Opis:
Objective. Evaluation of the neutrophil-lymphocyte ratio and mean platelet volume as predictive factors for liver fibrosis and steatosis in HBV patients qualified for antiviral treatment. Materials and method. The study comprised 38 CHB patients who had commenced antiviral treatment, and 20 healthy volunteers who constituted the clinical control group. All patients had their blood count taken and underwent hepatic assessment using transient elastography with CAP (controlled attenuation parameter). Results. It was found that the mean hepatic fibrosis was 8.7 kPa (±8.8) and the mean liver steatosis – 286 db/m (±64). Mean NLR – 2.78(±1.1), whereas in the control group the mean NLR value was 1.64(±0.98). A negative linear correlation (r= -0.34; p=0.035) was found between liver fibrosis and the NLR value in the study group. No correlation was observed between hepatic steatosis and the NLR. Mean MPV – 12.6fl (±3.1), which was considerably higher in the CBH patients than in the control group. A positive correlation (r= 0.79, p= 0.001) was found between MPV and disease severity evaluated with transient elastography. Conclusions. The NLR and the MPV were significantly higher in the CHB patients than in the healthy volunteers. Both the NLR and the MPV can be treated as predictive factors for liver fibrosis in this group of patients.

Background and Aim: Fibrosis, which develops during the progression of liver damage, is the previous stage of cirrhosis. Carbon tetrachloride is one of the most commonly used hepatic toxins in experimental animal models of liver fibrosis. It was investigated the hepatoprotective effects of Tribulus terrestris, Ashwagandha and N-acetylcysteine in an experimental model of liver fibrosis induced by carbon tetrachloride in this study. Methods: Fifty Wistar rats were divided into five groups of 10 each, as follows: 1) control, 2) carbon tetrachloride, 3) carbon tetrachloride plus N-acetylcysteine, 4) carbon tetrachloride plus T. terrestris, and 5) carbon tetrachloride plus Ashwagandha group. At the end of 6 weeks, the rats were sacrificed, and serum and tissue samples were collected. Aspartate aminotransferase, alanine aminotransferase, gamma-glutamyl transpeptidase, malondialdehyde, NF-κB, collagen 1, nuclear factor erythroid-2-related factor 2, tumor necrosis factor-α were analyzed, and histopathological evaluation was performed. Results: There were no significant differences in serum aspartate aminotransferase and alanine aminotransferase levels of the N-acetylcysteine-treated group versus those in the T. terrestris- and Ashwagandha-treated groups (p>0.05). Liver malondialdehyde levels were lower in the N-acetylcysteine-, T. terrestris- and Ashwagandha-treated groups than in the carbon tetrachloride-administered group (p<0.001). There were differences between groups in NF-κB, collagen 1, nuclear factor erythroid-2-related factor 2 and tumor necrosis factor-α levels (p<0.05). Conclusions: In conclusion, T. terrestris, Ashwagandha and N-acetylcysteine had protective effects on the liver in this experimental fibrosis model. T.Terrestris was a little more effective than Ashwagandha in combating liver fibrosis.

Opis:
Contemporary medicine should provide high quality diagnostic services while at the same time remaining as comfortable as possible for a patient. Therefore novel non-invasive disease recognition methods are becoming one of the key issues in the health services domain. Analysis of data from such examinations opens an interdisciplinary bridge between the medical research and artificial intelligence. The paper presents application of machine learning techniques to biomedical data coming from indirect examination method of the liver fibrosis stage. Presented approach is based on a common set of non-invasive blood test results. The performance of four different compound machine learning algorithms, namely Bagging, Boosting, Random Forest and Random Subspaces, is examined and grid search method is used to find the best setting of their parameters. Extensive experimental investigations, carried out on a dataset collected by authors, show that automatic methods achieve a satisfactory level of the fibrosis level recognition and may be used as a real-time medical decision support system for this task.

Introduction. Non-alcoholic fatty liver disease (NAFLD) is excessive fat build-up in the liver due to causes other than alcohol use. Aim. To evaluate the clinical efficacy and safety of Liv.52 DS tablets in the management of NAFLD. Material and methods. Prospective, interventional clinical study conducted on 60 patients of both sex, aged between 18-65 years, confirmed with NAFLD from clinical examination, laboratory test, ultrasound findings and those willing to give informed consent. All patients received Liv.52 DS at a dose of 2 tablets twice daily for 2 months. All patients were evaluated at baseline, end of 1st month, and end of 2nd month for liver function tests, hepatomegaly by ultrasound, NAFLD Fibrosis Score, lipid profile, hematology and biochemical investigations. Results. Study data was analyzed with GraphPad Prism Software Version 6.07. Data of those patients who completed the study was considered for analysis. Significant improvement in hepatomegaly, liver enzymes was observed. NAFLD fibrosis score revealed no progression of liver fibrosis due to NAFLD during the study period. No abnormal lab values were recorded and there were no adverse events reported during the study. Conclusion. Study concludes that Liv.52 DS is safe and beneficial in individuals suffering from NAFLD.

Opis:
Currently, liver disease is widespread and the awareness of these diseases is low. Early symptoms of liver disease do not necessarily indicate problems with this organ and patients are usually informed of their problems when the stage of the disease is already advanced. Invasive biopsies are the clinical diagnostic method most commonly used in the evaluation of liver disease. A biopsy is associated with a high risk of false results and additional complications. Finding new non-invasive imaging methods has led to the discovery of a new method called Magnetic Resonance Elastography (MRE). This technique allows one to evaluate the mechanical properties of tissues and to distinguish between pathological states. Testing using this technique can be performed on a conventional magnetic resonance system by using few additional components and properly prepared software. Studies have shown that there is a strong correlation between MRE-measured liver stiffness and the degree of fibrosis. MRE is also useful in characterizing liver tumors. Studies show that this technique is highly credible in both health volunteers and patients with liver fibrosis. MRE has tremendous diagnostic potential. The described technique is not currently widely used and has the 127-132potential to serve as a safe and accurate alternative in clinical diagnostics in the future.

