Introduction and objective. Matrix metalloproteinases (MMPs) are proteolytic enzymes responsible for the decomposition of extracellular matrix elements. They play an important role during embryogenesis, wound healing, endometrial epithelial exfoliation, the formation of new blood vessels, and also during cancer development. Throat and larynx tumours are included in a large group of head and neck cancers. These tumours are characterized by a poor prognosis. Despite advances in medical science, cancer treatment is difficult and often ineffective. The aim of the study was to evaluate the level of MMP-3 and MMP-9 in the serum and saliva of patients with pharynx and larynx tumours. Materials and method. Samples of saliva and serum were collected from 60 patients with larynx or throat cancer. Twenty patients without cancer comprised the control group. MMPs in saliva and serum were determined by the ELISA method. Results. In the study group, concentrations of MMP-3 in saliva were from 0.2 – 77.6 ng/ml. Patients with malignant tumours had higher saliva MMP-3 levels than healthy subjects. The concentration of MMP-3 in the serum of the study group ranged from 10.9 – 200.00 ng/ml, which was also higher than in the control group. There were no statistically significant difference in the MMP-9 level between the study and control groups (both in serum and saliva). Conclusions. This study is another element that shows the phenomena taking place at the cellular level during oncological disease. In serum and saliva samples, higher values of MMP3 were found in patients with cancer. The increase in the concentration of this enzyme in the risk group may be used for early detection of tumour transformation and evaluation of treatment.

The aim of the treatment of early laryngeal cancer is complete oncological cure and simultaneously voice and swallowing preservation. According to the European Laryngological Society (ELS) classification of CO2 laser cordectomy, full voice recovery is seen in subepithelial cordectomy (ELS Type I ) and near complete in subligamental cordectomy (ELS type II). Voice deterioration is usually seen after more extensive levels of cordectomy (ELS types III-V). Voice quality after microsurgical laser cordectomy depends on the presence or absence of synechiae in the anterior commissure and on the quantity of the removed thyro-arytenoid muscle. More extensive reduction of the vocal muscle quantity causes more intensive glottic incompetence. Contralateral healthy vocal fold, rudimentary, cicatrixial previously operated vocal fold and false ventricular folds may take part in postoperative supraglottic voice compensation. All patients should undergo speech and voice therapy after terminating the scaring process on the operated vocal fold. Patients routinely undergo a minimum of 6 months of voice rehabilitation which allows speech therapy to yield the best possible voice. Phonosurgical techniques i.e. medialization thyroplasty, augmentation techniques, Zeitels’s laryngoplasty or Lichtenberger’s technique in treatment of synechia in the anterior commissure are successfully performed to restore the vocal competence. Voice preservation after treatment of early laryngeal cancer of the vocal fold improves life quality of the patient.

Supracricoid partial laryngectomy (SCPL) with crico-epiglotto-hyoidopexy (CHEP) or crico- hyoidopexy (CHP) is technically difficult. Although SCPL is effective in patients with laryngeal cancer, postoperative rehabilitation of speech and swallowing is needed for good functional outcomes. We discuss what patients can benefit from SCPL, with CHEP or CHP, and when these procedures are contraindicated.

Introduction: The two major recognized risk factors for laryngeal cancer are tobacco and alcohol consumption. However, according to WHO and IARC, occupational factors are probably underestimated. Confirmed/highly probable occupational carcinogens related to laryngeal cancer include asbestos and strong organic acid mists. Probable carcinogens are cement, polycyclic aromatic hydrocarbons (PAH) and wood dust. Suspected carcinogens are organic solvents, formaldehyde, coal/silica dust, leather dust and cotton dust. An important problem of epidemiological studies appears to be the lack of differentiation between laryngeal and hypopharyngeal (squamous cell) carcinomas. Material and Methods: For the purposes of this study, a database of 164 well-documented cases of laryngeal and hypopharyngeal cancers was created by collecting data from 4 centres in Belgium, Italy and France. In order to identify confirmed, probable and suspected carcinogenic risks, jobs and exposures over the whole career of the patients were exhaustively identified and mapped. Confounding factors were taken in account. Results: In general, occupational exposure to known or suspected carcinogens is significantly higher in laryngeal than in hypopharyngeal carcinomas. As regards the exposed subjects, the global occupational exposure profiles differ significantly (10 categories). Asbestos and acid mist exposures are significantly more important in laryngeal carcinoma. Coal/silica dust exposure is significantly more important in pharyngeal carcinoma..

