Opis:
Retinal diseases account for the vast majority of ophthalmologic disorders. Over the past years, laser-based approach has been successfully used. Despite introduction of other potentially innovative, beneficial and successful therapeutic methods, lasers are still considered the gold standard. In this review, we discuss the spectrum of currently prevailing laser methods and new insights into novel perspectives and techniques regarding laser management of retinal disorders. This paper is divided into three sections. Two parts, already published in previous issues, have presented methodology with the spectrum of laser-based techniques and literature review investigating the existing knowledge in laser management of diabetic retinopathy, diabetic macular edema and vascular retinal disorders. Finally, the current one, consists of the principles guiding laser treatment of retinal tears, retinal degeneration, retinopathy of prematurity, age-related macular degeneration and other retinal diseases.
Choroby siatkówki stanowią dużą grupę schorzeń narządu wzroku. W leczeniu wielu z nich od lat z powodzeniem jest stosowana laseroterapia. Mimo prób wprowadzania innych metod leczniczych nadal pozostaje ona jedną z głównych opcji terapeutycznych. W pracy przedstawiono przegląd piśmiennictwa dotyczący zastosowania laseroterapii w chorobach siatkówki, z uwzględnieniem nowych rodzajów i metod tej formy leczenia. Pracę podzielono na trzy części. Dwie pierwsze, które ukazały się w poprzednich numerach, zawierały opis metodyki oraz rodzajów zabiegów laserowych i przegląd piśmiennictwa dotyczący laseroterapii w retinopatii cukrzycowej, cukrzycowym obrzęku plamki oraz chorobach naczyniowych siatkówki. W niniejszej części zaprezentowany został przegląd piśmiennictwa dotyczący laseroterapii w przedarciach oraz chorobach zwyrodnieniowych siatkówki, retinopatii wcześniaków, zwyrodnieniu plamki związanym z wiekiem oraz innych chorobach siatkówki.

Introduction. Authors of numerous publications have proved the therapeutic effect of laser irradiation on biological material, but the mechanisms at cellular and subcellular level are not yet well understood. Objective. The aim of this study was to assess the effect of laser radiation emitted by the MLS M1 system (Multiwave Locked System) at two wavelengths (808 nm continuous and 905 nm pulsed) on the stability and fluidity of liposomes with a lipid composition similar to that of human erythrocyte membrane or made of phosphatidylocholine. Materials and method. Liposomes were exposed to low-energy laser radiation at surface densities 195 mW/cm2 (frequency 1,000 Hz) and 230 mW/cm2 (frequency 2,000 Hz). Different doses of radiation energy in the range 0–15 J were applied. The surface energy density was within the range 0.46 – 4.9 J/cm2. Results. The fluidity and stability of liposomes subjected to such irradiation changed depending on the parameters of radiation used. Conclusions. Since MLS M1 laser radiation, depending on the parameters used, affects fluidity and stability of liposomes with the lipid content similar to erythrocyte membrane, it may also cause structural and functional changes in cell membranes.

Diabetes mellitus (DM) is a group of metabolic diseases which typically presents with frequent urination, increased thirst and increased hunger. DM be classified into three main types: type I (insulin-dependent DM), type-II (non-insulin dependent DM) and type III (gestational DM). Diabetes is a group of diseases of global health significance as 382 million people worldwide had diabetes in the year 2013 and this was projected to increase to an estimated 415 million in 2015. Damage to the nerves of the body (diabetic neuropathy) is the most common complication of diabetes. The signs and symptoms of diabetic neuropathy include numbness, diminished sensation, pain etc. Various types of electrotherapy, such as transcutaneous electrical nerve stimulation (TENS), pulsed-dose electrical stimulation, frequency-modulated electromagnetic neural stimulation, have been reported effective in managing diabetic neuropathy. This study is a systematic review of the evidence to enable the determination of the effectiveness of electrical stimulation and low-intensity laser therapy (LILT), and also aid their recommendation if proven to be effective. The outcome of this study was that TENS and other forms of electrical stimulation reviewed in this study may be effective and safe non-pharmacological treatment modalities in relieving the symptoms associated with diabetic neuropathy. The effectiveness of LILT couldn’t be determined due to the different parameters used to evaluate patients’ outcome and limited number of studies. Authors recommend that further randomized controlled trials with similar methodological parameters and studies with higher quality of evidences are needed to establish the true effectiveness of these modalities in diabetic neuropathy.