Mutation of the SERPINA1 gene is present in about 2% of patients with cystic fibrosis but is more common and accounts for about 5% in patients with cystic fibrosis and co-existing liver lesions. The SERPINA1 gene is responsible for the synthesis of a serine protease inhibitor. The protein related with this gene is accumulated within the endoplasmic reticulum of hepatocytes causing their damage, inflammation and cirrhosis. The aim was to assess the presumable effect of the SERPINA1 mutation gene in patients with diagnosed cystic fibrosis on damage to the liver and/or cholestasis. Material and method: The analysis included 30 children, 13 girls (43.3%) and 17 boys (56.6%), aged from 6 months to 18 years (the average age was 5.5 years) with diagnosed cystic fibrosis. All the patients have undergone a genetic test of the mutation of the SERPINA1 gene. The analysis included age, sex, clinical symptoms, type of mutation of the CFTR protein, abnormalities in laboratory tests (the activity of aminotransferases, GGTP, alkaline phosphatase , protein, the indicator of acid steatocrit, the rate of APRI) and abdominal ultrasonography. Results: Symptoms of damaged liver were concluded in 9/30 patients (30%) with diagnosed cystic fibrosis. Most commonly observed were increased activities of aminotransferases in 9/30 patients (30%) and of gamma glutamyl transferase in 6/30 (20%) of the assessed patients. In 4/30 patients the abdominal ultrasonography revealed an enlarged liver and increased echogenicity. Mutation within the SERPINA1 gene was observed only in 1/30 patients (3.3%) with diagnosed cystic fibrosis. As far as the patient is concerned, currently the activities of aminotransferases, GGTP and AF are normal, but there has been a considerable increase in the intensity of symptoms from the respiratory system. No corelation between the mutation of the SERPINA1 gene and clinical symptoms, type of mutation of the CFTR protein, laboratory results of the functions and damage to the liver and the abdominal ultrasonography was observed. Conclusions: We did not find a more frequent occurrence of the SERPINA1 gene mutation in children with cystic fibrosis and coexisting features of damaged liver and cholestasis. The obtained results suggest the contribution of other than SERPINA1 gene mutation factors responsible for the development of changes in the liver in patients diagnosed with cystic fibrosis. The studies on the subject should be extended and performed on a larger group of patients.
Mutacja genu SERPINA1 występuje u około 2% pacjentów z mukowiscydozą , natomiast u około 5% pacjentów z mukowiscydozą i współistniejącymi zmianami wątrobowymi. Gen SERPINA1 odpowiedzialny jest za syntezę inhibitora proteazy serynowej, a białko związane z tym genem jest gromadzone w obrębie retikulum endoplazmatycznego hepatocytów prowadząc do ich uszkodzenia, zapalenia i marskości. Celem pracy było poszukiwanie wpływu mutacji genu SERPINA1 na wystąpienie uszkodzenia wątroby i/lub cholestazy u pacjentów z rozpoznaną mukowiscydozą. Materiał i metoda: Analizą objęto 30 dzieci, 13 dziewczynek (43,3%) oraz 17 chłopców (56,6%), w wieku od 6 miesięcy do 18 lat (średni wiek 5,5 lat) z rozpoznaną mukowiscydozą. U wszystkich pacjentów wykonano badanie genetyczne w kierunku mutacji genu SERPINA1. W analizie uwzględniono wiek, płeć, objawy kliniczne, rodzaj mutacji genu białka CFTR, nieprawidłowości w wynikach badania laboratoryjnych (aktywności aminotransferaz, GGTP, FA, białka, wskaźnik kwaśnego steatokrytu, współczynnik APRI) oraz badanie usg jamy brzusznej. Wyniki: Cechy uszkodzenia wątroby stwierdzono u 9/30 pacjentów (30%) z rozpoznaną mukowiscydozą. Najczęściej obserwowano podwyższone aktywności aminotransferaz – u 9/30 (30%), natomiast gammaglutamylotransferazy u 6/30 (20%) badanych pacjentów. U 4/30 pacjentów stwierdzono powiększoną wątrobę o podwyższonej echogeniczności w badaniu usg. Mutację w obrębie genu SERPINA1 wykazano jedynie u 1/30 (3,3%) pacjenta z rozpoznaną mukowiscydozą. U pacjentki tej aktualnie stwierdza się prawidłowe aktywności aminotransferaz, GGTP, FA, natomiast obserwuje się znaczne nasilenie objawów ze strony układu oddechowego. Nie wykazano korelacji pomiędzy występowaniem mutacji genu SERPINA1 a objawami klinicznymi, rodzajem mutacji białka CFTR, wynikami badań laboratoryjnych funkcji i uszkodzenia komórki wątrobowej oraz badaniem ultrasonograficznym jamy brzusznej. Wnioski: Nie wykazano częstszego występowania mutacji genu SERPINA1 u dzieci z rozpoznaną mukowiscydozą i współistniejącymi cechami uszkodzenia wątroby i cholestazy. Uzyskane dane sugerują udział innych, niż mutacje genu SERPINA1 czynników odpowiedzialnych za rozwój zmian obrębie wątroby u pacjentów z rozpoznaną mukowiscydozą. Wskazane byłoby przeprowadzenie badań na większej grupie pacjentów.