Opis:
Objective: To assess the sensitivity and specificity of larngovideostroboscopy (LVS) in the diagnosis of precancerous and malignant lesions of the vocal folds. Material and methods: In 175 patients (128 men and 47 women), aged 19-88 years, mean age 61.5, who were admitted to the clinic with diagnosed premalignant conditions of vocal fold mucosa (leukoplakia, chronic hypertrophic inflammatory lesions) and thickening or tumor on the vocal fold, there was performed LVS before the laryngeal microsurgery. The LVS study included: localization of the leasion, movement of the vocal folds, mucosal wave, shape of glottis clousure, amplitude and symmetry of vocal fold vibration. In the evaluation, a point scale was applied for the individual functional parameters. The scale ranged from 0 to 14. Patients with impaired vocal fold motion or absent mucosal wave were positive on LVS for malignant lesions. Those with limitted mucosal wave were positive on LVS for dysplastic lesions. The results were compared with the final histopathological examination and the sensitivity, specificity, accuracy, positive (PPV) and negative (NPV) predictive value were calculated. Results: On the basis of histopathological examination, benign lesions (normal or inflammatory mucosa) accounted for 20% of diagnoses, hypertrophy and parakeratosis for 28%, low and middle grade dysplasia accounted for 10% and malignant lesions (high-grade dysplasia, pre-invasive cancer, Invasive cancer) was diagnosed in 42% of patients. The overall mean score for LVS was 4.5 and 8.0, respectively for benign and malignant lesions. Sensitivity, specificity, accuracy, PPV and NPV of LVS in detecting malignant lesions were respectively - 95.6%, 23.8%, 61.1%, 57.6% and 83.3% and in detecting both premalignant and malignant lesions were respectively – 90.7%, 31.4%, 78.9%, 84.1% and 45.8%. Conclusions: Because of the high sensitivity of LVS in detecting precancerous and malignant lesions, this method is a very good tool for screening of pathology within the larynx.

Aim: The aim of the study was to evaluate serum concentrations of total sialic acids (TSA) in patients with squamous cell carcinoma of the larynx preoperatively and 4 years postoperatively and to compare the results with the serum concentration of sialic acids in patients with non-neoplastic laryngeal lesions. Material and methods: The study comprised 52 patients with squamous cell carcinoma of the larynx and 25 patients with non-neoplastic laryngeal lesions. Serum TSA concentrations were assessed in all patients preoperatively. 4 years after surgery, serum TSA concentrations were assessed again in 21 patients from the study group in whom recurrence was not detected. Results: Significantly higher serum concentrations of TSA were found in patients with laryngeal cancer compared to the control group (P < 0.05). Changes in TSA concentrations in the study group depended only on cancer grade (G). Significantly higher TSA concentrations were found in G3 patients. Such a relationship was not found for other parameters assessed. Alcohol consumption was the only important factor that affected TSA concentration in the control group. Significantly higher TSA concentrations were found in patients who admitted alcohol abuse. Significantly lower TSA concentrations were observed in patients with laryngeal cancer who were assessed 4 years postoperatively (P < 0.05). Conclusion: Serum concentration of TSA was increased significantly in laryngeal cancer and decreased significantly after several years of successful treatment. Further studies are warranted to assess the possibility of using sialic acids as a marker or target for immunotherapy.
Cel: Celem pracy była ocena poziomu całkowitego kwasu sjalowego (TSA) w surowicy pacjentów z rakiem płaskonabłonkowym krtani przed leczeniem operacyjnym oraz 4 lata po leczeniu operacyjnym, a także porównanie uzyskanych wyników z poziomami kwasu sjalowego w surowicy pacjentów z nienowotworowymi zmianami w obrębie krtani. Materiał i metody: Badaniem objęto 52 pacjentów z płaskonabłonkowym rakiem krtani oraz 25 chorych z nienowotworowymi zmianami w krtani. U wszystkich pacjentów dokonano pomiaru stężenia TSA w surowicy przed leczeniem chirurgicznym. Po upływie 4 lat od leczenia chirurgicznego dokonano ponownej oceny poziomu TSA w surowicy 21 pacjentów z grupy badanej, u których nie stwierdzono nawrotu choroby. Wyniki: Zaobserwowano istotnie wyższe stężenia TSA w surowicy pacjentów z rakiem krtani, w porównaniu z pacjentami z grupy kontrolnej (p<0,05). Zmiany w stężeniach TSA w grupie badanej zależały jedynie od stopnia złośliwości nowotworu (G). Istotnie wyższe stężenia TSA odnotowano u pacjentów z nowotworami stopnia G3. Nie stwierdzono jednak podobnej zależności dla pozostałych ocenianych parametrów. Jedynym istotnym czynnikiem wpływającym na stężenie TSA było spożywanie alkoholu. Istotnie wyższe stężenia TSA obserwowano u pacjentów przyznających się do nadużywania alkoholu. U pacjentów z rakiem krtani, badanych 4 lata po leczeniu chirurgicznym, stwierdzono znaczne obniżenie stężeń TSA (p <0,05). Wnioski: Stężenie TSA w surowicy ulega znacznemu zwiększeniu w przebiegu raka krtani i znacznie obniża się w ciągu kilku lat od zastosowania skutecznego leczenia. Ocena możliwości wykorzystywania kwasu sjalowego jako markera lub punktu uchwytu immunoterapii wymaga przeprowadzenia dalszych badań.
