the globally changing cancer profile. State of knowledge. A range of clinical and epidemiological studies have shown the relationship between excess body fat and the most frequently occurring malignancies. Obesity is associated with many cancers, such as: breast, colorectal, liver, lung, kidney, oesophageal, pancreatic, endometrium, ovarian, prostate, thyroid, and gallbladder cancer. Conclusions. In the light of this information, the study supports the claimed statement that obesity is one of the major health problems of the 21st century. Considering the increase in the number of obese people worldwide, it is necessary to develop a strategy allowing to prevent it. Fighting against unhealthy lifestyle in order to reduce overweight and obesity in society may have an essential impact on decreasing the number of incidences of cancer.

Background: Ovarian cancer is one of the commonest gynecologic malignancy. It is the most common cause of death due to gynecologic tumors, and accounts for 50% of deaths due to all gynecologic cancer types. Objectives: The aim was to discuss and assess ovarian cancer in Misan province and to underline its impact for increased awareness and interest in screening and early diagnosis by the determination of the prevalence rates among Maysanian women. Methods: The study lasted six months. During this period, we obtained a lot of data from records of the Al-Shifaa Oncology Center in Misan province, Iraq. Between September 2016 and February 2017, 50 cases of ovarian cancer from 282 gynecologic cancers were recorded. Complete history was obtained for every case. Results: The study showed that ovarian cancer constituted 17.73% of all types of cancer. It usually occurred in patients aged 60–70 years (30%). Women lived in urban areas 1.5 times more frequently than in rural areas. The most common histopathological type of ovarian cancer was ovarian serous carcinoma (46%). The most common stages were stage III and IV, accounting for 76%. Conclusion: Ovarian cancer is the third most common gynecologic cancer type. It was more common in women aged 60–70 years. Regarding the family history, the results were insignificant. The most common histopathological type of ovarian cancer in this study was ovarian serous carcinoma. The most common stages of the disease were advanced stages III and IV.
Wstęp: Rak jajnika jest jednym z najczęstszych nowotworów złośliwych żeńskich narządów płciowych oraz główną przyczyną umieralności z powodu tych nowotworów – odpowiada za 50% zgonów wśród chorych z nowotworami kobiecego układu płciowego. Cele: Omówienie i ocena występowania raka jajnika w irackiej prowincji Majsan w celu zwiększenia świadomości i zainteresowania badaniami przesiewowymi i wczesnym rozpoznaniem choroby poprzez określenie częstości występowania omawianego nowotworu wśród kobiet zamieszkujących region Majsan. Metody: Badanie trwało sześć miesięcy. Uzyskano liczne dane z Centrum Onkologii Al-Shifaa w prowincji Majsan w Iraku. W okresie od września 2016 do lutego 2017 roku odnotowano 50 przypadków raka jajnika wśród ogółem 282 przypadków nowotworów narządów płciowych. Od każdej pacjentki zebrano pełny wywiad. Wyniki: Badanie wykazało, że rak jajnika stanowił 17,73% nowotworów. Rozpoznawano go zwykle u kobiet w wieku 60–70 lat (30%). Pacjentki częściej (o 1,5 razy) zamieszkiwały rejony wiejskie. Najczęstszym typem histopatologicznym był surowiczy rak jajnika (46%). Chorobę zwykle rozpoznawano w stadium III i IV, co stanowiło 76% przypadków. Wnioski: Rak jajnika to trzeci co do częstości nowotwór żeńskich narządów płciowych. Występował częściej u kobiet w wieku 60–70 lat. Wyniki dotyczące wywiadu rodzinnego nie były istotne. Najczęstszy typ histopatologiczny stanowił surowiczy rak jajnika. Chorobę rozpoznawano przeważnie w zaawansowanym stadium – III i IV.

It has been reported that concentrations of neopterin in the urine are changed according to the host immunological conditions. In the present study, we measured urinary concentration of neopterin in patients with malignant hematological disorders and investigated the relationship between urinary neopterin levels and laboratory indices for cellular immunity. Urine neopterin levels were correlated with serum sIL-2R levels in the patients with malignant lymphoma, and inversely correlated with lymphocyte reactivity with ConA in the patients with acute myelocytic leukemia. However, no significant correlation was observed between urine neopterin levels and lymphocyte reactivity with phytohemagglutinin and pokeweed mitogen, CD4/8 ratio, CD56+16+ subset or serum IFN-β levels. In the patients with malignant lymphoma, parallel changes in serum sIL-2R and urine neopterin were observed. The presented results suggest that urine neopterin levels are related to the activation of T cells in malignant lymphoma.