The aim of this study is to assess the influence of laser irradiation and incoherent infrared radiation on sensorimotor excitability and muscle pain after a workout. The research was carried out on 40 healthy volunteers who were randomly divided into four groups. A single phototherapy treatment, preceded by the triceps calf muscle workout, was applied in all the groups. Group 1: low-level laser therapy (LLLT), group 2: high-intensity laser therapy(HILT), group 3: infrared radiation (IR), group 4: placebo was applied. The electrodiagnostic examination was performed on the first day, before a workout and irradiation, as well as 48 hours after the workout. Moreover, pain intensity assessment (VAS) was performed 24 and 48 hours after workouts. A statistically significant increase in the average threshold limit value of accommodation after a workout was observed in the IR group, in contrast to the HILT group, in which a decrease in the average threshold limit value of accommodation was observed. In the IR and LLLT groups, the value of rheobase increased. A statistically significant decrease in pain was observed in the LLLT and IR groups. The post-exercise irradiation with LLLT and incoherent IR influenced a decrease in sensorimotor excitability and decrease in pain of the muscle after a workout.

Purpose: The aim of the study was the assessment of the influence of MLS laser therapy on morphological changes in nailfold videocapillaroscopy (NVC), clinical features, and the serum NO level in patients with primary and secondary Raynaud’s phenomenon (RP). Materials and methods: The analysis was performed on a group of 78 patients with RP and 30 healthy volunteers, who underwent NVC examination. NO concentration was assayed using the Griess method in blood serum before and after 3-weeks of laser biostimulation. MLS wasperformed with a Laser-M6 ASA Company device,for 3 weeks with weekend breaks, using thefollowing parameters: a frequency of 1500 Hz, adose of 25 J/cm2, and a time of 2.5 minutes on onehand. Results: After 3 weeks of MLS laser therapy, thebeneficial clinical effects manifested by a decrease of duration and number of RP attacks and degree of pain score on the visual analogue scale (VAS) in patients with primary and secondary RP. Clinical improvement after MLS laser therapy was reflected in the assessment of microcirculation disorders in NVC examination. Moreover, the tendency of normalization of NO concentration in the serum of patients with primary and secondary RP may suggest a favorable effect of laser biostimulation on the regulation of processes taking part in microcirculation disorders. Conclusions: The results showed that NVC is a useful diagnostic tool in the evaluation of dynamic microvascular involvement in RP patients. MLS laser therapy has a beneficial effect in patients with primary and secondary Raynaud’s phenomenon

Introduction and objective. Since the characteristic of chronic non-specific lumbar dysfunction presents a high prevalence and morbidity, and that there are still conflicting results with the use of photobiomodulation, the aim of this study was to analyze the effect of photobiomodulation application in patients with chronic non-specific lombalgy. Materials and method. A quantitative, experimental, randomized study composed of 21 volunteers randomly separated into two groups: 1) the intervention group (IG), who were given an application of photobiomodulation (LED (617nm ±10%, 1,500 mW) and low-level laser therapy (830nm, 150 mW, beam area 12.57mm), combined energy of 8.4 J per area for one minute in four different locations: in regions of greatest pain referred to palpation, on a bilateral basis, area of the applicator – 23,8 cm2). 2) the control group (CG), in which the device remained switched-off during therapy. All subjects were evaluated by McGill and Roland Morris questionnaires before and at the end of 6 interventions (3 weeks). Results. For both McGill and Roland Morris total pain rates, there was no inter-group interaction or interaction between evaluation and group (p>0.05). In the comparison between evaluations (before and after), there was a significant difference (p<0.001). Conclusions. The use of photobiomodulation in these parameters in young patients with chronic non-specific lombalgy, was not more effective than the placebo for the relief of painful symptoms. Nor did it promote a decrease in the repercussion of lombalgy in the performance of daily activities.

Nevoid basal cell carcinoma syndrome (NBCCS) is a rare genetic disease that is manifested in a number of disorders concerning the skin, skeleton, cardiovascular and nervous systems. Various defects which can be observed at first contact with a patient visiting a dermatologist or dentist may help to diagnose this syndrome. Frequent problems with odontogenic cysts and metastatic basal-cell carcinomas result in patients being under the constant care of a specialist. This short study presents the case of 31-year-old patient with Gorlin-Goltz syndrome treated with combination therapy using CO2 laser and photodynamic therapy.

Introduction. Periodontitis is a disease of multifactorial etiology and complex clinical manifestation. Removal of subgingival bacterial biofilm is a fundamental procedure in the treatment of periodontitis. Scaling and root planing (SRP) is currently the gold standard in the treatment of periodontitis, but in some cases such a procedure does not provide satisfactory results. The use of lasers in the treatment of periodontitis is one of the alternative adjunctive methods that has recently become more popular. Objective. The aim of the study was to evaluate the effectiveness of diode lasers in the treatment of periodontitis, based on the literature found in the Medline, Pubmed and Scopus databases published from June 2013 – June 2020. Results were based on a comparison of the effectiveness of SRP treatment in combination with photodynamic therapy or SRP in combination with laser irradiation of periodontal pockets. Results. The majority of studies confirmed the effectiveness of periodontal treatment combining SRP with photodynamic therapy or SRP with laser irradiation, compared to an SRP alone. Conclusions. Based on the recent literature, it can be concluded that SRP combined with photodynamic therapy or SRP in combination with a diode laser irradiation are effective methods to improve the outcomes of non-surgical periodontal treatment.