The umbilical area can present with a variety of signs associated with an intra abdominal malignancy. An umbilical/paraumbilical hernia might itself be a sign of an internal malignancy. The correlation between the presence of an umbilical/paraumbilical hernia and an intra abdominal malignancy has been previously based only on case reports. The aim of the study was to evaluate the significance of an umbilical/paraumbilical hernia as a symptom of an intraabdominal malignancy. Material and methods. A retrospective analysis was performed; review of the medical records of 145 patients (113 female and 32 male; mean age 66.4±11.9) with an umbilical/paraumbilical hernia treated during the period of 2005-2013. Twenty-three patients (15.9%) were diagnosed with an intra abdominal malignancy; 34% were in the age group over 75 years of age. Results. The most common malignancies were: colorectal cancer, followed by pancreatic cancer, and cancers of the adnexa and kidneys. The patients with a concomitant malignancy identified were significantly older than those without a malignancy. In 65% of patients, the diagnosis was made postoperatively. Logistic regression analysis demonstrated that age, the presence of preoperative symptoms, anemia, and weight loss were independent risk factors for concomitant abdominal cancer. Conclusion. The findings of this study support intensive preoperative diagnostic evaluation of elderly patients that are qualified for surgery for an umbilical/paraumbilical hernia. This is particularly important because most of these patients had a small/medium hernia orifice, which did not allow for accurate manual abdominal exploration. Currently, the routine preoperative diagnostic evaluation is often insufficient for an accurate diagnosis

Malignancy or its treatment affect kidney in several ways. The most common are acute kidney injury and chronic kidney disease. Other form of kidney diseases can also be present such as nephrotic syndrome, tubulointerstitial nephritis, thrombotic microangipathy etc. In addition, electrolyte abnormalities such as hypercalcemia, hyponatremia and hypernatremia, hypokalemia and hyperkalemia, and hypomagnesemia. are observed. Treatment of malignancy associated kidney disease is usually symptomatic. Cessation of the offending agent or other supportive measures if needed i.e. renal replacement therapy are also implemented.

The case of complete response of squamous cell oesophageal cancer to radiochemotherapy in patient previously operated due to hereditary retinoblastoma is presented. Second cancers in retinoblastoma survivors are the main cause of death in that group of patients in developed countries. There is little data on the outcome of treatment in those patients but the few papers available suggest rather poor prognosis. However, it is not clear whether the biology of cancer determined by RB gene mutation is responsible for that. The data suggesting decreased radio- and chemoresistance in cancers with decreased RB gene expression is discussed in the present paper. The role of that gene in the response to cancer treatment was showed in lung, breast and bladder cancer. Moreover, laboratory studies seem to confirm clinical observations and show that RB gene inhibition leads to increased cytotoxic effect of cisplatin, etoposide and 5-fluorouracil. It seems that without the incorporation of gene expression profiling into clinical practice further improvement in cancer treatment will be difficult to achieve.

Bazując na wynikach analiz metod statystyki wielowymiarowej przeprowadzono klasyfikację grupy badanych pacjentów ze względu na grupę badanych cech. Celem analizy jest próba wyodrębnienia charakterystycznych grup czynników wśród pacjentów chorujących na raka jelita grubego w różnym stopniu zaawansowania klinicznego. Analizie poddano wybrane dane epidemiologiczne pochodzące z dokumentacji medycznej chorych z ustalonym rozpoznaniem - rak jelita grubego. Do analizy wykorzystano zmienne jakościowe: płeć, stopień zaawansowania klinicznego choroby, typ i złośliwość histopatologiczną, podział na osoby z wagą prawidłową, nadwagą i otyłością, podział ze względu na stężenie glukozy na czczo w surowicy krwi oraz współistnienie występowania innych chorób.
In this paper a classification o f examined patients was carried out based on results o f multivariate analysis using classification trees. The aim o f the analysis was to identify characteristic factors describing groups o f patients suffering from colorectal cancer with different stage o f disease. Clinical data from medical documentation o f the patients with colon cancer were analyzed. Qualitative variables such as sex, clinical stage, histopathology type o f cancer and malignancy, weight class, glucose level class and coexistence with other illnesses were used in the analysis.

Venous thromboembolism is a multifactorial disease and a frequent complication in patients affected with malignant tumors. The risk of venous thrombosis in oncologic patients increases seven-fold and is particularly high during the first few months after diagnosis. Molecular mechanisms of tumor – thrombosis correlation are relatively well known. Main factors responsible for hypercoagulation state are tissue factor and tumor procoagulant. Incidence of venous thromboembolic syndrome differs depending on location of the tumor, being the most frequent in gastrointestinal tumors of the mucous variety, lung cancer, ovarian cancer, endometrial cancer, breast cancer and leukemias. Among patients with gynecologic malignancy, thromboembolic complications usually coexist with ovarian cancer and endometrial cancer, as these are frequently of the adenomatous tumors of the mucous variety. Patients affected with gynecologic malignancy are at high-risk of thromboembolism not only because of the disease itself, but also due to the fact that pelvic surgery is the type of surgery second only to orthopedic procedures in terms of risk of thromboembolic complications. Furthermore, there are several common risk factors for gynecologic malignancies and venous thromboembolism. Thromboembolic complications significantly contribute to worsen the prognosis of oncologic patients. Standards of management should be developed concerning antithrombotic prevention in the group of patients with gynecologic malignancies at a particularly high risk of venous thromboembolism, as to date there is no consensus as to the optimal way of preventing these complications.
Żylna choroba zakrzepowo-zatorowa (ŻChZZ) jest schorzeniem wieloczynnikowym, a zarazem częstym powikłaniem stwierdzanym u pacjentów z nowotworem złośliwym. Ryzyko zakrzepicy żylnej u pacjentów chorujących na nowotwory wzrasta siedmiokrotnie i jest ono szczególnie wysokie w pierwszych miesiącach po postawieniu diagnozy. Molekularna zależność pomiędzy nowotworami złośliwymi a zakrzepicą została dobrze poznana. Głównym czynnikiem odpowiedzialnym za mechanizmy prokoagulacyjne jest czynnik tkankowy i prokoagulant nowotworowy. Zapadalność na ŻChZZ różni się w zależności od umiejscowienia procesu nowotworowego, a powikłanie to towarzyszy najczęściej guzom układu pokarmowego typu śluzowego, rakowi płuc, jajnika, błony śluzowej trzonu macicy, piersi oraz białaczkom. Wśród pacjentek z rakiem narządu płciowego najczęściej powikłania zakrzepowo-zatorowe towarzyszą rakowi jajnika i rakowi błony śluzowej trzonu macicy, jako że są to często guzy gruczołowe typu śluzowego. Pacjentki cierpiące na raka narządu płciowego znajdują się w grupie podwyższonego ryzyka wystąpienia ŻChZZ nie tylko ze względu na samą chorobą, ale także z uwagi na fakt, że operacje przeprowadzane w miednicy są drugą po operacjach ortopedycznych grupą zabiegów obarczonych największym ryzykiem wystąpienia powikłań zakrzepowych. Ponadto istnieje wiele wspólnych czynników ryzyka dla nowotworów narządu płciowego i ŻChZZ. Problem powikłań zakrzepowo-zatorowych jest istotny także dlatego, że wpływają one w istotny sposób na pogorszenie rokowania u pacjentów z nowotworami. Istnieje więc potrzeba opracowania standardów postępowania w odniesieniu do profilaktyki przeciwzakrzepowej w grupie pacjentek chorujących na nowotwory złośliwe narządu płciowego, jest to bowiem grupa chorych obciążona szczególnie dużym ryzykiem wystąpienia ŻChZZ, a do tej pory nie ma zgodności co do idealnej terapii zapobiegającej tym powikłaniom.

According to the World Health Organization (WHO), breast cancer (BC) is one of the most deadly cancers diagnosed among middle-aged women. Precise diagnosis and prognosis are crucial to reduce the high death rate. In this paper we present a framework for automatic malignancy grading of fine needle aspiration biopsy tissue. The malignancy grade is one of the most important factors taken into consideration during the prediction of cancer behavior after the treatment. Our framework is based on a classification using Support Vector Machines (SVM). The SVMs presented here are able to assign a malignancy grade based on preextracted features with the accuracy up to 94.24%. We also show that SVMs performed best out of four tested classifiers.

Several years ago, the presence of macrophages in the tumor microenvironment was thought to be an inflammatory response to kill the cancer cells. Now, this is clear that the inflammatory cells that exit blood vessels and migrate to the tumor tissue play an important role in cancer progression. Various cells present in the tumor microenvironment enhance cancer growth and invasiveness by secretion of tumor-enhancing products. That is why tumors should not be treated as only aggregates of cancer cells but as separate structures. Macrophages form a major component of the inflammatory infiltration in tumors, where they are termed tumor-associated macrophages (TAMs). To the best of our knowledge, up-to-date there were no studies on tumor associated macrophages and the role of the tumor microenvironment in tumor invasion/metastasis in dogs. This is the first study performed to asses if the number of TAMs and expression of MCSF-R (macrophages colony stimulating factor receptor) and CD14 (LPS co-receptor) are associated with the grade of tumor malignancy and its ability to metastasize. We have performed immunohistochemical analysis of 50 canine mammary adenocarcinomas of various grade of malignancy (1st, 2nd, 3rd) and tumors that gave local or distant metastases. The results indicate that in dogs, similarly to humans and mice, the number of tumor associated macrophages is related to the cancer ability to metastasize. Our results also indicate that the expression of MCSF-R and, what is particularly new finding, CD14 is associated with tumor malignancy and its ability to metastasize. Hence, these molecules play a role in tumor progression, metastasis and microenvironment interactions. These results show that in dogs we should treat the tumor as a whole organ rather than just try to eliminate the cancer cells.
